2.THE EXPRESSION OF TELOMERASE IN ADRENAL CORTICAL TUMORS AND ITS CLINICAL SIGNIFICANCE

To study the telomerase expression in adrenal cortical tumors and its clinical signinificance. Telomerase expression was estimated in 8 cases of adrenal cortical carcinoma, 20 cases of adrenal cortical adenoma, 24 tissues adjacent to cancer, and 4 specimens of normal adrenal tissue by the PCR based telomeric repeat amplification protocol. Telomerase was found to be expressed positively in 7(87 5%) of 8 adrenal cortical carcinoma samples and 1(4 2%) of 24 tissues adjacent to tumor, while it was not expressed in all the 20 samples of adrenal cortical adenoma and normal adrenal tissue. The expression of telomerase in adrenal cortical tumors clearly demonstrated the malignant behavior of the tumor cells. Demonstration of telomerase expression appears to be a useful tool for the diagnosis of malignant adrenal cortical tumors.
4.A preliminary study of serum-deprivation-induced release of S100A13 and flbroblast growth factor 1 from thyroid cancer TT cell

Objective To investigate the effect of serum-deprivation on the changes of [ Ca2+ ] i and the protein release of S100A13 and fibroblast growth factor 1 ( FGF-1 ) in thyroid cancer TT cell,and to reveal the role of Ca2+in the protein release of S100Al3 and FGF-1.Methods The protein expressions of FGF-1 and S100A13 in TT cells under serum-deprivation were detected by Western blot.The released FGF-1 protein from TT cells in the supernatant fluid was detected by ELISA.Realtime dynamic examinations on the change of 1 h [ Ca2+ ] i in TT cells under serum-deprivation were detected by confocal laser scanning microscopy.Then,the effect of EGTA( 2.5 mmol/ L),BAPTA-AM (2.5 μmol/L)on distributions of the fluorescence of S100AI 3 and FGF-1 in TT cells under serumdeprivation for6 h were detected by indirect immunofluorescence.Results The expressions of FGF-1 and S100A13 in TT cells after serum-deprivation for4 h and 6 h were reduced( P<0.05 or P<0.01 ),but the released FGF-1 protein from TT cells in the supernatant fluid was elevated ( P<0.05 or P<0.01).Confocal laser scanning of Ca2+ imaging indicated that [ Ca2+ ] i of serum-deprivation TT cells maintained the relative stabilization within 23 win,but the rapid rise of [ Ca2+ ] i achieved peak value 1.6 μmol/L after 30 min,and remained stable for about 17 win,and thereafter 40 win slowly dropped to a low level From 40 win to 60 win the [ Ca2+ ] i was about 0.3-0.6 μ mol/L.The average [ Ca2+ ] i was higher than that in normal group,EGTA group,and BAPTA-AM group within 1 h.The protein expressions of S100A13 and FGF-1 did not drop obviously in EGTA group and BAPTA-AM group.Conlusion The release of S100A13 and FGF-1 from TT cell under serum-deprivation is possibly related with the change of [ Ca2+ ]i.Both Ca2+-chelating agents EGTA and BA PTA-AM are able to inhibit the rise of [ Ca2+ ] i and release of S100A 13 and FGF-1 from TT cells under serum-deprivation.
5.HPLC Determination of Chlorogenic Acid Content in Yinqiao Jiedu Oral Liquid

Objective To establish the HPLC determination method of chlorogenic acid for developing the quality standard of Yinqiao Jiedu Oral Liquid.Methods HPLC method was performed on an E.Merck LiChrospher 100RP-18 column(250 mm? 4 mm,5 ? m).The mobile phase consisted of acetonitrle and o.4 % phosphoric acid(10:100).The flow rate was 1.0 mL/min,column temperature was 30 ℃,and the detection wavelength was 327 nm.Results Chlorogenic acid was linear at the range of 0.12~ 0.63 ? g(r=0.999 6),the average recovery was 96.68 %,and RSD was 1.56 % .Conclusion This method is accurate,reliable,reproducible,and can provide evidence for the quality control of Yinqiao Jiedu Oral Liquid.
6.MRI assessment of mid-urethral ligament changes in female stress urinary incontinence

Objective To evaluate the MRI value in changes of mid-urethral ligament injury offemale stress urinary incontinence (SUI).MethodsComparison of MRI changes of mid-urethral ligament on 30 healthy female volunteers and 20 female SUI patients.Chi-square test was used to compare the form of SUI patient's mid-urethral support ligaments Results The female nid-urethral support ligaments were composed of 4 groups of ligaments,including the periurethral ligament and pubourethral ligaments (1 pair),and at both sides of the urethra's paraurethral ligaments (1 pair) and suburethral ligament lying dorsal urethra,connecting the urethra and pelvic arcus tendinous fasciae.In normal MRI,ligament was a thin strip and showed low signal on both T1WI and T2WI,T2WI sagittal and cross-section scan was the best combination to show the middle urethral support ligaments changes,with tension;6 patients (20%) in the 30 patients normal control group could be seen tortuously and slack like around the urethra ligaments.Twenty SUI patients mid-urethral support ligaments were performance laxity or rupture,rates were 39% (47/120) and 42% (50/120) (x2 =43.191,P < 0.05 ).On T2WI,the ligamentous laxity was floating,and losstension,also could performance one side extension and thinner than the other side.The ligament rupture was performance of the signal interruption,ligament contracture and one end of liganent attachment points separation.ConclusionMRIcanobjectiveeffectiveevaluatethemid-urethralsupportligaments' pathological changes in stress urinary incontinence patients.
7.Study of Abnormal Heart Rate Variability and Its Acupuncture Intervention in Anxiety Disorder

Heart rate variability (HRV) is a reliable, innocuous and sensitive new index to measure cardiac autonomic nervous function. Anxiety disorder is often accompanied by autonomic nerve dysfunction and its main sign is abnormal HRV. Acupuncture can affect HRV indices, correct abnormal HRV and improve cardiac autonomic nerve dysfunction to relieve anxiety. This article sorts out and analyzes recent years’ studies on abnormal HRV in anxiety disorder, the effect of acupuncture on HRV and acupuncture regulation of abnormal HRV in anxiety disorder to provide a therapeutic basis for clinical acupuncture intervention in abnormal HRV in anxiety disorder.
8.The value of narrow band imaging laryngoscope for laryngopharyngeal reflux disease.

