1.Clinical application of continuity care model in stroke patients

ObjectiveTo probe into the clinical application effect of continuity care model in stroke patients. Methods58 patients with stroke in our hospital during June 2008 to June 2011 were selected.According to block randomization method,the selected patients were divided into the study group and the control group with 29 patients in each group.The control group received routine care interventions,while patients in the study group was given routine care interventions plus continuity care model.Activities of daily living,compliance behavior,quality of life,nursing satisfaction degree and other related indicators of patients in two groups were comparatively analyzed. ResultsCompared with the control group,the activities of daily living of the study group at discharge did not show significant differences,while 1 month,3 months,6 months after discharge,the activities of daily living was significantly increased.Regular medication,exercise time,timely return visit,proper diet and other indicators of compliance behaviors were significantly improved.The quality of life score did not show significant differences at discharge,while 3 months after discharge,the quality of life score were significantly higher in the study group than that of the control group.The satisfaction degree of patients was significantly higher in the study group than that of the control group. ConclusionsApplication of continuity care model in stroke patients can improve activities of daily living,compliance behavior,quality of life,satisfaction degree with nurses.It's worthy of further clinical application and promotion.
2.Research progress on the relationship between sleep disorders and Alzheimer's disease

Sleep disorders are common diseases with various dys-function during sleep-wake process, including difficulty falling or staying asleep, falling asleep at inappropriate times, excessive total sleep time, or abnormal behaviors associated with sleep. Sleep disorders can lead to the deposition of amyloid beta protein ( Aβ) by affecting the normal metabolism of amyloid beta protein in the brain. Patients with Alzheimer’s disease ( AD) often suffer from sleep disorders, and its pathology always results in more se-vere sleep disorders, which leads to a risk of cognitive impair-ment and hypofunction. Sleep disorders could interact closely with AD, forming a positive feedback loop, which causes serious damage to the body health. This review summarized the current research about sleep disorder in the onset of AD and the current status of medication.
3.Association of a miRNA-137 rs1625579 polymorphism with neurocognitive function in patients with schizophrenia

Objective To investigate the aassociation of a microRNA-137 (miR-137) polymorphism, single nucleotide polymorphism (SNP) rs1625579, with neurocognitive function in patients with schizophrenia. Methods A total of 250 patients with schizophrenia were included in this study. The positive and negative syndrome scale (PANSS) was used to evaluate patients. The brief assessment of cognition in schizophrenia (BACS) scale was used to determine neurocongnitive functions in patients. Blood samples of patients were collected, and SNaPshot technique was used to compare the neurocognitive functions of different genotypes of rs1625579. Results The genotypes of TT, GT and GG were 221 (88.4%), 28 (11.2%) and 1(0.4%). There was no significant difference in PANSS score between TT genotype carriers and G allele (GG+GT) carriers. The detection of BACS showed that the digit sequencing score was significantly lower in patients with TT genotype than that of G allele (GG+GT) carrier ( P<0.05). There were no significant differences in other scores of BACS evaluation between two groups of patients. Conclusion The miR-137 polymorphism influences the working memory performance of schizophrenic patients in Chinese Han population.
4.Evaluation of the sedative and hypnotic effects of H1208.

This study is to investigate the sedative and hypnotic effects of a novel compound H1208. The sedative activity of H1208 was investigated by recording the spontaneous locomotor activity of mice. The hypnotic property was evaluated by the latency and duration of sleep (loss of righting reflex) in mice and the effect of hypnotics on sleep pattern of electroencephalogram were studied in conscious, freely moving mice with chronically implanted electrodes. The brain monoamine neurotransmitters levels in mice were measured by high performance liquid chromatography-electrochemical detection. The spontaneous locomotor activity was decreased by 56.7% and 80.2% in H1208 (5 and 25 mg x kg(-1), ip) treated mice, respectively. The loss of righting reflex was directly induced in mice after H1208 (60 mg x kg(-1), ip) administration. The non-rapid eye movement sleep increased significantly by 131% and 259%, respectively, within 3 hours after H1208 (30 and 60 mg x kg(-1), ip) administration. However, the rapid eye movement sleep decreased significantly. The contents of DA in the striatum and cortex and 5-HT in the cortex decreased significantly. These results demonstrated that H1208 has potent sedative and hypnotic effects, which may be closely related to the decreased contents of DA and 5-HT in mouse brain.

6.Oropharyngeal Bacterial Carriage in Hospitalized Elderly Patients Without Acute Infections

OBJECTIVE To investigate the oropharyngeal bacterial carriage in hospitalized elderly patients without acute infections and the antimicrobial resistance,and the risk factors for the bacterial carriage.METHODS An oropharyngeal swab was taken from each patient after they rinsed their mouths with sterile saline.Bacteria were cultured and identified with routine methods and the antimicrobial susceptibility testing was carried out with disk diffusion method.RESULTS The oropharyngeal bacterial carriage rate was 55.2% in elderly patients.Sixty two pathogens were isolated including 56(90.3%) strains of Gram-negative bacilli,5(8.1%) Gram-positive cocci and 1 Candida albicans.All of 34 strains of Haemophilus spp were susceptible to antimicrobials tested such as ampicillin and cefaclor,a few strains were resistant to gentamicin and ciprofloxacin.Of 10 strains of Klebsiella pneumoniae,4 strains were extended-spectrum ?-lactamases positive and resistant to cefotaxime.Logistic analysis indicated that denture-wearing was the risk factor for the oropharyngeal bacterial carriage in elderly.CONCLUSIONS Oropharyngeal bacterial carriage rate is high in hospitalized elderly patients.The major colonized bacteria are Gram-negative bacilli.Denture-wearing is the risk factor for the oropharyngeal bacterial carriage in elderly.
7.Characteristic and clinical significance of DNA methyltransferase 3B overexpression in endometrial carcinoma

