1.A meta-analysis of countious femoral nerve block versus continuous epidural analgesia after total knee arthroplasty

BACKGROUND:Pain is the significant cause for patients with early rehabilitation after total knee arthroplasty. Continuous epidural analgesia and continuous femoral nerve block are effective analgesic methods after total knee arthroplasty, however, which method has better effects and less complications remains controversial. OBJECTIVE:To compare the efficacy and safety of countious femoral nerve block and continuous epidural analgesia after total knee arthroplasty. METHODS:We searched Cochrane Library, PubMed, EMBASE, Web of Science, CBM, CNKI, VIP, and WangFang. Meanwhile, we also searched conference papers and academic dissertation. The retrieval time was from database establishment to October 1, 2014. Studies of randomized control ed trials on countious femoral nerve block and continuous epidural analgesia after total knee arthroplasty were included. We evaluated the quality of these included studies and analyzed data by Cochrane Col aboration’s RevMan 5.0 software. RESULTS AND CONCLUSION:A total of 12 randomized control ed trials (4 English articles and 8 Chinese articles) involving 680 patients were included. There were 343 patients with countious femoral nerve block and 337 patients with continuous epidural analgesia. Meta-analysis results revealed that no significant differences in visual analog scale scores were detected between the countious femoral nerve block and continuous epidural analgesia groups at 6, 12, 24 and 48 hours after total knee arthroplasty. However, compared with the continuous epidural analgesia group, countious femoral nerve block could decrease the incidences of nausea/vomiting (RR=0.36, 95%CI:0.21-0.63, P=0.003), urine retention (RR=0.08, 95%CI:0.04-0.16, P<0.001) and dizziness (RR=0.24, 95%CI:0.06-0.99, P=0.05). These results indicate that compared with epidural analgesia, countious femoral nerve block after total knee arthroplasty provided a strong analgesia effect, contributed to early functional training, had less adverse reactions, and was a safe and effective analgesic method.

4. Effect of social psychological factors on the mental health among enterprise workers using structural equation modeling

OBJECTIVE: Structural equation modeling was used to explore the relationship between mental health and social psychological factors. METHODS: Using multi-stage sampling method,1 200 first-line production workers working in enterprises were chosen as study subjects. Chinese version of the WHO-5 well-being Index,and Social Psychological Factors Questionnaire(a self-designed questionnaire based on job demands control model) were used to investigate the mental health and social psychological factors. The structural equation modeling of social psychological factors on mental health was constructed. RESULTS: Among the 1 200 workers,about 34. 8% workers had poor psychological well-being(the score of the WHO-5 well-being Index is equal or less than 13). Job demands and job autonomy were available and have direct effect on work-related pressure [standardized path coefficient(β) were 0. 162 and-0. 186,P < 0. 05]. Job demands,job autonomy and work-related pressure were available have direct effect on mental health(β were-0. 136,0. 235 and-0. 135,P < 0. 05). Job demands can have an indirect effect on mental health through work-related pressure(β =-0. 022,P < 0. 05),and job autonomy can have an indirect effect on mental health through work-related pressure(β = 0. 025,P < 0. 05). CONCLUSION: The results of mental health and social psychological factors are consistent with the job demand control model. Job demands and work-related pressure are risk factors and job autonomy is a protective factor for mental health. Mental health intervention should be taken according to the related factors.
5.Driver gene status and first line treatment of advanced lung adenocarcinoma:A single centric real-world study

Objective To analyze driver genes status and its clinical characteristics of advanced lung adenocarcinoma, then evaluate the status of first-line treatment in a single centric real-world. Methods EGFR, ALK, ROS-1 gene in 204 advanced lung adenocarcinoma tissue were tested by ARMS-PCR method. And the relationship between driver genes status and clinical characteristics was analyzed as the first line treatment in real clinical practice. Results The positive rate of driver genes status in 204 advanced lung adenocarcinoma was 53.9% (110/204) , including EGFR mutation 46.1% (94/204) , ALK positive 6.4% (13/204) and ROS1 positive 1.5% (3/204). The driving genes status was significantly correlated with gender, smoking history, tumor staging and serosal invasion (P < 0.05). There were significantly differences among the proportion of first-line standard treatment in different subgroup (P = 0.000) , the first-line standard treatment rate of EGFR mutation, ALK/ROS1 positive and drive gene negative were 77.7%, 37.5%, and 46.8% respectively. And the ratio of using 1 st generation EGFR-TKIs in all patients is 70.6% (60/85). Conclusion More than half of advanced lung adenocarcinoma have driver genes changes, and EGFR-TKI first-line treatment has higher acceptability in real-word.
6.Analysis of trends and hot spots in the research of atrial fibrillation and cognitive impairment

