2.Investigation progress of molecular biomarkers from body fluids in early gastric cancer

The mechanism of gastric cancer (GC) remains unclear. The diagnosis and treatment of GC undergo rapid development, but the prognosis of the patients remains dismal. The key procedure to improve overall survival is early detection and treatment. However, classic tumor markers have several limitations in clinical practice because of their low sensitivity and specificity. Given the expansion of high throughput technologies, investigators have attained considerable progress in obtaining the benefits of molecular biomarkers through genomics and proteomics analysis. These findings provide new directions for the diagnosis and treatment of early-stage GC. Markers from body fluids, which can be easily obtained , are increasingly gaining attention. This article summarizes the progress of the investigation of molecular biomarkers in early GC using body fluids.
3.Expression changes of insulin-like growth factor-1 and its receptor as well as extracellular signal regulated kinase 1/2 in the vestibular epithelium of guinea pigs following gentamycin toxicity

BACKGROUND:Insulin-like growth factor-1 (IGF-1) is an important mitogen and an indispensable regulator during normal hair cell development. Extracellular signal regulated kinase 1/2 (ERK1/2) is also expressed in mammalian vestibular organs.OBJECTIVE: To investigate the changes of the expression and the distribution of IGF-1, IGF-1 receptor (IGF-1R) and ERK1/2 in guinea pig's vestibular epithelium following gentamycin injury.DESIGN: A randomly controlled study.SETTING: Department of Otorhinolaryngology, General Hospital of Chengdu Military Area Command of Chinese PLA.MATERIALS: Twenty healthy and adult guinea pigs, weighing 300 to 350 g, provided by the Experimental Animal Center,Fourth Military Medical University of Chinese PLA, were randomly divided into four experimental groups and a control group with 4 in each group.METHODS: The experiment was performed at Department of Otorhinolaryngology Research Laboratory of Xijing Hospital,Fourth Military Medical University of Chinese PLA from January 2002 to May 2002. In the experimental groups,gentamycin was intraperitoneally administered daily (80 mg/kg) for 10 consecutive days. For the control group, 1 mL of 0.9% sterile saline was administered in the same way. The guinea pigs, in which unsteady gait or nystagmus appeared,could be involved in the further experiments. Before being sacrificed, the animals' gait, nystagmus and the amount of food-intake were observed every day. The animals were anesthetized deeply with 50 mg/kg pentobarbital 1, 7, 14 or 21days after the last gentamycin administration and then decapitated. Bilateral otic vesicles were harvested and immersed in 4% paraformaldehyde. Then the specimens were decalcified in 10% ethylenediamine tetraacetic acid (EDTA),thereafter embedded in OCT gel and serially sectioned at a thickness of 10 μm in the cryostat. The behavioral changes of guinea pigs were observed. The morphological changes of vestibular epithelium were examined by hematoxylin and eosin (HE) staining. The expression and distribution changes of IGF-1/1R and ERK1/2 were investigated immunohistochemically. MAIN OUTCOME MEASURES: The behavioral, morphological, expression and distribution changes of IGF-1/1R and ERK1/2 of guinea pigs in each group.RESULTS: Twenty involved guinea pigs entered the stage of final analysis. ① All the guinea pigs' vestibular functions were impaired after gentamycin treatment, then partially restored without any pharmaceutical treatment. ②Morphological injury was obvious after gentamycin treatment, and improved 1 week later. ③The expression of IGF-1/1R was low in the control group, and in the 1-day group it increased significantly to its maximum. After that, its expression decreased gradually, but was still higher in the 21-day group than that in the control group. There were significant differences in the expression of IGF-1/1R among different groups (F =51.8,45.7,P < 0.05). The expression changes of IGF-1 and its receptor were similar. ④The expression of ERK1/2 was low in the control group, and increased gradually after gentamycin toxicity. In the 7-day group, the immunoreactivity reached its maximum. Then the levels of ERK1/2 decreased gradually, but were still higher in the 21-day group than that in the control group. There were significant differences in the expression of ERK1/2 among different groups (F =103.7,106.4, P < 0.01), but no significant differences existed in the expression between ERK1 and ERK2 among different groups (P > 0.05).CONCLUSION: The expression of IGF-1/1R and ERK1/2 in vestibular epithelium increased after gentamycin treatment,and IGF-1 may play an important role as an endogenous mitogen through a paracrine or autocrine manner in the early stage of hair cell self-repair of guinea pigs. ERK1/2 may play an important role in signal transduction during vestibular hair cell self-repair after gentamycin toxicity in guinea pigs.

