1.The value and limitation of primary MSCT examination for rib fractures in a short interval

Objective To evaluate the value and limitation of primary MSCT examination for rib fractures in a short interval. Methods Seventy-two cases with rib fractures were analyzed retrospectively.All of them underwent primary MSCT examination within the three days after trauma,and MSCT examination of chest,epigastria,rib or X-ray examination of rib for 1-6 times in the following 8 weeks.41 cases underwent X-ray examination firstly before MSCT examination.The total number of rib fractures was determined according to the compositive results of every examination.Diagnosis rate of the primary MSCT and X-ray examination were compared with each other.Results Among 72 cases,404 fractures in 325 ribs were diagnosed.Primary MSCT diagnosed 305 confirmed fractures and 28 doubtful fractures,and 84 fractures were not detected with diagnosis rate of 75.5%.13 of 28 suspected fractures were ruled out by the following re-examinations.The MSCT and X-ray diagnostic rates were 72.1%、50.3%,respectively for 41 cases who underwent X-ray examination firstly.Four fractures showed by X-ray plain film were not shown by primary MSCT.Conclusion There is a certain proportion of cases diagnosed as obscure or doubtful rib fracture by the primary MSCT. Therefore,diagnosis should be closely combined with the re-examination results of MSCT and X-ray plain film,especially MSCT ex-amination between the fourth and eighth weeks after trauma.
2.Clinical result and follow-up of incidental prostate cancer after radical prostatectomy

Objective To investigate the long-term outcome of radical prostatectomy (RP) in the patients with incidental prostate cancer (IPCa) detected by surgery of benign prostatic hyperplasia (BPH) and to evaluate the risk factors for residual tumour after BPH surgery and biochemical recurrence in patients with IPCa treated with RP.Methods We retrospectively analyzed the clinical and follow-up data of 45 patients with IPCa detected by surgery of BPH and undergoing RP from January 2004 to October 2016.The age,PSA before and after BPH surgery,prostate volume,T stage and Gleason score after the BPH surgery,T stage at RP (pT0,pT2,pT3),Gleason score at RP and status of biochemical recurrence were recorded.Multivariate logistic regression analysis addressed the association between the factors and the presence of residual cancer after the surgery for BPH.Cox regression was used to analyzed the relationship between the factors and the rate of biochemical recurrence after RP.Results Among 45 IPCa patients,21 patients were stage T1a and 24 were stage T1b.After RP,7 (15.6%) patients had no residual tumor (pT0).PSA before BPH surgery (RR =2.58,95% CI 1.27-5.42,P =0.04),PSA after BPH surgery (RR =4.26,95% CI 2.57-7.64,P =0.01) and Gleason score after BPH surgery (RR =3.98,95% CI 1.85-5.77,P =0.02) were significant associated factors with the residual cancer after BPH surgery.With a mean follow-up of 54 months(ranging 5-144 months),the 5-and 10-years.biochemical recurrence-free survival rates were 95.6% and 86.7%,respectively.PSA after surgery for BPH (RR =4.79,95% CI 2.57-7.64,P =0.02) and Gleason score after RP(RR =2.01,95% CI 1.74-5.21,P =0.04) were the only independent risk factors for biochemical recurrence.Stage (T1a-T1b) did not predict residual cancer or the rate of biochemical recurrence (P > 0.05).Conclusions RP in the patients with IPCa detected by BPH surgery had a good outcome of long-term oncological control.PSA before and after BPH surgery and Gleason score at BPH surgery were the significant associated factors of residual cancer after BPH surgery.PSA after BPH surgery and Gleason score at RP were the only independent risk factors for biochemical recurrence.
3.Impact of Community Popular Opinion Leader Intervention on Ocular Epidemic Study

Objective To observe the role of community popular opinion leader (POL) intervention on ocular epidemic study. Methods During the process of epidemiological survey of eye disease in Shenyang City,the community POL was interviewed to observe the change of the follow-up population from 2015 to 2016 and the effects of the POL intervention were analyzed. Results The study population follow-up rate was significantly improved after POL intervention (χ2 = 85.42,P < 0.01),the male-to-female ratio was balanced (χ2 = 6.51, P = 0.01),and more participants were willing and hoped to participate in the epidemic study for a long time. Conclusion Community POLs can improve the follow-up rate of epidemiological surveys and mobilize the enthusiasm of the population to participate in epidemiological investigations.
4.Discovery and identification of EIF2AK2 as a direct key target of berberine for anti-inflammatory effects.

Using chemoproteomic techniques, we first identified EIF2AK2, eEF1A1, PRDX3 and VPS4B as direct targets of berberine (BBR) for its synergistically anti-inflammatory effects. Of them, BBR has the strongest affinity with EIF2AK2 via two ionic bonds, and regulates several key inflammatory pathways through EIF2AK2, indicating the dominant role of EIF2AK2. Also, BBR could subtly inhibit the dimerization of EIF2AK2, rather than its enzyme activity, to selectively modulate its downstream pathways including JNK, NF-κB, AKT and NLRP3, with an advantage of good safety profile. In EIF2AK2 gene knockdown mice, the inhibitory IL-1β, IL-6, IL-18 and TNF-α secretion of BBR was obviously attenuated, confirming an EIF2AK2-dependent anti-inflammatory efficacy. The results highlight the BBR's network mechanism on anti-inflammatory effects in which EIF2AK2 is a key target, and inhibition of EIF2AK2 dimerization has a potential to be a therapeutic strategy against inflammation-related disorders.
