1.Investigation of Glucohemoglobin in Gestational Diabetes Mellitus

Objective:To evaluate the value of glucohemoglobin Alc (HbAIe) in selecting gestational diabetes mellitus(GDM)and monitoring blood glucose in gestation. Methods:FPG,HbAlc,and OGTT were retrospectively analyzed in 88 pregnant wornen at 24~28 weeks. Results: The identification rate of HbAlc in GDM was 82 %. Conclusion:HbAlc is a suitable method in selecting GDM.
2.Construction and identification of HSV-1 vector vaccine carrying HIV-1 antigen.

To construct an HSV-1 vector vaccine carrying HIV-1 antigens, HIV-1 gp160, gag, protease and the expression elements were chained together, and then inserted into the internal inverted repeat sequence region of HSV-1 by bacterial artificial chromosome technology. Firstly, HIV-1 gp160 (including type B and C), gag and protease genes were cloned into pcDNA3 in series to generate the pcDNA/gBgp and pcDNA/gCgp, then the recombinant plasmids were transfected into 293FT cells, and HIV-1 antigen was detected from transfected cells by Western blotting. Then the expression cassettes from pcDNA/gBgp and pcDNA/gCgp, comprising HIV-1 antigen genes and expression elements, were cloned into pKO5/BN to generate the shuttle plasmids pKO5/BN/gBgp and pKO5/BN/gCgp. The shuttle plasmids were electroporated into E. coli cells that harbor an HSV-BAC, the recombinant bacteria were screened, and the recombinant DNA was extracted and transfected into Vero cells. The recombinant virus was purified through picking plaques, the virus' DNAs were identified by Southern blotting; HIV-1 antigen was detected from the recombinant HSV-1 infected cells by Western blotting, and the virus' replication competent was analyzed. As the results, gp160 and gag proteins were detected from 293FT cells transfected with pcDNA/gBgp and pcDNA/gCgp by Western blotting. The recombinant bacteria were generated from the E. coli electroporated with pKO5/BN/gBgp or pKO5/BN/gCgp. The recombinant HSV was purified from the Vero cells transfected with the recombinant DNA, the unique DNA fragment was detected from the genome of recombination HSV by Southern blotting; gp120 and gp41 were detected from the infected cells by Western blotting, and the recombinant HSV retained replication competent in mammalian cells. The results indicate that the recombinant HSV carrying HIV-1 gp160, gag and protease genes was generated, the virus retains replication competent in mammalian cells, and could be used as a replicated viral vector vaccine.

3.Effects of exercise rehabilitation in aged patients with chronic heart failure

Objective: To explore safety and effects of exercise rehabilitation in aged patients with chronic heart failure (CHF). Methods: A total of 83 aged CHF patients were randomly divided into exercise rehabilitation group (n=42, received exercise training based on usual care) and usual care group (n=41, received usual care). Period of treatment was eight weeks and patients were followed up for 12 months. New York heart association (NYHA) classification was used to represent cardiac function. Left ventricular ejection fraction (LVEF), left ventricular end-diastolic dimension (LVEDd) were determined by ultrasound cardiography, 6 min walking distance (6MWD) and oxygen metabolic equivalent (METs) also were determined , plasma level of brain natriuretic peptide (BNP) was examined. Minnesota living with heart failure questionnaire (MLHFQ) was used to represent quality of life. Rehospitalization rate and mortality rate within 12 months were recorded in all patients. Results: On 8 th weeks after treatment, the LVEF, LVEDd and NYHA class of two groups all significantly improved(P<0.05 all),compared with usual care group, there were significant improvement in LVEF [(54.7±6.2)% vs. 65.4±8.7)%], LVEDd [(49.6±8.3)mm vs.(40.2±9.3)mm] and NYHA class [(2.7±0.8)classes vs.(1.9±0.9)classes], P<0.05 all; 6MWD [(122.7±9.2) m vs. (175.6±8.7) m] and METs [(5.8±1.8) vs. (8.4±2.4)] also significantly increased (P<0.01), and plasma level of BNP [(43.4±9.8) pg/ml vs. (31.7±8.9) pg/ml, P<0.05] significantly decreased in exercise rehabilitation group. No severe adverse events occurred in exercise rehabilitation group. After 12 months, compared with usual care group, there were significant increase in MLHFQ score [(45.6±8.2) scores vs. (68.9±7.9) scores], significant decrease in rehospitalization rate caused by heart failure (24.4% vs. 9.5%) , P<0.05 all in exercise rehabilitation group. Conclusion: Exercise rehabilitation is safe and effective in aged patients with chronic heart failure, which can significantly improve cardiac function, enhance exercise capacity and increase quality of life.
4.MRI findings of Ramsay Hunt syndrome