Objective:To explore the research trends and hot spots of atrial fibrillation and cognitive impairment.Methods:Research papers and review articles related to atrial fibrillation and cognitive impairment was retrieved from the Web of Science core collection database from inception to November 17, 2021. The bibliometric and visualization analysis on the publishing times,journals and countries,related disciplines and key words was conducted with Excel and VOSviewer software.Results:A total of 1 496 articles were collected,showing a parabolic rise in the number of published articles in recent years. During this period,journal Stroke published more high quality articles in the field than others. The academic areas with the most publications were neurology, cardiovascular research, peripheral vascular disease and geriatrics. "Stroke" and "risk factors" were the keywords with the highest frequency. The research hot spots of atrial fibrillation and cognitive impairment mainly focused on anticoagulant drugs, mechanism research, epidemiology catheter ablation and status quo investigation. Conclusion:Through visualization, the development trend and research hot spots on atrial fibrillation and cognitive dysfunction are displayed intuitively, to provide reference for future studies.
7.Effects of different health education methods on patients with enterostomy: a network Meta-analysis

Objective:To explore the effects of different health education methods on patients with enterostomy using a network Meta-analysis, so as to provide a theoretical basis for patients with enterostomy to choose the health education method.Methods:Randomized controlled trials (RCTs) on the effects of different health education methods in patients with enterostomy were electronically searched in PubMed, Web of Science, Embase, Cochrane Library, Medline, CINAHL, China National Knowledge Infrastructure, China Biology Medicine disc, VIP, and WanFang Data, supplemented by other manual search methods. The search period was from database establishment to May 31, 2023. This study extracted data, conducted methodological quality evaluation, and used Stata 15.0 software for network Meta-analysis.Results:A total of 30 RCTs were included, involving 7 health education methods. The probability ranking of the network Meta-analysis showed that in terms of enhancing self-care ability, the effects were ranked from high to low as personalized education, internet platform education, nursing pathway education, visual education, mind mapping education, peer support education, and routine health education. In terms of promoting the level of stoma adaptation, the order of effects from high to low was visual education, internet platform education, peer support education, and routine health education. In terms of improving quality of life, the order of effects from high to low was internet platform education, personalized education, peer support education, and routine health education.Conclusions:Personalized education has certain advantages in enhancing the self-care ability of patients with enterostomy, visual education is advantageous in promoting the social adaptation of patients, and internet platform education has certain advantages in improving the quality of life. This study can effectively guide clinical nurses to choose appropriate health education methods based on the purpose of health education, in order to heighten the effectiveness of patient health education.
8.Status and influencing factors of positive feelings among caregivers of patients with enterostomy

Objective:To explore the status and influencing factors of positive feelings of caregivers of patients with enterostomy, so as to provide a reference for targeted interventions.Methods:From May to October 2021, convenience sampling was used to select 130 caregivers of patients with enterostomy who received follow-up visits in the Stoma Clinic of the First Affiliated Hospital of Xi'an Jiaotong University as the research subject. The survey was carried out using the General Information Questionnaire, the Positive Aspects of Caregiving (PAC) , and the Care Preparedness Scale (CPS) . A total of 130 questionnaires were distributed, and 127 valid questionnaires were recovered, with a valid recovery rate of 97.69%.Results:Among 127 caregivers of patients with enterostomy, the total score of PAC was (32.69±4.99) , and the self-affirmation dimension score was (18.39±2.84) , and the life outlook dimension score was (14.36±2.53) , and the total score of CPS was (17.76±4.36) . Multiple linear regression analysis showed that caregiver gender, education level, work status, and readiness were the influencing factors of positive feelings of caregivers of patients with enterostomy ( P<0.05) , which explained 43.7% of the total variance. Conclusions:The positive perception of caregivers of patients with enterostomy is at an upper-middle level. Nurses should formulate targeted humanistic care strategies according to the influencing factors, increase the positive feelings of caregivers, help patients adapt to the stoma as soon as possible, and then improve the quality of life of patients.
9.Analysis of Ethical Review of Clinical Study on Defecation Function of Patients with Rectal Cancer after sphincter-preserving Surgery

In order to understand the status quo of ethical review of clinical research on the defecation function of patients with rectal cancer after sphincter-preserving surgery, analyze its causes and put forward corresponding suggestions, to arouse researchers’ attention to ethical review in subsequent relevant clinical studies. The ethical review of literatures related to the defecation function of patients with rectal cancer after sphincter-preserving surgery published on CNKI in recent 10 years was sorted out and summarized. The results showed that the ethical review of clinical research papers on defecation function of patients with rectal cancer after sphincter-preserving surgery was not optimistic. We should strengthen the ethical training of researchers, improve the ethical awareness of researchers, strictly implement the ethical norms of paper publication, strengthen the ethical requirements of manuscript contracts, perfect the ethical review system, and pay attention to the examination and supervision of informed consent, so as to promote the construction of ethical examination and approval norms of clinical research documents.
