1.Association betweenHER-2 gene polymorphism and protein expression in breast cancer

Background and purpose:Pathogenic gene polymorphism may affect the function of gene, leading to the difference of individual tumor susceptibility and heterogeneity of bioactive substances in individuals. The purpose of this study was to investigate the interrelationship betweenHER-2 gene polymorphism and its protein expression, and to evaluate their association with the clinicopathological characteristics of breast cancer.Methods:The data from a total number of 303 female breast cancer patients of Han ethnicity were collected. The MassARRAY platform was used to examineHER-2 gene rs2517954 and rs2517955 single nucleotide polymorphisms. Meanwhile immunohistochemistry was used to detect HER-2 protein expression and corresponding estrogen receptor (ER), progesterone receptor (PR), P53 and Ki-67 expressions in breast cancer tissues. Pearson chi-square test was used to study the relationship of the two loci and the protein expression, and their correlation with clinicopathological features of breast cancer was analyzed.Results:Under the codominant model,HER-2 gene rs2517954 and rs2517955 loci polymorphisms were associated with its protein expression (χ2=9.613,P=0.008;χ2=9.613,P=0.008). And under the dominant model,HER-2 gene rs2517955 loci TT homozygous and CT heterozygous mutant was associated with its protein expression (χ2=8.894,P=0.003). There were no signiifcant correlations betweenHER-2 gene rs2517954, and rs2517955 loci polymorphisms, and breast cancer patients’ clinical stage, tumor size, histological grade, lymph node metastasis, ER, PR, Ki-67 and P53 expressions (P>0.05).Conclusion:HER-2 gene rs2517955 loci polymorphism is correlated with its protein expression. Further studies may be helpful to elucidate the mechanism of HER-2 protein expression in breast cancer.
2.Effects of Berberine on Inflammatory Factors, Adipokines and Fatty Acid Metabolism in 3T3-L1 Adipocytes

Objective To observe the effects of berberine on inflammatory factors, adipokines and fatty acid metabo-lism in 3T3-L1 adipocytes, and to investigate the molecular mechanism underlying berberine’s role of improving insulin re-sistance. Methods mRNA level of inflammatory molecules, adipokines, key enzymes and protein in fatty acid metabolism in 3T3-L1 cells were determined by quantitative real time polymerase chain reaction (qRT-PCR) after cells were treated with different concentrations of berberine (0, 5, 10, 20, 40μmol/L) for 24 hours and with 10μmol/L berberine at different du-rations (0,4,8,24,48 h). These factors mainly included interleukin-6 (IL-6), tumor necrosis factor-α(TNF-α), leptin, adipo-nectin, visfatin, fatty acid synthase (FAS), acetyl-CoA carboxylase (ACC), adipose triglyceride lipase (ATGL) and adipocyte fatty acid binding protein (AFABP). Results In 3T3-L1 adipocytes, transcription level of IL-6, TNF-α, leptin, FAS, AT-GL, AFABP reduced with addition of berberine dosage at 10~40μmol/L(P<0.05)while visfatin mRNA level increased(P<0.05)compared with the control group. No significant difference was found in expression of adiponectin(P>0.05). Tran-scription level of IL-6, TNF-α, leptin, AFABP, ATGL, FAS decreased with time after 10μmol/L berberine intervention (8-48 h) compared with the control group(P<0.05). On the other hand, visfatin mRNA level increased(P<0.05)compared with the control group. Adiponectin mRNA decreased only after cells were treated with berberine for 48 h(P<0.05). No sig-nificant difference was found transcription of ACC between each groups treated with berberine(P > 0.05). Conclusion mRNA level of inflammatory factors, adipokines, key enzymes and protein in fatty acid metabolism in 3T3-L1 adipocytes can be affected by berberine and this effect depend on its dose and time . This might be the mechanisms underlying berber- ine to improve insulin resistance.
3.Related factors of pre-dialysis blood pressure variability in under-age patients undergoing maintenance hemodialysis

Objective To explore the blood pressure variability (BPV) in pediatric patients undergoing maintenance hemodialysis (MHD) and to assess the factors associated with pre-dialysis BPV (pre-HD BPV).Methods The pediatric patients who undergone regular dialysis for more than twelve months from Oct 2005 to Oct 2011 in hemo dialysis center of Guizhou Provincial People's Hospital were divided into high pre-HD BPV group and low pre-HD BPV group.Baseline characteristics,biochemical indexes and cardiac function parameters measured by echocardiography were collected in both groups and multiple linear regression analysis was performed.Results Pediatric patients in high pre-HD BPV group demonstrated significantly higher inter-dialytic weight growth rate (IDWG),pre-dialysis systolic blood pressure and average amount of dehydration than those in low pre-HD BPV group (P < 0.05),while significantly lower hemoglobin and albumin levels than those in low pre-HD BPV group (P < 0.05).Comparison among laboratory indicators,serum phosphorus and parathyroid hormone demonstrated significant difference between groups (P < 0.05).For all pediatric patients,pre-HD BPV was positively correlated with IDWG (β=0.165),pre-dialysis systolicblood pressure (β=0.259),and iPTH (β=0.187),while negatively correlated with hemoglobin level (β=-0.199).Conclusions Increasing IDWG,higher pre-dialysis systolic blood pressure,anemia and secondary hyperparathyroidism influence BPV in pediatric patients on MHD.
4.Clinical effect of high flux hemodialysis foRAECOPD patients complicated with renal insufficiency

