1.Clinical Study on Rosiglitazone Monotherapy of Early Type 2 Diabetes

OBJECTIVE:To evaluate the efficacy and safety of domestic rosiglitazone monotherapy in newly diagnosed patients whose type 2 diabetes is inadequately controlled.METHODS:In a double-blind,randomized,with placebo controlled phase Ⅱ clinical trial,patients were assigned to receive 4mg/d of rosiglitazone(n=27) or placebo(n=27) for 8 weeks.RESU-LTS & CONCLUSION:Fasting and postprandial plasma glucose levels of the rosiglitazone group decreased by 27.1% and 55.7%,and fasting and postprandial plasma glucose levels of the control group decreased by 10.6% and 19.8% respectively.These changes of fasting and postprandial plasma glucose levels in the rosiglitazone group were significantly higher than those in the placebo group(P
2.Analysis of apelin level and related factors in patients with obesity and different glucose tolerances

Objective To evaluate apelin level in patients with obesity and different glucose tolerances, investigate relevance of apelin with body mass index(BMI), blood lipid and blood glucose. Methods According to different glucose results of oral glucose tolerance test, 120 subjects were divided into three groups: normal glucose tolerance(NGT),impaired glucose tolerance(IGT) and type 2 diabetes(T2DM). All of the subjects were also divided into obese and non-obese subgroups based on their BMI(≥25kg/m~2 or <25kg/m~2 ). Plasma apelin levels were compared among groups of NGT, IGT and T2DM. Fasting plasma glucose(FPG)and 2h postprandial plasma glucose (2hPG) after OGTT, lipid profile and BMI were measured. Results Both fasting and 2h postprandial plasma apelin had significant differences in these groups (P<0.05,or P<0.01). The overweight and obese subgroups in the IGT group and diabetes group had higher apelin levels than their counterpart subgroups in the NGT group. BMI, FPO, systolic blood pressure(SBP) were independent influencing factors for apelin (P<0.05 ,or P<0.01). Conclusions Plasma apelin level is related to obesity and type 2 diabetes.
3.The ReIationship Between Glycemia,Lipidemia and Proteinuria in Different Stages of Diabetic Nephropathy.

Objective To establish the prevalence of renal involvement in a large group of patients with type 2 diabetes and evaluate several risk factors of early stages and advanced stages of albuminuria. Methods Five hundred and sixty-six patients(265 males, mean age:57.8?13.3 years, diabetes duration 8.35?6.4 years;301 females, mean age:60.0?12.4 years, diabetes duration 9.9?7.3 years) from the endocrinology unit of Beijing Tongren hospital were included in this study. Urinary albumin excretion rate(UAER),age ,diabetes duration, arterial blood pressure, body mass index(BMI),waist to hip ratio(WHR),fasting blood glucose(FBG),HbA1c,lipid profile were evaluated. Results The prevalence of microalbuminuria, macroalbuminuria was 24.6% and 16.8% respectively. On logistic regression analysis, using the presence or absence of microalbuminuria as dependent variable, arterial blood pressure and, fasting blood glucose were included in the model in both males and females. When analyzing patients with microalbuminuria vs those with macroalbuminuria, in females, arterial blood pressure and triglycerides were included in the model. In males, only blood pressure was included in the mole. Conclusions High fasting blood glucose and arterial blood pressure were independent risk factors of microalbuminuria in type 2 diabetic patients .High arterial blood pressure and disorder in lipid profile were independently associated with macroalbuminuria.
4.Association between SUR1 Ser1369Ala polymorphism and the efficacy of sulfonylurea

Objective To investigate the association between a missense mutation (T/G, Ser 1369 Ala) in exon 33 of sulfonylurea receptor 1(SUR1) gene in type 2 diabetes patients and the glucose-lowering effect of Gliclazide. Methods One hundred and four type 2 diabetes patients were selected and orally administered with Gliclazide for 56 days. Venous fasting plasma glucose levels (FPG), plasma glucose levels half an hour after taking 75 g glucose (OGH) and two hours (OGT) after taking Gliclazide were measured. Ser1369Ala polymorphism genotypes(TT, TG, GG) of SUR1 gene were determined by Taqman method. The glucose-lowering effect of Gliclazide was compared among different groups. If FPG was reduced more than 20%, we define it as effective. People whose blood sugar level was effectively reduced and not significantly reduced were compared to find whether their genotype groups were different.Results The change of FPG,OGH and OGT on day 1 and day 57 among TT,TG and GG genotype groups had no statistically significant difference. There was no statistically significant difference of the efficacy of Gliclazide in recessive model (TT+TG vs GG). The combination of TG and GG was more effective in reducing blood sugar level than TT. Conclusion Patients carrying one or two copies of the G allele were more sensitive to Gliclazide than patients carrying T allele.

