2.Effect of interventional occlusion for the treatment of residual leak after repaired of patent ductus arteriosus

Objective To observe the effect of interventional occlusion for the treatment of residual leak after repaired of patent ductus arteriosus (PDA).Methods A total of 23 patients with residual leak after repaired of PDA underwent interventional occlusive procedures.Original operative methods:15 patients performed surgical ligation of PDA,4 patients performed neoplasty under cardiopulmonary bypass,4 patients performed transcatheter occlusion.Twenty-three patients were treated by standard interventional occlusion.The lateral descending aortographies and echocardiography were performed to evaluate the immediate results after occlusion 15 min.Echocardiography was rechecked after occlusion 1 week,1 month to find whether there was residual shunt and recanalization.Results Twenty-three patients per formed aortographies in operation showed residual leak.The mean minimum diameter of the residual leak was 2-8 (3.7 ± 1.4) mm.After occlusion 15 min,heart murmur disappeared,the form of interventional occlusive procedures was better.One patient had little residual leak,22 patients (95.65%,22/23) residual leak disappeared.Aftertreated 1 week,1 month,aortographies and echocardiography showed the form of interventional occlusive procedures was better,all patients had no residual leak.Conclusion Residual leak after repaired of PDA can be treated safely and effectively with interventional occlusive procedures.
3.A comparative study on coronary arterial fistula's treatment between surgical and interventional procedure

ObjectiveTo discuss the effect of surgical procedure and interventional procedure on coronary arterial fistula.Methods The clinical data of 25 cases of coronary arterial fistula were retrospectively analyzed.Seven cases received surgical procedures,18 cases received interventional procedures.ResultsOf the 7 cases with surgical procedures,coronary arterial fistula shunt disappeared in 6 cases after review of echocardiography,coronary arterial aneurysms was found in 1 case who had limited residual flow.Of the 18 cases with interventional procedures,12 cases were completely cured,2 cases had limited residual flow,4 cases did not succeeded.ConclusionsSurgical procedures and interventional procedures on coronary arterial fistula are safe and efficient.The choice of method should be based on the specific situation.
4.Long-term efficacy of surgical treatment of slow transit constipation with pelvic floor hernia and rectal mucosal prolapse

Objective To evaluate the long-term therapeutic efficacy of subtotal colectomy,ascending colon-rectum anastomosis, pelvic floor hernia repair, functional rectal suspension, and uterine suspension surgery for slow transit constipation with pelvic floor hernia and rectal mucosal prolapse.Methods From June 2007 to May 2008, 35 patients with intractable constipation caused by slow colonic transit combined with pelvic floor hernia and rectal mucosal prolapse underwent subtotal colectomy and ascending colon-rectum anastomosis, pelvic floor hernia repair, functional rectal suspension, and uterine suspension surgery. Postoperative defecation, complications, quality of life, and degree of satisfaction were followed-up. Results The average follow-up period was two years. At one month after the operation, the average defecation frequency was five times (2 -8 times) a day, with a semi-liquid stool consistency; After two years the frequency was twice ( 1 -3 times) a day, with solid stool consistency. Of the 35 patients, 19 were satisfied with the surgical efficacy, and 16 were very satisfied. All the patients' quality of life improved significantly. Conclusions For patients suffering from slow transit constipation with pelvic floor hernia and rectal mucosal prolapse subtotal colectomy, ascending colon-rectum anastomosis, pelvic floor hernia repair,functional rectal suspension, and uterine suspension surgery has satisfactory results.
5.Clinical application of prostaglandin E1 in the treatment of hypertensive renal damage of early stage

Objective To observe the effect ofprostaglandin E1 intravenously in the improvement of the function of renal glomeruli and renal tubules in hypertensive patients complicated with microalbuminuria.Methods Totally 60 hypertensive patients complicated with microalbuminuria and nocturia who had received losartan for above 12 weeks,according to random number table method were divided into treatment group and control group,with 30 cases in each group.Original antihypertensive drugs were used continuously in treatment group and control group,and patients in treatment group received prostaglandin E1 intravenously for 3 courses of treatment,with losartan and other antihypertensive drugs routinely.Using self-contrast before and after treatment and contrast with control group,the change of urinary level of microalbumin (mALB),osmotic pressure,β2-microglodulin (β 2-MG) and N-acetyl-β-D-glucosaminidase (NAG) were compared.Results The blood pressure after treatment in both groups were decreased,but there was no statistically significant difference compared with that before treatment (P > 0.05).After treatment of 12 weeks,urinary levels of mALB,β 2-MG and NAG in treatment group were lower than those in control group [(88.6 ± 14.2)μg/min vs.(123.9 ± 14.8) μg/min,(0.84 ±0.41) mg/L vs.(1.61 ±0.49) mg/L,(30.2 ± 10.5) U/L vs.(75.7 ± 12.8) U/L],and osmotic pressure was higher than that in control group [(591 ± 71) mmol/L vs.(454 ± 67) mmol/L],the difference had statistical significance (P < 0.01).Conclusion Prostaglandin E1can reduce the renal ischemic state in early stage of hypertensive renal damage and can improve renal function.
6.A research of the application of evidence-based nursing in the patients with cerebrospinal fluid leakage after vertebrae surgery

