1.Epidemiological survey of TCM syndromes distribution related to kidney deficiency in 612 patients with DUB

Objective:To investigate the syndromes distribution in patients with dysfunctional uterine bleeding(DUB) related to kidney deficiency, and to provide the basis for standardizing the DUB syndrome differentiation of kidney deficiency. Methods: DUB Patients with kidney deficiency were chosen and accepted epidemical survey through the questionnaire, then the frequency and constituent ratio of each syndrome were calculated. Results: In 612 cases of DUB patients with kidney deficiency, there were 102 patients with simple syndromes covered 16.67%, and 510 patients with compound syndromes covered 83.33%. In patients with simple syndromes, the common syndromes were kidney-yin deficiency(45,44.12%) and kidney-qi deficiency(45,41.18%). And the syndromes of kidney-qi deficiency and kidney-yin deficiency were often respctively complicated with liver stagnation(68,11.09%; 113,18.43%), blood defi ciency(57,9.30%; 99,16.15%) and blood stasis(57,9.30%; 99,16.15%), the syndrome of kidney-yang deficiency usually complicated with spleen-qi deficiency(75,12.23%). The syndrome of kidney-qi deficiency(186,57.23%) was commonly-seen in the patients with ovular DUB, while the syndromes of kidney-qi(146,50.69%) and kidney-yin deficiency(111,38.54%) were frequently-seen in the patients with anovular DUB. Conclusion: In clinic, the compound syndromes were commonly seen in patients with DUB due to kidney deficiency, the complicated syndromes of kidney deficiency were liver stagnation, spleen-qi deficiency, blood deficiency and blood stasis..
2.Analysis of the routine blood counts and blood biochemical parameters and hemorheology in the rats with congenital cataract

Objective To measure the range of routine blood counts and blood biochemical parameters ,to analyze hemorheology of the rats with congenital cataract .Methods Blood samples were taken from 90 rats with congenital cataract weight about 185 ~211 g.Routine blood analysis was performed and blood biochemical and hemorheology paramenters were determind using an automatic blood biochemical and hemorheology analyzer .Results There were no significant difference ( P >0.05 ) between the cataract rats and the normal rats in Blood test results; but there were significant difference between the microphthalmos cataract rats and the normal same -sex rats ( P <0.01 or P <0.05 ) . The biochemical results is the cataract rats and the normal rats were different significantly in ALB group ( P <0.01 or P<0.05), and the female microphthalmos cataract rats compared with the control rats had significant difference in Ure (P<0.01) , the female cataract rats ompared with the normal rats were very significant difference in Cr group ( P <0.05 or P <0.01).The erythrocyte counts of the male cataract rats and male microphthalmos cataract rats were significantly lower than that in the female ones, respectively(P <0.05, P <0.01).The platelet counts of the male cataract rats and the male microphthalmos cataract rats were significantly higher than that in the female ones , respectively(P <0.01), and the creatinine of the male cataract rats and the male microphthalmos cataract rats were significantly lower then that in the female ones, respectively(P <0.01).There were no significant difference in every group on hemorheology .Conclusions There were significant differences in some blood indexes between the congenital cataract rats and the normal rats .These data may become useful reference for biomedical researcher in this field .
3.Development of a computer-aided-controlling and image analysis system for light/dark box test in mice and rats

Objective To develop a computer-aided-controlling and analysis system for light/dark box in mice and rats with a high degree of automation and intelligence.Methods Video recording and image processing were applied to develop the computer-aided-controlling and image analysis system for light/dark box test in mice and rats. The artificial environment was developed. The stability and reliability of the system was validated by male rats. Results The percentage of time spent in the lit chamber in total time was above 79.40%. The data showed that the artificial environment was successful. When the threshold was set at 18 cm/s, the data showed a high correlation coefficient of movement time between the computer and manual recordings(r > 0.99). Classical indexes including transition and time spent in both the lit and dark chambers also showed a high correlation. The model group showed a significantly decrease in the transitions and time spent in the lit chamber compared with the control group, indicating a high stability and reliability of the light/dark box test. Conclusions A stable and highly intelligent computer-aided-controlling and image analysis system for light/dark box test of mice and rats has been developed,and it could be used for pathological mechanism studies of anxiolytics.
4.A case of nasopharyngeal carcinoma with autonomic nervous dysfunction and literature review

