1.Nardosinone reduces neuronal injury induced by oxygen-glucose deprivation in primary cortical cultures.

The aim of the study is to investigate the effect of nardosinone (Nar) on neuronal injury induced by oxygen-glucose deprivation (OGD) in primary cortical cultures isolated from embryos at gestational day 14. MTT method was used to determine the dosage regimen of Nar in primary neuronal cultures and observe the influence of Nar on the neurons suffering OGD; Western blotting analysis was used to detect expressions of protein kinase A (PKA), Ras related protein 1 (Rap1), mitogen-activated protein kinase kinase 1 (MEK1) and phospho-extracellular signal-regulated kinase 1/2 (p-ERK1/2) of OGD-injured or uninjured primary cultured neurons after Nar treatment. Results showed that Nar (50 and 100 micromol x L(-1)) improved the cell viability during OGD damage (P < 0.01) and increased the expression of PKA, Rap1, MEK1 and p-ERK1/2 in injured neurons. Additionally, elevations of PKA, Rapl, MEK1 and p-ERK1/2 in uninjured neurons were caused by Nar (50, 100 and 200 micromol x L(-1)) with a dose-dependent tenclency as well (P < 0.01). In conclusion, Nar could protect against the neuronal injury exposed to OGD, which may be relevant to the promotion of PKA and ERK signaling pathway.
2.A systematic review of diagnostic value of miRNA for breast cancer

Objective To systematically review the diagnostic value of microRNA(miRNA) quantitation in breast cancer .Meth-ods Literatures about miRNA and breast cancer diagnosis were selected by retrieving Medline ,Embase and Cochrane Library .Ac-cording to the inclusion and exclusion criteria ,the literatures were independently screened ,and a 2 × 2 contingency table was con-structed .Quality of literatures was assessed by quality assessment of diagnostic accuracy studies(QUADAS) .Statistical analysis was performed by employing Meta-Disc 1 .4 software and STATA 11 .0 .Results 16 studies were included ,which contained 1 303 patients and 711 control samples .There were threshold effects among these studies (the spearman′s correlation coefficient was-0 .758 ,P=0 .001) .A random effects model was used for meta-analysis .The summary sensitivity ,specificity ,positive likelihood ratio ,negative likelihood ratio ,and diagnostic odds ratio for miRNA in breast cancer diagnosis were 0 .77(95% CI:0 .75 -0 .79) , 0 .77(95% CI:0 .74-0 .80) ,4 .19(95% CI:2 .79-6 .30) ,0 .25(95% CI:0 .19-0 .35) ,19 .91(95% CI:9 .68-40 .95) .The area un-der curve of SROC was 0 .895 0 .Conclusion These results suggest that miRNAs have potential value to diagnose breast cancer . However ,effective diagnosis of breast cancer still needs to be conducted with assistance of clinical findings and traditional lab inves-tigations .
3.Aberrant expression and mutation of FOXQ1 in colorectal carcinoma and adenoma

Objective To investigate the clinical pathological association of FOXQ1 with colorectal carcinoma (CRC) and colorectal adenoma (CRA). Colorectal mucosa specimens were collected between Jane 2007 and June 2009 in the First hospital of Yunnan prorince, and consisted of CRC (n=76), CRA (n=50) and normal (n=48,≥8 cm apart from cancer) tissues. Methods The expression of FOXQ1 in colorectal mucosa with was detected using immunohistochemistry (SP method). PCR amplification and direct DNA sequencing were used to identify FOXQ1 gene mutations in 23 CRC, 22 CRA and 18 normal specimens. Results There was no expression of FOXQ1 in normal specimens. Aberrant expression of FOXQ1 in either CRC or CRA specimens was significantly higher than in normal colorectal mucosa tissue (76.3% vs 0.0%, 26.0% vs 0.0% respectively, P<0.01).Aberrant expression rates of FOXQ1 was significantly higher in CRC than that in CRA (76.3% vs 26.0%, P<0.01). Expression level of FOXQ1 was increase gradually along with the pathological process of colorectal adenoma-carcinoma sequence. In CRC, the aberrant expression of FOXQ1 was not only distributed in nuclear of the tumor cells, but also found in the cytoplasm and the extracellular matrix. For CRC patients, the aberrant expression rates of FOXQ1 in extracellular matrix in Dukes C/D was significantly higher than that in Dukes A/B (61.5% vs 3.3%, 61.5% vs 5.0% respectively,P<0.01);The aberrant expression of FOXQ1 in extracellular matrix in patient with lymph node metastasis was significantly higher than those without lymph node metastasis (61.5 % vs 4.0 %, P<0.01). FOXQ1 gene mutation was only identified in one out of 23 CRC specimens, but not found in 22CRA nor 18 normal colorectal mucosa samples. Conclusions Aberrant expression of FOXQ1 may play an important role in the carcinogenesis of CRC and FOXQ1 gene may be involved in the cancer erosion and lymph node metastasis. FOXQ1 mutation is not the primary cause of aberrant FOXQ1 expression in CRC.
4.AngⅡin paraventricular nucleus contribute to chronic intermittent hypoxia induced-hypertension in rats

