1.Impairment of left ventricular function in patients with mild-to-moderate chronic obstructive pulmonary disease

Objective To determine whether the extent of airflow obstruction is associated with left ventricular function in mild-to-moderate chronic obstructive pulmonary disease (COPD) patients.Methods Left ventricle end diastolic volume ( LVEDV ),left ventricle end systolic volume ( LVESV ),left ventricle stroke volume( LVSV),left ventricle ejection fraction( LVEF),heart rate ( HR),cardiac output ( CO) and cardiac index (CI) were measured by ultrasonic cardiogram.Thirty-one patients with chronic bronchitis,42 mild-to-moderate COPD patients and 16 controls with normal lung function were recruited.The relations between the extent of airflow obstruction and the impairment of left ventricular function were analyzed.Results There were no significant differences of LVEDV,LVESV,LVSV,HR,CO or CI between the control and chronic bronchitis groups.LVEDV,LVESV,LVSV,CO and CI of chronic bronchitis group were significantly higher than those of mild-to-moderate COPD group while HR was lower.LVEDV,LVESV,LVSV,CO and CI had a positive correlation with forced expiratory volume in 1 second ( FEV1 ),forced vital capacity (FVC) and FEV1/FVC ratio.And HR had a negative correlation with FEV1 and FEV1/FVC.LVEF was positively correlated with FVC,but not with FEV1 and FEV1/FVC.LVEDV,LVESV,LVSV,HR,CO and CI were linearly related with FEV1.Conclusions Left ventricular function is maintained in chronic bronchitis patients. Left ventricular function,especially left ventricular end diastolic filling,deteriorates among the mild-to-moderate COPD patients.The extent of airflow obstruction may reflect the impairment of left ventricular function in COPD patients.
2.Detect of P53,bax and caspase 3 genes expression after optic nerve injury in rats with SYBR green I fluorescence quantitative PCR

Objective Previous study showed that the histopathological basis of visual function damage caused by optical nerve injury is apoptosis of retinal ganglion cells(RGCs). This procedure is regulated by P53, bax and caspase 3 genes. Present study aimed to observe the expression of bax, P53 and caspase 3 mRNA in RGCs after traumatic optic nerve damage in the rats by SYBR green I fluorescence quantitative PCR method. Methods The animal model of optic nerve injury was established in the right eyes of 56 adult Wistar rats by a fluid percussion brain injury device (FPI) . Animal were killed on days 1, 3, 5, 7, 9, 14, 28 days separately after injury. Other 16 Wistar rats were divided into normal control group and sham operation group. The total RNA was isolated from rat fresh retina tissue by Trizol method and was treated by reverse transcription to cDNA using 01igo(dt) 18 as primer and then amplified. The target fragments of bax, P53 and caspase 3 cDNA were linked with carrier pTZ57 R/T to construct recombined plasmids which were transformated to E. Coli DH5α by T/A clone method. Recombined plasmids were extracted with alkaline lysis method and the plasmids were selected in white colonies by ampicillin screening, EcoR I restrictive enzyme analysis, and their specificity was evaluated using DNA sequencing. The standard curves were created by plasmid DNA and the precise expression level of target genes in samples were determined using software. The results were expressed as the ratios of target gene mRNA to GAPDH mRNA. Results The standard curve drawn by pTZ57R/T and target gene presented a good linear tendency with the higher sensitivity and specificity. The expression of P53 and bax mRNA began to increase on the third day after the injury of optic nerve and peaked on the fifth day and started to decline on the seventh day. The expression of caspase 3 mRNA increased from the fifth day through the ninth days after injury and declined on the fourteenth day. The significant differences were found in the expression of P53, bax and caspase 3 between model group and control group (P < 0. 05) . Conclusion The pro-apoptotic protein P53, bax and caspase 3 play an important role in RGCs apoptosis.
3.Clinical effect of recombinant human growth hormone for interventing central precocious puberty in children

Objective To investigate clinical effect of recombinant human growth hormone (rhGH)for inter-venting central precocious puberty in children,to provide reference for clinical treatment.Methods 80 patients with growth deceleration slows CCP were selected,when height growth rate below 4cm/year,40 patients on the basis of joint GnRHa used rhGH treatment were selected as the observation group,and the remaining 40 patients refused to use rhGH,continued to use of GnRHa treatment were selected as the control group.After treatment predicted height,height,bone age,growth rate and other indicators were compared.Results In observation group,the actual height Ht, predicted height PAH,the growth rate Gv,serum insulin -like growth factor -1 (IGF -1 )after treatment were (1 44.48 ±6.59)cm,(1 54.94 ±4.52)cm,(4.43 ±0.64)cm/6months,(1 32.25 ±8.84)ng/mL,compared to before treatment,the differences were statistically significant (t =6.548,P <0.01 ;t =5.734,P <0.01 ;t =28.869,P <0.01 ;t =20.65,P <0.01 ),compared with the control group,the differences were significant (t =3.943,P <0.01 ;t =4.759,P <0.01 ;t =28.247,P <0.01 ;t =20.882,P <0.01 ),there were no differences in other indices;hor-mones FSH,LH and other indicators of the two groups before and after treatment,and blood sugar,thyroid indicators showed no abnormality.Conclusion Recombinant human growth hormone in the treatment of central precocious puberty deceleration in children does not increase bone age,which will help improve the growth rate and predicted height,which worthy of clinical application.
4.Change of bacteria and enzymes in the drainage fluid in patients with intestinal fistulas