Abnormal enhancement of ganglion geniculi (4/4) and labyrinthine segment (3/4) was relatively frequent, abnormal enhancement of distal part of internal auditory segment, tympanic segment, and mastoid segment was seen in 2 cases, respectively.Conclusion MRI doesn't have diagnostic value unless the enhancement of segments of the affected facial nerve side is more evident than the unaffected side for unilateral cases. Labyrinthine segment and ganglion geniculi are more susceptible to Ramsay Hunt syndrome. Gd-DTPA enhanced MRI can demonstrate segmental lesions of facial nerves objectively, thus it can be helpful in the diagnosis and differential diagnosis of Ramsay Hunt syndrome. The relationship between MRI findings and clinical representation still needs further study.

The paper designs and implements a digitalized information resources platform for Mongolian medicine based on ASP.NET.The system adopts VS2008 + SQL SERVER2005 and the 3-tier architectural pattern,integrates functional modules such as News Information,Mongolian Medicine,Mongolian Doctors and Q&A,realizes the digitalization of Mongolian medicine resources and establishes a sharing system.Upon testing,the system operates well and achieves the expectations.

OBJECTIVE:
To investigate the expressions of CD4+CD45RA+T cells and CD4+CD45RO+T cells in peripheral blood of patients with acute coronary syndrome (ACS) and their significance.
METHODS:
A case-control study was conducted. Ninety-four patients receiving coronary angiography (CAG) admitted to Tianjin Chest Hospital from March 5th to April 27th in 2018 were enrolled. They were divided into non-coronary heart disease (CHD) group (n = 12), unstable angina pectoris (UAP) group (n = 27), acute non-ST elevation myocardial infarction (NSTEMI) group (n = 27) and acute ST elevation myocardial infarction (STEMI) group (n = 28) according to the patients' symptoms, electrocardiogram, troponin test and angiographic results. General data, blood routine parameters, and biochemical indicators were collected. The ratios of CD4+CD45RA+T cells and CD4+CD45RO+T cells were determined by flow cytometry. Multivariate Logistic regression was used to evaluate whether CD4+CD45RA+T cells and CD4+CD45RO+T cells were associated with STEMI.
RESULTS:
Ninety-four patients were included initially. After excluding the patients who died during the intervention, 93 patients were enrolled in the data analysis finally, with 12 patients in the non-CHD group, 27 patients in the UAP group, and the same as the NSTEMI group and the STEMI group. Compared with the non-CHD group, white blood cell count (WBC) was decreased (×109/L: 6.03±1.30 vs. 6.60±1.30, P < 0.05), and lymphocyte ratio was increased (0.273±0.059 vs. 0.269±0.070, P > 0.05) in patients of the UAP group; however, in the NSTEMI group and STEMI group, WBC was increased (×109/L: 8.29±2.28, 9.86±2.76 vs. 6.60±1.30, both P < 0.05), and lymphocyte ratio was decreased (0.236±0.076, 0.173±0.094 vs. 0.269±0.070, P > 0.05 and P < 0.05), especially in the STEMI group [WBC (×109/L): 9.86±2.76 vs. 6.60±1.30, lymphocyte ratio: 0.173±0.094 vs. 0.269±0.070, both P < 0.05]. There was no significant difference in biochemical indicators among all of the groups. Flow cytometry results showed that the ratios of CD4+CD45RO+T cells in the UAP group and NSTEMI group were higher than those in the non-CHD group (0.323±0.074, 0.319±0.078 vs. 0.314±0.058, both P > 0.05); however, the ratio of CD4+CD45RO+T cells in the STEMI group showed a decreased tendency (0.270±0.057 vs. 0.314±0.058, P > 0.05), and it was significantly lower than that in the UAP group and the NSTEMI group (0.270±0.057 vs. 0.323±0.074, 0.319±0.078, both P < 0.05). There was no significant difference in the ratio of CD4+CD45RA+T cells among all of the groups. Multivariate Logistic regression analysis showed that CD4+CD45RA+T cells ratio was not significantly correlated with the occurrence of STEMI [odds ratio (OR) = 0.976, 95% confidence interval (95%CI) was 0.907-1.050, P = 0.518], but CD4+CD45RO+T cells ratio was significantly correlated with the occurrence of STEMI (OR = 0.888, 95%CI was 0.821-0.961, P = 0.003).
CONCLUSIONS
There was no significant difference in the ratio of CD4+CD45RA+T cells among UAP, NSTEMI and STEMI patients, and CD4+CD45RO+T cells ratio in the STEMI group was significantly lower than that in the UAP group and NSTEMI group. CD4+CD45RO+T cells ratio may be risk factor of STEMI.

