AIM: To study the immunity modulation mechanism of achyranthes bidentata polysaccharides (ABPS). METHODS: (1) PBMC from patient with asthma or lung cancer were cultured with 800 mg/L ABPS for 18 h. IFN-? and IL-4 mRNA was detected by RT -PCR. (2) Th cells were cultured with serially diluted ABPS for 48 h and with 800 mg/L ABPS for various times. Cultured supernatant was harvested at diffevent time points and RNA was extracted, respectively. IFN-? or IL-4 transcriptional and translational levels were assay by RT -PCR and ELISA. Objective To evaluate the effects of fraction of inspired oxygen (FiO2) and positive end-expiratory pressure (PEEP) on the gradient between arterial and end-tidal carbon dioxide (D(a-ET)CO2).Methods Sixty ASA Ⅰ or Ⅱ patients (body mass index < 30 kg/m2),aged 25-50 yr,weighing 45-75 kg,scheduled for elective gynecological laparoscopic surgery,were randomized into A,B and C groups (n =20 each).The patients were mechanically ventilated with pure oxygen in group A,and with air and O2 (FiO2 =50%) in groups B (PEEP =0) and C (PEEP =5 cm H2O).PETCO2 was monitored during mechanically ventilation.Blood samples were obtained from the radial artery immediately after tracheal intubation (T1) and at 1 h of pneumoperitoneum (T2) for blood gas analysis.D(a-ET) CO2 and Qs/Qt were calculated.Results D(a-ET) CO2 and Qs/Qt were significantly lower at T2 in groups B and C than in group A (P < 0.05).Compared with group B,D(a-ET)CO2 was significantly decreased at T2 (P < 0.05),and no significant change was found in Qs/Qt in group C (P > 0.05).Conclusion Decreased FiO2 and PEEP of 5 cm H2O can decrease D(a-ET)CO2 and increase the accuracy of PErCO2 for reflection of PaCO2,which is due to reduced intrapulmonary shunt.
3.Prevention and treatment of cognitive dysfunction caused by radiotherapy to the brain

Radiation-induced brain injury is a serious untoward effect of radiotherapy for malignant tumors. Patients received radiotherapy frequently occur cognitive dysfunction which seriously affects the quality of life. Although the exact mechanisms regarding radiation-induced cognitive dysfunction remain unclear, prevention strategies targeting cognitive dysfunction are increasingly applied to clinical intervention, including whole brain radiotherapy with hippocampus avoidance, stereotactic radiosurgery in patients with multiple brain metastases, and pretreatment with neuroprotective drugs such as memantine and donepezil. In addition, measures including appropriate radiotherapy management models, regular cognitive tests, and therapeutic measures at the appropriate time are critical to improve the quality of life for brain tumor patients.
4.Pathological effect of Anti-stenosis No.1 on restenosis after carotid artery angioplasty in experimental rats

ObjectiveTo explore the pathological effect of Anti-stenosis No.1 which had effect of vivid Qi and promoting blood flow,eliminate sputum and circulating collaterals on restenosis after carotid artery angioplasty in experimental rats.Methods48 rats were randomly divided into the blank group,the hyperlipid group,the small-dose group and the large-dose group.Restenosis model in carotid artery after angioplasty was built by high-dose cholesterol diet and three times balloon injury.Chinese herbs were administered for three months.Lumen area,intimal area,media area and intimal area/media area in the restenosis region were calculated.ResultsLumen area and intimal area were very more significantly changed in the hyperlipid group,the small-dose group,and the large-dose group compared to the blank group(P<0.01).Media area and intimal area/media area were very more significantly changed in the hyperlipid group and the small-dose group compared to the blank group(P<0.01).There was no significant difference in the large-dose group.Compared to hyperlipid group,lumen area significantly increased in the large-dose group(P<0.01)),intimal area significantly decreased in the small-and large-dose groups(P<0.05),and media area significantly decreased in the large-dose group(P<0.01).There was a significant difference in media area between the small-and large-dose groups(P<0.05).ConclusionAnti-stenosis No.1 can significantly inhibit restenosis by inhibiting intimal and media hyperplasty,and the effect is dose dependent.
5.Clinical observation on 308-nm monochromatic excimer light for the treatment of amelanotic nevus

