Objective To discuss the technical points and the clinical application of single wire-guided inverted Y-shaped tracheal stent implantation under general anesthesia in treating complex tracheal diseases. Methods During the period from January 2014 to October 2014 at authors’ hospital, a total of 6 patients with complex tracheal diseases received inverted Y-shaped tracheal stent implantation. The diseases included trachea-pleural fistula(n=1), trachea-esophageal fistula(n=2) and complex tracheal stenosis(n=3). Under general anesthesia and guided by DSA, inverted Y-shaped tracheal stent implantation was carried out in all the 6 patients. The results were analyzed. Results A total of 6 Y-shaped tracheal stents were used in the 6 patients, and single wire-guided implantation technique was employed in all procedures. In one case , the right branch of the Y-shaped tracheal stent was placed in the right upper lobe bronchus by mistake , and in the remaining 5 cases the stent implantation was successfully accomplished with single manipulation. Conclusion Under general anesthesia, Y-shaped tracheal stent implantation can effectively obstruct the trachea-pleural fistula and left main bronchus-esophageal fistula, and it can also quickly and significantly relieve the complex airway stenosis located at the tracheal carina region. This treatment is safe and reliable with satisfactory short-term effect. Moreover, single wire-guided manipulation is technically simpler, easier and faster than dual wire-guided manipulation. Therefore, this technique should be recommended in the clinical practice.

Peritoneal dialysis is an important approach to the treatment of renal failure diseases such as acute renal failure, chronic renal failure and uremia. In this paper are presented the application of the multifunction squirm pump developed by authors, the use of PLC and single-chip to control harmonically, and the instituition of full automatic dialysis therapy. Man-machine interface with integrative touch liquid crystal display is adopted in the system, and Chinese interface is used to display and operate so that user can manipulate it in a guiding way. Hence, the whole process is simple and clear. At the same time, it is very easy to set dialysis parameters suited for medical treatment demands of all kinds of patients. Through squirm pump, electric valve, electronic weighing system and electrical heating system, the accurate control of input and output dialysis volumes, flow velocities, temperature, and other parameters are achieved and automatic alarm function is also implemented. Meanwhile, particular intelligent card system that can access automatically the user's treatment information is syncretized, which is convenient for doctor to make a diagnosis and give treatment or to carry out remote treatment. This system plays an important role in the improvement of peritoneal dialysis technique for renal failure treatment and clinical implementation.

Objective:To establish the fingerprint analysis method for the root of Rosa laevigata Michx from different regions by UPLC. Methods:The column was ACQUITY UPLC? Phenyl(2.1 mm × 100 mm,1.7 μm). The mobile phase consisted of methanol-water with gradient elution. The flow rate was 0. 2 ml·min-1 , the detection wavelength was 210 nm, the column temperature was 30℃, and the injection volume was 3 μl. Results:The fingerprint consisted of 15 common peaks. The range of similarity for twelve bat-ches of the root of R. laevigata Michx was 0. 489-0. 942. And the reference fingerprint of the root of R. laevigata Michx was estab-lished by UPLC. Conclusion:The fingerprint method is simple and reproducible, which can provide basis for the quality control and the medicinal resources exploration.

Objective To observe the effects oftelmisartan (TEL) on learning and memory functions of animal models of Alzheimer's disease combined with hypertension. Methods Seventy male spontaneously hypertensive rats were randomly assigned into 7 groups:control group,sham-operated group, model group, low TEL group (1 mg/[kg· d]),high TEL group (10 mg/[kg· d]),low TEL+GW9662 group (Tel 1 mg/[kg·d]+GW 1 mg/[kg·d]),and high TEL+GW9662 group (Tel 10 mg/[kg·d],GW 10 mg/[kg·d],n=10);the lateral cerebral ventricle of rats in the later 5 groups were injected with beta-amyloid peptide 1-42 (Aβ1-42);rats in the sham-operated group were given the same volume of normal saline, and those in the control group did not give any treatment. The learning and memory abilities of these rats were detected by Morris Maze test;microglia level and casepase-3 expression were assessed by immunohistochemical stainning. Results Constant-bearing navigation indicated that as compared with the model group, the high TEL+GW9662 group, control group and sham-operated group had significantly shorten escape latency on the 3rd d of experiment (P<0.05),and as compared with the model group, the low TEL group, high TEL group and high TEL+GW9662 group had significantly shorten latency (P<0.05).Spatial probe test showed that the times of crossing the platform, target quadrant residence time in the low TEL group, high TEL group and high TEL+GW9662 group were significantly larger/longer than those in the model group (P<0.05).The casepase-3 expression and microglia level in the CA1 area of model group were significantly higher than those in the control group (P<0.05);The Ibal and caspase-3 expressions in the low TEL group, high TEL group and high TEL+GW9662 group were significantly weaker than those in the model group (P<0.05).Conclusion TEL has preventive effect on cognitive decline in rat models of Alzheimer's disease complicated with hypertension, and can be restrained by GW9662,which suggests that these benefits might be partly resulted from PPAR-γ activation and intracranial infection inhibitation.