Aim To determine whether AngⅡin para-ventricular nucleus (PVN)was involved in the chronic intermittent hypoxia (CIH ) induced-hypertension in rats.Methods Male Sprague-Dawley rats were ran-domly divided into Sham and CIH groups,the Sham rats were exposed to continuous normoxia,while the CIH rats were submitted to CIH (8 h per day for 15 days).The conscious noninvasive method with tail cuff was performed in rats to record the systolic blood pres-sure during establishing the model of CIH induced hy-pertension.Mean arterial pressure (MAP)and heart rate (HR)were recorded in vivo on a PowerLab data acquisition system after CIH.Rats were fixed on the stereotaxic instrument to conduct microinjection in the PVN.We used Western blot to measure Ang Ⅱ level and AngⅡtype 1 receptor (AT1 R)protein expression in PVN.Results The level of PVN Ang Ⅱin CIH rats was significantly higher than that in Sham rats,a-long with increased AT1 R protein expression.Microin-jection of Ang Ⅱ(0.03,0.3,3 nmol)in bilateral PVN dose-dependently increased MAP in both CIH and Sham rats,and this response was significantly augmen-ted in CIH rats.Losartan (50 nmol),AT1 R antago-nist,had no effect on MAP in Sham rats,but caused significant MAP decreases in CIH rats,and prevented Ang Ⅱ-induced increases in MAP in both CIH and Sham rats.Conclusion The results suggest that the increased AngⅡrelease and enhanced AT1 R activation in the PVN contribute to CIH induced-hypertension in rats.
5.Expression of plasma miR-199a-5p and miR-200c-3p and its clinical relevance in gastric carcinoma

Objective To explore the relative expression of plasma miR-199a-5p and miR-200c-3p in gastric adenocarcinoma cancer(GAC) patients and its clinical value.Methods Case-control study was used in this research.The relative expression of plasma miR-199a-5p and miR-200c-3p from 47 GAC patients and 50 healthy controls were determined by RT-PCR ( TaqMan Probe method).Meanwhile, the association with age, gender, tumor location, size, degree of differentiation, TNM stage, lymph node metastasis and other clinical pathological parameters were analyzed.The expression of these two miRNAs in plasma of 30 GAC patients during preoperation was compared with their expression 6-8 days after radical surgery.The sensitivity and specificity of plasma miRNAs expression for the diagnosis of GAC were analyzed using the receiver operating characteristic ( ROC ) curve.SPSS20.0 statistical software was used for statistical analysis.T-test, paired t-test and one-factor ANOVA were used for normal distribution of quantitative data.Results The plasma level of miR-199a-5p in GAC patients was significantly lower(1.05 ±0.22) (t =3.058,P =0.003), while miR-200c-3p was significantly higher(15.15 ±3.02) (t =-2.854,P=0.006), when they were compared with those in controls(26.80 ±8.38, 3.39 ±0.87).Low miR-199a-5p expression in GAC patients were associated with lymph node metastasis ( F =4.725, P =0.029) and the differentiation degree of gastric cancer(F=3.854,P=0.032).The relative expression of miR-199a-5p in postoperative plasma was significantly increased(t=-3.814,P=0.001), but the relative expression of miR-200c-3p was significantly reduced when compared to the preoperative samples(t=2.978, P=0.006).Area under the ROC curve of miR-199a-5p, miR-200c-3p and combined miR-199a-5p and miR-200c-3p were 0.692, 0.792 and 0.798, the sensitivity and specificity were 87%,97%,92.5% and 43%,54%, 65%, respectively.Conclusion Combined detection of miR-199a-5p and miR-200c-3p in plasma has a higher sensitivity and specificity than the conventional tumor marker CEA and CA19-9, and may be a useful combination for gastric cancer diagnosis.
6.Clinical analysis of 18 cases of neurosyphilis