Objective To explore effect comparison between high flux hemodialysis and conventional hemodialysis therapy on acute exacerbation chronic obstructive pulmonary diseases (AECOPD) patients with renal insufficiency.Methods Clinical data of 51 AECOPD combined renal insufficiency (BUN≥20 mmol/L,CREA≥400 μmol/L) cases admitted in ouRhospital and Guiyang First People′s Hospital from 2010 to 2015 were retrospectively analyzed.And were randomly divided into high flux hemodialysis (HFHD) group with 25 cases and conventional hemodialysis (CHD) group with 26 cases according to different dialysis method and dialysis,and the two groups before treatment all used the same internal medicine comprehensive treatment.Blood leukocyte count,neutrophil percentage,blood gas index[pH、SaO2、p(O2)],c-reactive protein (CRP) and interleukin-1 (IL-1),interleukin-6 (IL-6) and renal function (BUN and CREA) before and afteRtreatment were detected,and the difference of the above indexes before and afteRtreatment in both groups were compared.Results Leukocyte count,neutrophil percentage,CRP,IL-1,IL-6,BUN and CREA in two groups afteRtreatment were significantly decreased than treatment before (P<0.01),but HPHD group decreased more than CHD group (P<0.05).Blood and gas index in HPHD group and CHD group afteRtreatment fell,but there was no significant difference (P<0.05).Conclusion High flux hemodialysis (HPHD) has superioRcurative effect in the treatment of AECOPD and in removing inflammatory mediators,infection control,improving renal function,so it is worthy of clinical application.

6.Comparative analysis of influenza etiology surveillance in general hospital and children′s hospital

Objective By comparing and analyzing the etiological monitoring results of influenza in Xicheng District general hos-pital and children′s Hospital ,to explore the epidemic regularity of influenza in children ,so as to provide the basis for developing in-fluenza prevention and control measures .Methods A total of 40 swabs of influenza samples were collected every week from two sentinel hospitals ,and the virus RNA was extracted and detected by RT-PCR method .Results There was a statistically significant difference in positive rate of influenza pathogen surveillance between general hospitals and children′s hospitals (21 .8% vs .14 .9% ) (P<0 .001) .In the general hospital ,the positive rate was highest in people aged ≥60 years old(31 .3% ) .In the children′s hospital , the positive rate was highest in people aged >4—14 years old(26 .7% ) .Both hospitals had an epidemic peak of influenza from No-vember to December of next year ,it was typical influenza epidemic of the North area .But the children′s hospital had a epidemic trough in January and February .In both hospitals ,the circulating strains were H3 ,H1N1 ,type B ,the predominant type was H3 and type B .Conclusion Taking the general hospital as a reference ,the quality of influenza surveillance in the children′s hospital was relatively stable and reliable .Lack of the people aged >24— <60 years old and aged ≥60 years old in children′s hospital ,is the key reason of the statistically significant difference of the positive rate ,between the general hospital and the children′s hospital .Chil-dren′s influenza vaccination rate is higher than the other age group ,and the protective effect of vaccine is better .
7.Relationship of rest metabolic rate with anthropometry in maintenance hemodialysis patients

Objective To explore the feasibility of bioelectrical impedance analysis in assessing resting energy metabolism(REM)and the association of REM with anthropometry in hemodialysis patients.Methods Adult hemodialysis patients from 11 hemodialysis facilities in Guizhou Province were enrolled in this study.Bioelectrical impedance analysis was used to measure the rest metabolic rate(RMR)and body composition of 765 patients undergoing regular dialysis.A standard method was used to measure their height,weight,upper arm circumference,triceps skinfold thickness,left calf circumference and grip strength.The upper arm muscle circumference was also calculated.The level of RMR and body composition in hemodialysis patients were compared by gender grouping.The patients were then divided into four groups according to the RMR quartile.Spearman correlation analysis and multiple linear regression analysis were derived to analyze the relationship between RMR and anthropometry.Results The level of RMR in male patients was significantly higher than that in female patients[1591(1444,1764)Rcal/d and 1226(1104,1354)Rcal/d,P < 0.001].Compared with the first quartile of RMR group,upper arm circumference,upper arm muscle circumference,left calf circumference and grip strength in the fourth quartile group were increased significantly(all P < 0.05).Multiple linear regression analysis showed that height(β=0.572),left calf circumference(β=0.273),upper ann muscle circumference(β=0.092)and suffering from protein energy wasting(PEW,β=-0.184)were independent influential factors of RMR(all P < 0.05).Conclusions RMR levels in patients with maintenance hemodialysis are associated with left calf circumference and upper ann muscle circumference,which may become a new index to evaluate energy consumption and malnutrition in MHD patients.
8.Effects of 1,25-dihydroxyvitamin D3 on the expression of connective tissue growth factor and heat shock protein 47 in peritoneum fibrosis rats

