Lymphorrhea and lymphocele are the common complications after urological pelvic lymph node dissection, which are difficult to diagnose and treat. Up to now, a few studies of lymphorrhea and lymphocele were reported, and there is no relevant diagnosis and treatment guideline. The pathogenesis, treatment and prevention of lymphorrhea and lymphocele formation after urological pelvic lymphadenectomy were reviewed in this article.The mechanism of occurrence includes leakage and exudation of lymph-vessels. The high risk factors are old age, open surgery and perioperative use of anticoagulants. Most patients can be cured by nutrition management and percutaneous catheter sclerotherapy. Closing lymph-vessels and stumps may paly an important role in prevetion of lymphorrhea and lymphocele.

Objective To discuss the effect of small needle knife therapy on collagensⅠ and Ⅲ in the hypertrophic scar tissues that was subcutaneously transplanted into nude mice. Methods Six samples of human hypertrophic scar tissues without cuticle were subcutaneously transplanted into the back of 24 nude mice to create the animal models of hypertrophic scar. Ten days after of the operation,the mice were divided into control,0.1 mg/ml triamcinolone,0.2 mg/ml triamcinolone,and small needle-knife groups with 6 mice in each. Specimens of the scar tissues were collected in 14 days for immunohistochemistry to detect the collagen Ⅰ and Ⅲ. Results In all the groups,collagens Ⅰand Ⅲ distributed extensively in the cytoplasm of fibroblast and tissues. Image analysis showed that the concentration of collagens Ⅰand Ⅲ in 0.1 mg/ml and 0.2 mg/ml triamcinolone groups were significantly lower than those in the control (0.09?0.03,0.11?0.05 and 0.12?0.02,0.11?0.01 vs. 0.17?0.04,0.19?0.03,P0.05). Conclusions Small needle-knife therapy can reduce the concentration of collagens Ⅰand Ⅲ in transplanted hypertrophic scar tissues in nude mice.

This study was aimed to observe the effect of Softening Liver and Reducing Enzyme Mixed Agent (SLREXA) in the prevention of acute liver injury rats induced by carbon tetrachloride (CCl4). A total of 60 male SD rats were randomly divided into 6 groups, which were the SLREXA low-, middle-, high-dose group, glucurolactone group, normal group and model group. Intraperitoneal injection of CCl4 was used to induce acute liver injury rat mod-el. Intragastric administration of SLREXA was given to each Chinese medicine group. Intragastric administration of distilled water was given to the normal group and the model group. Intragastric administration of glucurolactone aque-ous solution was given to the glucurolactone group. On the 12th day of the experiment, after 16-hour fasting, rats were killed. Pathological changes in liver tissues were examined. Blood serum was determined for alanine aminotrans-ferase (ALT) and aspartate aminotransferase (AST). The liver homogenate was determined for superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), glutathione (GSH), catalase (CAT) and malondialdehyde (MDA) in liver tis-sues of rats. RT-PCR was used to detect the expression level of mRNA in liver heme oxygenase-1 (HO-1). The re-sults showed that in the microscopic examination of liver tissues, compared with the model group, different doses of SLREXA can alleviate pathological damages of liver in varying degrees. Levels of blood serum ALT and AST content in different doses of SLREXA groups and glucurolactone group were significantly lower than those of the model group (P < 0.05 or P < 0.01). Compared with the model group, contents of GSH-Px, GSH, SOD, CAT in the liver ho-mogenate were significantly increased, and MDA content was decreased significantly (P< 0.05 or P< 0.01) in differ-ent doses of SLREXA groups and glucurolactone group; compared with the model group, the HO-1 mRNA relative expression quantity in the normal group and each treatment group increased obviously, with statistical significance (P< 0.05 or P< 0.01). It was concluded that SLREXA can prevent CCl4-induced liver injury rats with definite thera-peutic effect.

Early use of statins after onset of acute ischemic stroke can improve the functional prognosis of patients and reduce in-hospital mortality.This article reviews the preclinical and clinical trials of statins for the treatment of ischemic stroke.

Objective To investigate the operative methods and their efficacy of the modified decompressive craniectomy in combination with indirect revascularization for the treatment of massive cerebral infarction. Methods The clinical data of 15 patients with massive cerebral infarction who underwent modified decompressive craniectomy in combination with encephaloduroarteriomyosynangiosis were analyzed retrospectively. The clinical efficacy was assessed according to the mortality, Barthel Index and modified Rankin scale. Results All the patients were followed up, and 12 patients survived 3 months after operation, of those, 4 had a good functional outcome; 11 patients survived 6 month after operation, of those, 7 had a good functional outcome. Conclusions The modified decompressive craniectomy in combination with indirect revascularization for the treatment of massive cerebral infarction could effectively treat massive cerebral infarction and increase the survival rate and life quality of patients, however, the surgical timing, surgical indications and surgical skills are needed to master accurately.

