ObjectiveTo investigate the clinical effect of the double stapling technique for anus-preserved (Dixon) and Miles procedure of rectal carcinoma. MethodsFifty-eight cases were divided into 29 cases as control group and 29 cases as observation group by digital table method. The distance of their carcinoma was 4-7 cm to anus. The control group was treated with Miles procedure of rectal carcinoma, the observation group was treated with double stapling technique for Dixon. ResultsThe operation time of observation group was ( 180 ± 56) min, the control group was (240 ± 73)min(P < 0.05). Five-year survival rate of observation group was 77.9%, the control group was 79.3% (P >0.05);the recurrence rate of observation group was 6.9%(2/29),the control group was 6.9%(2/29) (P > 0.05). In the observation group, nobody suffered encopresis after 1 year, postoperative stool frequency was ≤5 times/d,while the control group occurred encopresis gradually. ConclusionIf the distance of carcinoma is 4-7 cm to anus, the way of the double stapling technique for Dixon can get the similar short term effect with the way of Miles, and can get more ability to control it.

Objective To discuss effect of different kinds of dressings on complications after periph-erally inserted central catheter(PICC). Methods Patients with skin hypersensitiveness after PICC adopt-ed sterile bandage and Anshutuo IV3000 transparent dressing to carry out local dressing change respective-ly. Incidence of catheter infection, slippage and complications were compared. Results The rate of catheter infection was 19.4%( 0 cases) and catheter slippage was 6.5%(7 cases) by using sterile bandage. While the rate of catheter infection was 3.2%(1 case) and catheter slippage was 3.2%(1 case) by using Aashutuo IV3000 transparent dressing. Conclusions Using Anshutuo IV3000 transparent dressing on local dressing change solved the shortcomings of common dressings and sterile bandage, reduced the inci-dence of complications after PICC and prolonged the using time of catheter.

Objective To investigate the relationship of uteroglobin gene polymorphism to the sus-ceptibility to, clinical type and pathological type of Henoch-Schsnlein purpura (HSP) and Henoch-Schonlein purpura nephritis (HSPN). Methods Totally, 118 patients with clinically diagnosed HSP, including 80 cases of HSPN and 38 cases without renal involvement were recruited in this study together with 100 normal human healthy controls. Genomic DNA was isolated from peripheral blood leucocytes of all subjects. The uteroglobin G38A polymorphism was determined by PCR-restriction fragment length polymorphism (RFLP). Results The frequencies of genotypes 38GG, 38GA and 38AA in normal human controls did not differ from those in patients with HSP, patients with HSP but without nephritis, patients with HSPN, patients with HSP and joint involvement, patients with HSP and gastrointestinal involvement (all P > 0.05). Also, no sig-nificant difference was observed between patients with HSPN and patients with HSP but without nephritis (P > 0.05). Furthermore, the frequency of genotypes 38GG, 38GA and 38AA had no significant correlation to the clinical phenotype of HSP, the occurrence of gross hematuria and nephrotie syndrome or the degree of renal damage (all P > 0.05). A significant increase was observed in the frequency of genotype 38AA in patients with HSP with elevated serum IgE compared with those with normal serum lgE (58.82% vs 8.43%, χ2 = 21.946, P < 0.05, OR = 15.51, 95% CI range: 4.93% - 48.84%), whereas the frequency of genotype 38GG was significantly increased in patients with HSPN and hypertension than in those with HSPN but without hypertension (75.68% vs 18.60%, χ2 = 26.172, P < 0.05, OR = 13.61, 95% CI range: 5.01% -37.01%). Conclusions The uteroglobin G38A polymorphism seems unrelated to the susceptibility to and degree of renal damage in patients with HSP and HSPN. The genotype 38AA may be associated with elevated level of serum IgE In patients with HSP, while genotype 38GG is associated with a high incidence of hyper-tension in patients with HSPN.

Objective To observe influence of atorvastatin combining with berberine on blood cholesterol level and carotid atherosclerotic plaques in patients with acute artery atherosclerosis cerebral infarction disease. Methods Fif-ty-five cases of acute cerebral infarction patients were randomized into 3 groups: group A (n=28), group B (n=11) and group C (n=16). Group A, B or C received atorvastatin 20 mg (qn), atorvastatin 40 mg (qn) or atorvastatin 20 mg (qn) +berberine 0.4 g (tid), respectively. All groups were then followed up for 3 months. The total cholesterol(TC), triglyceride (TG), low-density lipoprotein-cholesterol (LDL-C),high-density lipoprotein-cholesterol(HDL-C and the changes of carotid atherosclerotic plaques including total plaque area (TPA), crouse score, carotid intima-media thickness (IMT) and the stability of carotid plaques as well as the serum levels of creatinine(Scr), alanine aminotransferase(ALT), aspartate aminotransferase(AST), were compared among three groups. Results After 3 months, the TC, TG and LDL-C were de-creased in all three groups. There were statistically significant differences in the TC and LDL-C among these three groups (P=0.011,P=0.033) after treatment. The compliance rate of group C (75.0%) was significantly higher than group A ( 32.1% ) and group B (45.5%) in LDL-C (P=0.026). Both berberine combining with atorvastatin and atorvastatin monotherapy(20mg could significantly reduce crouse score. Conclusions Berberine can significantly enhance the benefi-cial effects of atorvastatin on LDL-C and the crouse score in patients with acute artery atherosclerosis cerebral infarction patients.