Objective To investigate the expression of connective tissue growth factor (CTGF) and heat shock protein 47 (HSP47) in peritoneum fibrosis rats,and the mechanism of 1,25-dihydroxyvitamin D3 [1,25-(OH)2-VitD3] in inhibiting the peritoneum fibrosis.Methods Adult male Sprague-Dawley rats were randomly divided into 3 groups:control group (n=8),model group (n=8) and 1,25-dihydroxyvitamin D3 group (VitD3,n=8).The model of peritoneum fibrosis rats were induced by daily intraperitoneally injection of 15% chlorhexidine gluconate (CHX) 0.2 ml/d with 0.1% glucose for 4 weeks.Rats in VitD3 group were also treated with 1,25-(OH)2-VitD3 [i.p.6 ng· (100 g) 1 · d-1].Peritoneal transport function,renal function,peritoneum thickness and serum level of 25hydroxyvitamin D3 were detected.In vitro,primary cultured peritoneal mesothelial cells were divided into control group,high glucose group (HG,2.5%),CTGF siRNA intervention group (CTGF siRNA+HG),VitD3 intervention group (VitD3+HG) and combined intervention group (CTGF siRNA+VitD3+HG).Real-time PCR,Western blotting and immunofluorescence were applied to measure the expression of CTGF and HSP47,also ELISA was used to detect the protein level of FN in peritoneum and peritoneal mesothelial cells.Results Compared with control group,the peritoneal ultrafiltration in peritoneum fibrosis rats were significantly decreased (P < 0.05),the absorbance level of peritoneal fibrosis,peritoneum thickness,the rate of dialysate urea nitrogen and blood urea nitrogen (DUN/BUN) and the expressions of CTGF and HSP47 were increased (all P<0.05).After application of 1,25-dihydroxyvitamin D3,peritoneal fibrosis lesion was significantly improved,the peritoneum thickness,the expressions of CTGF and HSP47 were decreased (all P < 0.05).In vitro,2.5% high glucose induced-peritoneal mesothelial cells were respectively treated by CTGF siRNA,1,25-(OH)2-VitD3 and combined interventions,the expression of FN,CTGF and HSP47 was significantly lower than that in high glucose group (all P < 0.05).Conclusions The expression of CTGF and HSP47 is significantly increased in peritoneal fibrosis rats.1,25-(OH)2-VitD3 may ameliorate the progression of peritoneal fibrosis via reducing the expression of CTGF,decreasing the expression of HSP47 and FN.
9.Fabrication and pre-clinical application of patient-specific 3D silicone rubber bolus for chest wall

Objective To apply 3D printing technology to fabricate patient-specific silicone tissue compensators for the chest wall and compare the advantages and clinical characteristics between conventional bolus and 3D-printed PLA materials. Methods The chest wall data of two breast cancer patients undergoing mastectomy were obtained based upon the CT images. A patient-specific 3D printing silicone rubber bolus (3D-SRB) was designed and fabricated. The conformability of 3D-SRB,3D-PLA and conventional bolus to the chest wall were validated. Ecipse8. 6 planning system was adopted to statistically compare the dosimetric parameters of virtual plan with those after using three tissue compensators. Results The 3D-SRB was successfully designed and fabricated with a similar hardness to conventional bolus. During the process of validating conformability and radiotherapy planning,3D-SRB and 3D-PLA were superior to conventional bolus in terms of conformability to chest wall and planning dosimetric distribution.3D-SRB was advantageous in repeatability, conformability and comfortable experience compared with 3D-PLA. Regarding dosimetric parameters,3D-SRB yielded the highest repeatability with the virtual plan, followed by 3D-PLA and conventional bolus. Conclusion It is applicable to utilize 3D-SRB as the patient-specific compensators for the chest wall,which is of significance in clinical practice.

The medical insurance is provided by the Unified Health System (SUS) covering most of Brazilians and alsocombined with private insurance, and every Brazilan could get access to basic health services. The major diseaseas leading to death in Brazil are ischemic heart disease, stroke and lower respiratory infection. The incidence of AD, chronic kidney disease and diabetes are increasing in recent years. Traditional Chinese Medicine (TCM), which was represented by acupuncture, was introduced to Brazil in the early 19th. At present, acupuncture has been widely accepted and used, which was included in SUS. A number of universities have acupuncture courses. However, the specialities and advantages of TCM needs to be strengthened, the local practioners lack the understanding of TCM theory, the appropriate use of herbal medicine, and there lacks TCM education standards, all these limit the development of TCM. Thus, in order to promote the development and dissemination of TCM in Brazil, it is suggested to give full play of the characteristics of TCM in preventing disease, increase the research and development input of herbal medicine, standardize the education stystem of TCM, etc.