Objective To evaluate the efficacy and safety of 308-nm monochromatic excimer light for the treatment of amelanotic nevus.Methods Eighteen patients with amelanotic nevus were treated with 308-nm monochromatic excimer light twice a week for five consecutive weeks.Melanin content index (MCI) was measured in the lesions before and after the treatment.After the final exposure,a three-month follow-up was carried out.Results After the start of treatment,the MCI in lesions increased in a session-dependent manner and approximated to 100% of that in perilesional normal skin after six sessions of treatment.The follow-up revealed a decrease trend in MCI after the end of the treatment,which was nearly equal to that in perilesional normal skin one month later,and dropped to about 90% of that three months later.No side effects such as blisters or scar were observed during the treatment.Conclusions The 308-nm monochromatic excimer light is safe and effective for the treatment of amelanotic nevus.Re-exposure to 308-nm monochromatic excimer light after three months is recommended as consolidation therapy.
6.The role of 99mTc-MIBI SPECT/CT in predicting response to neoadjuvant chemotherapy with docetaxel-based regimen for locoregionally advanced nasopharyngeal carcinoma

Background and purpose:Neoadjuvant chemotherapy with docetaxel was gaining more attention in the treatment for locoregionally advanced nasopharyngeal carcinoma (NPC). The prediction of the sensitivity to neoadjuvant chemotherapy with docetaxel could enable clinicians to individualize treatment protocols for NPC. Recently we found that 99mTc-MIBI imaging in NPC could predict tumor response to chemotherapy with cisplatin plus 5-FU. However, there was no study to support similar findings in NPC patients receiving chemotherapy containing docetaxel. This study was to evaluate the value of double-phase 99mTc-MIBI SPECT/CT in predicting response to neoadjuvant chemotherapy with docetaxel-based regimen for nasopharyngeal carcinoma. Methods:Thirty-one nasopharyngeal carcinoma patients participated in this prospective study. Before treatment, early and delayed single-photon emission computed tomography/compute tomography (SPECT/CT) images were obtained instantly and 2 hours after an intravenous injection of 25-30 mCi 99mTc-MIBI. All patients received neoadjuvant chemotherapy consisting of docetaxel, cisplatin plus 5-FU for two cycles. The relationships between efficacy of neoadjuvant chemotherapy and the early uptake ratio, late uptake ratio and washout rate of 99mTc-MIBI were evaluated.Results:According to the MRI, the early uptake of 99mTc-MIBI (2.67±0.83) in the lesions which were sensitive to chemotherapy was significantly higher compared with that (1.69±0.46) in the insensitive lesions(P=0.003). The difference of the late uptake between the sensitive(1.46±0.39) and the insensitive (1.06±0.62) was also statistically significant (P=0.026). However, the washout rate of 99mTc-MIBI was not significantly different between the two groups (P=0.23). Through ROC curve analysis, the AUC for early uptake of 99mTc-MIBI and late uptake were 0.84. The sensitivity, specificity, positive predictive value and negative predictive value were 74.2%, 87.5%, 95.8%and 46.7%for early uptake when the cut off value of 1.97 was used. Conclusion:The uptake of 99mTc-MIBI in both early phase and late phase could predict the efficacy of neoadjuvant chemotherapy with docetaxel-based regimen.
7.Clinical characteristics and etiological analysis of lung nodules in 98 children

Objective To analyze the clinical features and common etiology of lung nodules in children. Methods The etiology, diagnosis, radiological features from 98 hospitalized children of lung nodules were analyzed. Results Of them, 58 were male and 40 were female aged from 0.2 years old to 14.8 years old. Pulmonary infection were found in 41 cases (41.8%) including tuberculosis in 15 cases (15.3%), pulmonary fungal infection in 13 cases (13.3%), pneumonia in 11 cases (11.2%), lung trematode in 2 cases (2.0%). Pulmonary metastases were found in 28 cases (28.6%), multiple pulmonary arteriovenous fistula in 1 case (1.0%), and pulmonary contusion in 1 case (1.0%) and unknown etiology in 27 cases (27.6%). Conclusions The etiology of lung nodules is complicated, in which infectious diseases are the most commonly seen, followed by pulmonary metastases. Biopsy is the golden standard of diagnosis.
8.Determination of Nicotinate-curcumin Ester and Its Nanoparticles by HPLC