Objective To evaluate the effects of two different short-term intensive therapies on ?-cell function in newly diagnosed type 2 diabetic patients.Methods Twenty newly diagnosed type 2 diabetic patients were divided into two groups of 10 patients each and randomly treated with glargine or glimepiride plus metformin for 4 weeks.The improvement of ?-cell function,FPG,PPG,HbA1c were measured before and after intensive therapy in each group.The difference in the improvement of ?-cell function,blood glucose and the frequency of hypoglycemia were compared between groups.Results After the treatment,FPG(12.25 vs.6.05 and 11.83 vs.6.35mmol/L),2hPG(18.67 vs.8.11 and 16.95 vs.8.73mmol/L),HbA1c(8.98% vs.7.38% and 9.17% vs.7.71%) were significantly decreased(all P
6.Treatment of hallux valgus with modified Austin operation

[Objective] To retrospectively study and discuss the indications,surgical techniques and effect of treating hallux valgus with modified Austin operation-Chevron-Gerbert osteotomy or Chevron-Youngswick osteotomy.[Method]From October 1998 to April 2006,34 patients(62 feet)with hallux valgus were treated with modified Austin operation.The X-ray films of all patients were collected preoperatively,two weeks postoperatively and during the follow-up.The hallux valgus angle(HVA),intermetataral angle(IMA),proximal articular set angle(PASA)and distal articular set angle(DASA)were measured and analyzed on radiogram.[Result]All patients were followed up for 1-8 years(mean 3.6 years).HVA before and after surgery was 33.50??1.02?and 13.6??0.826 respectively.IMA was 16.0??0.837?before and 8.6??1.078 after surgery.AOFAS score was 44.8?5.7 before,and 87.6 4.2 after surgery.Significant difference was found between the results.According to AOFAS rating system,46 feets had a excellent result and 10 feet had a good result and the other 5 feet had a fair result.The rate of excellent or good results was 90.3%.[Conclusion]The modified Austin operation is a good,simple and reliable operation means for treatment of hallux valgus,which especially applies to severe hallux valgus with big PASA.The adductor hallucis or the sesamoid is not necessarily interfered in patients with slight or moderate hallux valgus.
7.Magnetic Resonance Venography for Diagnosis of Cerebral Venous Sinus Thrombosis:A Meta-analysis

Purpose cerebral venous sinus thrombosis (CVST) has no specific manifestations in clinic, it is often misdiagnosed and the treatment is delayed. This paper aims to evaluate magnetic resonance venography (MRV) in the detection of CVST by meta-analysis. Materials and Methods The articles concerning the diagnosis of CVST by using MRV were searched in the databases such as Cochrane Library, PubMed, Embase, Web of Science, CBM, VIP, Wanfang and CNKI. The QUADAS items were used to evaluate the quality of the included studies. The results were analyzed with software Metadisc1.4 and RevMan 5.0. The pooled weighted sensitivity, specificity, and the diagnostic odds ratio were calculated, and complete summary receiver operating characteristic curve (SROC) was drawn. Results Twelve studies of eight articles met the inclusion criteria. The pooled sensitivity, specificity, diagnostic odds ratio and area under SROC curve for the MRV were 0.86 (0.80-0.91), 0.87 (0.83-0.93), 29.65 (12.48-70.47), and 0.9125, respectively. Conclusion MRV can be used as an effective method with its high value in the diagnosis of CVST.
8.Expression of angiotensin-converting enzyme 2 in rat pancreas

