1.Relationship between bone relapse in pediatric acute lymphoblastic leukemia and gene mutation

Acute lymphoblastic leukemia is a major type of childhood cancer.It is an obstacle to cure for young patients with relapsed acute lymphoblastic leukemia.Recent investigations into the mechanism of relapse in pediatric acute lymphoblastic leukemia indicate the relationship between gene abnormalities and the relapse of pediatric acute lymphoblastic leukemia.An increasing number of gene abnormalities have been confirmed to be related with relapse of this disease, including deletion of IKZF1 and mutations of JAK, CREBBP,CEBPE and ARID5B.This paper reviews the details about the influence of the various gene mutations on the relapse of the pediatric acute lymphoblastic leukemia.

Objective: To study the effect of Tang Ke Decoction on tumor necrosis factor ?(TNF?) and glucose transporter 4(GLUT4) expression in tissues of model rats of insulin resistance of type two diabetes mellitus,and to investigate the mechanism of the function of Tang Ke Decoction to insulin resistance.Methods: Rat model of insulin resistance of type two diabetes mellitus was duplicated by using strepozotocin(STZ) and high caloric diet,and using Tang Ke Decoction of compound Chinese herbs and rosiglitazone acted on model rats individually.The degree of insulin resistance was measured by insulin sensitivity index(ISI).The expression of TNF? mRNA was assayed in adipose tissue by RT-PCR.GLUT4 protein expression was assayed by immunohistochemical method in skeletal muscule tissue,and its distribution in skeletal muscle cell was observed with electron microscope.Results: ISI of model group was significantly lower than that of control group(P
5.RELATIONSHIP BETWEEN ERYTHROCYTE SORBITOL AND DIABETIC MICROAN-GIOPATHY IN PATIENTS WITH NIDDM

To explore the relationship between the activity of polyol pathway and diabetic microangiopathy, the level of red blood cell sorbitol(RBCS)in 106 patients with NIDDMwas determined. The results indicated that the level of RBCS in diabetic patients was significantly increased, and was positively correlated with 24-h urinary albumin excretion rate(UAER)and the degree of diabetic retinopathy(DR). Besides,the value of RBCS in patients with both pro-teinuris and DR or proteinuria was markedly higher as compared with that in patients with either proteinuria or DR,and the concentration of RBCS in the patients with DR of proteinuria was also significantly higher than those wth neither proteinuria nor DR. In addition.it was also found that the value of RBCS gradually elevated with the aggravation of UAERand DR. These findings suggest that the elevation of RBCS level plays an important role in the formation and development of diabetic microangiopathy.
6.Effect of Simvastatin on Mobilization and Migration of Endothelial Progenitor Cells

Objective To investigate the effect of simvastatin on the mobilization and migration of endothelial progenitor cells(EPCs).Methods EPCs were harvested from the bone marrows of two rabbits,cultured with M199,and identified by immunohistochemistry.The identified EPCs were then treated with simvastatin with different concentrations(0,0.01,0.1,1.0 ?mol/L),and their migration induced by simvastatin was determined with Transwell chamber assay.Six rabbits models of cranial bone defect were established and divided into control and experiment groups(3 in each).In order to elicit the effects of simvastatin on mobilization of EPCs,simvastatin was embedded in polylactic acid compound,and implanted into the cranial bone defect area in the experiment group.Meanwhile,polylactic acid was implanted in the control animals.After 10 days,the expression rate of CD34+/CD133+ EPCs in the rabbit peripheral blood was counted by flow cytometry to determine the motivating effect of simvastatin.Results In Transwell experiment,16 hours after adding simvastatin(0,0.01,0.1 or 1.0 ?mol/L),the cell migration ability was obviously increased showing a dose-dependent trend(OD value:0.077?0.014 in control group and 0.075?0.013 in 0.01 ?mol/L group vs 0.097?0.011 in 0.1 ?mol/L group and 0.099?0.019 in 1.0 ?mol/L group,P
7.Effect of Simvastatin-Polylactic Acid Compound on Critical Calvarial Defect

Objective To investigate the effect of simvastatin-polylactic acid compound on critical calvarial defects in rats.Methods Twenty male SD rats(150 g?10 g),were used to establish critical cranial defect(10 mm in diameter)model.The animals were randomly divided into control and experiment groups(10 in each).In the control group,40 mg of polylactic acid were implanted into the defect area;whereas in the experiment group,simvastatin-polylactic acid compound were used(20 mg simvastatin and 40 mg polylactic acid).Four and eight weeks after the implantation,the defect area of the rats was observed by X-ray and toluidine blue staining.Results Eight weeks after the operation,X-ray examination showed high-density regions in the defect area in the experiment group,while low-density regions in the control group.The radiopacity of cranial defects were 27.33%?2.54% in the control group,and 74.63%?2.42% in the experimental group(n=5,t=-30.148,P=0.000).Toluidine blue staining showed a few new bone tissues at 4 weeks and fully filled bone defect at 8 weeks in the experiment group.Meanwhile,in the control group,only a small quantity of new bone tissue could be seen on the edge of the cranial defects.Conclusion Locally implanted simvastain-polylactic acid compound is a promising method for the treatment of bone defect owing to its high osteogenic ability.
8.A research of the application of evidence-based nursing in the patients with cerebrospinal fluid leakage after vertebrae surgery