ObjectiveTo investigate the amount of bacteria and the expression of amylase and lipases in the drainage fluid in patients with intestinal fistulas with time courses.MethodsThe samples were collected from 16 patients with high intestinal fistulas from July 1998 to January 2008.The amounts of bacteria from the drainage fluid were measured 0,2 and 4 hours after taking out from the patients.At the respective time points,the intestinal juices were also collected to measure the amylase and lipase expressions.After reinfusion of succus entericus,thelevels of albumin,prealbumin,transferring,and fibronectin were measured at 0,7,and 14 days,ResultsThere was no significant increase of bacteria in the drainage fluid within 4 hours ( F(0,2) =18 812.50,P > 0.05 ; F(0,4) =387 625.00,P > 0.05).and there was no change in the expressions of amylase ( F(0,2) =190.60,P > 0.05 ;F(0,4) =631.75,P>0.05) and lipase within 4 hours (F(0,2) =204.10,P>0.05; F(0,4) =1080.05,P>0.05).After succus entericus reinfusion,the fibronectin (F(0,14) =74.24,P < 0.01 ; F(7,14) =59.78,P <0.01),transferring (F(0,14) =0.46,P < 0.01 ; F(7,14) =0.39,P < 0.05 ),and prealbumin ( F(0,14) =54.37,P < 0.05) were increased significantly.ConclusionsBacteria and enzymes do not increase in the drainage fluid within 4 hours in patients with intestinal fistulas.Therefore,it is safe and effective to reinfuse succus entericus.
5.Effects of succus entericus reinfusion with continuous enteral nutrition on the barrier function of intestinal mucosa in patients with stomal type fistulas

ObjectiveTo observe the effect of succus entericus reinfusion with continuous enteral nutrition on the barrier function of intestinal mucosa and nutritional status in patients with stomal type fistulas. Methods Sixteen patients with stomal type fistula from July 1995 to May 2008 were enrolled in the study. A]l patients met the following conditions: gut function returned normal; abdominal infection was controlled; total enteral nutrition was provided ; and the length of small intestine for succus entericus reinfusion was more than 50 cm. Intestinal mucosa was taken at 25 to 30 cm away from stoma of fistula by endoscope 0, 7, and 14 days after reinfusior. Hematoxylineosin staining was performed to count the number of intestinal intraepithelial lymphocytes (IIELS). In addition,proliferating cell nuclear antigen (PCNA) was measured with immunohistochemical staining. Serum protein levels were determined by immunonephelometry. ResultsThe percentage of IIELS in intestinal mucosa ( 19.06% ±4.81% vs. 12.81% ±2.95%, P=0.000) and the percentage of PCNA positive cells ( 12.13% ±4.33% vs.6.44% ± 2.34%, P =0.000) 14 days after succus entericus reinfusion were significantly higher than those on the day of reinfusion. Serum fibronectin level increased from ( 152.80 ± 16.50 ) to ( 227.05 ± 45.36 ) mg/L ( P =0.000), and transferring protein level increased from ( 2.16 ± 0.52 ) to ( 2.62 ± 0.41 ) g/L ( P =0.017 ) 14days after succus entericus reinfusion. ConclusionSuccus entericus reinfusion is effective in protecting the intestinal mucosa in patients with stomal type fistulas.
6.Antimicrobial-resistant Profile of Staphylococcus aureus from Different Origins

OBJECTIVE To investigate the antimicrobial-resistance of Staphylococcus aureus isolates which obtained from different origins.METHODS The isolates of S.aureus were derived from clinical samples by routine methods.The identification and susceptibility test of the isolates were determined by Automated Microbiology Analyzer.Disk-diffusion test(K-B method) was used for detecting meticillin-resistant S.aureus(MRSA).?2 Test was made to identify the significance of difference.RESULTS A total of 248 S.aureus strains were isolated from blood(25),medical internal catheter(27),pus(89),sputum(93),and urine(14).All isolates of S.aureus were sensitive to nitrofurantoin and vancomycin.The resistant rates of S.aureus isolates from pus to chloramphenicol,gentamicin,oxacillin,penicillin,rifampicin,tetracycline and levofloxacin all significantly lower than those of isolates from sputum(P
7.Drug Resistance of Aminoglycoside-resistant Klebsiella pneumoniae