OBJECTIVE:
To investigate the value of narrow-band imaging (NBI) laryngoscope for the diagnosis and therapy of the laryngopharyngeal reflux disease (LPRD).
METHOD:
Analyzed the NBI laryngoscope characteristic findings of the throats of 46 LPRD patients which confirmed the diagnosis according to the reflux symptom index (RSI) and reflux finding score (RFS) both positive and the proton pump inhibitor therapy effective.
RESULT:
The NBI laryngoscope findings as follows: erythema and edema between the arytenoid cartilages (71.7%), the epiglottis congestion (67.4%), pharyngeal isolation/integration erythema (65.2%), pharyngeal pebbles--like changes (65.2%), hypertrophy of the posterior commissure (52.2%), vocal cord erythema and edema (47.8%), vocal nodules or vocal polyps with erythema, or edema (39.1%), arytenoid cartilages edema and erythema (21.7%), ventricular edema, erythema and laryngeal ventricle disappeared (17.4%), granuloma (6.5%), ulcers (4.3%), false vocal cord ditch (4.3%).
CONCLUSION
LPRD has characteristic findings. NBI laryngoscope is easy to operate and has a high value on the diagnosis and the treatment of the LPRD.

9.Effects of enteral nutrition preparations on glucose and lipid metabolism in elderly diabetic patients

Objective To investigate the effects of enteral nutritional emulsions on glucose and lipid metabolism in elderly patients with diabetes. Methods This randomized controlled trail recruited 80 elderly diabetic patients whose MNA-SF scores were between 0-11 with dysphagia or inability for oral feeding and who were randomized into a TPF-DM group ,a TPF-D group ,a TPF-T group ,and a TPF group.Glycolipid metabolism indexes and nutritional indexes were measured before intervention and at 1 m ,2 m and 3 m after intervention. Results Compared with baseline levels , FBG and HbA1c declined in the TPF-DM group[(5.70 ± 0.69)mmol/L vs .(7.90 ± 1.71)mmol/L and (6.39 ± 1.11)% vs .(7.92 ± 1.50)%,all P<0.05]and the TPF-D group[(5.72 ± 1.12)mmol/L vs . (7.63 ± 1.73 )mmol/L and (6.86 ± 0.97 )% vs .(7.81 ± 1.04 )%,all P< 0.05 ]after 3 months of treatment ,but no significant difference was found between these two groups. The TPF-DM ,TPF-D , TPF-T groups showed significantly narrower fluctuation ranges of 24 h blood glucose levels after 3 months[(2.20 ± 0.88)mmol/L vs .(4.43 ± 2.07)mmol/L ,(2.94 ± 1.16)mmol/L vs .(4.22 ± 1.60) mmol/L ,(3.21 ± 1.64)mmol/L vs .(4.36 ± 1.88)mmol/L ,all P < 0.05).Serum albumin levels increased in all four groups ,compared with baseline levels (all P<0.05) ,but there was no significant difference between the groups(all P>0.05). Furthermore ,the serum HDLC level increased in all four groups than baseline (all P< 0.05). Conclusions Enteral nutritional emulsions containing slow release starch and fructose ,dietary fibers and proper ω6 and ω3 ratio can stabilize blood sugar ,regulate blood lipids ,and improve nutritional status.
10.Application of humanistic literacy and clinical practice ability training in postgraduate courses of geriatrics

Objective:To explore the application value of humanistic literacy and clinical practice ability training in postgraduate courses of geriatrics.Methods:Postgraduates in Zhejiang University School of Medicine were taken as research objects. The teaching curriculum combining humanistic literacy and clinical practice ability training was set as the course of experimental group, and the teacher-led teaching course was set as the course of control group. The questionnaire survey was conducted to the evaluate the teaching effect among the postgraduates from three aspects: classroom situation, knowledge application and ability training. SPSS 17.0 was used for t-test. Results:The teaching of combining humanistic literacy and clinical practice ability training in classroom atmosphere, participation, teacher-student communication, helping memory, knowledge comprehension, knowledge application, collaboration ability, expression ability, organizational ability, knowledge acquisition ability, and autonomous learning ability scores of the experimental group were (3.96±0.20), (3.92±0.28), (3.75±0.44), (3.92±0.28), (3.96±0.20), (3.96±0.20), (3.83±0.38), (3.92±0.28), (3.79±0.41), (3.88±0.34), and (3.83±0.38), which were better than those of the control group, especially in classroom atmosphere, participation, knowledge application, expression ability, and autonomous learning ability.Conclusion:The combination of humanistic literacy and clinical practice ability training can significantly improve the classroom atmosphere of geriatric postgraduate courses, enhance students' medical humanistic literacy, familiarize students with the core skills of geriatrics, and improve students' ability to express and learn independently.
