1.Diagnosis and treatment of primary hyperparathyroidism: 115 cases

Objective To analyze clinical features of primary hyperparathyroidism(PHPT)and to evaluate the diagnosis and treatment for PHPT.Methods 115 patients with PHPT undergoing surgery and confirmed by pathology from Jan.2006 to Mar.2014 were retrospectively analyzed.Results The clinical manifestations of PHPT were various.The rate of misdiagnosis was 70.0%(77/115).The positive rate of ultrasonography was 61.0% (64/105),99Tcm-MIBI 88.3%(68/77)and CT 75.4%(46/61),respectively.The difference among the 3 tested methods had statistical significance (P<0.05).89 cases with parathyroidoma underwent unilateral neck exploration,6 cases of parathyroid hyperplasia underwent bilateral neck exploration,3 cases with parathyroid carcinoma underwent carcinoma resection,ipsilateral thyroid lobe resection and ipsilateral central lymph node dissection,and 21 cases with thyroid benign or malignant lesions underwent suitable operations.97 cases developed hypocalcaemia of various degrees after operation,and the symptoms were relieved after use of calcium gluconate.Conclusions PHPT can be diagnosed according to the lever of serum calcium and PTH.Ultrasonography and 99Tcm-MIBI should be the first choice for preoperative localization.Unilateral neck exploration can be used for parathyroidoma with accurate localization.
2.Effects of Epigallocatechingallate on Lipid Metabolism Related Gene and Long Non-coding RNA Expression Profile in HepG2 Cells

Objective: To investigate the effects of epigallocatechingallate (EGCG) on lipid metabolism related gene and long non-coding RNA (lncRNA) expression proifle by biochip technology, and to explore the possible relationship between the two elements.
Methods: HepG2 cell was cultured with EGCG at 25μmol/L for 24 hours, the total RNA was extracted and hybridized into the biochip of Human Transcriptome Array 2.0 for mRNA and lncRNA expression profile analysis. Bioinformatics technology was used to establish the possible relationship between lncRNA and the predicted target genes;the data obtained from biochip microarray was conifrmed by real time RT-PCR examination.
Results: The microarray revealed that EGCG treated HepG2 cell expressed 27 differential lipid metabolism genes and 11 of them involved in cholesterol metabolism. In addition, there 285 lncRNA expressions were up-or down-regulated. Bioinformatics technology indicated that the predicted target genes for lipid metabolism might be cis-or trans-regulated by lncRNA;the data from real-time RT-PCR was consistent with the data from biochip microarray.
Conclusion: Tea polyphenols improves lipid metabolism and lncRNA might be involved in the regulation of lipid metabolism related gene.
3.Changes of TNF-alpha and malondialdehyde content in brain tissue and therapeutic effect of edaravone on rats after intracerebral hemorrhage

Objective To observe the protective effect of edaraven on brain tissue and the therapeutic mechanism in rats following intracerebral hemorrhage(ICH).Methods In this series,96 Sprague-Dawley rats were divided into three groups at random(n=32 in each group):sham-operative group,ICH model group and edaraven therapy group.ICH model was induced by infusion of autologous whole blood into the caudate-putamen.Rats in sham-operative group were treated the same as above,but no blood injected.Rats in edaraven therapy group were treated with edaraven(3 mg/kg)by peritoneal injection 30 min before operation and once per 12 hours after operation.These rats were killed at 6,24,72,and 168 h after neurologic impairment test with Longa criteria.Brain water content was measured by dry-wet weight method.Malondialdehyde and TNF-alpha in the vicinity of the hematoma were measured by thio-barbituric acid method and ELISA respectively.Results Compared with sham operative group,neurologic impairment score,brain water content,malondialdehyde and TNF-alpha in brain tissues were obviously higher in ICH model group(P
4.Relation between single Nucleotide Polymorphisms of CYP3A5 Gene and MDR1 Gene Loci and Risk of CML Cytogenetic Relapse.

