2.Ultrasonic wave in combination with quadriceps exercise for the treatment of senile knee osteoarthritis

BACKGROUND: The pain and the unstability of the knee joint are the main clinical manifestation of knee osteoarthritis.OBJECTIVE: To verify the efficacy of ultrasonic drug penetration combiued with quadriceps exercise in treating senile knee osteoarthritis.DESIGN: Randomized controlled study.SETTING: Rehabilitation Department of the Affiliated Hospital of Luzhou Medical College.PARTICIPANTS: Seventy-six in-patient and out-patient cases of the knee osteoarthritis, 28 males and 48 females and total 109 knees, were chosen in the Orthopaedic Department and Rehabilitation Department in the Affiliated Hospital of Luzhou Medical College in theperiod between August,2001 and August, 2004. The cases were randomly divided into the experimental group and the control group with 38 cases each.therapy apparatus made in Mianyang was adopted and the diameter of the handpiece was 3 cm. 10 g/L dichloride sodium emulsion (made by Beijing Nuohua Pharmaceutical co.,Ltd) was taken as the coupling agent to smear around the knee joint about 0.75 -1.25 W/cm2, once a day, and 8-10 minutes per time. Meanwhile the static isometric contraction exercise of the knee joint in the extension position was done. Using the quardriceps isometric exercise apparatus, the Delorme progressive resisThe same ultrasonic therapy was done as the therapy group. The nonthermal and microthermal ultrasonic wave was adopted on the suffered location 10 -15 minutes per time, and the tolerance dose of the mid-frequency current was given 20 minutes per time and once a day using the CM-efficacy was assessed in the two groups 4 weeks after the therapy. During the therapy period the anti-inflammatory and analgesic drugs were not patient's knee joint activities of daily life (0 score signifies no pain, 10 scores severe pain, the pain relief better than 40% that before the therapy means the effectiveness). The lower limb activity of daily life was chosen to evaluate the patient's knee joint activities of daily life (5 items,total 10 scores, high score means good, low score means poor). The maximal loading dose in the extension position of the knee joint was measured on the quadriceps isometric exercise apparatus using the suspension weight method.The maximal loading dose in the extension position of the knee joint in the two groups.result of the walking pain scale in the two groups before and after the therapy: The score of pain evaluation after therapy was significantly lower than that before the therapy (P < 0.01), and there was no significant differs knee joint activities of daily life in the two groups before and after the therapy: the result after the therapy was siguifieantly better than that before the therapy, and the efficacy after the therapy was significantly higher than that before the therapy, the efficacy of the experiment group was better than that of the control group (9.61±1.82, 8.77±2.28), while there was tion results of the maximal loading dose in the extension position of the knee joint in the two groups: the result of the experimental group after the therapy was significantly better than that of the experimental group before the therapy and the control group [ (10.05±2.41) kg vs (7.86±1.98) kg and (8.76±1.65) kg, P < 0.01 ].CONCLUSION: The ultrasonic wave using the dichloride sodium emulsion as the ultrasonic coupling agent in combination with quadriceps strengthening exercise in treating the senile knee osteoarthritis is efficacious in relieving pain and improving the ADL ability of the knee joint, increasing the maximal loading dose in the extension position of the knee joint and the stability of the knee joint. The efficacy of the combination therapy is better than the single ultrasonic therapy.
3.Feixin mixture for vascular remodeling in rats with monocrotaline-induced pulmonary hypertension

BACKGROUND: Pulmonary hypertension induced by alternation of pulmonary function and structure is the prerequisite of pulmonary-cardiac disorder. Chinese herb, fenxin mixture(FXM) reduces effectively pulmonary hypertension, but the mechanism on which has not been completely explained yet.OBJECTIVE: To comp are the reversal effects between feixin mixture and nifedipine on vascular remodeling in rats with pulmonary hypertension.DESIGN: Randomized controlled experimental study in which experimental animals are taken as the objects.SETTING: Nursing department of affiliated hospital of a medical university;a medical college affiliated hospital and department of traditional Chinese medicine of a provincial hospital.METHODS: Monocrotaline(MCT) (50 mg/kg) was used to duplicate pulmonary-cardiac model in rats. Totally 60 Wister male rats were randomized into 6 groups, namely normal control group, model group, FXM groups of low, middle and high dosages successively and nifedipine group, 10 rats in each group. Since the 14th day of modeling, the corresponding medications were administrated with gastric infusion, continuously for 8 days. After the rats sacrificed, the special staining collaborating with pathological image analysis was used to determine the measurement alternations on path-morphology of pulmonary small artery.wall square(VWS)/total vessel square(TVS), cavity square(CS)/TVS of pulmonary small artery in rats of every group.RESULTS: Both feixin mixture and nifedipine alleviate pulmonary vascular remodeling remarkably( P < 0.01 ), of which, the effect was the best in feixin mixture group of high dosage. But, the every index in experimental groups had not recovered to the level in normal control group completely.CONCLUSION: Feixin mixture reverses partially the remodeling of pulmonary vascular structure and reduces effectively pulmonary hypertension.
4.Effect of acupuncture and moxibustion combined with quadriceps exercise on knee osteoarthritis

