1.Tachycardia-induced cardiomyopathy in children:a report of 20 cases

ObjectiveTo explore the clinical features, treatment, and outcomes of tachycardia induced cardiomy-opathy (TIC) in children.MethodsThe clinical data of 20 children with TIC hospitalized from January 2007 to October 2014 were retrospectively analyzed.ResultsIn 20 patients with TIC, there were 11 infants, one toddler, 5 pre-school age children, and 3 adolescent patients were as follows: 15 cases of atrial tachycardia (distributed in each age group), 3 cases of paroxysmal supraventricular tachycardia (2 in infancy and one in adolescence), and 2 cases of ventricular tachycardia (2 in infancy). After the treatment of anti-arrhythmic drugs, sinus rhythm was restored in 11 patients and ventricular rate was controlled in 5 patients while poor effect of drug was found in one patient who received radiofrequency ablation eventually and got cured. Three patients received radiofrequency ablation after admission immediately. Compared with those before treatment, left ventricular ejection fraction (LVEF) and left ventricular diastolic diameter (LVDD) measured by cardiac ultrasonography were signiifcantly improved after treatment (P<0.01).ConclusionsTIC is common in infancy. Atrial tachycardia is the main type of arrhythmia. Generally drug therapy is the ifrst choice in the treatment of TIC but in older children and those refractory to drug therapy the radiofrequen-cy ablation is chosen.
2.THE PROTECTION OF ANTIOXIDANT MICRONUTRIENTS ON MYOCARDIAL ULTRASTRUCTURE IN TYPE 1 DIABETIC MICE

Objective To observe the effects of combined antioxidant micronutrients (CAM) on myocardium ultrastructure of type one diabetic mellitus (T1DM) mice. Method T1DM mice model was made by injection of multiple low dosages of streptozotocin (MLDS). The supplementation of CAM was designed CAM4 and CAM7 two groups. The ultrastructure of myocardium in T1DM mice was observed by electron microscope and measured by computer image analysis method. Results The blood glucose level of mice in CAM4 and CAM7 groups was significantly decreased and the depth of microvascular basement membrane was smaller as compared with that of T1DM model group. The structure of mitochondria and myofilament was improved and the swelling of mitochondria was lightened in both CAM4 and CAM7 group. Conclusion Combined antioxidant micronutrients may protect ultrastructure of myocardium in T1DM mice.
3.Clinical research of Xingnaojing injection combined with naloxone in the treatment of hypoxic ischemic encephalopathy

Objective To observe the clinical effect of Xingnaojing injection combined with naloxone in the treatment of hypoxic ischemic encephalopathy.Methods 62 patients with hypoxia ischemia encephalopathy were randomly assigned into the control group(31 cases)and the treatment group(31 cases).The control group used naloxone treatment on the basis of the conventional treatment.The treatment group received Xingnaojing injection combined with naloxone treatment on the basis of routine treatment.The changes in different time of Glasgow Coma Scale(GCS) scores of the two groups after treatment were compared.Results After treatment for 1 ,5,1 0,1 5d,the GCS scores in the control group were (5.27 ±0.87)points,(9.03 ±0.72)points,(1 0.03 ±0.72)points,(1 3.03 ±0.72)points respectively,which in the treatment group were (5.1 4 ±1 .03)points,(9.24 ±1 .06)points,(1 3.31 ±2.83)points, (15.31 ±0.93)points.The differences of GCS scores after treatment for 15,10d between the two groups were statistically significant(t =1 5.1 1 3,1 0.501 ,P =0.00,0.00).In the control group,the total effective rate was 50.0%,which of the treatment group was 82.7%,the total effective rate between the two groups had statistically significant difference (χ2 =6.437,P <0.05).Conclusion On the basis of routine treatment,Xingnaojing injection combined with naloxone in the treatment of hypoxic ischemic encephalopathy has better effect than the single use of naloxone treatment.
4.Application of intravascular ultrasound in analysis on influencing factors of prognosis in patients with different coronary artery in-stent restenosis

