Objective To investigate dilatancy effect of the Hypertonic Sodium Chloride Hydroxythyl Starch 40 injection (HSH) for the treatment of hemorrhagic shock. Methods 70 cases with trauma induced hemorrhagic shock were randomly divided into two groups:Hypertonic Sodium Chloride Hydroxythyl Starch 40 injection group (H group), Ringer's group (R group), 35 cases in each group.Patients of H group were infused HSH10ml/kg within 30 minutes, the R group were infused Ringer's 10ml/kg at the same time.Central venous pressure (CVP), systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR) and oxygen saturation (SpO2) were monitored before the infusion, after the infusion and finished 30 minutes separately.Na+、Cl-、K+ were examined before infusion and after infusion respectively. Results The CVP、SBP、DBP、HR and SpO2 in both groups were improved than before infusion, but those in H group is much better than R group, with statistical significance (p<0.05).The potassium was reduced moderately and sodium ions and chloride ions were increased in H group than in R group, but it was still within normal range. Conclusion Appling HSH (10ml/kg) to patients with hemorrhagic shock can be better than the compound prescription of sodium lactate.

Objective To investigate the concentration of blood uric acid and the prevalence of hyperuricemia in staff of a univer-sity so as to provide scientific basis for prevention and treatment of hyperuricemia. Methods Uric acid(UA)of 4 893 persons were detected by enzymatic method. For male whose UA>428 μmol/L,For female whose UA>357μmol/L were considered as hyperuri-cemia.People enrolled in the study were grouped according to age and analyzed. Results The average UA of males was (357.23± 75.17)μmol/L and was significantly higher than that of females(P <0.05).The average UA concentration and hyperuricemia rati-os in males of 20-<30,30-<40 years old groups were the highest, that of 60 -<70 group was the lowest, the difference was statistically significant(P <0.05).The average UA and hyperuricemia ratios of the male group decreased with age before 70 year old.But The average UA and hyperuricemia ratios of 70-<80 and ≥80 groups were higher than that of the 60-<70 group, the difference was statistically significant(P <0.05).The average UA and of hyperuricemia ratio of the female group increased with age. Conclusion The average UA concentration of males was significantly higher than that of female,in addition to that,hyperuri-cemia patients become younger and younger in male population.

Objective To investigate the effect of prophylaxis against respiratory and circulation depression caused by propofol during painless colonoscopy by intravenous doxpram with small dose. Methods 80 patients undergoing painless colonoscopy were randomly divided into the control group(A group) and the doxpram group(B group). All patients used propofol, and then used propofol after induction. In group B,doxpram was injected before anesthesia. The degree and incidence rate of respiratory and circulotion depression, the dose of propofol, the awake time, the examination time, the complication during anesthesia and awake were monitored. Results The SBP,DBP,SpO2,RR during anesthesia were obviously lower in group A than in group B(P

60 years old). 43 cases of antiepileptic drugs combination accounted for 10% and the therapeutic plasma concentration of 27 cases deviated from normal range(62.8%). CONCLUSION:Results of plasma concentration monitoring provide an important basis for clinical drug use. Monitoring data and other clinical index can promote rational use of antiepileptic drugs.

Objective To review the anatomic and visual outcomes of a series of patients with macular holes who underwent vitreous surgery without adjunctive therapy. Methods We performed a retrospective review of 16 patients who underwent vitrectomy, fluid-gas exchange and perflu oropropane(C3 F8) gas temponade for 14 eyes with idiopathic and 2 eyes with traumatic macular holes. Results Anatomical closure of the macular holes was achieved in 15(93%) of 16 eyes with an average follow-up of 7 months. Only one eye needed performing second surgery. Three(19%) eyes achieved postoperative visual acuity of 0.4, four (25%)eyes 0.2~0.3,and nine(56%)eyes 0.1 or worse. Conclusions Vitreous surgery without adjunctive therapy with intraocular gas tamponade and postoperative posturing is a well tolerated and effective intervention for idiopathic or some traumatic macular holes. Anatomical closure of the macular hole is associated with a significant improvement in visual acuity in most eyes.

Objective To investigate the effect and clinical valve of target-controlled infusion of remifentanil for conscious intubation of patients under fibrobronchoscopy. Methods Forty patients were randomly divided into two groups(F and R),each group contained 20 cases. All of the patients were intubated on regional anaesthesia,as the first step, then the patients in F group were given fentanyl 2?g/kg by vein.In remifentanil target-controlled infusion group(R group),the patients were kept infusing remifentanil 2ng/ml.Results (1)Haemodynamics:in R group,SBP and HR kept steadily; In F group, while the two indices increased significantly.(2)Static pulmonary function:in R group,RR,VT deceased remarkablely. There were no difference between two groups in SpO_2 and P_ ET CO_2.(3)Tracheal intubating condition and Ramsay score in R group were better than in F group;Cardiovascular response in R group was lower. Conclusion Target-controlled infusion of remifentanil for conscious intubation can achieve excellent result, with less cardiovascular response.

Objective:To analyse the evaluation and experience of clinical effect of minimally invasive drainage and craniotomy in the treatment of patient with epidural hematoa.Methods:100 cases of patients who were diagnosed as epidural hematoma from January 2015 to January 2016 were selected and randomly divided into two groups, where the control group were given craniotomy, and the observation group were given minimal invasive drainage. The effect of the surgery and the life qualities before and after treatment of the two groups were compared. Results:The effect of observation group is better than that of control group (P < 0.05);craniocerebral defect and the re-hemorrhage incidence occurrence in the observation group were obviously lower than those in the control group,(0 %,6 % vs 12 %,20 %) (P<0.05).Conclusion:Compared with craniotomy, minimal invasive drainage in clinical treatment of epidural hematoma showed advantage in at smaller trauma, faster recovery, and better effect, thus deserving further promotion of clinic application.