Objective To investigate the application of evidence-based nursing in the patients with cerebrospinal fluid leakage (CSFL) after vertebrae surgery.Methods 60 cases of patients were randomly divided into the observation group and the control group (30 cases in each),the control group was given conventional nursing,the observation group used evidence-based nursing,which was established by referring to some data and literatures according to a series of related nursing problems,getting direct and indirect evidence,and through analysis and summarizing to set up the nursing project for implementation.The incidence of intracranial hypotension headache,hyponatremia,wound healing disorders,infection of intervertebral space were compared.Results In the observation group,incidence of postoperative intracranial hypotension symptoms,postoperative hyponatremia,the incidence of wound healing disorders was statistically different from that of the control group.The incidence of infection in two groups showed no significant difference.Conclusions Using evidence-based nursing can reduce a series of complications in patients with CSFL after cervical surgery,control infection effectively,improve patients' comfort and improve the quality of nursing.

Objective To investigate the electrophysiological characteristics of atrial tachycardia (AT) originating from the side of the bundle of His, and to analyze the efficacy and safety of transcatheter radiofrequency ablation via the right side of the bundle of His and left noncoronary sinus of aorta for the treatment of AT. Methods A total of 12 patients with AT originating from the side the bundle of His, which was confirmed by conventional electrophysiological study and atrial activation patterns, were included in this study. The patient’s age ranged from 12 to 64 years old with a mean of (47.4±14.6) years. The mapping was carried out at the right side of the bundle of His and radiofrequency ablation was performed. When the ablation procedure failed, or the junction zone rhythm or atrioventricular block occurred, ablation via the left noncoronary sinus of aorta was employed. Results Atrial stimulation could repeatedly induce and terminate AT in all the 12 patients, the average cycle length was (327±76) ms. TA was terminated within 10 seconds after the start of ablation in 10 patients during their AT attacks. Ablation was unsuccessful in 2 patients. Ablation via the right side of His bundle was successfully accomplished in 2 patients, and the ablation via the left noncoronary sinus of aorta was successfully carried out in 8 patients. The average follow-up time was 1-6 years, and no recurrence of AT was observed. Conclusion Atrial tachycardia originating from the side of the bundle of His has certain electrophysiological characteristics, and transcatheter radiofrequency ablation is safe and effective for its treatment. Ablation via the left noncoronary sinus of aorta should be considered as a strategy of priority.

10.Role of spinal c-Jun N-terminal kinase signaling pathway in incisional pain in rats

Objective To evaluate the role of spinal c?Jun N?terminal kinase ( JNK ) signaling pathway in incisional pain in rats. Methods Sixty?three adult male Sprague?Dawley rats, weighing 200-250 g, were randomly divided into 3 groups ( n=21 each) using a random number table: incisional pain group ( IP group) , dimethyl sulfoxide ( DMSO) group, and JNK inhibitor SP600125 group ( SP group) . A 1?cm longitudinal incision was made through skin, fascia and muscle of the plantar aspect of the hindpaw in anesthetized rats. In group DMSO, 10% DMSO 10 μl was injected intrathecally at 30 min before surgery. In group SP, SP600125 25 μg (in 10 μl of 10% DMSO) was injected intrathecally at 30 min before sur?gery. Six rats in each group were sacrificed, and the mechanical paw withdrawal threshold ( MWT) and thermal paw withdrawal latency ( TWL) were measured at 24 h before establishment of the model and 2, 6, 24, 48 and 72 h after establishment of the model. After measurement of the pain threshold at 24 h before establishment of the model and 6, 24, 48 and 72 h after establishment of the model, the lumbar segment of the spinal cord was removed for determination of the expression of phosphorylated JNK ( p?JNK) by im?munofluorescence. Results The MWT was significantly lower, the TWL was shorter, and the expression of p?JNK was lower at each time point after establishment of the model than at 24 h before establishment of the model in group IP (P<0?05). Compared with group IP, the MWT was significantly increased, the TWL was prolonged, and the expression of p?JNK was down?regulated at each time point after establishment of the model in group SP ( P<0?05) , and no significant change was found in the parameters mentioned a?bove in group DMSO ( P>0?05) . Conclusion Spinal JNK signaling pathway is involved in the develop?ment and maintenance of incisional pain in rats.
