Objective To compare the effect of entecavir and lamivudine combined with adefovir in treatment of patients with e antigen -positive chronic hepatitis B.Methods 72 patients with HBeAg positive chronic hepatitis B were randomly divided into group Ⅰ (36 cases)and group Ⅱ (36 cases).Group Ⅰ was treated by grace entecavir,group Ⅱ was treated with adefovir and lamivudine,two groups were treated for 24 months.The HBV -DNA negative rate,ALT recovery rate,HBeAg seroconversion rate,negative conversion rate of HBsAg,HBeAg and HBeAb conversion rate of the two groups were compared after 6 months,12 months and 24 months treatment.Results After treatment for 6 months,12 months and 24 months,the HBV -DNA negative rate,HBeAg negative conversion rate, HBeAg and HBeAb rate in the two groups had no statistically significant differences.After 6 months and 12 months treatment,the ALT recovery rates of group Ⅰ were 52.78%,83.33%,which were significantly higher than 27.78%, 38.89% of group Ⅱ,the differences were statistically significant(χ2 =4.677,14.961,all P <0.05 ).The total response rates of 6 months,12 months and 24 months after treatment were 47.22%,77.78%,88.89%,72.22%, 83.33% and 44.44%,respectively.There were no significant differences between the two groups(all P >0.05). Conclusion There is similar effect of entecavir and lamivudine combined with adefovir in treatment of patients with e antigen -positive chronic hepatitis B,which can inhibit HBV replication and improve liver function,and the security was good,but entecavir has better ALT normalization rate at early,the treatment should be selected according to clini-cal practice.

It’s worthy of popularization.

Objective:To design a simple, energy saving, environmental protection, safety of the use of a new type of medical lamp with infrared light disinfection.Methods: Set the controller in the middle of the upright column of lamp, the controller side is provided with a flexible hose, and the end of the flexible hose is provided with a patch, the patch is placed in the patient's side. The back side of the skin is provided with a temperature sensor, a controller and a temperature sensor and a baking lamp. The controller is provided with an alarm, when over temperature will alarm.Results: The new type of infrared light disinfection and baking lamp is reasonable structure, mobile convenience. The alarm can automatically turn off the lights and alarm when the chip temperature is too high.Conclusion: The developed lamp device has the advantages of convenient use, strong practicability and long service life. It’s very worthy of promotion.

Laparoscopic minimally invasive surgery technology is rapidly developing in the tumor treatment field. Due to the lack of space perception of the previous 2 D laparoscopic surgery, it has been unable to meet the current needs, and then 3 D laparoscopic technology emerged and got widely development. This paper summarizes the difference between the 3 D laparoscopic technique and traditional 2 D laparoscopic technology, and then analyzes the classification of 3 D signal display technology and common format. At the same time, this paper introduces the integration operating room structure with 3 d technology, signal format, and signal transmitting method. Finally, this paper prospected the application foreground for 3 D laparoscopic technology in the operating room.

Objective To study clinical features of patients with Parkinson's disease (PD)with chronic pain and its correlation with cognitive function.Methods 100 PD patients with chronic pain were selected as the observa-tion group.100 PD patients without chronic pain were selected as disease control group.90 healthy subjects with pain were selected as control group.Statistical and clinical features of PD patients with pain,and the relationship between cognitive function were made.Results 100 patients with chronic pain with PD skeletal muscle pain in 42 cases (42.0%),motility pain in 23 cases (23.0%),23 cases of nerve root pain (23.0%),9 cases of central pain (9.0%),akathisia in 3 patients (3.0%);Control pain in patients with chronic pain with part mainly in the head and neck and the waist,PD patients with chronic pain with mainly in the limbs,pain in the two groups the incidence of head and neck pain,waist pain,limb pain had statistical differences (χ2 =10.017,4.990,21.770,all P <0.05);Observation group of daily life,emotion,ability to walk,normal work,sleep,life fun scores were better than those in control group,with statistical significance (t =7.021,7.011,3.167,7.016,7.014,8.036,all P <0.05);PD with pain group and PD without pain in gender,age,course of the disease symptoms,PD with pain group and PD without pain in UPDRS score,H -Y in installment,HAMD scores,PDQL score,LED aspects comparison was statistically significant (χ2 =5.307,4.880,all P <0.05),PD with pain group smell disorders,motion sickness were higher than the proportion of the PD without pain group,with statistical significance (t =-1.689,-1.561,all P <0.05);PD patients with pain group named ability score,delayed recall below PD without pain group,the difference was statistically significant (P <0.05),PD patients with pain and PD without pain group in the MoCA scores and other project score had no statistical significance (t =-0.967,-0.272,-0.791,-0.837,-0.671,-0.658,all P >0.05).Conclusion PD associated with chronic pain is given priority to with skeletal muscle pain,PD associated with chronic pain pain is different from normal people,PD associated with chronic pain has influence on daily life, sleep quality and depression of patients bad feelings,PD associated with chronic pain are closely related to the depres-sion and cognitive dysfunction of patients.Cognitive and emotional disorders may be involved in the pathogenesis of chronic pain.

Objective:To analyze the relationship between the concentration of glucose and the impedance of human blood, and study the relationship between human blood glucose detection and human blood impedance.Methods:The system adopts the AD5933 digital frequency synthesizer (DDS) in the excitation signal is produced, imposed on the impedance under test, the ADC acquisition signal and sent to the corresponding chip DFT digital processing module, measurement results through the I2C sent to the microcontroller, again by the single-chip microcomputer and PC computer communication, the computer shows that the impedance values. Results:The instrument can realize the resistor, capacitor, inductance, glucose solution impedance measuring rapidly and accurately, through experimental verify that resistive impedance deviation from the mean is 0.04, phase deviation from the mean is 0.15°.Conclusion:The glucose concentration of aqueous solution was linearly correlated with the impedance, the correlation coefficient greater than 0.99.

