1.Vertebroplasty and kyphoplasty: biomaterials and biomechanics
Chinese Journal of Orthopaedic Trauma 2002;0(04):-
Percutaneous vertebroplasty (PVP) and kyphoplasty (PKP) are perf or med by image-guided spinal procedures and consists in percutaneously transpedic ular injection of the polymer polymethylmethacrylate (PMMA) into a fractured ver tebral body to relieve the pain and to strengthen the bone. Advantages of the te chnology include its relative simplicity, fewer complications and minimally inva sive surgery. PKP can also restore vertebral height. This paper is to sum up the results of biomechanical comparison of different cements, different volumes and different distribution in the vertebras. This article also indicates the disadv antages of PVP and PKP at present. To achieve the goal in the future, the operat ions and ideal biomaterials should be improved and developed.

Objective To observe pathological changes of varicose veins after electrocoagulation and endovenous laser treatment(EVLT) and to compare the efficacy between electrocoagulation and EVLT for the treatment of varicose veins of the lower extremity. Methods Twenty patients with varicose veins of the lower extremity were treated with either electrocoagulation(n=10) or EVLT(n=10).Histopathological changes of varicose veins were observed.Eight dogs were randomly divided into 2 groups and treated with electrocoagulation or EVLT.The treated veins were taken out at 0,7,and 14 days after operation, for histopathological examinations. Results After electrocoagulation or EVLT,the endothelial cells of varicose veins were absent, the structure of smooth muscle cells was indistinct, broken collagenous fibers and elastic fibers were observed, and thrombosis leading to occlusion was observed in the lumen.Incomplete thrombus organization and conglutination with the walls were observed 7 days after operation.Mild recanalization was found in both groups. On the 14th postoperative day,the hyperplasia of smooth muscle cells, collagenous fibers, and elastic fibers was detected.Complete thrombus organization was seen, and recanalization presented no obvious changes.ConclusionsBoth electrocoagulation and EVLT can lead to the occlusion of varicose veins, being effective therapeutic alternatives for varicose veins of the lower extremity.

[ ABSTRACT] AIM:To observe the inhibitory effect of madecassoside on the LPS-stimulated microglia and to inves-tigate its possible mechanism.METHODS:Microglia cells of neonatal Sprague-Dawley ( SD) rats were cultured, isolated and purified.Microglia cells were activated with lipopolysaccharide ( LPS) .The inhibitory effect of madecassoside on micro-glia was measured by MTT assay.Tumor necrosis factor alpha (TNF-α), interleukin 1β(IL-1β) were detected by ELISA. Cell cycle and apoptotic rate were evaluated by flow cytometry. The expression of TLR4 was detected by Western blotting. The expression of NF-κB was detected by RT-PCR.RESULTS: LPS induced the proliferation of microglia and release in-flammatory cytokines significantly.Compared with LPS group, madecassoside inhibited the proliferation of microglia induced by LPS in a dose dependent manner. The IC50 value of madecassoside was 10.97 nmol/L to microglia after incubation for 48 h.Madecassoside also decreased the levels of TNF-αand IL-6, increased the ratios of microglia at the G2 phase and the ap-optotic rate, decreased the expression of TLR4 and NF-κB significantly (P<0.05).CONCLUSION:Madecassoside has in-hibitory effects on the proliferation of LPS-stimulated microglia, by which the mechanism may be related to inhibition of the expression of TLR4 and NF-κB, change of cell cycle distribution and induction of microglia apoptosis.

Objective To investigate the kinetics and phenotype of spleen dendritic cells (DC) in lipopolysaccharide (LPS)-induced acute lung injury (ALI) mice. Methods Thirty-six C57BL/6 mice were randomly (random number) divided into two groups:control group and ALI group.Spleens were harvested at the following intervals of 6,12,and 24 h after LPS or PBS administration.Lung wet weight / body weight ratio (LW/BW) was recorded to assess lung injury.Meanwhile, pathological changes were examined under optical microscope. The IL-6 level in the lung was measured by using ELISA (enzymelinked immuno sorbent assay). The DC in the spleen was measured by flow cytometry (FCM).Results (1) LPS-ALI resulted in a significant increase in LW/BW ratio.(2) Histologically, extensive alveolar wall thickening resulted from edema, severe hemorrhage in the interstitium and alveolus, and marked and diffuse interstitial infiltration with inflammatory cells were observed in the ALI group.(3) Meanwhile, the levels of IL-6 in lung tissue were significantly increased in the LPS-induced ALI mice.(4) LPS-induced ALI led to divergent kinetics of spleen DC in ALI mice.In ALI mice, spleen DC only showed a transient increase at 12 h.(5) All DC within spleens had a modest maturation in ALI mice. Conclusions LPS-induced ALI provokes a transient increase as well as modest maturation of spleen DC.

The necessity and superiority of the surgical operation on children with floating knee injury and the fracture union and complications were investigated. Twenty-eight children with floating knee injury were subjected to open reduction and internal fixation or external fixator. The patients were followed up for 18 months to 7 years. The curative effectiveness was scored by Karlstrom criteria. The results showed that no nonunion or deformity was found. The affected limb was 1.2 cm to 1.5 cm longer in 2 cases, 0.8 to 1.2 cm shorter in 3 cases than the contralateral. No severe dysfunction of knee joint occurred. The excellent-good rate was 92.8% and the curative rate 71.4% respectively. So for children whose age is older than 5 years, it's a good way to treat the fractures of femur and tibia with open reduction and internal fixation or external fixator. The method can be advantageous for the nursing care, early function recovery, shortening of the hospital stay and avoidance of severe complications.

60 years)group and younger(≤60)group. Analysis of the pathogenesis of DVT in the 2 groups was made and the relationship of DVT and CS (between) the 2 groups was compared. There were 43 patients in elder group and 46 in younger group. All (patients) were treated by thrombectomy. Results There were 25 (58.1%) patients with concomitant CS in elder group and 37 (80.4%) in younger group. There was a significant difference between the two groups (?~2=5.2274, P

Objective: To establish a determination method for the enantiomer in safinamide mesilate. Methods: A Chiralpak ASH (250 mm ×4. 6 mm, 5 μm) column was used with the mobile phase of n-hexane-ethanol-diethylamine (75 ∶ 25 ∶ 0. 1). The flow rate was 1. 0 ml·min-1 . The wavelength was set at 225 nm. The column temperature was 35℃. Results: The resolution of safinamide mesilate and the enantiomer was above 2. 0. The linear range of them was 1. 007-2 517. 500 μg·ml-1 and 0. 909-2273. 200 μg·ml-1 ,respectively(r = 0. 999 0). The average recovery of the enantionmer was 104. 9% with RSD of 2. 3% (n = 9). Conclusion: The method is accurate and rapid, and suitable for the determination of the enantiomer in safinamide mesilate.