Objective:To determine the clinicopathological significance of the DNA methyltransferase 3B (DNMT3B)overexpression in endometrial carcinomas and to evaluate its correlation with hormone re-ceptor status.Methods:Immunohistochemistry was performed to assess the expression of DNMT3B and hormone receptors in 104 endometrial carcinomas.Results:DNMT3B overexpression occurred frequently in endometrioid carcinoma (EC,54.8%)more than in nonendometrioid carcinoma (NEC,30.0%) with statistical significance (P =0.028).Furthermore,there was a trend that EC with worse clinico-pathological variables and shorter survival had a higher DNMT3B expression,and the correlation between DNMT3B and tumor grade reached statistical significance (P =0.019).A negative correlation between DNMT3B and estrogen receptor (ER)or progesterone receptor (PR)expression was found in EC. NMT3B overexpression occurred frequently in the ER or PR negative subgroups (78.9%,86.7%)more than in the positive subgroups (47.7%,47.8%)with statistical significance (P =0.016,P =0.006). In addition,the DNMT3B overexpression increased in tumors with both ER and PR negative expression (92.9%,P =0.002).However,no such correlation was found in NEC (P >0.05).Sequence analyses demonstrated multiple ER and PR binding sites in the promoter regions of DNMT3B gene.Conclusion:This study showed that the expression of DNMT3B in EC and NEC was different.DNMT3B overexpres-sion in EC was associated with the worse clinicopathological variables and might have predictive value. The methylation status of EC and NEC maybe different.In addition,in EC,DNMT3B overexpression negatively correlated with ER or PR expression.In NEC,the correlation between DNMT3B and ER or PR status was not present.
8.Clinical and molecular characteristics of invasive community-acquired methicillin-resistant Staphylococcus aureus infection in Chinese neonates

Objective To analyze the clinical and molecular features of community-acquired methicillin-resistant Staphylococcus aureus (CA-MRSA) infection in neonates and to investigate their antibiotic resistance profiles.Methods A total of 35 invasive CA-MRSA strains were collected from six hospitals in 2014.Multilocus sequence typing (MLST), staphylococcal cassette chromosome mec (SCCmec) typing and spa typing were used to analyze these isolated CA-MRSA strains.In vitro antibiotic susceptibilities of those strains to 15 antibiotics were analyzed by using agar dilution method.Results Up to 88.6% patients were late-onset infection and septicemia (24, 68.5%) was the most common infection among the 35 cases.A total of 16 patients (45.7%) suffered from complications.Caesarean section and premature birth were risk factors for invasive CA-MRSA infection.ST59-MRSA-SCCmecⅣa-t437 (14, 40%) was the most predominant CA-MRSA clone, followed by ST59-MRSA-SCCmecⅤ-t437 (13, 37.1%).The incidence of severe complications caused by ST59-MRSA-SCCmecⅤ-t437 was higher than that caused by ST59-MRSA-SCCmecⅣa-t437 (P<0.05).Up to 85.7% of the isolated CA-MRSA strains were multidrug-resistant strains.Conclusion This study shows that neonatal invasive CA-MRSA infections mainly result in septicemia and are often accompanied by complications and involve multiple organs.Multidrug-resistant CA-MRSA strains are prevalent in neonates.ST59-MRSA-SCCmecⅣa-t437 is the predominant clone causing neonatal invasive CA-MRSA infection.
9.Design, synthesis and pharmacological investigation of isoindoline derivatives as 5-HT/NE double reuptake inhibitors.

A series of isoindoline derivatives were designed, synthesized, and evaluated for their double inhibitory activities. All of them were new compounds, and their structures were confirmed by 1H NMR and HR-MS. Preliminary in vitro pharmacological tests showed that all compounds exhibited 5-HT or NE reuptake inhibition activity. Among the tested compounds, compound I-3 exhibited potent inhibitory activity against 5-HT and NE reuptake in vitro, and exhibited potent antidepressant activity in vivo. These compounds designed can be further optimized for finding more potent 5-HT/NE dual reuptake inhibitors and antidepressant candidates as well.
10.Clinicopathological screening of Lynch syndrome:a report of 2 cases and literature review

Lynch syndrome is an autosomal dominant genetic disease characterized by the early onset of colon cancer, endometrial cancer and other tumors caused by a genetic mutation within DNA mismatch repair (MMR) genes.A small subgroup (approximately 3% -5%) of endometrial cancer and colorectal cancer is related to Lynch syndrome .Identification of these patients in clinical practice will be of great benefit to the relatives and patients themselves .We reported two cases, and reviewed the literature and clinical diagnostic guideline.MMR protein was lost in the tumors.Meanwhile the two cases had different clinicopathological characteristics.Together with the literature, our findings may suggest that the MMR protein expression, associated molecular alterations and clinicopathological features and biological behavior of endometrial cancer and colorectal cancer related to Lynch syndrome are different .Thus the algorithm for detection the patients at highest risk is different .To detect the MMR loss by immunohisto-chemistry is a practicalscreening method.