OBJECTIVE:
To analyze the relation between the signle nucleotide polymorphisms (SNP) of CYP3A5 gene and MDR1 gene loci and the risk of cytogenetic relapse in chronic myeloid leukemia (CML).
METHODS:
The clinical data of 90 patients with CML treated with imatinib in our hospital were collected.The patients were divided into 2 groups: non-relapse and relapse according to relapse and non-relapse, then the relation between the SNP of CYP3A5 gene and MRD1 gene loci and the risk of cytogenetic relapse in CML patients.
RESULTS:
The grouping result showed that the patients with non cytogenetic relapse accounted for 41 cases those were enrolled in non-relapse group, and patient-with cytogenetic relapse accounted for 49 cases those were enrolled in relapse group. The follow-up time was 36 months. The detection showed that the incidence of cytogenetic relapse in the patients with CC genotype was significantly higher than that in the patients with TT+CT genotype of C3435T and C1236T at MDR1 gene loci (P<0.05).Compared with the patients with CT+CC genotype in C3435T locus of MDR1 gene, the rate of cytogenetic relapse in the patients with TT genotype decreased significantly (P<0.05). Compared with patients with CT+CC phemotype of C3435T in MDR1 gene locus, the non-relapse survival time of TT genotypes was significantly prolonged (P<0.05). Compared with non-relapse group, the incidence of neutropenia (29.27% vs 71.43%) and blood toxicity (39.02% vs 61.22%) in the relapse group increased significantly (P<0.05). The imatinib dose (OR=2 95, 95% CI:1.37~7.76) and the C3435T genotype in MDR1 genes (OR=0.09, 95% CI:0.05~0.72) were the factors affecting the cytogenetic relapse of the patients with CML (both P<0.05).
CONCLUSION
The therapeutic dose of imatinib and the C3435T and C1236T genotypes in MDR1 gene have a certain effect on the cytogenetic relapse of CML patients. C3435T genotypes in the.MDR1 gene showed a certain predictive value for evaluating the risk of cytogenetic relapse, which can be used as a clinical biomarker.

5.Role of Th9, Th17, Treg Cells levels and IL-9, IL-17 and TGF-β Expression in Peripheral Blood of Patients with ITP in Pathogenesis of ITP.

OBJECTIVE:
To detect the levels of Treg, Th17, Th9 cells and expression of transforming growth factor-β (TGF-β), interleukin-17 (IL-17) and interleukin-9 (IL-9) in peripheral blood of patients with immune thrombocytopenia (ITP) and to explore its role in the pathogenesis of ITP.
METHODS:
Fifty-four patients with ITP (ITP group) and 40 healthy volunteers (control group) were selected in our hospital. The of Treg, Th17 and Th9 cells in peripheral blood of 2 groups were measured by flow cytometry, and the expression of cytokines, such as TGF-β, IL-17 and IL-9 in the peripheral blood of 2 groups were detected by enzyme linked immunosorbent assay (ELISA).
RESULTS:
The level of Treg cells in the peripheral blood of the ITP group was significantly decreased in comparison with the control group, while the levels of Th17 and Th9 cells significantly increased in comparison with the control group (all P<0.01). The expression of cytokine such as TGF-β in the peripheral blood of the case group significantly decreased in comparison with the control group, while the expression levels of IL-17 and IL-9 significantly increased in comparison with the control group (P<0.01). The results of Pearson correlation analysis showed that there was a positive correlation between the level of Treg cells and platelet count (PLT) in peripheral blood of the ITP group (r=0.35, P<0.05), and there were negative correlation between the level rate of Th17, Th9 cells and Plt count (r=-0.37, -0.43, P<0.05); there was a positive correlation between the expression of the TGF-β in the ITP group and Plt count (r=0.46, P<0.05), while the expression of IL-17 and IL-9 showed negative correlation with PLT (r=-0.48, -0.54, P<0.05).
CONCLUSION
The percentage of Treg, Th17 and Th9 cells in the peripheral blood of patients with ITP is abnormal, and the expression of TGF-β, IL-17 and IL-9 also is abnormal, which may play an important role in the pathogenesis of ITP.

6.Medical surgical navigation through DSA using electromagnetic tracking system

Objective In order to meet the needs of more precise surgical navigation during interventional procedures and minimally invasive surgery, Aurora electromagnetic tracking technology-assisted surgical navigation with free combination of various surgical instruments was tested, trying to further solve problems encountered in surgery. Methods Aurora electromagnetic tracking system was used to match the medical surgical environment with metal-containing material. The built-in control software in this electromagnetic tracking system, which was equipped with the software development kit, was used to write a set of VC programming language suitable for real-time display of clinical surgical navigation system, and relative performance tests and comparative experiments were conducted for different medical surgical navigation application in the hospital to measure the precision degree of the tracking system, as well as the statistics of various surgical instruments corresponding to the application in different surgeries. Results 160 patients in interventional surgery with coronary angiography were randomly selected, 80 patients treated by digital subtraction angiography(DSA) with electromagnetic tracking systems as the experimental group, and 80 patients by DSA only as the control group. It was found out that with obviously less operating time, the amount of contrast agent was significantly reduced in the experimental group, and the degree of controllability for interventional surgical procedure was increased from 95% success rate to 99%. Conclusions The comparative experiments proved that Aurora electromagnetic tracking technology could significantly improve the accuracy in surgery, with the mean value of navigation less than 2 mm. In terms of interventional procedures and minimally invasive surgery, this electromagnetic tracking technology can greatly relieve patients' suffering and reduce medical risks to make medical care safer, and enhance the surgical technology level in hospital.
7.Establishment and evaluation of clinical nursing pathway after glossectomy and forearm flap reconstruction in patients undertaking tongue cancer surgery