Objective: To provide basis for the preparation process study by establishing an HPLC determination method for nicotinate-curcumin ester.Methods: A C18 column(250 mm×4.6 mm,5 μm) was used at 30℃ with the mobile phase of acetonitrile-0.5% acetic acid glacial(65∶35) at a flow rate of 1.0 ml·min-1.The UV detection wavelength was 280 nm.Results: The good linear relationship of nicotinate-curcumin ester was shown within the range of 5.200-104.000 μg · ml-1(r=0.9999 4), The average recovery of nicotinate-curcumin ester and its nanoparticles was 100.1% and 100.9%with RSD of 1.260% and 0.995 0%(n=6), respectively.Conclusion: The method is convenient, accurate and reproducible, and suitable for the determination of nicotinate-curcumin ester nanoparticles and the studies on the preparation technology.
9.Fungal and virus infection following kidney transplantation

BACKGROUND: Infection following kidney transplantation has become one of the main reasons for graft failure and death of allograft recipients. However, there is not a standard therapeutic scheme for infection following kidney transplantation. OBJECTIVE: To investigate the clinical features and treatment measures of infection, additionally, to increase the cure rate of infection following kidney transplantation.DESIGN, TIME AND SETTING: A retrospectively analysis was performed at the Organ Transplantation Center, the First Affiliated Hospital of Kunming Medical College from February 2006 to February 2008.PARTClPANTS: Eighteen cases of infections in 84 kidney allograft recipients.METHODS: All cases were checked by chest X-ray. Patients who had no significant lung infection symptoms or obvious signs received lung CT scan. Pathogen detection was performed, including hemoculture, urine culture, sputum culture, nose swabs culture, throat swab culture, checking clinically important cytomegalovirus (CMV), EB-DNA and mycoplasma in blood, acid-fast bacilli and eumycete culture in sputum. All cases of pulmonary infection underwent a comprehensive treatment-antiviral drugs, antibiotics and antifungal. Depending on the individual condition and absolute values of lymphocytes and CD4+T cells, the immunosuppressant was adjusted individually. The occurrence time, clinical symptom, auxiliary examination and treatment strategies were analyzed.MAIN OUTCOME MEASURES: The occurrence time of infection and clinical symptoms; imaging manifestation and results of pathogenic detection; selection of antibiotics and immunosuppressant adjustment.RESULTS: Among 18 cases, 11 cases (61.2%) were deceased-donor kidney transplant recipients. Inflection following kidney transplantation occurred in 12 cases (66.7%) within 3 months, and increased to 15 cases (83.3%) within 3-6 months. Of the 18 infection cases, 14 cases (77.8%) had a main symptom of fever. There were 15 cases (83.3%) of respiratory tract infection, including 13 cases (72.2%) of pulmonary infection. Fungal cultivation, especially Monilia, was positive in 6 cases. Three out of the 18 cases (16.7%) died, two of whom had CMV infection. Mixed infection occurred in all cases.CONCLUSION: Infected patients following kidney transplantation present with diversity pathogens, which are dominated by bacteria, fungus and virus. Severe pneumonia combined with CMV infection demonstrates that poor prognosis, fungal and virus infection following kidney transplantation should be given more attention. Combined de-escalation therapy is the main method, and timely adjustment and even discontinuance of immunosupprassive agents is one of the key points in the treatment of infection following kidney transplantation
10.Research progress on proteasome subunits in regulating occurrence and development of hepatocellular carcinoma.

Proteasome is the eukaryotic organelle responsible for degradation of short-lived proteins and involved in maintaining cellular protein homeostasis. It has been reported that during the occurrence and development of hepatocellular carcinoma (HCC), the regulatory particle subunits of proteasome regulate a series of tumor-related proteins, and proliferation, survival-associated signaling molecules, including PTEN gene, P53, Bcl-2, Bcl-2 interacting mediator of cell death (Bim), cyclin-dependent kinase 4(CDK4), transforming growth factor β receptor (TGFBR), E2F1, growth factor receptor-bound protein 2 (GRB2) . Meanwhile, these subunits regulate some tumor-associated pathway protein, such as signal transducer and activator of transcription 3 (STAT3) and protein kinase B (AKT), inducing their malfunction to promote the occurrence, proliferation, invasion and metastasis of HCC. The core particle subunits are more to perform the degradation of HCC-related proteins, so inhibitors targeting the core particle show a good anti-tumor effect. This review summarizes the current research progress on the regulation and mechanism of proteasome subunits in promoting the occurrence and development .
