1.The expression of interleukin-25 in patients with inflammatory bowel disease and its clinical significance

Objective To explore the function and significance of interleukin(IL)-25 in the pathogenesis of inflammatory bowel disease (IBD) through testing its expression in the intestinal mucosal and serum of patients with IBD. Methods Intestinal or colonic mucosal biopsies of 12 patients with ulcerative colitis (UC), 16 patients with Crohn's disease (CD) and 10 healthy controls were collected. The expression of IL-25 at mRNA level was detected by real-time PCR and the situ expression of IL-25 in intestinal mucosa was analyzed with immunohistochemistry. At same time,serum of 20 UC patients, 24 CD patients and 20 controls was collected, and IL-25 concentration in the serum was determined by enzyme-linked immunosorbent assay (ELISA). Results Compared with healthy controls, the expression of IL-25 at mRNA level in inflamed mucosa of CD and UC patients was significantly decreased (P<0.05), no statistic difference between UC and CD groups (P>0.05).The immunohistochemistry results indicated that more IL-25 positive cells in normal lamina propria,the expression of IL-25 in mucosal epithelia cells was low, the expression of IL-25 protein in the intestinal or colonic mucosa of UC and CD patients significantly decreased ( P < 0.05), no statistic difference between UC and CD groups (P>0. 05). ELISA results showed that the level of IL-25 in the serum of UC and CD groups was significantly lower than healthy controls (P < 0.05). Conclusion The expression of IL-25 in the intestinal mucosa and serum of IBD patients was significantly decreased,which suggested that IL-25 expression defects may closely related to the pathogenesis of IBD. IL-25 may be a new target for the IBD treatment.
2.Iatrogenic foreign body of the ethmoid sinus misdiagnosed as tumor.

Foreign bodies in the paranasal sinuses are rare. Foreign bodies in the ethmoid sinus are easily misdiagnosed as tumor. There is no typical display in CT and MRI tests. The described case showed the misdiagnosis of an ethmoid foreign body as tumour on the basis of CT and MRI results. It should be emphasised that the tumour diagnosis should be based on further physical and histopathological examinations and not only on the results of CT and MRI tests.

3.Research Progress in New Dosage Forms of Nevirapine

Nevirapine ( NVP) is the first-generation non-nucleoside reverse transcriptase inhibitor, which is a front-line drug for the treatment of AIDS and the prevention of mother-to-child transmission of HIV-1. NVP is with poor solubility and low bioavailability. With the development of new dosage forms, such as solid dispersion, self-emulsifying, microparticles, micelles, sustained-release tab-lets and floating microspheres, the bioavailability of NVP can be increased and the adverse reactions can be decreased. Research pro-gress in the new dosage forms of NVP at home and abroad was reviewed through searching literatures in order to provide basis for the development of new dosage forms of NVP.

5.Study Progress in Vaginal Mucoadhesive Drug Delivery Systems

Objective:To review the progress in vaginal mucoadhesive drug delivery systems. Methods:Based on the recently published papers,the research on the application of vaginal mucoadhesive drug delivery systems was classified and summarized. Re-sults:Vaginal mucoadhesive drug delivery systems not only showed safety,effectiveness and strong permeability but also avoided the first pass effect,prolonged drug residence time and improved bioavailability and compliance of patients. Conclusion:Vaginal mucoad-hesive drug delivery systems are becoming research hotspots and exhibit broad application prospects to replace the traditional delivery systems.
6.Research Progress in Bioavailability Improvement Methods for Curcumin

Curcumin is the main active component in turmeric, which possesses many pharmacologic effects, including anti-in-flammatory, antioxidant, anti-tumor, anti-atherosclerosis, liver and kidney protection and so on. However, due to its poor bioavail-ability, its clinical application is limited. Therefore, the methods for improving the bioavailability of curcumin were reviewed by refer-ring to a large number of literatures. The bioavailability of curcumin can be improved by different administration routes and various dos-age forms. The review provides theoretical basis and research ideas for the development of new drugs.
7.Application of tramadol combined atropine in oocyte retrieval operation

Objective To observe the analgesic effects of tramadol combined with atropine during the oocyte retrieval operation in assisted reproduction treatment(IVF/ICSI-ET)? Methods Three hundred patients(four hundred cycles of infertility totally)in the Center for Reproductive Medicine were equally assigned into treatment group and control group according to the different analgesia: the former were treated with intramuscular injection of 100 mg tramadol combined with 0?5 mg atropine for analgesia before the operation and the latter with 50 mg pethidine hydrochloride? The two groups were compared in terms of blood pressure,pulse,degree of pains, rate of fertility,rate of cleavage and rate of transplantable embryo? Result There were no differences between them in terms of blood pressure,pulse,pain degree,rate of fertility and rate of cleavage and rate of transplantable embryo(all P > 0?05),but the incidence of adverse effect in the treatment group was significantly lower than that in the control group(P < 0?05)? Conclusion The application of tramadol hydrochloride combined with atropine sulphatev for analgesia during oocyte retrieval operation of IVF-ET is advantageous for its safety,lower incidence of advers effect and stable vital signs?
8.Analysis of risk factors and drug-resistant associated with nosocomial bacteremia