Objective To investigate the expression of angiotensin-converting enzyme 2(ACE2) in rat endocrine and exocrine tissue of pancreas and to compare its expression level with that in other visceral tissues. Methods The expression of ACE2 in rat visceral organs was detected by immunohistochemistry and reverse transcriptase polymerase chain reaction (RT-PCR). Results ACE2 was positively expressed in all the test tissues, including pancreas, heart, liver, kidney, lung, spleen, duodenum, ileum and bladder. The expression of ACE2 mRNA was detected in whole pancreas and ACE2 protein was found in both endocrine tissue and exocrine tissue of rat pancreas, while in the endocrine tissue much less ACE2 protein was expressed. The absorbance value of exocrine tissue of rat pancreas increased significantly as compared to that in pancreatic islet. Conclusion Our data demonstrated a tissue specific pattern of ACE2 expression in rat pancreas and this indicated ACE2 might be present in islet of pancreas of human, which might cause the elevation of blood glucose levels in SARS patients. Since ACE2 works as a negative regulator of the angiotensin system, the expression of ACE2 protein in endocrine tissue of pancreas may facilitate the treatment of diabetes.
9.Construct eukaryotic expression vector with aFGF gene and transfect muscle satellite cells

Objective To construct human acidic fibroblast growth factor (aFGF) recombinant eu karyotic expression vector and transfect it into muscle satellite cells(MSCs) of rat,in purpose of further study the method to set up cell bank.Methods The aFGF gene was cloned from human total RNA which was obtained from human skeletal muscle tissue by RT-PCR method.Human interleukin 2 (IL-2) signal peptide sequence (SPS) was obtained by direct chemosynthesis method.Then aFGF and SPS were fused to obtain SPS-aFGF.Finally,directional cloning SPS-aFGF into pEGFP-N1,the recombinant (pEGFP-N1-SPS-aFGF) was obtained.The recombinant was confirmed by endonuclease digestion and DNA sequencing.MSCs were purified by difference-speed adherence method and were ideontified by immunofluorescence assay.The correct cells were divided into 3 groups:Experimental group (aFGF +N 1),control group (N 1),blank group (blank).All the groups were transfected by Lipofectamine 2000TM Reagent,and pEGFP-N1-SPS-aFGF,pEGFP-N1 were respectively added in experimental group and control group while blank group was added none plasmid.Fluorescence microscope was employed to detect transfection efficiency tendency along with time changes.The expression of target gene was detected by fluorescent quantitation PCR and Western blot.Results (1) The sequencing of pEGFP-N1-SPS-aFGF was completely correct and the outcome of endonuclease was equal to actual ban s-ize.(2)The expression of GFP in transfected cells were observed by fluorescencemicroscope and transfection efficiency reached the peak at 72 h.(3)Real-time fluorescent quantitation PCR proved strong aFGF mRNA expression in transfected cells (the average relative expression of experimental group was 1464.95)with aFGF gene,while it was detected a little in the other groups (the average relative expression of control group was 1.016 and blank group was 1.000) (P < 0.05).Western blot also proved strong expression in Experimental group then the other two groups.Conclusion aFGF eukaryotic expression vector was successfully constructed and transfected into MSCs.This study may be expected to obtain some specific functions cells.
10.Clinical application of LI-RADS in diagnostic evaluation of hepatocellular carcinoma

Objective To evaluate the reliability and validity of hepatocellular carcinoma (HCC)using the liver imaging reporting and data system (LI-RADS).Methods By using the LI-RADS v2013.1,two radiologists evaluated 1 62 patients with cirrhosis or with a risk of HCC according to an inclusion criteria.The Kappa value was used to evaluate the consistency between two different diagnoses and was compared with pathological and follow-up results.The accuracy of the LI-RADS was assessed including sensitivi-ty,specificity,positive predictive value,negative predictive value,positive likelihood ratio,negative likelihood ratio and accuracy rating.Results In all 1 62 patients,there were HCCs in 97 patients including 7 patients belonging to the LR3,benign diseases in 50,and other kinds of malignancy in 1 5.The Kappa value was 0.882 (P =0.000)between two observers on LI-RADS grading. The sensitivity,specificity and accuracy rating of LI-RADS grading in diagnosing HCC was 100.00%,91.30%,and 97.06%,re-spectively.Conclusion The LI-RADS has high consistency and stability in evaluation and diagnosis of HCC by enhanced CT.LR3 di-agnosis should be cautious because of a susceptible development to HCC,which can be improved through the combination of clinic and laboratory examination.
