1.Observations on the Efficacy of Electroacupuncture plus Acupoint Application in Treating Diabetic Constipation

Objective To investigate the clinical efficacy of electroacupuncture plus acupoint application in treating diabetic constipation of yin deficiency and blood stasis type.Methods One hundred and fifty-six patients with diabetic constipation of yin deficiency and blood stasis type were randomly allocated to treatment and control groups, 78 cases each. The treatment group received electroacupuncture plus acupoint application and the control group, oral administration of mosapride citrate tablets. The TCM syndrome scores were recorded in the two groups before and after treatment. The clinical therapeutic effects were evaluated using the Constipation Scoring System (CSS), the Patient Assessment of Constipation Quality of Life (PAC-QOL) questionnaire and the TCM syndrome scores.Results There was a statistically significant post-treatment difference in the total efficacy rate based on CSS, PAC-QOL and the TCM syndrome scores between the two groups (P<0.05,P<0.01). There were statistically significant pre-/post-treatment differences in the TCM syndrome scores in the treatment group (P<0.05). There were statistically significant pre-/post-treatment differences in TCM syndrome dry mouth and tongue, thirst and dry stool scores in the control group (P<0.05). There were statistically significant post-treatment differences in TCM syndrome short breath, weakness, lassitude, stabbing pain, scaly dry skin, fixed pain, sore and weak loins and knees, and chest, palm and sole vexing heat scores between the two groups (P<0.01,P<0.05).Conclusion Electroacupuncture plus acupoint application is an effective way to treat diabetic constipation of yin deficiency and blood stasis type.
2.The relationship of serum vitamin D3 level and inflammatory factor in primary hypertensive patients

Objective To study the relationship between serum 1,25 -dihydroxy vitamin D3 level and inflammatory factor in elderly primary hypertension patients.Methods Sixty -eight primary hypertension patients were selected as hypertension group,and 60 healthy subjects were selected as control group in this study.The serum 1,25 -dihydroxy vitamin D3 level and plasma inflammatory factor were measured and compared.Results The serum 1,25 -dihydroxy vitamin D3 level in the hypertension group was significantly lower,while plasma inflammatory factor in the hypertension group was significantly higher than in the control group(t =6.445,7.326,8.174,8.556,all P <0.01).Pearson correlation analysis showed that the serum 1,25 -dihydroxy vitamin D3 level was negatively related with systolic blood pressure,plasma diastolic blood pressure and inflammatory factor (r =-0.425, -0.367, -0.254, -0.263, -0.296,all P <0.05).Logistic repression analysis revealed that the inflammatory factor was a risk factor for hypertension,while the 1,25 -dihydroxy vitamin D3 level was a protecting factor for primary hypertension. Conclusion The serum 1,25 -dihydroxy vitamin D3 level is significantly lower in elderly primary hypertension patients than in healthy subjects.The serum 1,25 -dihydroxy vitamin D3 level and the inflammatory factor are predic-tive factors for hypertension.
3.Effect of Ultrasound-guided Subcostal Transverses Abdominis Plane Block with Dexmedetomidine after Laparoscopic Radical Operation on Colon

Objective To investigate the effect of ultrasound-guided subcostal transverses abdominis plane (TAP) block with dexmedetomidine after laparoscopic radical operation. Methods 40 patients underwent laparoscopic radical operation for colorectal cancer were randomized into dexmedetomidine group (group DEX) and control group (group CON). All the patients received ultrasound-guided subcostal TAP block after operation, Group DEX with dexmedetomidine 1 μg/kg and 0.25% ropivacaine to 20 ml, and group CON with 0.25% ropivacaine 20 ml. All the patients were assessed with Ramsay scores and the pain at rest and on coughing were assessed with Visual Analogue Scale (VAS), 2, 6, 12, 24 and 24 hours after operation. The highest level and the duration of sensory blockade, the first time and the total times of pressing the analgesia pump in the first day after operation, and the requirements of sufentanil were recorded. First flatus time, first diet time and the length of hospital stay were compared. Results The scores of VAS were significantly less (P<0.001), and the Ramsay scores were more in the group DEX than in the group CON (P<0.01) 2, 6 and 12 hours after operation; with the longer time of sensory blockade (P<0.001), the later to first press the analgesia pump (P<0.001), the less frequence of pressing the analgesia pump (P<0.001), and less dosage of sufentanil (P<0.001). The first flatus time, first diet time were significantly earlier in the group DEX than in the group CON (P<0.001), with the less length of total hospital stay (P<0.001). Conclusion Dexmedetomidine can promote the anaesthesia of ultrasound-guided subcostal TAP block with ropivacaine and improve the recovery after laparoscopic radical operation.
4.The application of time to positivity in blood culture

