1.Advances in platinum-intercalators of DNA as antitumor agents

Platinum antitumor drugs are widely used for clinical treatment because of their unique antitumor mechanisms, significant antitumor effects, and broad antitumor spectrum. Yet shortcomings such as toxic side effects, drug resistance and cross-resistance of platinum-based drugs have limited their further application. Platinum-intercalator conjugates possess different antitumor mechanisms from those of classic platinum drugs, and have unique advantages in overcoming the disadvantages of classic platinum antitumor drugs. The platinum-intercalator conjugates can be classified into six categories based on the different DNA-intercalator: platinum-acridine, platinum-quinoline, platinum-indole, platinum-naphthalimides, platinum-anthraquinone and platinum-based antitumor agents containing other types of intercalating groups. This article summarizes the research progress of platinum-based antitumor drugs containing DNA insertion groups in recent years.
2.Epidemiology of intussusception related hospitalizations in children aged <2 years in Suzhou, 2007-2013

Objective To understand the epidemiological and clinical characteristics of < 2 years old children hospitalized due to intussusceptions.Methods Clinical and demographic data of <2 years old children hospitalized due to intussusception between January 2007 and August 2013 were retrospectively collected in Affiliated Children' s Hospital of Soochow University in Suzhou.The incidence data,age distribution,seasonality and clinical characteristics of hospitalized intussusceptions cases were analyzed.Results A total of 594 intussusception-related hospitalizations were identified during this period in children aged <2 years,no death occurred.The crude incidence of hospitalized intussusception was 57.3 per 100 000 in children aged <2 years (95%CI:52.8-62.1),and 100.6 per 100 000 in children aged <1 year (95%CI:92.1-109.8).The male to female ratio was 1.90 ∶ 1.Up to 85.4% (507/594) of the cases were aged < 1 year,and 66.2% (393/594) of the cases were aged 3-8 months.The incidence peaked in age group 5-8 months.The median age of the cases was 6.8 months (QR=4.4),and increased from 6.3 months (QR=4.2) in 2007 to 7.3 months (QR=4.0) in 2013.No obvious seasonality was observed.Main symptoms or signs included vomiting (83.2%,494/594),abdominal mass (81.1%,482/594),and bloody stool (64.5%,383/594).Abdominal ultrasonic testing was the most frequently used diagnostic approach (98.7%,586/594).Up to 86.2% (512/594) of patients were successfully treated by surgical intervention.The main sites for acute intussusception in children aged <2 years were ileo-colic (34.5%,183/530),ileo-ileo (30.8%,163/530) or ileo-ileo-colic (27.9%,148/530).Conclusion The incidence of hospitalized intussusception in children aged <2 years was high in Suzhou.It is necessary to establish an active surveillance system to provide baseline data for the evaluation of rotavirus vaccine safety.
3.Comparison of ultrasound-guided dynamic needle tip positioning versus long axis in-plane techniques for axillary vein puncture and catheterization

Objective:To compare the effects of ultrasound-guided dynamic needle tip positioning (DNTP) and long axis in-plane (LAX-IP) techniques for axillary vein puncture and catheterization.Methods:One hundred Society of Anesthesiologists physical statusⅠ-Ⅲ patients of both sexes, aged 18-64 yr, with body mass index of 20-28 kg/m 2, scheduled for elective axillary vein cannulation, were divided into 2 groups ( n=49 each) using the random number table method: DNTP group and LAX-IP group.Axillary vein puncture was performed using DNTP technique and LAX-IP technique under ultrasound guidance in DNTP group and LAX-IP group, respectively.Successful puncture at first attempt, overall successful catheterization, the number of needle tip redirection, and axillary vein puncture time and catheterization time were recorded.The occurrence of complications such as axillary artery puncture, posterior wall penetration of axillary vein, hematoma formation, pneumothorax, and nerve injury was recorded. Results:Compared with group LAX-IP, the success rate of puncture at first attempt was significantly increased, the number of cases required needle redirection was decreased, and the puncture time was shortened ( P<0.05), and no significant change was found in the logarithm of the posterior wall penetration of axillary vein in group DNTP ( P>0.05). No complications such as arterial puncture, hematoma, pneumothorax, or nerve injury occurred in two groups. Conclusions:Compared with LAX-IP technique, ultrasound-guided DNTP technique can dynamically observe the position of the needle tip, the operation is simple and safe, and it is worthy of clinical promotion when used for axillary vein puncture and cannulation.
4.Analgesic efficacy of pericapsular nerve group block in elderly patients with hip replacement under subarachnoid block

Objective:To evaluate the analgesic efficacy of pericapsular nerve group (PENG) block in elderly patients undergoing hip replacement under subarachnoid block.Methods:Fifty patients of both sexes, aged 65-89 yr, of American Society of Anesthesiologists physical status Ⅱ or Ⅲ, with body mass index 20-30 kg/m 2, undergoing unilateral hip arthroplasty, were divided into 2 groups ( n=25 each) by a random number table method: PENG group and fascia iliaca compartment block (FICB) group.In PENG group, 0.4% ropivacaine hydrochloride 20 ml was injected around the nerve innervating the hip joint capsule under ultrasound guidance.In FICB group, 0.4% ropivacaine 30 ml was injected around the nerve innervating the fascia iliaca compartment under ultrasound guidance.Subarachnoid block was performed in both groups.Visual analog scale scores and scores for satisfaction with analgesia at rest and during activity were recorded before blockade (T 0), at 10, 20 and 30 min after blockade (T 1-3) and when placed in the position for spinal anesthesia (T 4). The cumulative consumption of sufentanil, effective pressing times of analgesic pump, and development of related complications were recorded at 6, 12, 24 and 48 h after operation (T 5-8). Results:Compared with FICB group, the VAS scores at rest and during activity were significantly decreased at T 1-4, and scores for satisfaction with analgesia during activity were increased in PENG group ( P<0.05). There was no significant difference between the two groups in the cumulative consumption of sufentanil and effective pressing times of analgesic pump ( P>0.05). One patient developed postoperative delirium in group FICB, and no patients developed puncture site infection and nerve damage after operation in two groups. Conclusion:PENG block produces better analgesic efficacy than FICB when used for elderly patients undergoing hip replacement under subarachnoid block.
5.Progress in roles and mechanisms of deacetylase SIRT7.

As a member of the Sirtuins family in mammals, SIRT7 locates in nucleus and is a highly specific H3K18Ac (acetylated lysine 18 of histone H3) deacetylase. Recent studies showed that SIRT7 could participate in the ribosomal RNA transcription, cell metabolism, cell stress and DNA damage repair through various signaling pathways. In addition, SIRT7 is also closely related with aging, heart disease and fatty liver. In particular, SIRT7 plays important roles in the regulation of initiation and development of various tumors, such as liver cancer, gastric cancer, breast cancer, bladder cancer, colorectal cancer, and head/neck squamous cell carcinoma. This review describes the cellular and molecular functions of SIRT7, and systematically summarizes recent progress of SIRT7 in human disease.
