1.Study on relationship between expression of survivin, galectin-3 and TPO and BRAFV600E mutation in patients with papillary thyroid microcarcinoma and its clinical significance

Objective:To detect the expressions of survivin, galectin-3 and thyroid peroxidase in papillary thyroid microcarcinoma ( PTMC) and cancer adjacent normal tissue to explore the clinical significant.The correlations between the expressions of survivin,Gal-3 and TPO with BRAFV600E gene mutation in PTMC were analyzed.Methods: The expressions of survivin,Gal-3 and TPO were measured by immunohistochemical ( IHC ) method in 56 cases of PTMC tissue and adjacent normal tissue; BRAFV600E mutation was detected by PCR amplification and subsequently sequencing.Chi square test was used to analyse the relation in the expression rates of survivin,Gal-3 and TPO and BRAFV600E gene mutation.Results:The survivin and Gal-3 were strongly expressed but TPO was negatively expressed.The survivin and Gal-3 were negative in adjacent normal tissues but TPO was expressed.There were sig-nificant differences in the expression rates of survivin, Gal-3 and TPO between PTMC tissue and adjacent normal tissue (all P<0.001).The BRAFV600E mutation rate (32.1%) and the expression rates of survivin,Gal-3 and TPO in PTMC tissue were found to be positively related to lymph node metastases (P=0.009,P=0.025,P=0.007,P=0.008),and negatively related to gender and age (all P>0.05).There were no correlation was found between the expressions of survivin,Gal-3,TPO and the BRAFV600E gene mutation in PTMC(all P>0.05).Conclusion: The strong expressions of survivin and Gal-3,the mild expression of TPO and BRAF mutation may be important in the development of PTMC,and the detection of BRAFV600E gene mutation and the expressions of survivin, Gal-3 and TPO could be used to the judgment of pathogenetic condition and prognostic outcomes.
2.Construction of Question Prompt List of information needs of patients with interstitial lung disease based on ERG theory

Objective:To construct a Question Prompt List of information needs for patients with interstitial lung disease, providing a tool for patients with interstitial lung disease to actively obtain disease-related information.Methods:A literature study was conducted based on the three core needs (ERG) theory framework of existence,relatedness and growth. A purposive sampling method was used to select 15 patients with interstitial lung disease from Drum Tower Hospital, Medical School of Nanjing University from December 2021 to January 2022 for descriptive research. Based on literature research and qualitative research, the initial items of the Question Prompt List were determined through expert inquiry, and the items were extracted, discussed, and adjusted, and finalized the list of information needs and prompts for patients with interstitial lung disease.Results:The 19 experts participated in the consultation, with a positive coefficient of 100.0% and 94.7% for two rounds of consultation, an authoritative coefficient of 0.91 and 0.92, and the Kendall coordination coefficient of 0.31 and 0.23, respectively. Finally, a list of interstitial lung disease patients′ question prompts containing 4 primary indicators and 34 secondary indicators was formed.Conclusions:Based on the ERG theory, the research method for the Question Prompt List of information needs for patients with interstitial lung disease is scientific and reliable, and can be used in clinical practice to help patients inquire and obtain the necessary information.
3.Impact of self-management support on the behavior and metabolic parameters of type 2 diabetic patients with obesity

Objective To explore the impact of self-management support on the behavior and metabolic parameters of type 2 diabetic patients with obesity.Methods A total of 96 inpatients of type 2 diabetes with obesity,who hospitalized in the Second Hospital of Hebei Medical University from October 2014 to March 2015, were randomly assigned to control group and intervention group with 49 cases and 47 cases respectively.The patients in the control group received routine health education,while the patients in the intervention group were given self-management support by educational team with eight members constituted by physicians,nurse educators,dietitians,psychologists,etc.additionally.The patients in both groups were followed up for 3 months,self-management behaviors,body weight,body mass index (BMI ), waist circumference and glycosylated hemoglobin(HbA1c)that measured before and 3 months after the intervention.Results The self-management behaviors in the intervention group were significantly better than those in the control group after the intervention (P <0.05 ).The values of HbA1c and waist circumference [(7.14 ±0.68 )%,(94.82 ± 8.90)cm]in the intervention group were significantly lower than [(7.70 ±0.58)% ,(98.31 ±6.38)cm]in the control group after the intervention (P <0.05).The body weight and body mass index(BMI)changed were not significantly compared with the control group (P >0.05 ).Conclusions Self-management support can improve the patients′self management behavior,reduce blood sugar,waist circumference and the risk factors for the occurrence of chronic complications.
4.Application of nursing cases in promoting accuracy of nursing grading