6.Zero-profile interboby fixation system for multi-level cervical spondylotic myelopathy

BACKGROUND: Anterior cervical decompression and fusion has been widely used in the treatment of multi-level cervical spondylotic myelopathy, but accompanied with lots of complications.OBJECTIVE: To investigate the efficacy of zero-profile interboby fixation system for multi-level cervical spondylotic myelopathy.METHODS: Seventy-one patients with multi-level cervical spondylotic myelopathy were randomly divided into two groups, and the patients in group A accepted zero-profile interboby fixation system, and group B accepted cage interboby fixation system. The Japanese Orthopaedic Association score, fusion rate, as well as the incidence of dysphagia and esophageal fistula were detected to compare the efficacy between two groups.RESULTS AND CONCLUSION: (1) All cases were followed-up for 3-34 months, average of 17.5 months. The excellent and good rate at the last follow-up showed no significant difference between two groups. (2) The final fusion rate did not differ significantly between two groups, but the fusion rate in the group A was significantly higher than that in the group B at 6 and 9 months postoperatively (P < 0.05). (3) There was one patient with mild dysphagia in the group A (3%), three mild, five medium, and two severe dysphagia in the group B (29%), which showed significant difference between two groups (P < 0.05). No internal fixation loosening occurred in the group A, but three cases in the group B. The blood loss, operation time and radiology times in the group A were significantly lower than those in the group B (P < 0.05). (4) These results suggest that the effect of these two surgical methods in promoting functional recovery of spinal cord and final fusion rate show no significant differences; however, the zero-profile interboby fixation system exhibits better postoperative stability and interim fusion rate, with lower incidence of dysphagia.
7.Systematic evaluation of brucea javanica oil adjuvant therapy of primary hepatocellular carcinoma

Objective To review the efficacy and safety of brucea javanica oil in the adjuvant therapy of primary hepatocellular carcinoma (HCC).Methods China National Knowledge Infrastructure (CNKI),China Biology Medicine (CBM),VIP,Wanfang database,Pubmed,Web of Science,ScienceDirect,and Cochrane Library were searched from their inception to December 2015.Then contact with the field experts and correspondence authors for gray literature.Two reviewers independently searched the databases,performed data extraction,and appraised the publications.The Reviewer Manager 5.3 software was employed for data analysis.Results Fifteen clinical trials with 1 128 HCC patients were included.Meta-analysis confirmed that the brucea javanica oil group,compared to the control group,was more advantageous to reduce the incidence of postoperative fever,bone marrow suppression,and gastrointestinal reaction.In addition,it might reduce the level of alpha fetoprotein (AFP),enhance immunity,and improve clinical symptoms.However,more evidence would be needed to support these results.Conclusions Brucea javanica oil is considered to reduce toxicity and increase efficiency in the adjuvant therapy for the HCC,but more high quality,multi-center,large sample,randomized,double-blind clinical trials are also needed for supporting this view.
8.Contrast analysis of FTIR fingerprints of Radix Paeoniae Rubra from different areas

Object To develop a new method of discriminating the principle components of Radix Paeoniae Rubra (RPR) from different areas, and to find the reason for forming high-quality of RPR in the place of the genuine. Methods Using Foruier transform infrared (FTIR) absorption spectrometry, the characteristic peaks of fingerprint infrared spectra of RPR samples from 18 habitats were recognized and compared. Results Frequency, intensity and shape of infrared absorption spectra were obviously different between wild and cultivated groups of RPR. The infrared absorption peaks of RPR in Duolun, a famous Chinese orthodox drug, were of distinct characteristics. Conclusion FTIR technique is first applied to rapid analysis of principle component of RPR from different areas. So an operable method in the quality control and discrimination of RPR in the place of the genuine is provided.
9.Cause of Anemia and Correlative Nursing Care in Senility Patients after Hip Arthroplasty

Objective To analyze the cause of anemia in patients after hip arthroplasty and to explore correlative nursing intervention. Methods 47 patients undergoing hip arthroplasty in our hospital from 2007 to 2010 were reviewed, they were divided into elderly group (≥70 years old) and non-elderly group (<70 years old). Intra-operative blood loss, post-operative visible blood loss, hidden blood loss, total blood loss, perioperative hemoglobin and number of anemia were compared. Results The average volume of intra-operative blood loss and extra- operative blood loss were significantly lower in elderly group than in non-elderly group (P<0.001); the hidden blood loss was significantly higher in elderly group and in non-elderly group (P<0.001); the incidence of anemia in elderly group was higher than in non-elderly group (P<0.05). Conclusion More post-operative hidden blood loss can be found in patients over 70 years old after hip arthroplasty, so as to the incidence of anemia. The perioperative nursing care and observation should be strengthened to reduce the incidence of anemia.