Objective:A case of advanced nasopharyngeal carcinoma with autonomic dysfunction was reported and its pathophysiological mechanism was discussed.Methods:The diagnosis and treatment of a nasopharyngeal carcinoma patient with autonomic nervous dysfunction such as paroxysmal syncope was summarized, and the pathophysiological mechanism of this case was analyzed by searching related literature.Results:Nasopharyngeal carcinoma characterized by autonomic dysfunction was rare and had a poor prognosis. Autonomic dysfunction caused by nasopharyngeal carcinoma was associated with carotid sinus syndrome, parapharyngeal space syncope syndrome, glossopharyngeal nerve reflex and paraneoplastic neuropathy.Conclusions:Early detection and treatment is a key factor affecting the prognosis of nasopharyngeal carcinoma. Clinicians should consider nasopharyngeal carcinoma as one of the differential diagnoses in the diagnosis and treatment of patients with autonomic nervous dysfunction combined with cranial nerve damage.
5.Application effect of IDEAS mode visit in patients with colon polyps undergoing endoscopic mucosal resection

Objective:To explore the application effect of IDEAS model visit in patients with colon polyps undergoing endoscopic mucosal resection (EMR) .Methods:Using the convenient sampling method, a total of 80 patients with colon polyps who underwent EMR in Affiliated Hospital of Jiangnan University from June 2020 to June 2021 were selected as the research subjects. According to the random number table method, they were divided into the control group and the observation group, with 40 cases in each group. The control group was given routine preoperative visit, while the observation group was given IDEAS mode visit on the basis of the control group. Physiological indicators and Amsterdam Preoperative Anxiety and Information Scale (APAIS) scores were compared between the two groups.Results:After the intervention, the pulse rate, systolic blood pressure, and diastolic blood pressure of the two groups were lower than those before the intervention, and the pulse rate, systolic blood pressure and diastolic blood pressure of the observation group were lower than those of the control group, and the differences were statistically significant ( P<0.05) . The APAIS scores of the two groups were lower than those before the intervention and the APAIS scores of the observation group were lower than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Application of IDEAS model visit in EMR patients with colon polyps can reduce the preoperative anxiety level and improve the adverse physiological indicators of patients.
6.Prediction model of platelet transfusion refractoriness in patients with hematological disorders

Objective To explore the risk factors for platelet transfusion refractoriness(PTR)in patients with hemato-logical disorders,construct a prediction model and validate the model efficacy.Methods Patients with hematological disor-ders who received platelet transfusion therapy in the Chengdu Second People's Hospital from December 2021 to December 2022 were retrospectively included to judge the effectiveness of platelet transfusion and screened for risk factors by univariate and multivariate logistic regression.A prediction model for PTR was constructed using receiver operating characteristic(ROC)curve,calibration curve and decision curve(DCA)to assess the differentiation,calibration and clinical value of the model,respectively.Results A total of 334 hematological patients were included,including 168 males and 176 females,with a PTR incidence of 40.4%.Univariate and multivariate logistic regression analysis showed that platelet transfusion vol-ume,erythrocyte transfusion volume,and neutrophil ratio were risk factors for PTR(P<0.05).A prediction model for PTR in hematological patients was established based on these risk factors.The area under the model's curve was 0.8377(95%CI:0.723-0.772),the sensitivity was 58.52%,and the specificity was 89.95%.The calibration curve showed that the S∶P was 0.964,the maximum absolute difference Emax was 0.032,and the average absolute difference Eavg was 0.009.The DCA a-nalysis showed that the model had clinical application value when the risk threshold ranged from 0.2 to 0.9.Conclusion The PTR prediction model based on platelet transfusion volume,erythrocyte transfusion volume and neutrophil ratio can pro-vide a basis for effective platelet transfusion in hematological patients.
7.Efficacy of apheresis platelet transfusion in 310 patients with haematological diseases

