1.Challenges and solutions for the elderly care industry featuring combination of medical service and daily care in Anhui province

A picture of great market potential and mismatch of supply and demand in Anhui was drawn in the paper,with five challenges analyzed for the industry in general.The authors proposed to integrate medical resources and aging care resources,to build a diversified and tiered health care and aging care supply system,an integrated administrative coordination mechanism,and enhance fiscal support among other policy suggestions.These proposals were designed to encourage the development of the aging care industry featuring a combination of medical resources and aging care.
2.Echocardiographic diagnosis of congenital supravalvular aortic stenosis in children

Objective To study the echocardiographic characteristics and its diagnosis value on congenital supravalvular aortic stenosis (SVAS) in children. Methods Thirty-one patients with SVAS diagnosed by multiplane echocardiography were enrolled in the study. Their echocardiographic characteristics were compared with cardiac catheterization, operation, and gene detection results. Echocardiographic changes were mainly observed in aortic valve, supravalve, descending aortic arch, pulmonary artery valve, main pulmonary artery and its branches,and coronary artery. Results Of the 31 patients,26 had hourglass type SVAS,4 hypoplastic type,and 1 membranous type; 2 patients had extremely mild stenosis (defined as a Doppler gradient <25 mm Hg) ,20 mild (25~49 mm Hg) ,5 moderate (50~75 mm Hg) ,and 4 severe C>75 mm Hg) ones. Nineteen patients were diagnosed with Williams syndrome by gene detection. Three patients were associated with aortic valve stenosis including one missed at the initial diagnosis; 10(32.26%) patients with pulmonary stenosis, including pulmonary valve stenosis in 6, left and right pulmonary artery stenosis in 3 ,and branch stenosis in 1:6 patients with coronary stenosis. Conclusions The sternal border and five chamber apical views are the best to detect SVAS. Williams syndrome patients are prone to SVAS.Pulmonary stenosis echocardiography forms a great proportion of the SVAS patients. Routine examination is necessary for coronary stenosis in cases of SVAS.

Objective To compare the efficacy of supraclavicular approach and infraclavicular approach in ultrasound-guided brachial plexus block(BPB).Methods One hundred and twenty patients,ASA Ⅰ-Ⅲ,aged 18-80 yr,scheduled for upper extremity surgeries of both gender,were randomized into two groups(n =60):supraclavicular group(group SCB)and infraclavicular group(group ICB).The anesthetic mixture consisted of ropivacaine 0.375% and lidocaine 1% in equal volumes with epinephrine 1∶200 000,the total dose was 0.5 ml/kg.The block performance time,duration of anesthesia and success of anesthesia(surgery was accomplished without supplementary block)were recorded.A blinded observer assessed pinprick sensory block in the seven distal nerve territories(axillary,radial,musculocutaneous,median,ulnar,medial antebrachial and medial brachial cutaneous nerves)every 5 min up to 30 min after injection.Success of nerve block,side effects and complications were recorded during and after operalion.Results Group ICB was superior in success rate of anesthesia and nerve block of ulnar,medial antebrachial and medial brachial cutaneous nerves,the rate of parasthesia was lower and the block performance time was longer compared with group SCB(P < 0.05).No major complications occurred in both groups.Conclusion Under ultrasound guidance,infraclavicular BPB is superior to supraclavicular approach.
5.Relationship between pulmonary hypertension and serum uric acid level in patients with systemic sclerosis

Objective To explore the relationship between serum uric acid and pulmonary hypertension (PH) in patients with systemic sclerosis (SSc). Methods The echocardiography, electrocardiogram, nailfold videocapillaroscopy and laboratory parameters of 62 patients with SSc were retrospectively analyzed . Patients were divided into two groups according to presence of PH . Statistical analysis was performed using SPSS 17 software . Results Compared to patients without PH , patients with PH had significantly higher serum uric acid levels ( P < 0 . 01 ) , systolic pulmonary arterial pressure ( P < 0 . 01 ) , abnormality of electrocardiogram (P < 0.01), abnormality of nailfold video capillaroscopy and lower serum albunin levels (P < 0.01). Systolic pulmonary arterial pressure had correlation with Serum UA ( r = 0 . 26 , P < 0 . 01 ) as well as serum ablumin (r = -0.28, P < 0.03). Moreover, the mean value of serum UA was significantly different in two ECG groups (P < 0.01) and two nailfold videocapillaroscopy groups (P < 0.01). At the cutoff level of 374 μmol/L, serum uric acid had reasonable accuracy for predicting the presence of PH in SSc patients ( sensitivity 66 . 7% and specificity 84 . 0%) . Conclusion The serum uric acid may be useful as a practicable marker for predict PH in patients with SSc .
6.Effect of magnetic labeled endothelial progenitor cell transplantation on renal function of diabetic rats:a MRI imaging verification