Objective:To collect the imaging data and related materials of the patients with in-stent restenosis (ISR)after coronary artery stent operation with intravascular ultrasound (IVUS),and to analyze the risk factors of ISR,and to propose the reasonable intervention strategies.Methods:Fifty patients with ISR were divided into ISR ≤ 50% drug group (n = 14 )and ISR > 50% drug group (n = 36),including drug-coated balloon therapy group (n=16)and stent treatment group (n=20);IVUS virtual organization technology was used to compare the plaque area,location,tissue composition,thrombus and other factors of the patients in various groups after treatment;the data changes after 6 months of follow-up were analyzed.Results:The IVUS results showed the plaque areas and plaque loads of the ISR patients treated with intervention were significantly reduced compared with before operation (P <0.05);the plaque compression degree of the patients in drug-coated balloon therapy group was lower than that in stent treatment group (P <0.05),but the differences were not found between drug-coated balloon therapy group and stent treatment group in fibrous tissue components and calcified tissue proportion (P >0.05).Conclusion:The ISR rate is higher in the patients with high degree of fiber components,plaque composition heterogeneity and distribution of diffuse tortuous and calcified lesions.ISR has no significant correlation with the plaque wall thickness and lipid content and plaque instability and necrotic tissue proportion.
5.Using parallel line assay method based on quantitative responses in methodology evaluation for determining lumbrokinase potency in Rongshuan capsules.

OBJECTIVETo develop an assay methodology for determination of lumbrukinase potency in Rongshuan capsules.
METHODThe agarose-fibrin plate assay methodology for determination of Lumbrukinase potency in Rongshuan capsules was studied including durability, specificity, linearity range, product's handling method, accuracy , repetitiveness, solution stability, recovery and statistical method. The method of parallel line assay based on quantitative responses in statistical methods for biological assays was used in the statistics of potency assay.
RESULTThe durability and specificity of assay accord with the requirement; The linearity range was 12.5 to approximately 400 U, the RSD of accuracy tests was 3.2%, the RSD of repetitiveness tests was 8.3%, the solution is stable under 4 degrees C for 72 hours, the recovery rate was 97.0% and the RSD of recovery assays was 16.5%.
CONCLUSIONThe agar-fibrin plate assay is rapidly, feasible, simple, convenient and accurate way for determining the Lumbrukinase potency. The method of parallel line assay based on quantitative responses in statistical methods for biological assays can control the error of determination.

Carrier-free multi-component self-assembled nano-systems have attracted widespread attention owing to their easy preparation, high drug-loading efficiency, and excellent therapeutic efficacy. Herein, MnAs-ICG nanospike was generated by self-assembly of indocyanine green (ICG), manganese ions (Mn2+), and arsenate (AsO4 3-) based on electrostatic and coordination interactions, effectively integrating the bimodal imaging ability of magnetic resonance imaging (MRI) and fluorescence (FL) imaging-guided synergistic therapy of photothermal/chemo/chemodynamic therapy within an "all-in-one" theranostic nano-platform. The as-prepared MnAs-ICG nanospike had a uniform size, well-defined nanospike morphology, and impressive loading capacities. The MnAs-ICG nanospike exhibited sensitive responsiveness to the acidic tumor microenvironment with morphological transformation and dimensional variability, enabling deep penetration into tumor tissue and on-demand release of functional therapeutic components. In vitro and in vivo results revealed that MnAs-ICG nanospike showed synergistic tumor-killing effect, prolonged blood circulation and increased tumor accumulation compared to their individual components, effectively resulting in synergistic therapy of photothermal/chemo/chemodynamic therapy with excellent anti-tumor effect. Taken together, this new strategy might hold great promise for rationally engineering multifunctional theranostic nano-platforms for breast cancer treatment.