Objective To investigate the application of evidence-based nursing in the patients with cerebrospinal fluid leakage (CSFL) after vertebrae surgery.Methods 60 cases of patients were randomly divided into the observation group and the control group (30 cases in each),the control group was given conventional nursing,the observation group used evidence-based nursing,which was established by referring to some data and literatures according to a series of related nursing problems,getting direct and indirect evidence,and through analysis and summarizing to set up the nursing project for implementation.The incidence of intracranial hypotension headache,hyponatremia,wound healing disorders,infection of intervertebral space were compared.Results In the observation group,incidence of postoperative intracranial hypotension symptoms,postoperative hyponatremia,the incidence of wound healing disorders was statistically different from that of the control group.The incidence of infection in two groups showed no significant difference.Conclusions Using evidence-based nursing can reduce a series of complications in patients with CSFL after cervical surgery,control infection effectively,improve patients' comfort and improve the quality of nursing.
9.Clinical analysis of 33 cases of neonatal group B streptococcal sepsis

Objective To explore the clinical characteristics of neonatal group B streptococcal sepsis (GBS)sepsis in order to provide the guide for early diagnosis and appropriate treatment.Methods A retro-spective review was performed and a total of 33cases of neonatal GBS sepsis were identified in the NICU of Children′s Hospital of Quanzhou from March 2011to October 2014.The perinatal factors,clinical characteris-tics,laboratory finding,treatment and prognosis were analyzed.Results A total of 33cases of neonatal GBS sepsis were identified.The incidence of neonatal GBS sepsis was 2.0‰(33/16448)among all the NICU pa-tients admitted at the same period.All 21cases of early-onset GBS sepsis were term infants,which had 13ca-ses of respiratory distress,11cases of anhelation and 10cases of cyanosis as main initial clinical symptoms. Among 12late-onset cases,8occurred in term infants,10with ardent fever as the main initial clinical symp-toms,6combined with purulent meningitis.All the GBS strains were sensitive to vancomycin,then penicillin combined with meropenem therapy was effective.Of the 33patients,18cured,9discharged with improve-ment,2died,4patients died during hospitalization after being given up because of serious complication,total mortality was 18.2%.Conclusion The clinical manifestations of neonatal GBS sepsis are usually obviously and fatally,and with a high mortality.Antepartum prophylaxis,early diagnosis and timely sensitive antibiotics therapy are vital for reducing the incidence of complications and mortality of neonatal GBS sepsis.
10.Application research of quantitative dynamic contrast enhanced MRI in preoperative grading of brain glioma

Objective To study appliction value of quantitative dynamic contrast enhanced MRI(T1-DCE MRI)in preoperative grading of brain glioma.Methods 80 patients who were pathologically confirmed with a tumor grade (WHO grade Ⅰ 20 cases, grade Ⅱ 20 cases,grade Ⅲ 20 cases and grade Ⅳ 20 cases).All patients were examined with MR enhancements and T1-DCE MRI. The original perfusions imaging datas were analyzed using the GE Omni Kinetic software,which produced the transfer constant (Ktrans )map,the rate constant (Kep )map and fractional volume (Ve )map.Choose ROI and get values of Ktrans ,Kep and Ve .Pearson correlation was carried out to analyze the correlation between values of Ktrans ,Kep ,Ve of different grades of gliomas and pathology classifications.The Ktrans ,Kep and Ve values of the different grade gliomas were statistically analyzed using an ANOVA .Receiver operator characteristics (ROC)curve was used to analyze sensitivity and specificity of permeability parameters.Results The Ktrans ,Kep and Ve values of each levels has a strong correlation with pathological grading (r=0.95 1,0.804,0.766).There was obviously statistically significant difference between different grade groups(P < 0.01 )by Ktrans .Kep values have statistically difference between different grades except grade Ⅱ and Ⅲ.Ve values were different between different grades except grade I andⅡand grade Ⅲ and IV.Accord-ing to ROC curve,Ktrans seemed to be a better parameter for evaluating the tumor grade with the highest sensitivity and specificity. With the cutoff thresholds of Ktrans of 0.1 60,0.420 and 0.935,different grades of glioma can be differentiated with sensitivities of 90%,95%,95% and specificities of 95%,95%,85% respectively.Conclusion Quantitative analysis of microcirculation perfusion status of different grade gliomas by Ktrans values obtained from T1-DCE MRI can assessment the degree of the destruction of the blood brain barrier and evaluate the grade of gliomas more accurately before operation.