Time to positivity(TTP) is a new parameter in blood culture field.The article shows us the concepts of TTP,differential time to positivity (DTTP),and introduces their relation with bloodstream infection (BSI),catheter-related bloodstream infection (CRBSI).Besides,it stresses TTP' s clinical application,including determining the severity of disease;identifying the isolates whether pollution or not; identification of isolate strains ; detection of the drug resistance of isolates ; evaluating the effect of antibiotic ; helping to adjust the therapeutic drug; diagnosing or excluding CRBSI by means of DTTP;deciding whether the catheter is the source of infection in patients with candidemia; understanding the epidemiological distribution of strain.At the same time,the article also describes the shortcomings and domestic current status.
5.Titanium specimens with different surface character affect the proliferation and IL-6 and Cbfα1 expres-sion in osteoblasts

Objective:To investigate the effects of titanium spcimens with different surface character on the proliferation and mRNA expression of IL-6 and Cbfα1 in osteoblasts.Methods:Titanium surface was treated by smooth pretreatment(PT),sandblast and acid etch(SLA)and anodic oxidation(AD)respectively.The morphology and the elements analysis of the spcimens were inspected and detected by SEMand EDS.The surface contact angle was measured by contact angle meter.MC3T3-E1 cells were cultured on the titanium surface and cells cultured on tissue culture plate were served as the control group.The proliferation was measured by MTT assay.The mRNA expression of IL-6 and Cbfα1 was quantified by RT-qPCR.Results:The sample surface in PT group showed scrat-ches,in SLA group showed multiple three dimensional structure,in AD group exhibited porous structure.The elements of the sample surface of group PT,SLA and AD were Ti,Ti/Al and Ti/O respectively;the contact angles were 54.47°±3.33°,75.42°±8.32° and 38.91 °±4.00°respectively(P<0.05).The cells in AD group showed higher proliferation than those in PT and SLA groups(P<0.05).In AD group IL-6 mRNA expression decreased and Cbfα1 mRNA increased more than in PT and SLA groups(P<0.05). Conclusion:Titanium spcimens treated with AD may promote cell proliferation,decrease IL-6 mRNA expression and increase Cbfα1 mRNA expression in MC3T3 cells.Implats treated with AD might have some advantages in early osseointegration.
6.Comparative analysis between ultrasonic characteristics and pathology of phyllodes tumors of the breast

ObjectiveTo investigate the ultrasonic features of phyllodes tumors of the breast (PTB) with different pathologic types.MethodsTwo-dimensional ultrasound and CDFI were performed in 16 patients with pathologically proved PTB,and the ultrasonic characteristics of different pathologic types PTB were reviewed retrospectively.ResultsAccording to WHO criteria,there were 12 benign PTB,2 borderline and 2 malignant PTB.Associated breast fibroadenoma was detected in 7 patients,including 6 benign and 1 borderline PTB.All of 16 PTB had clear boundary.Benign PTB had regular or irregular shapes,homogeneous or unhomogeneous internal echo and posterior acoustic enhancement in a few cases.Most borderline and malignant PTB showed regular or irregular shapes,mixed cystic and solid internal echo or irregular internal echo,as well as posterior acoustic enhancement.No calcification of PTB was found in all 16 patients.Conclusion The ultrasonograghic features of PTB with different pathologic types are characteristic to some extent,therefore being helpful to the differential diagnosis of PTB.
7.Increase of correctness in revascularization of coronary artery disease by first assessment of coronary computed tomography angiography

