Objective To explore the clinical effect of the small incision with laser comprehensive treatment for patients with the great saphenous vein varices. Methods Small incision surgical ligation was done at the top and end of the great saphenous vein. Then took out the whole part or subsection of the great saphenous vein, with laser to treat the main part of the great saphenous vein and varicose vein group. Results 210 cases of 230 limbs all cured, cure rate was 100%. Conclusion The small incision with laser comprehensive treatment was an effective method for the great saphenous vein varices. It was worthy of clinical application and dissemination.

Objective To analyze postoperative complications and its prevention of small incision with laser comprehensive treatment the great saphenous vein varices. Methods Clinical data of 210 cases (total 230 limbs) in our hospital used small incision with laser comprehensive treatment the great saphenous vein varices were retrospectively analyze. Results 210 cases of 230 limbs,60 cases have concurrent subcutaneous congestion after surgery,6 cases have funicular induration, red and swallon, pain in the local crus,6 cases have skin paraesthesia due to saphenous nerve injure,4 cases have skin burns,2 cases have ulcer amalgamate infection. All of the cases have healed after the corresponding treatment. Conclusion This is a effective method for the great saphenous vein varices but may have complications. It should be taken seriously and processing.

Objective To explore the influence of different blood gas strategies on cerebral protection and blood gas analysis during moderate hypothermic cardiopulmonar. Methods Patients under cardiac valve replacement with extracorporeal circulation (ECC) were performed in this study. The cerebral blood flow (CBF) and regional cerebral oxygen saturation (rSO2) were monitored at 5 points:(1)Induction of anesthesia (T1),(2) after 10 min of the beginning of cardiopulmonary bypass (T2),(3) during the moderate hypothermic phase of CBP managed by the alpha-stat after calibrate blood gas 15 min (T3),(4) pH-stat followed, after calibrate blood gas 15 min (T4),and (5) 10 min after CBP (T5).pH and pCO2 in patients were recorded and analyzed at T3 and T4.Results The CBF of T2 and T3 was lower than that of T1,but the difference was not significant (t =2.841,2.711;P =0.062,0.080).The CBF of T4 and T5 was higher than that of T3,but the difference was not significant (t =1.793,2.135;P =0.119,0.066).The CBF of T5 resumed to before operation. The rSO2 of T2 and T3 was lower than that of T1,but the difference was not significant (t =1.821,2.032;P =0.132,0.267),the rSO2 ofT4 and T5 was higher than that ofT3,but the difference was not significant (t =1.879,2.021;P =0.312,0.075).The rSO2 of T5 resumed to before operation. However, the pH was much lower in pH-stat than alpha-stat (t =17.541,P =0.000) and the pCO2 was much higher than alpha-stat (t =13.914,P =0.000).Conclusions Both pH-stat and alpha-stat have cerebral protection effects. However, compared to pH-stat,alpha-stat could reduce the possibility of acidosis, and maintain the acid-base equilibrium.

Objective To investigate the relationship between cellular immune response which is related with Th1/Th2 cells and the different severities of tuberculosis pleural effusion adhesion. Methods A total of 66 in-patients diagnosed with different severities of tuberculosis pleural effusion adhesion by internal thoracoscope were enrolled from August 2014 to December 2016.ELISA was used to determine levels of INF-γ,TNF-α,IL-2 and IL-4. The ratio of Th1 and Th2 cells was detected by flow cytometry. Results All the patients were divided into 3 groups:9 cases without pleural adhesion,32 cases with mild pleural adhesion,25 cases with severe pleural adhe-sion. The levels of 4 cytokines in pleural fluid were significantly higher than those in serum in each group(P <0.05,respectively).The concentrations of INF-γ and TNF-α were increased with the severity of tuberculosis pleu-ral effusion adhesion. The levels of INF-γ and TNF-α in the severe pleural adhesion group were markedly higher than those in the other two groups(P<0.05,respectively).The proportion of Th1 cells in the severe pleural adhe-sion group was significantly higher than that in the none pleural adhesion group and in the mild pleural adhesion group(P<0.05,respectively).Conclusions The proportion of Th1 cells and levels of INF-γ and TNF-α are pos-itively related with pleural adhesion severity. Cellular immune response which is related with Th1 cells contributes to pathological immune pleural damage and intensify the severity of pleural adhesions.

Objective Though investigating the causes of untreated congenital heart disease among children aged 3–14 years in Panzhihua city, to provide the basis for formulating intervention measures in the next step. Methods A survey was conducted on the children aged 3 to 14 years in Panzhihua city from March 2015 to December 2017. The causes of untreated congenital heart disease were analyzed. Results Of 148 children with untreated congenital heart diseases, 82 were found to have congenital heart disease or heart murmur before screening. Among them, 59 parents (71.95%) thought that hospitalization was expensive; 42 parents (53.84%) thought that their children were asymptomatic and did not need treatment. Doctors suggested that 13 children (15.85%) be observed. Sixty-six patients of congenital heart disease were found for the first time in this screening, with an average age of 7.11±1.23 years. The patients in the rural population (n=44, 81.82%) were significantly more than the patients in the urban (n=12, 18.18%, P<0.01). Atrial septal defect ranked the first among the diseases, and 45 (68.18%) children did not regularly participate in child health care after birth. Conclusion Most families with untreated congenital heart disease in Panzhihua city think that the cost of treatment is high and there is no need for treatment. And most of them do not participate in child health care regularly. Atrial septal defect ranks first among the diseases. Free screening and treatment of congenital heart disease is of great significance in Panzhihua city.