Objective To explore the effect of acupuncture and moxibustion combined with quadriceps exercise on osteoarthritis of knee joint. Methods 82 cases of knee osteoarthritis were randomly divided into the acup-moxibustion group (41 cases, treated with acup-moxibustion and quadriceps exercise treatment) and routine treatment group (41 cases). Lower limb ability of daily life respectively and knee pain of patients before and 30 days after treatment were evaluated. Results Clinical symptoms of patients in both groups were improved significantly after treatment. The acup-moxibustion group was better than traditional group in relieving pain and improving the lower limb ability of daily life (P<0.01). Conclusion The acup-moxibustion therapy combined with quadriceps exercise has preferable clinical curative effect on knee osteoarthritis.
5.Effect of comprehensive rehabilitation on craniocerebral trauma

ObjectiveTo study the effect of comprehensive rehabilitation on the patients with craniocerebral trauma. Methods85 cases with craniocerebral trauma were subjected to the rehabilitation training supplemented with hyperbaric oxygen and acupuncture and ultrasound therapy in addition to routine neurosurgical operation. Before and after treatment,Glasgow coma scale (GCS), Fugl-Meyer assessment (FMA) and Barthel index were assessed and compared. ResultsAfter comprehensive rehabilitation therapy, the degree of coma, limbs motion and activities of daily living were improved significantly (P<0.01).ConclusionComprehensive rehabilitation is effective on the patients with craniocerbral trauma.
6.Study on delineation of tumor volume of primary locally advanced nasopharyngeal carcinoma after induction chemotherapy

ObjectiveTo investigate the delineation of gross tumor volume (GTV) in locally advanced nasopharyngeal carcinoma (LANC) according to imageological changes before and after induction chemotherapy (IC) in order to decrease high dose area and protect normal tissue better.MethodsBetween Mar 2010 to Jan 2011,11 patients with LANC were enrolled and treated with TPF regimen followed by intensity-modulated radiotherapy (IMRT) with concurrent chemotherapy,target volumes were delineated based on fused CT imaging before and after IC following project determination.Tumor target volumes after and before IC were respectively delineated according to imaging tumor residues and were overlaid by CTVnx in order to ensure radical doses for the imaging tumor volume before IC,the resulting differences of tumor target volumes of IC before and after were measured and analyzed by paired t-test.ResultsBefore and after IC,the average volumes of GTVnx were respectively 44.72 cm3 and 28.87 ( t =3.89,P =0.003 ),the average volumes of GTVnd were respectively 32.76 cm3 and 19.82 cm3 ( t =2.47,P =0.033 ),the volumes of maximum dose area in brainstem and spinal cord as well as eyeball decreased ( t =2.93-4.59,all P <0.05).ConclusionsLANC treated by 3 cycle TPF regimen followed by IMRT with concurrent chemotherapy showes significant shrinkage of tumor volume.The volume of high dose region which caused by normally recovered tissues were decreased by re-delineation of target volume in brainstem and spinal cord as well as eyeball of CT images after IC.
7.Effects of continuously motivational interviewing on further consultation compliance post discharge in cataract patients

Objective?To explore the effects of continuously motivational interviewing on further consultation compliance post discharge in cataract patients and to observe the application effects. Methods?From January 2016 to January 2017, we selected 168 cataract inpatients in the Affiliated Hospital of Southwest Medical University. All of the patients were divided into observation group and control group with the random number table, 84 cases in each group. Patients of two groups were treated with routine health education by health care providers during hospitalization as well as telephone follow-up one time every month lasting three months after discharge. The visit content of control group included physical status, medication condition of patients and guidance for patients according to the feedback. Observation group carried out continuously motivational interviewing involving analyzing the influencing factors of further consultation compliance post discharge, formulating targeted interview scheme of continued visit. The primary nurse was in charge of interview and health preach. We compared the further consultation compliance between two groups. Results?One month post discharge, there were 62 patients with a good level of further consultation compliance in observation group and 33 in control group; three months post discharge, there were 68 patients with a good level of further consultation compliance in observation group and 40 in control group; the differences were significant between two groups (P<0.01). Conclusions?Continuously motivational interviewing can effectively improve the further consultation compliance post discharge in cataract patients. The continuity of interview ensures the nursing care after discharge. It is worth to be widely generalized in clinic.
8.An analysis on the combination of chrono-chemotherapy with different speed rate and concomitant intensity-modulated radiotherapy in the treatment of locally advanced nasopharyngeal carcinoma