Objective To analyze the clinical features, diagnosis, treatment and prognosis of neurosyphilis. Methods Clinical data on and laboratory findings in 18 cases with neurosyphilis collected in the Affiliated Provincial Hospital, Anhui Medical University from 2006 to 2008 were retrospectively studied.Results Among the 18 patients, 3 sufferred from asymptomatic neurosyphilis, 1 from meningeal syphilis, 7 from meningovascular syphilis, 5 from paralytic dementia, and 2 from intracranial space-occupation. Toluidine red unheated serum reagin test (TRUST) and Treponema pallidum particle agglutination test (TPPA) of sera were positive in all the patients; cerebrospinal fluid (CSF) TRUST was positive in 16 patients, and CSF TPPA in all patients. An increase was observed in CSF leukocyte count in 7 patients and in CSF protein in 13 patients.The findings on cerebral magnetic resonance imaging (MRI) mainly included demyelination, brain atrophy,cerebral infarction, etc. All the patients, except 2 with a TRUST titer of 1:4, experienced a 4-fold decrease in TRUST titer within a 3-month follow up. Clinical symptoms of neurosyphilis improved in all patients except 1 with paralytic dementia. Conclusions The diversity of clinical manifestations usually leads to the misdiagnosis of neurosyphilis, which should be diagnosed based on comprehensive analysis of clinical characteristics as well as laboratory and imaging findings. Early diagnosis and treatment are beneficial to its prognosis.
7.Changes of bulbocavernosus sphincter reflex in male rabbit models of sacral spinal ischemia during early stage

Objective To explore the changes of bulbocavemosus sphincter reflex (BCR) in male rabbit models of spinal cord ischemia injury of different levels at acute stage and their values in predicting spinal cord functions,and provide theoretical basis for protecting the spinal cord function by neurophysiology examination in clinical microsurgical operation.Methods Thirty-six adult New Zealand rabbits were randomly divided into 6 groups:a control group (n=6) was used to eliminate the effects of anesthesia and surgery on BCR;rabbits in the experimental group (n=30) were randomly divided into 5 groups (n=6) according to different levels of lumbar arteries being ligatured between the left renal artery and arterial bifurcate (subgroups of one,two,three,4 and 5 lumbar arteries being ligatured).BCR was recorded continuously within two h of ligation;rabbits in each group were subjected to double-blind Tarlov lower limb motor function scale before and 2 d after surgery;and ischemic spinal cord specimens were performed hematoxylin and eosin (HE) staining to observe pathological changes in sacral spinal cord ischemic areas after 2 days.Results There was no significant difference in BCR amplitude and latency between different time points after anesthesia (P>0.05);there was no significant difference in BCR amplitude and latency between different time points before and after operation (P>0.05).There was no significant change in the amplitude of BCR before and after one lumbar artery ligation;however,the BCR amplitude immediately changed after ligaturing two,three,4 or 5 lumbar arteries:the latency of BCR immediately began to extend and amplitude immediately began to reduce;amplitude was stable respectively after (0.7±0.4),(0.7±0.3),(0.7±0.3) and (0.6±0.3) min of ligation.The amplitude variation of BCR was positively correlated with Tarlov lower limb motor function scale scores 2 days after ligation (r=0.791,P=0.0001).HE staining showed that the normal sacral spinal cord structure was observed in the control group,subgroups of one and two lumbar arteries being ligatured;the basically normal sacral spinal cord structure was observed in the subgroup of three lumbar arteries being ligatured;the normal sacral spinal cord structure basically disappeared in the subgroup of 5 lumbar arteries being ligatured;the spinal cord injury of subgroup of 4 lumbar arteries being ligatured was between that of subgroup of three lumbar arteries being ligatured and subgroup of 5 lumbar arteries being ligatured.Conclusion The amplitude and latency of BCR in male rabbits are sensitive to sacral cord ischemic injury,which can help find ischemic injury in the reversible phase of the sacral cord ischemia and provide theoretical basis for the prevention of occurrence of male irreversible sacral spinal cord injury during microsurgery.
8.A survey on clinical practice of digestive endoscopy in county-level hospitals of Yunnan Province