Objective To evaluate the effect of applying nursing cases in promoting accuracy of nursing grading . Methods A specialized-group of nursing cases was built to clear up the problems in nursing grading by using quality control system. The researchers surveyed the accuracy of nursing grading of 84 patients with activities of daily living ( ADL) score less than or equal to 60 ( ADL≤60) in two wards before and after the improvement project. The researchers compared the relative knowledge of nursing grading and sense of dynamically adapting nursing grading of 20 resident doctors, and self-ability of 7 nurses in specialized-group of nursing cases before and after the improvement project. Results The relative knowledge of nursing grading, ability of dynamically adapting classification and accuracy of classification were higher than before (χ2=42. 15, 8. 09,12.2;P<0. 01). The abilities of promoting nursing cases, communication, solving problem, reading literatures were significantly higher than before(t=-3.16,-4.90,-2.57,-4.90,-2.93;P<0.05). The dimension of responsibility had no significant difference ( t=-1.84,P=0.090) . Conclusions Nursing cases is effective in improving the accuracy of nursing grading, and promoting the ability and sense of improving nursing cases actively.
5.Screening of Anti-breast Cancer Active Ingredients in Famous Classical Formula Yanghetang

ObjectiveBased on ultra performance liquid chromatography-quadrupole-time-of-flight mass spectrometry(UPLC-Q-TOF-MS/MS), the combination of serum pharmacochemistry, response profile of absorbed components in serum, network pharmacology and drug-likeness prediction was used to screen the potential active ingredients of Yanghetang against breast cancer. MethodsUPLC-Q-TOF-MS/MS was used to identify the main components in different solvent extracts of Yanghetang, and serum pharmacochemistry was applied to analyze the absorbed components from the serum of female SD rats after 0.5, 1, 2 h of administration. Combined with the response characteristic values of serum drug components obtained from UNIFI 1.8.2, the absorbed prototype components and metabolites were screened to get the absorbed components of Yanghetang with a significant patterns of elimination and growth. Network pharmacology was applied to construct a drug-component-pathway-target-disease network, and molecular docking was performed between absorbed components and key targets of breast cancer, and the drug similarity was analyzed by SwissADME. ResultsForty-two compounds were identified in Yanghetang samples extracted with different solvents, of which 16 compounds were common to the three different extraction solvents(methanol, 50% methanol and water). The results of drug-containing serum analysis showed that there were 16 absorbed components in serum, including 5 prototypes and 11 metabolites. Network pharmacology results showed that Yanghetang against breast cancer involved 15 key targets such as proto-oncogene tyrosine-protein kinase Src(SRC), epidermal growth factor receptor(EGFR) and phosphoinositide 3 kinase catalytic alpha polypeptide(PIK3CA). Molecular docking results showed that 16 potential active ingredients were well combined with the predicted targets. Combined with drug likenesses, 12 compounds in the absorbed components of Yanghetang were considered to have potential for anti-breast cancer activity, mainly including α-pinene and γ-eudesmol and their metabolites, of which one was from Ephedrae Herba, one was from Rehmanniae Radix, and eight were from Cinnamomi Cortex. ConclusionThe chemical components of Yanghetang mainly include polysaccharides, monoterpene glycosides and coumarins, and its prototype components mainly undergo oxidation, hydrolysis and acetylation after entering the blood. Its anti-breast cancer mechanism may be related to the regulation of signaling pathways such as the mitogen-activated protein kinase(MAPK) and phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt). The results of this study can lay a foundation for further exploration of Yanghetang in the treatment of breast cancer.