BACKGROUND:Endothelial progenitor cel s have been shown to play an important role in the pathogenesis of traumatic diseases in recent years. OBJECTIVE:To explore the effect of magnetic labeled endothelial progenitor cel transplantation on renal function of diabetic rats through a MRI imaging study.METHODS:Sixty Wistar rats were randomly divided into normal (no treatment), control and experimental groups. Intraperitoneal injection of 40 mg/kg streptozotocin was performed to make a rat model of type 1 diabetes in the control and experimental groups. Four weeks after modeling, rats in the experimental group were given intravenous injection of magnetic labeled endothelial progenitor cel s (0.15 mL, 1×109/L). Fasting blood glucose, serum insulin, serum creatinine, urea nitrogen and 24-hour urinary protein levels in rats were measured at 8 weeks after cel transplantation. MRI was used to trace transplanted cel s in vivo in comparison with renal biopsy findings, and rat body mass and kidney weight were measured to calculate kidney weight index. RESULTS AND CONCLUSION:After modeling, fasting blood glucose, serum creatinine, urea nitrogen and 24-hour urinary protein levels as wel as kidney weight index were increased significantly (P<0.05), while the insulin level decreased (P<0.05). Compared with the model group, the endothelial progenitor cel transplantation reversed these indices (P<0.05). Additional y, in the experimental group, there was slightly longer T1 and shorter T2 signals as wel as marked lesion edge, and the FLASH sequence became more remarkable compared with the T2-weighted RARE sequence. The other groups showed no significant low signal changes. Magnetic-labeled positive cel s in the experimental group showed by the MRI were consistent with the tissue biopsy results, while no positive cel s were found in the model and normal groups. To conclude, the magnetic labeled endothelial progenitor cel transplantation can improve renal dysfunction in diabetic rats to a certain extent.
7.Evidence-based Study on Probiotics for Ulcerative Colitis

OBJECTIVE:To evaluate the efficacy of Probiotics in the treatment of ulcerative colitis(UC).METHODS:The recent pertinent literature about the use of Probiotics in the treatment of UC was retrieved from PubMed and Medline and the methodology and literature quality were evaluated in accordance with the evaluation criteria for the quality of literature of evidence-based medicine.RESULTS & CONCLUSION:The majority of the studies showed that Probiotics are of positive value for the remission of the US at acute stage and maintaining of remission induction of drugs in the treatment of UC.Strictly designed and randomized controlled trials(RCTs)remain to be done to tackle the problems encountered by the micro-ecological preparations as a new therapy in the treatment of UC.
8.Chemical constituents in butanol extraction from stems and leaves of Arachis hypogaea

Objective To investigate the chemical constituents in the stems and leaves of Arachis hypogaea.Methods The isolated compounds Ⅰ—Ⅳ were obtained by the combination of silica gel,ODS-18,and Sephadex LH-20 column chromatographies.Structural elucidation was conducted by the moedrn spectral method.Results Seventeen compounds were isolated and identified from the water extraction,and there were four norsesquiterpene compounds from butanol extraction.They are arachiside A(Ⅰ),cucubalactone(Ⅱ),roseoside(Ⅲ),and citroside A(Ⅳ).Conclusion Compound Ⅰ is a new compound named arachiside A and compounds Ⅱ—Ⅳ are isolated from the plants of Arachis Linn.for the first time.
9.Chemical constituents in leaves of Piper laetispicum

Objective To study the chemical constituents in the leaves of Piper laetispicum.Methods Compounds were separated and purified by silica gel column and macroporous resin.Their structures were identified on the basis of spectral methods.Results Sixteen compounds were isolated and identified from the leaves of P.laetispicum:(2E,4E)-N-isobutyl-11-phenylundecadienamide(Ⅰ),(2E,4E)-N-isobutyl-15-phenylpentadecadienamide(Ⅱ),laetispicine(Ⅲ),(2E,4E)-N-isobutyl-7-(3',4'-methylene-dioxyphenyl)heptadienamide(Ⅳ),brachystamide A(Ⅴ),dihydropipercide(Ⅵ),pipermacramide A [(2E,4E)-N-isobutyleicosadienamide,Ⅶ],(+)-spathulenol(Ⅷ),d-sesamin(Ⅸ),piperitol(Ⅹ),vitexin(Ⅺ),vitexin-2″-O-?-D-glucoside(ⅩⅡ),N-p-coumaroyltyramine(ⅩⅢ),?-sitosterol(ⅩⅣ),stigmasterol(ⅩⅤ),and 1-dotriacontanol(ⅩⅥ).Conclusion Compounds Ⅰ and Ⅱ are isolated for the first time as natural substance,compounds Ⅴ,Ⅵ,and Ⅷ are first isolated from P.laetispicum,compounds Ⅺ,ⅩⅡ,and ⅩⅥ are obtained from the plants of Piper L.for the first time.
10.Concentration, purification and drying of Yuxianling Granules under aqueous extr action alcohol precipitation

AIM: To specify optimization of the concentration, purification and drying for prepar ati on of Yuxianling Grannles and decrease the dosage on condition that is retentive of potency in order to provide a basis for mass production. METHODS: The contents of ferulic acid and the amount of extract wer e used as marker, the optimium of concentration, purification and drying for pr eparation were selected by orthogonal design and contrast test during aqueous ex tract alcohol precipitation process. RESULTS: Condition was optimum of herb-to-extract ratio in the range of 0.5 ∶1, pr ecipitation with ethanol containing 40% water and drying temperature not to exce ed 60℃ . CONCLUSION: The highest yield of active priciples is obtained by c ondition above.