Objective:To study the best opportunity for minimal invasive puncture in the treatment of hypertensive intracerebral hemorrhage.Methods:Forty-one patients with hypertensive intracerebral hemorrhage were randomly divided into four groups:<6 h,6-12 h,12-18 h and 18-24 h from the onset of symptoms.The comparison among groups in outcome measures were performed,including the rebleeding rate,short-term efficacy,long-term efficacy,mortality,and excellent and good rate.Results:The incidence of complication such as rebleeding etc had no significantly difference in the four groups.The excellent and good rates of clinical outcomes at 3 months were the<6 h group>6-12 h group,and 12-18 h group>18-24 h group(P<0.05).The 18-24 h group had the highest mortality(30%,3/10).Conclusions:Within 6 h after the onset of hypertensive intracerebral hemorrhage was the best opportunity for the minimal invasive puncture.

Objective To observe the efficacy,toxic side effects, survival time and quality of life (QOL) of vinorelbine and vinorelbine plus carboplatin in elderly patients with stage Ⅲ b/Ⅳ non-small cell lung cancer(NSCLC).Methods Eighty patients aged 65 years or over with stage Ⅲ b/Ⅳ non-small cell lung cancer were randomly divided into two groups.One group was treated with vinorelbine (vinorelbine 25 mg/m2 iv d1,8, repeated every 21 days), the other group was treated with vinorelbine plus carboplatin(vinorelbine 25 mg/m2 iv at day 1 and 8 and earboplatin AUC5 iv at day 1, repeated every 21 days).Results The response rate(RR), median survival time(MST) and 1-year survival rate were 35.0%, 9.0 months and 35.0% in vinorelbine group and were 42.5%, 10.0 months and 37.5% in vinorelbine plus carboplatin group respectively.There was no significant difference between two groups(χ2 =0.296,P=0.586).The incidences of Ⅲ-Ⅳ degree granulocytopenia (χ2 =7.168,P=0.014), Ⅲ-Ⅳ degree thrombocytopenia (χ2 = 5.165,P=0.048)and Ⅲ-Ⅳ degree nausea and vomiting (χ2 =6.275, P =0.025) were significantly higher in the combined chemotherapy group than in the vinorelbine treatment group.The scores of lung cancer symptom scale (LCSS) of appetite loss(χ2 =2.600,P=0.011), fatigue(χ2 =3.169,P=0.002) and pain(χ2 =2.257,P=0.027) were more higher in the vinorelbine treatment group than in the combined chemotherapy group.Conclusions Vinorelbine regimen is effective, well tolerated and more favorable for the elderly NSCLC patients.

Objective To evaluate the value of transient elastography (TE) for predicting severity of liver fibrosis in patients with chronic hepatitis B (CHB).Methods A total of 969 patients with CHB was enrolled and recruited for analysis,which had been received TE scan,including 258 patients of liver biopsy,and 117 patients of gastric endoscopy.Results A total of 35 patients was excluded from analysis due to TE failure or unreliable TE.Liver stiffness measurement (LSM) was independently influenced by bilirubin,AST,liver fibrosis and inflammation,ultrasonic score and albumin.TE predicted Child-Pugh C,B/C,liver fibrosis S4,≥S3 and ≥ S2 with respective area under receiver operating characteristics curves (AUROC)0.907 (95% CI 0.886-0.928 ),0.920 ( 95% CI 0.899-0.940 ),0.871 ( 95% CI 0.819-0.923 ),0.852(95%CI0.805-0.899) and 0.807(95% CI0.749-0.865),respectively.While LSM ＜32.2 kPa excluded Child-Pugh C with 99.4% probability,LSM ≥35.3 kPa determined Child-Pugh B/C with positive predictive value (PPV) 0.820.For compensated CHB,cut-offs of LSM 23.3,15.2 and 10.8 kPa diagnosed cirrhosis,liver fibrosis ≥S3 and ≥S2 with positive likelihood ratio nearly 10.0 and PPV 0.692,0.882 and 0.980,respectively; and cut-offs 8.8 kPa,6.6 kPa excluded cirrhosis,liver fibrosis ≥ S3 with negative likelihood ration nearly 0.1 and negative predictive value 0.977 and 0.903,respectively.Correlation coefficient between LSM and grades of esophageal varices was only 0.180,and AUROC for TE predicting EV was of no clinical value.ConclusionTE relatively make accurate prediction in the severity of liver fibrosis and classification of Child-Pugh.Patients with LSM ≥ 10.8 kPa should be considered for receiving antivirus treatment.
