Chinese Journal of Rehabilitation Theory and Practice 2008;14(3):247-248
Objective To value the application of surface electromyography(sEMG)in assessing the functional electrical stimulation(FES)used in children with spastic diplegia. Methods 20 children with spastic diplegia were treated with FES.They were assessed with the sEMG and the clinical assessment before and after treatment. Results After treatment, the iEMG of the biceps brachii and quadriceps decreased significantly on the functional position, but increased significantly when the muscles contracted. Conclusion FES can reduce the spasticity and increase the strength of the limbs in children with spastic diplegia. The sEMG can be used as an objective assessment tool.

Objective To evaluate the changes in cerebral oxygen delivery/consumption and glucose metabolism during laparoscopic gynecologic surgery under propofol-fentanyl anesthesia. Methods Fifteen ASA Ⅰ or Ⅱ patients aged 20-59 yrs weighing 47-65 kg undergoing laparoscopic gynecologic surgery under propofol-fentanyl anesthesia were studied. The patients were premedicated with scopolamine 0.3 mg, midazolam 0.02 mg?kg-1 and fentanyl-droperidol mixture 0.03 ml?kg-1 Ⅳ. Anesthesia was induced with fentanyl 2 ?g?kg-1, propofol 2 mg? kg-1 and vecuronium 0.1 mg?kg-1 and maintained with TCI of propofol with target effect-site concentration set at 3-4 ?g? ml-1 and intermittent Ⅳ boluses of fentanyl and vecuronium. The patients were mechanically ventilated (VT = 7 ml? kg-1 RR = 14 bpm, I: E = 1:2). PET CO2 was maintained at 35-45 mm Hg. Radial artery was cannulated for BP monitoring and blood sampling. Left internal jugular vein was cannulated and advanced cephalad until a resistance was met. The depth of insertion was about 11-14 cm. After the pneumoperitoneum was established ( Ultra-abdominal pressure = 15 mm Hg) , the patients were placed in head-down position. Blood samples were taken from radial artery and internal jugular vein simultaneously before anesthesia ( T1 , baseline), before pneumoperitoneum (T2) and at 10, 20 and 60 min after pneumoperitoneum was established (T3-5) for blood gas analysis and determination of plasma glucose and lactic acid concentration. Arterio-jugular oxygen content difference ( Da-jv O2 ), arterio-jugular plasma lactic acid difference ( Da-jv. Lac ), cerebral oxygen consumption and cerebral glucose extraction rate (GluER) were calculated. Jugular bulb pressure (JBP) was recorded at blood sampling. Results After pneumoperitoneum was established JBP increased significantly. Jugular bulb oxygen saturation (Sjv O2 ) increased significantly. Da-jv, O2 and cerebral oxygen extraction rate ( OER) decreased significantly after pneumoperitoneum (T3-5 ) . Plasma glucose level increased along with the pneumoperitoneum, while the Da-jv Glu, Da-jv Lac remained constant. Conclusion During laparoscopic gynecologic surgery under propofotfentanyl anesthesia cerebral venous pressure is elevated and cerebral oxygen supply exceeds oxygen consumption. Blood glucose level increases during pneumoperitoneum. However brain energy metabolism is not affected during pneumoperitoneum with patient in head-down position.

Results of Meta-analysis showed the total effective rate [RR=1.56,95% CI(1.29,1.88),P<0.001],US National Institutes of Health Chronic Prostatitis Symptom Index scores(NIH-CPSI scores)[MD=-6.97,95% CI(-8.67,-5.26),P<0.001],pain symptom scores[MD=-3.18,95%CI(-3.92,-2.44),P<0.001)],voiding dysfunction scores [MD=-1.62,95%CI(-1.85,-1.38), P<0.001],life quality scores[MD=-2.16,95% CI(-2.89,-1.43),P<0.001],EPS leukocyte count [MD=-8.90,95% CI (-11.79,-6.02),P<0.001] and recurrence rate[RR=0.22,95%CI(0.12,0.42),P<0.001] in test group were significantly better than control group, there were statistically significant differences between 2 groups;and there was no significant difference in the incidence of adverse reactions between 2 groups [RR=0.49,95%CI(0.11,2.13),P=0.34]. CONCLUSIONS:The efficacy of Qian-lie shutong capsule combined with Quinolone antibiotics is better than only Qianlie shutong capsule or Quinolone antibiotics in the treatment of chronic prostatitis, with similar safety.

Objective To explore the surface electromyography (sEMG) assist for the rehabilitation training for children with cerebral palsy. Methods 60 children with spastic diplegia were divided into treatment group (n=30) and control group (n=30). All the children received comprehensive rehabilitation, while the rehabilitation program were designed with the assist of sEMG in the treatment group. They were assessed with Gross Motor Function Measure (GMFM) before and 3 months after rehabilitation. Results There was no statistically improvement in dimension A, B, and C of GMFM (P>0.05), but was in the total score and the dimension D and E (P<0.0001). There was no statistical difference in dimension A, B, and C between 2 groups (P>0.05), but was in the total score and the dimension D and E (P<0.01) after rehabilitation. Conclusion The sEMG might be helpful for the rehabilitation for children with cerebral palsy.

Objective To explore the surface electromyography(EMG)characteristics of biceps brachii during isometric contraction in children with hemiplegic cerebral palsy. Methods The surface electromyograms signals of biceps brachii of 10 children with hemiplegic cerebral palsy during passive isometric contraction were recorded. Results The intact side registered a stronger EMG than that of the affected side, the integrated electromyography(iEMG)and root mean square(RMS)levels from the intact side were higher than those of affected side significantly(P<0.05).Conclusion The results of electromyographic analysis are consistent with Modified Ashworth Scale.Electromyographic analysis can provide a satisfactory evaluation of the muscle tone in children with hemiplegic cerebral palsy.

Objective To observe the effect of static balance test and training equipment on the equilibrium function in children with spastic diplegia. Methods 24 children with spastic diplegia were treated with visual feedback balance training besides routine rehabilitation training. All cases were assessed with Berg Balance Scale(BBS) and the static balance test and training equipment when eyes opened and closed, before and after the 8-week training. Results After the 8 week-training,the BBS scores of all children were higher than that before training(P<0.05).In the segment of the static balance test, fall index, weight distribution index and stability index decreased significantly(P<0.05~0.01).Conclusion The equilibrium function of the children with cerebral palsy can be assessed and pertinent training can be performed with balance equipment.

The classification messages of non-powered suction apparatus device (NPSAD) intended for negative pressure wound therapy by CFDA have been analysis and generalized. A set of classification regular patterns of NPSAD have been generalized from its intended use, composition, mechanism, contact area and resorbable characteristic. It is helpful to draw a more consistent classification in NPSAD.