OBJECTIVE To investigate the antimicrobial-resistant profile of aminoglycosides-resistant Klebsiella pneumoniae isolated from Wenzhou.METHODS Between Jan 2006 and Sep 2007,non-replicate clinical isolates of K.pneumoniae were consecutively collected from inpatients in a teaching hospital in Wenzhou,China.Antibiotic susceptibility test was determined by the disc diffusion method recommended by CLSI.Sixty-four gentamicin-resistant isolates were investigated for the presence of ESBLs by the CLSI-recommended confirmatory tests.RESULTS Of the 64 gentamicin-resistant isolates,the resistant rates to ciprofloxacin,levofloxacin cefoxitin,tetracycline,trimethoprim/sulfamethoxazole,ceftazidime and cefotaxime were 7.5%,29.7%,39.1%,85.9%,76.6%,67.2%,and 73.4%,respectively.All isolates were susceptible to imipenem.73.4% of gentamicin-resistant isolates produced ESBLs.CONCLUSIONS Gentamicin-resistant K.pneumoniae is multi-resistant to antibiotics.
8.Effectiveness and safety of bronchoscope combined with cryotherapeatic equipment in treatment of bronchial foreign body in children

Objective To explore the effectiveness and safety of fiberbronchoscope combined with cryotherapeatic equipment in treatment of bronchial foreign body in children.Methods 72 children with bronchial foreign body received fiberbronchoscope combined with cryotherapeatic equipment, then the complications were observed during and after the operation.Results Bronchial foreign bodies were successfully removed in all the patients. Among the 72 cases, transient hypoxia was the most common complication during the operation which happened in 37 cases and most occurred in the small age group, and decreased with the age increasing; cough or worsening of cough was the most common complication after the operation which happened in 20 cases; No serious complications were found during the follow-up.Conclusion Although it has some complications when bronchial foreign body received the fiberbronchoscope combined with cryotherapeatic equipment, but the operation time is short, and the complication is mild. It has great application value and security.
9.Effect of Percutaneous Autogenous Bone Marrow Stem Cell Transplant on Bone Nonunion

Objective To investigate the clinical effect of percutaneous autogenous bone marrow stem cell transplant on bone nonunion. Methods 140 patient with bones nonunion were divided into the autogenous bone transplants (A) group and the autogenous bone marrow stem cell transplant (B) group, with 70 cases in each group. They were assessed 2 and 5 months after operation. Results A few calluses could be observed at the end of fracture in 42 cases in group A 2 months after operation, and massive callus formation could be observed in 46 cases 5 months after operation. However, massive callus formation could be observed in 60 cases in group B 2 months after operation, and continual callus formation could be observed in 45 cases 5 months after operation. The mean healing time was (8.4±1.8) months in group A, and (6.5±2.0) months in group B (P<0.05). No side-effects have been observed during the treatment. Conclusion The percutaneous autogenous bone marrow stem cell transplant is more effective on bone nonunion compared with the traditional treatment.
10.Establishment of animal model of traumatic optic neuropathy and its function evaluation by F-VEP

Background Visual electrophysiology is a sensitive index for the evaluation of visual function.It has an important value in the assessment of traumatic optic neuropathy.Rabbit is an ideal animal model of traumatic optic neuropathy,and it is simple for the record of flash visual evoked potential(F-VEP)in rabbits.ObjectiveThe present study is to establish the animal model of traumatic optic neuropathy with or without lens injury and observe the repairing procedure using F-VEP. MethodsModels of traumatic optic neuropathy associated with lens injury were established in the right eyes and only traumatic optic neuropathy were created in the left eyes of 64 healthy SPF Chinese white rabbits using fluid percussion brain injury device(FPI).F-VEP was recorded based on the Proposal of International Visual Electrophysiology on 1,2,4,7,10,14,21,28 days after injury of optic nerves.Experimental animals were sacrificed in above time points for the histopathological examination.Macrophages were labeled by ED-1 antibody and survival retinal ganglion cells (RGCs)were stained by Nissl method.Results At the first day after injury,the latencies of P_(100) in both group were longer,and the amplitudes of P_(100) in both group were lower than before injury,showing statistically significant differences among different time points(P<0.05),but no significant difference was seen between the two groups(P>0.05).The duration of latency in traumatic optic neuropathy associated with lens injury group was shorter than that in only traumatic optic neuropathy group(P<0.05).The restore of latency in traumatic optic neuropathy associated with lens injury group was much faster than that in only traumatic optic neuropathy group(P<0.05).The numbers of macrophages were significantly increased and numbers of survival RGCs were considerably decreased with lapse of injury time (P<0.05).The abnormalities of VEP P_(100) and RGCs were obviously improved in 28 days after injury in both groups. ConclusionThis animal model can be established successfully by FPI.The result of retinal histopathological examination confirms F-VEP findings in this model.