Objective To investigate the clinical efficacy and the influence on T cell subsets of telbivudine in the treatment of HBeAg -positive chronic hepatitis B , guide clinical treatment .Methods 70 patients HBeAg -positive chronic hepatitis B in our hospital were randomly divided into the telbivudine group which were received telbi -vudine 600mg/d and lamivudine group which were received lamivudine 100mg/d,35 cases of each group ,both group had been treated for 48 weeks,the liver function (ALT,AST) were detected for making a statistics of ALT normaliza-tion,the serum were extracting for detecting HBV -DNA and making a statistics of HBV -DNA negative rate by PCR,the HbeAg were detected for making a statistics of HBeAg seroconversionby ECLIA ,T cell subsets ( CD3+,CD4+, CD8+,CD4+/CD8+) were detected by flow cytometry ,observed adverse events .Results The ALT,AST,HBV-DNA of the telbivudine group and lamivudine group after treatment were (36.04 ±7.62)U/L and (63.53 ±14.13)U/L, (34.26 ±8.51)U/L and (55.31 ±15.48)U/L,(2.22 ±0.73)copies/mL and (3.28 ±0.95)copies/mL,all indi-cators significantly lower than before treatment (t =24.826,18.564,7.495 and 17.413,13.331,5.442,all P<0.05),and the ALT,AST of the telbivudine group were significantly lower than the lamivudine group ( t=3.867, 3.774,3.498,all P<0.05).The ALT normalization,HBV-DNA negative rate,HBeAg seroconversion rate of the telbivudine group and lamivudine group were 97.14%and 74.29%,94.29%and 68.57%,62.86%and 37.14%, telbivudine group,the telbivudine group were significantly higher than that of lamivudine group (χ2 =7.467,7.652, 4.629,all P<0.05).The CD3+,CD4+,CD4+/CD8+of telbivudine group and lamivudine group after treatment were (66.3 ±3.8)%and (60.6 ±3.5)%,(38.8 ±5.4)% and (35.2 ±4.4)%,(1.33 ±0.16)% and (0.95 ±0.11)%,all indicaitons significantly higher than before treatment ( t =5.442,6.173,3.753 and 3.404,3.635, 3.222,all P <0.05),the telbivudine group were significantly higher than lamivudine group (t =3.473,3.207, 3.422,all P<0.05).Conclusion The telbivudine has better efficacy than the lamivudine in the treatment of HBeAg-positive chronic hepatitis B because of enhancing immune function ,can improves ALT normalization ,HBV-DNA negative rate and HBeAg seroconversion rate ,inhibit HBV replication .

AIM:To evaluate the influence of scutellarin on the expression of vascular endothelial growth factor ( VEGF) in high glucose-treated human retinal pigment epithelial cell line ARPE-19 and to observe the effects of scutellarin on the protein expression of VEGF, p-ERK and VEGFR2 in the retinas of type II diabetic rats.METHODS: Cultured ARPE-19 cells were divided into normal control group, scutellarin group, high glucose group and high glucose+scutellarin group.The protein levels of VEGF, p-ERK and VEGFR2 were measured by Western blot.The VEGF release in ARPE-19 cells was detected by ELISA.Normal rats were randomly divided into normal control group and scutellarin group.Diabetic rat model was established by feeding with high-fat diet and injecting with streptozocin, and randomly divided into diabetes group and diabetes treated with scutellarin group.After 16 weeks, the eyes were removed.The morphological changes of the retinas were observed under light microscope with HE staining, and histopathological score was recorded.The expres-sion of VEGF in the retinas was observed by the method of immunohistochemistry.RESULTS:Compared with normal con-trol group, the protein levels of VEGF, p-ERK and VEGFR2 in the ARPE-19 cells decreased in scutellarin group, but in-creased in high glucose group.The histopathological score of the retinas showed significant difference among diabetes group, diabetes treated with scutellarin group and normal control group, and no significant difference between normal con-trol group and scutellarin group was observed.The expression of VEGF increased in diabetic group and was significantly higher than that in scutellarin treatment group (P<0.05).CONCLUSION:Scutellarin inhibits the increased protein le-vels of VEGF, p-ERK and VEGFR2 in ARPE-19 cells, and decreases the expression of VEGF in the retinas of diabetic rats.The suppression of the diabetic retinopathy development by scutellarin may be partly involved in the ERK/MAPK pathway.

Detection of the possible role of ERp46,new endoplasmic reticulum protein,on palmitic acid-inducedendoplasmic reticulum stress-apoptosis pathway in βTC6 cells for the new treatment of type 2 diabetes.Results showed that ERp46 played a protective role in palnutic acid-induced cell apoptosis by decreasing the endoplasmic reticulum stress response through three pathway.

Objective:In order to provide a new area of bone flap were supplied for non-unions, osseous defect of upper limbs and chondral defect of elbow joint.Methods:The origin,course,distribu- tion and anastomosis of inferior ulnal collateral artery and ulnal recurrent artery were observed on 40 adult cadaver upper limbs.Based on the results,transposition of pedicled medial lower part of humerus bone flap with vessels was designed.Results:Satisfactary results were achieved in 9 case of patients (non-unions of 3 case and osseous defect of 2 case on medial part of humerus bone;non-unions of 3 case on medial upper part of ulna and radius;chondral defect of 1 case on elbow joint.)Conclusion:This pe- riosteal flap that there are constant vessels and abundant anastomosis with inferior ulnar collateral artery form many area of bone flap were supplied with blood supply for repairing the non-unions,os- seous defect of humerus,ulna,radius and chondral defect of elbow joint.