Objective:To analyze general information, diagnosis and treatment of digestive endoscopy departments in county-level hospitals of Yunnan Province, thus to provide evidence for improving the endoscopic diagnosis and therapy in these hospitals.Methods:An online survey was performed to collect quality-related information of digestive endoscopy centers (departments) at county-level hospitals of Yunnan from January 2019 to January 2020, including endoscopic equipment, endoscopic techniques, staffing, number of operations, and quality control.Results:A total of 143 county-level hospitals were involved in this study. Each hospital owned only 1.74 endoscopy operation rooms on average, 1.42 regular endoscopy workstation, 4 endoscopes. There were only 10 endoscopic ultrasonography workstations and 2 enteroscopy workstations respectively installed in these hospitals. Endoscopic retrograde cholangiopancreatography was independently performed in only 4 hospitals. There were 392 professional endoscopists in all 143 hospitals, 18.62% (73/392) of whom were able to perform endoscopic therapy of grade 4, while only 6.12% (24/392) of whom could perform endoscopic submucosal dissection (ESD) independently. In 2019, the early diagnostic rate of gastrointestinal tract cancer in these hospitals was 19.48% (1 133/5 817). The early diagnostic rate was 21.04% (276/1 312) for esophageal cancer, 19.53% (397/2 033) for gastric cancer, and 18.61% (460/2 472) for colorectal cancer. The colorectal adenoma detection rate was 12.83% (12 207/95 148). The rate of reaching cecum during colonoscopy was 70.49% (67 067/95 148). The complete resection rate of ESD was 12.54% (221/1 763).Conclusion:The general situation of digestive endoscopy in county-level hospitals of Yunnan is far from being well-developed, which may be attributed to slow upgrading of equipments, inadequate training of endoscopic professionals and undemanding quality control. It is highly significant to improve medical service and quality of diagnosis and treatment of digestive endoscopy in these hospitals, on the basis of a complete endoscopy quality control system with a high standard.
9.Application research of mixed teaching based on DingTalk platform in clinical practice teaching of respiratory department

Objective:To explore the application effect of online and offline mixed teaching based on DingTalk platform in clinical practice teaching of respiratory department.Methods:The Batch 2016 ( n= 40) and Batch 2017 ( n=47) undergraduates of clinical medicine in Qiqihar Medical College were selected as the research objects, and they were divided into a control group and a research group. The control group adopted traditional offline theoretical teaching, while the research group adopted online-offline mixed teaching, focusing on teaching design and teaching objectives, teaching subjects, teaching resources, network platforms and teaching activities (classroom learning and evaluation). The theoretical and practical test scores of the two groups of interns were compared, and the evaluations of the students in the research group on the effect of online and offline mixed teaching were collected. Excel 2013 was used to sort out the data, and R software was used for data processing and statistical analysis. T-test was used for measurement data and Chi-square test was used for counting data. Results:The operational test scores [(28.16±1.70) points] and theoretical scores [(82.07±6.40) points] of the students in the study group were higher than those in the control group [(23.35±2.88) points and (76.20±5.67) points], with significant differences ( P<0.001). Compared with the students in the control group, the students in the research group were more satisfied with the mixed teaching method. Conclusion:The online and offline mixed teaching is conducive to improving students' comprehensive performance, learning effect, and promoting students' clinical diagnosis and treatment skills, self-study, innovation abilities and clinical competence.