Objective To discuss the influence of coronary computed tomography angiography(CCTA)on correctness of assessing revascularization in patients with coronary artery disease. Methods A retrospective study method was conducted,605 cases underwent CCTA before coronary angiography(CAG)from 2008 to 2009 in Chinese PLA General Hospital were selected as CCTA before CAG group,and meanwhile 616 cases examined by CAG directly were selected as direct CAG group. Patients with multiple procedures of CAG were excluded. The proportions of various treatment strategies were compared,including per-cutaneous coronary intervention(PCI),coronary artery bypass grafting(CABG),medical therapy(MT),normal rate of CAG and the correctness of assessing revascularization between the two groups. Results The comparison between the baseline of the two groups showed that in the CCTA before CAG group,there were more severe lesions than those in the direct CAG group,such as Syntax score(11.31±8.90 vs. 10.23±9.73,P<0.05). Compared with direct CAG group,the triage of PCI and CABG in the CCTA before CAG group was significantly increased〔PCI:65.3%(395/605)vs. 57.1%(352/616),CABG:16.5%(100/605)vs. 3.4%(21/616)〕,while the percentages of medical treatment and normal CAG were obviously reduced〔medical treatment:11.7%(71/605)vs. 19.3%(119/616),normal rate of CAG:6.4%(39/605)vs. 20.1%(124/616),all P<0.01〕. With the guidance of CCTA,the correctness of assessing revascularization was increased〔81.8%(495/605)vs. 60.6%(373/616),P<0.01〕. Conclusion Compared with the direct induction by CAG,the CCTA examination carried out before CAG is capable of increasing the rate of correctness in the determination of revascularization in coronary heart diseases.
8.Effect of rhDCN on the expression of HLA molecules on HepG2 cell lines

Objective To observe the regulatory effect of recombinant human DCN on the expression of HLA class Ⅰ, Ⅱ molecule on hepatoma carcinoma cell HepG2 and investigate the relatively immune mechanism of human DCN on enhancing anti-tumor effect. Methods After transfected HepG2 cells with pcDNA3.1 (+)-DCN by liposome transfection, the expression of HLA class Ⅰ , Ⅱ molecule and the apoptotic indexes were analyzed by the flow cytometer. Results The expression of HLA class Ⅰ , Ⅱ molecule on the HepG2 cells transfected with pcDNA3.1 (+)-DCN was up-regulated with the apoptosis indexes being significantly higher than that of other control groups. Conclusion The human DCN can induce HepG2 apoptosis and up-regulate the expressions of HLA class Ⅰ , Ⅱ molecule which may contribute to its antitumor effect.
9.Impact of individualized diabetes education on disease awareness and blood glucose control in type 2 diabetic patients

This study was to evaluate the effect of individualized health education on blood glucose control in type 2 diabetic(T2D)Patients.Two hundred and thirty-eight T2D patients were randomly received individualized diabetes education(intervention group)or general education(control group).At 48 week,change of blood glucose control from baseline was observed.The blood glucose level and HbA1c in the intervention group were lower than those in the control group[(7.1±0.8)vs (9.3±6.5)mmol/L,(7.0 ±1.3)%vs(8.0%±1.0)%;P<0.05].Thus,individualized diabetes education might be helpful for blood glucose control in T2D patients.

4-Hydroxybenzoate (4HBA) is an important chemical compound used for synthesis of liquid crystal. Production of 4HBA from renewable resources is an effective mean to solve problems such as environmental pollution and petroleum shortage. This review briefly introduces the chemical synthesis of 4HBA from oil compounds, and mainly describes the progress in 4HBA biosynthesis from renewable resources by plants and microorganisms. In most intriguing aspect of plant-based synthesis of 4HBA is the appeal of directly synthesizing a chemical from CO2. However, the glucosylation system in plant cells converting 4HBA to glucose conjugates, causing the post treatment a problem. The recombinant microorganisms produce pure 4HBA, but less efficient. A new strain of Microbulbifer has ability to naturally accumulate 4HBA from glucose. Elucidation of the metabolic pathways and regulation systems would improve 4HBA synthesis efficiency.