Objective To research the significance of application of clinical nursing pathway in patients undertaking glossectomy and forearm flap reconstruction.Methods Sixty patients with tongue cancer who undertook glossectomy and forearm flap reconstruction were enrolled.The patients were randomly divided into the experimental group and the control group (30 cases in each group).The patients of the experimental group received nursing following clinical nursing pathway,and the patients in the control group received routine nursing service.The patients and doctors satisfaction evaluation were compared between the two groups with a self-designed nursing quality evaluation form.The qualities of care achieved in 2 groups were compared.Results It took less in-ward time and expenditure in the experimental group compared with the control group.The quality of care,the satisfactory degree of patients and doctors in the exipermental group were significantly higher than those of the control group.Conclusions The establishment and application of the clinical nursing pathway in patients undertaking glossectomy and forearm flap reconstrucition contributes to improve the satisfaction of patients and doctors and the quality of nursing service.
8.Diagnostic value of multi-slice spiral computed tomography for acute pulmonary embolism

Objective To evaluate the value of multi-slice spiral computed tomography (CT) for diagnosing acute pulmonary embolism, and to provide a reference for the precise diagnosis of acute pulmonary embolism. Methods We enrolled a total of 102 patients pathologically diagnosed with acute pulmonary embolism from January 2019 to October 2023. All the patients underwent CT scanning of central and segmental pulmonary arteries with a GE 64-slice spiral CT scanner. The diagnostic efficacy of multi-slice CT scans for acute pulmonary embolism was evaluated with the pathological results as the gold standard. Results Of the 102 patients pathologically diagnosed with acute pulmonary embolism, multi-slice CT detected 92 cases, with an accuracy of 90.20% (92/102), including 17 cases (18.48%) of embolism in left pulmonary arteries, 31 cases (33.70%) of embolism in right pulmonary arteries, and 44 cases (47.82%) of embolism in both pulmonary arteries. Multi-slice CT visualized a total of 9905 pulmonary artery branches, and 304 of them (3.07%) had embolism, with the highest embolism rate in lobar arteries (43.89%). Conclusion Multi-slice spiral CT has high accuracy for diagnosing acute pulmonary embolism by directly and clearly visualizing embolism in the arteries of the lungs, which deserves clinical promotion.
9.Relationship between glutamate in the limbic system and hypothalamus-pituitary-adrenal axis after middle cerebral artery occlusion in rats.

OBJECTIVETo investigate the features of glutamate activity in the limbic system and the effects of glutamate on the activation of the hypothalamus-pituitary-adrenal (HPA) axis throughout both acute cerebral ischemia and reperfusion.
METHODSThe changes in glutamate content in the nervous cell gap, in corticotrophin releasing hormone (CHR) mRNA expression level in brain tissue, and in adrenocorticotropic hormone in blood plasma at different time-points after middle cerebral artery occlusion (MCAO) in rats were determined respectively with high-performance liquid chomatography (HPLC) and in situ hybridization.
RESULTSGlutamate content in the hippocampus and the hypothalamus increased rapidly at ischemia 15 minutes, and reached peak value (the averages were 21.05 mg/g +/- 2.88 mg/g and 14.20 mg/g +/- 2.58 mg/g, respectively) at 1 hour after middle cerebral artery occlusion. During recirculation, it returned rapidly to the baseline level. At 24 hours after reperfusion, it went up once more, and remained at a relative high level until 48 hours after reperfusion, and then declined gradually. CRH mRNA expression levels in the temporal cortex, hippocampus and hypothalamus were enhanced markedly at 1 hour ischemia and were maintained until 96 hours after reperfusion. At the same time, adrenocorticotropic hormone level in plasma was relatively increased. In the peak stage of reperfusion injury, there was a significantly positive correlation (n = 15, r = 0.566, P < 0.05) of the glutamate contents in the hypothalamus with the number of cells positive for CRH mRNA expression level in the hypothalamus.
CONCLUSIONIt is probable that the CRH system in the central nervous system is mainly distributed in the limbic system, and glutamate might be one of the trigger factors to induce excessive stress response in the HPA axis.

10.Identification and Characterization of Gonatobotryum apiculatum CausingLeaf Spot and Blight on Sinowilsonia henryi

Sinowilsonia henryi is a rare and endangered plant, as well as an endemic species in China.In July 2018, leaf spot and blight disease was observed on S. henryi in Yichang, Hubei,China. A fungus isolated from disease tissues was identified as Gonatobotryum apiculatumbased on morphology and sequence analyses of ITS and LSU regions. Phylogenetic analysesindicated that the species belongs to Dothioraceae (Dothideales). Morphologically, the speciesproduced two distinct types of conidia from authentic media, both conidia weredescribed here. Pathogenicity tests showed that the fungus is a pathogen causing leaf spotson S. henryi. This is the first report of leaf spot and blight disease on S. henryi caused byG. apiculatum in China.