7.Bloom's syndrome in two brothers

The patient 1 was a 9-year-old boy of Hui nationality who presented with recurrent erythema over the face and auricles for 8 years.Telangiectasis and erythema developed on both cheeks 7 months after birth,which gradually spread to the auricles,forearms and both legs with a mild ichthyosiform appearance.At 5 years of age blisters and erosions appeared on the lips,which occurred and were aggravated after exposure to strong sunlight during summer and spontaneously subsided or disappeared in winter.The patient 2,a 3-year-old boy who was the younger brother of the patient 1,presented with recurrent erythema for 2 years.At 1 year of age,erythema began to appear on both cheeks and gradually spread to the buccal region,with mild pruritus and scaling.Meanwhile,the skin of both legs was dry and scaling and gave an ichthyosifonn appearance.The lesions usually appeared in early summer and gradually subsided in winter.No abnormality was found by systemic,laboratory,pathological or other auxiliary examination.Chromosomal abnormality was undetected.Body height was normal.The parents of the two siblings were first cousins.Both children were diagnosed with Bloom's syndrome.
8.The clinical and laboratory features of acute promyelocytic leukemia: an analysis of 513 cases

Objective To investigate the clinical and laboratory features of acute promyelocytic leukemia (APL).Methotis 513 APL patients in the last two decades were retrospectively analyzed in this research.We investigated the clinical features including age,sex,abnormality of peripheral hemogram before treatment.therapeutic effect and follow-up and laboratory data such as morphology,immunology,cytogenetics and molecular biology(MICM).Results The median age of the APL patients was 33 years old and the ratio of male and female was 1.21:1.Before treatment,the median level of WBC was 4.3×109/L and the deteetion rate of abnormal promyelocyte on blood film was 85.8%;with immunophenotypie detection,the expression levels of CD117、CD34、HLA-DR、CD7、CD14 and CD19 in APL were found to be lower and the expression 1evels of CD2、CD33 and MPO higher than those in other subtypes of acute myelocytie leukemia(AML)(beth P<0.01).Specific abnormal chromosome t(15;17)was detected in 91.7%of the patients,of whom 75.9%had standard translocation of t(15;17),being the most common one and 15.8% of the patients had t(15;17)with additional abnormal chromosome.There was only 7.5%of the patients with nolnlal karyotype.However,the presence of both simple translocation and complex translocation was seldom seen.With molecular biological detection.PML/RARα fusion gene positive rate was 99.6%.In a relativelv long clinical follow-up,we found that the complete remission(CR)rate in APL patients was 84.7%.incidence of DIC was 13.4%and five-year survival rate was 30.7%.111e median count of WBC in CR group was lower than that non-remission group(P<0.01).There were no significant differences on expressions of CD34 and CD2 and changes of cytogenetics between the two groups(P>0.05).Conclusions Comprehensive evaluation of MICM could be of important significance in the diagnosis and prognosis iudgrnent for APL patients.The CR rate in these patients with high WBC eount was considerable low.

10.A preliminary study on the rule of secondary correction demand for unilateral cleft lip patients.

OBJECTIVETo explore the secondary correction demand rule for unilateral cleft lip patients.
METHODSThe population in parts of Henan province were investigated by census method. The unilateral cleft lip patients underwent the primary correction were selected and be photographed, and 100 patients were selected randomly into study. The subjective satisfaction for the patients' facial appearance was evaluated by three groups: Professional group(cleft lip and palate specialist), non-professional group (the hospital administrative staff), and family members of patients group. The facial symmetry rate was measured for the patients with consistent subjective evaluation.
RESULTSThe rate of facial symmetry and subjective evaluation was not convergence. The subjective evaluation disaffect rates of profes-sional and non-professional groups were higher than that in family members of patients group (P<0.05).
CONCLUSIONThe rate of facial symmetry is not the most suitable method to evaluate the demand of the secondary correction for unilateral cleft lip patients.