Objective To evaluate the differences of toxicities,therapeutic efficacy and immune function between induction chemotherapy followed by sinusoidal chrono-modulated infusion and flat intermittent infusion of cisplatin (DDP)with intensity-modulated radiotherapy (IMRT) in patients with locoregionally advanced nasopharyngeal carcinoma (NPC).Methods Seventy patients with biopsydiagnosed stages Ⅲ and Ⅳ B NPC (according to the 2010 UICC staging system) were treated with two-cycle induction chemotherapy before chemoradiotherapy in Guizhou Cancer Hospital.The TPF chemotherapy regimen was administered as follows:The TXT and DDP with the dose of 75 mg/m2 was carried out by bolus infusing for the first day,the 5-FU with 750 mg · m-2 · d-1 was carried out by continuous intravenous pumping for the first day to fifth day(120 h).The induction chemotherapy was 21 days per cycle,for two cycles.After that all patients were randomly treated with 2-3 cycles of sinusoidal chronomodulated infusion or flat intermittent constant rate infusion of DDP with IMRT.Using a multi-channel programmed pump,the patients were given 12 h continuous infusions of DDP (100 mg/m2) for day one,repeated every 3 weeks for 2-3 cycles.DDP was administered from 10:00 am to 10:00 pm.Concurrent radiotherapy regimen was administered as follows:GTVnx 69.96-73.92 Gy/33 f,PTVnx 69.96 Gy/33 f,PTVnd 69.96 Gy/33 f,PTV1 60.06 Gy/33 f,PTV2 50.96 Gy/28 f.Results The main toxicities of chemoradiotherapy in the group of sinusoidal chrono-modulated infusion were bone marrow suppression:leukocytes,and then nausea,oral mucositis and hemoglobin.The main toxicities of chemoradiotherapy in the group of flat intermittent constant rate infusion were bone marrow suppression:hemoglobin,leukocytes,and then nausea,oral mucositis.No significant differences were observed for toxicities(P > 0.05).After concurrent chemoradiotherapy,the complete response rate (CR),partial response rate (PR),stable disease rate(SD),progressive disease rate (PD) and overall response rate (ORR) were 11.4%,85.7%,2.9%,0 and 97.1% in the group of sinusoidal chrono-modulated infusion.The CR,PR,SD,PD,ORR in the group of flat intermittent constant rate infusion were 22.9%,74.2%,2.9%,0,97.1%,respectively.However,there was no significant differences of effect in the two Arms (P > 0.05).For sinusoidal ehrono-modulated infusion and flat intermittent infusion group,the 2-year overall survival(OS) were 82.9% and 94.3% respectively,the 2-year progression-free survival(PFS) were 77.1%,91.4% respectively,and the 2-year distant metastasis free survival (DMFS) were 82.9%,91.4% respectively.The value of CD3 + in the group of sinusoidal chrono-modulated infusion was higher than the group of flat intermittent constant rate infusion after therapy (Z =3.254,P < 0.05).The value of CD4 +,CD8 +,CD16 + CD56 +,CD19 +,and CD4 +/CD8 + had no differences in two Arms (P > 0.05).Conclusions No significance differences on the toxicities,therapeutic efficacy and survival were observed between the two groups,but immune function might be improved in the sinusoidal chrono-modulated infusion group.
9.Neuroprotective effects of voluntary exercise and Yisaipu after traumatic brain injury in mice.

The mechanisms underlying exercise-induced neuroprotective effects after traumatic brain injury (TBI) remained elusive, and there is a lack of effective treatments for TBI. In this study, we investigated the effects of an integrative approach of exercise and Yisaipu (TNFR-IgG fusion protein, TNF inhibitor) in a mouse TBI model. Male C57BL/6J mice were randomly assigned to a sedentary group or a group that followed a voluntary exercise regimen. The effects of 6-week prophylactic preconditioning exercise (PE) alone or in combination with post-TBI Yisaipu treatment on moderate TBI associated deficits were examined. The results showed that combined treatments of PE and post-TBI Yisaipu were superior to single treatments on reducing sensorimotor and gait dysfunctions in mice. These functional improvements were accompanied by reduced systemic inflammation largely via decreased serum TNF-α, boosted autophagic flux, and mitigated lesion volume after TBI. Given these neuroprotective effects, composite approaches such as a combination of exercise and TNF inhibitor may be a promising strategy for facilitating functional recovery from TBI and are worth further investigation.

10.Advances in Classification and Research Methods of Lung Epithelial Stem and Progenitor Cells

Isolation and characterization of lung epithelial stem and progenitor cells and understanding of their specific role in lung physiopathology are critical for preventing and controlling lung diseases including lung cancer. In this re-view, we summarized recent advances in classification and research methods of lung epithelial stem and progenitor cells. Lung epithelial stem and progenitor cells were region-specific, which primarily included basal cells and duct cells in proximal airway, Clara cells, variant Clara cells, bronchioalveolar stem cells and induced krt5+ cells in bronchioles, type Ⅱ alveolar cells and type Ⅱ alveolar progenitor cells in alveoli. The research methods of lung epithelial stem and progenitor cells were mainly focused on lung injury models, lineage-tracing experiments, three dimensional culture, transplantation, chronic labeled cells and single-cell transcriptome analysis. Lastly, the potential relationship between lung epithelial stem and progenitor cells and lung cancer as well as lung cancer stem cell-targeted drug development were briefly reviewed.