【Objective】 To observe the effect of platelet transfusion in inpatients with haematological diseases, analyze the possible causes of platelet transfusion refractoriness (PTR), in order to further improve the efficacy of platelet transfusion. 【Methods】 A total of 310 patients with blood disease in our hospital from August 2020 to November 2021 who received platelet transfusion were retrospectively analyzed. Possible influencing factors of platelet transfusion, including gender, age, platelet preservation time, number of platelet transfusions, complication and red blood cell product transfusion were analyzed. 【Results】 Patients were divided into effective group and refractory group according to percentage platelet recovery (PPR) and corrected count increment (CCI). PTR was defined as PPR <20% or CCI <5 000 after two consecutive transfusions in 24 h or clinical bleeding symptoms or tendency not significantly controlled. Statistical differences were noticed between the two groups in terms of gender, pretransfusion white blood cell count, anemia, and whether antibiotics were used (P<0.05). The type of disease, gender, anemia and number of comorbidities were associated with PTR. The incidence of PTR was the highest in patients with myelodysplastic syndrome, and the incidence of PTR was higher in men than in women. Transfusion units of suspended red blood cells and the number of comorbidities were negatively correlated with the transfusion efficacy (P<0.05). 【Conclusion】 Possible influencing factors of platelet transfusion included the level of white blood cells before transfusion, use of antibiotics, anemia and transfusion of red blood cells, number of comorbidities, and type of disease, while no significant differences were found in age, hemolysis, hypersplenism, platelet preservation time, and number of platelet transfusions on transfusion efficacy.
8.Application effectiveness of PDCA in emergency blood management

【Objective】 To explore the effectiveness of PDCA (Plan-Do-Check-Act Cycle Management) in clinical emergency blood management. 【Methods】 The data of emergency blood-using cases from January 2021 to June 2022 in each clinical department of our hospital were collected to observe the blood matching time, blood retrieving time, and emergency bloodusing rate. They were divided into PDCA experimental group (Experimental group, July to December 2021, n=287), pre-PDCA experimental group (Control group 1, January to June 2021, n=516) and post-PDCA experimental cessation group (Control group 2, January to June 2022, n=277). Subgroup analysis was performed according to different departments, which were Internal Medicine Department, Surgery Depatment, and ICU. The situation of non-emergency blood use occupying emergency lanes in the pre-implementation period was continuously improved using PDCA, and the differences in blood matching time, blood retrieving time, and emergency blood-using rate among the three groups were compared and analyzed by Kruskal-Wallis test and chi-square test. 【Results】 The blood matching time and blood retrieving time (M, min) in the experimental group, control group 1 and control group 2 were 19.00 vs 45.50 vs 23.00 and 22.00 vs 44.00 vs 25.00, respectively (P< 0.05), and were 19.00 vs 47.00 vs 24.00 and 23.00 vs 56.00 vs 30. 50 in Internal Medicine Department, 18.00 vs 57.50 vs 14.00 and 32.00 vs 41.00 vs 24.00 in Surgery Department, 20.00 vs 42.00 vs 23.00 and 16.50 vs 34.00 vs 12.50 in ICU (P<0.05). The rate of emergency blood use in the experimental group, control group 1, and control group 2 were 6.9%(287/4 141) vs 11.0%(516/4 689) vs 6.8%(277/4 089), respectively (P< 0.05), and were 6.3%(175/2 769) vs 11.8% (297/2 512) vs 6.7% (186/2 789) in Internal Medicine Department, 5.9%(24/405) vs 3.6 %(44/1 213) vs 7.4% (37/501) in Surgery Department, and 9.1% (88/967) vs 18% (175/973) vs 6.8%(54/799) in ICU (P<0.05). 【Conclusion】 The adoption of PDCA in Blood Transfusion Department can effectively shorten the blood matching time and blood retrieving time for clinical emergencies and improve the success rate of emergency blood transfusion.