Objective To investigate the risk factors for nosocomial bacteremia and decrease the prevalence of nosocomial bacteremia.Methods We collected the data of bacteremia patients in our hospital from January,2006 to December,2009.According to the criterion of nosocomial infection,the patients were divided into nosocomial bacteremia group (83) and community-onset bacteremia group (119).The influence of a series of variables on the development on both types of bacteremia was analyzed by Student's t test and x2 test.The risk factors were performed using multivariate logistic regression.Results Compared to that of community-onset bacteremia group,the proportion of malignancy (21/83 vs 12/119,x2 =8.2846,P < 0.01 ),venous catheter ( 28/83 vs 3/119,x2 =36.67,P < 0.01 ),diabetes ( 37/83 vs 0/119,x2 =68.226,P <0.05),surgical operation(37/83 vs 0/119,x2 =68.226,P <0.01),previous antibiotics(78/83 vs 10/119,x2 =173.5657,P < 0.01 ) in nosocomial bacteremia group were higher.Multivariable logistic regression analysis showed that only 4 factors were significantly and independently responsible for nosocomial bacteremia,They were malignancy ( P < 0.05,OR =3.186),diabetes ( P <0.001,OR =4.821 ),venous catheter( P < 0.05,OR =2.135),previous antibiotics ( P < 0.05,OR =2.135 ).The bacteria in nosocomial bacteremia group showed more ability to resist to antibiotics.Conclusions We should pay more attention to the patients with diabetes or malignancy or venous catheter or previous treated with antibiotics.These patients have more chances to develop to nosocomial bacteremia and infect by the drug-resistant bacteria.

Pulmonary drug delivery has become a research focus in recent years, while the action between particles and pulmonary macrophages after particles transportation to lung tissue was paid little attention. Therapeutic efficacy can be enhanced by regulating the particles uptake action of pulmonary macrophages according to different diseases. Referring to a lot of articles, the relationship between common diseases and macrophages was reviewed and the influencing factors in the particles uptake of alveolar macrophages were sum-marized. The review laid foundation for the development of preparations and clinical application of pulmonary drug delivery systems.
10.Effects of cyclamic acid on dynamic monitoring D-Dimer and thromboembolic prophylaxis during total hip arthroplasty

Objective To study the effects of use of cyclamic acid on monitoring D-Dimer and predicting deep venous thrombosis on lower limbs during total hip arthroplasty.Methods Ninety-three cases patients who received total hip arthroplasty operations at Joint Surgery Department of the Central Hospital of Zhuzhou city and Zhuzhou Hospital Affiliated to Xiangya Medical College of Central South University from December 2015 to May 2016 were selected as subjects and randomly assigned into study group with 50 cases and control group with 43 cases.During the operations,cyclamic acid was used intravenously and locally as a routine in the study group while saline was utilized instead in the control group.The D-Dimer was dynamically monitored before operation and 1,3,5,7,9 d after the operation,and venous color ultrasonography of both lower limbs were taken 3,6,9 d after the operation to check the conformation of thrombosis.Results The total blood loss after treatment in the study group was (350.5 ± 65.2) ml,intraoperative blood loss of (129.3 ± 43.1) ml,postoperative drainage volume of (80.9± 12.6) ml,occult blood loss of (141.9± 20.6) ml,corresponding to the control group were (560.8±60.6) ml,(208.9± 57.8) ml,(150.8 ± 18.9) ml,(202.9±23.9) ml.The above indicators were lower in the study group than in the control group,the differences between the two groups were statistically significant(t =16.02,7.59,21.24,13.22,P<0.05).There were significant differences in terms of D2-dimer level of 1,3,5 d after the operation between the study group and the control group (P<0.05),but at 7,9 d after the operation,the difference between the two groups were not statistically significant(P>0.05).The study group and the control group at 9 d after operation with color Doppler ultrasound examination showed that there were 1 cases of patients with calf vein thrombosis both in two groups,there was no significant difference between the two groups(2.00% vs.2.32%,x2 =0.012,P> 0.05).Conclusion The proper use of cyclamic acid can reduce blood loss and will not increase the risk of thrombosis.Monitoring dynamically on D-Dimer and deep venous color ultrasonography on lower limbs is helpful for early detection of thrombosis after total hip arthroplasty.However,the use of cyclamic acid during total hip arthroplasty will affect the monitoring on D-Dimer and therefore needs to be taken seriously.
