1.A STUDY OF GASTRIC MUCOSAL HISTOLOGY AND GASTRIC ACID SECRETION AFTER VAGOTOMY

3.5) .Ninteen patients with duodenal ulcer underwent HSV(12) or SV+A (7).BAO and PAO(pentagastrin)decreased by 76.8% and 60% after HSV as compared to 83.4% and 90.4% after SV + A . Geeral clinical postoperative results were appro-ximately the same in the two groups, wih 83.3 % and 86% visick I or II for HSV and SV + A respctively.Results of both animal experiments and clinical investigations showed that HSV caused neither histolo-gical changes in gastric mucosa nor the development or augmentation of gastritis. In contrast bath TV + PP and SV + A induced augmentation of gastritis (especially in the antrum and around the stoma) which might have resulted from reflux of duodenal contents. There was, howver, no relationship between reduction of acicse-cretion and the presence or absence of gatritis and its severity.No reduction of gastric glands were observed. There were no significant changes in size. shape and density of parietal cells postoperatively either. These results provided no histological evidence for post-vagotomy acid secretory reduction.
2.Expressions of Kidney β-Klotho and Fibroblast Growth Factor Receptor 1 in Experimental Rats Model With Vascular Calcification

Objective: To observe the impact of vascular calcification on kidney injury rats with the expressions of β-Klotho, fibroblast growth factor receptor 1 (FGFR1) in kidney tissue in order to find the predictor for early chronic kidney disease (CKD), to provide the prevention and investigation basis of vascular calcification and CKD. Methods: Vascular calcification model was induced by vitamin D3 and nicotine injection in experimental rats and the animals were divided into 2 groups: Normal control group and Calcification group. n=6 in each group. Serum levels of creatinine and urea nitrogen were examined by sarcosine oxidase method and UV-glutamate dehydrogenase method respectively; blood levels of calcium and phosphorus were detected by biochemistry method; kidney tissue alkaline phosphatases (ALP) activity was measured by ALP detection kit, protein expressions of β-Klotho and FGFR1 were assessed by ELISA.Results: Compared with Normal control group, Calcification group showed increased serum levels of creatinine (35.200±4.087) umol/L vs (26.000±5.0990) umol/L and urea nitrogen (6.900±0.623) mmol/L vs (5.400±0.803) mmol/L, both P<0.05; elevated kidney tissue ALP activity (60.510±31.090) U/g vs (26.590±8.664) U/g and β-Klotho protein expression (9.052±1.238) ng/mg vs (6.860±1.036) ng/mg, both P<0.05. Blood levels of calcium, phosphorus and kidney tissue FGFR1 protein content were similar between 2 groups. Conclusion: Large dose vitamin D3 and nicotine injection may induce vascular calcification and early CKD symptom in experimental rats; β-Klotho protein expression was significantly increased suggesting that β-Klotho had been involved in the early regulation of vascular calcification and it could be used for the early diagnosis of CKD at certain point.
3.Construction and expression of inset expression vector pIZT/V5-His harboring mIL-4

Objective To establish stable insect cell line expressing mIL4 on the basis of construction of inset expression vector pIZT/V5-His.Methods Amplified mIL-4 and pIZT/V5-His were treated with EcoRI and XbaI,and mIL-4 were ligated into pIZT/V5-His vector using T4 DNA ligase.Sf9 cells were transfected with recombinant DNA and ELISA was employed to detect soluble mIL-4 production by transfected Sf9 cells.Results Transfected Sf9 cells could significantly produce mIL4(1 mg?L~(-1)) compared!with control group(0.003 mg?L~(-1)).Conclusion Inset expression vector pIZT/V5-His is an ideal expression vector for mIL-4 production in transfected cells.
4.Construction of prokaryotic expression vector harboring different domains of STAT6

Objective To explore the influence of different domains of STAT6 expressed by the vector pGEX-6p-2 on signal transduction.Methods Different domains of STAT6 were amplified by PCR technique and ligated respectively into the prokaryotic expression vector.Recombinant DNA was transformed into HB101 cells.Transformants were lysed by supersonic method and lysates run on SDS-PAGE.Results Western blotting analysis showed a high level expression(60 000,56 000 and 50 000) of different domains of STAT6 SHST,DST and NST,and the empty vector only had GST expression.Conclusion Expression vector pGEX-6p-2 can be used for high level expression of different domains of STAT6.
5.Morphological changes of sinusoidal endothelial cells in the patients with chronic hepatitis B

Objective To observe the morphological changes of sinusoidal endothelial cells(SECs) in the patients with chronic hepatitis B, and to study the relationship between the SECs changes and the development of hepatic microcirculation disorders. Methods The liver biopsy tissues from fifty three cases with hepatitis B were observed with light microscope and transmitted electronic microscope. Results The morphologies of SECs in patients with chronic hepatitis B changed significantly. The main manifestations included decreased sizes and reduced numbers of penestrate on SECs, basal membrane formation, cellular connection development between SECs, occurrence of WP bodies in SECs and the disappearance of SECs. The intimate contact occurred between SEC and lymphocyte or Kupffer cell. Conclusions The morphologies of SECs in patients with chronic hepatitis B develop significant change, which might be the initial step in the development of hepatic microcirculation disturbances.
6.Relationship between pain and nerve fibers distribution in multiple endometriosis lesions

Objective To investigate the relationship between the distribution of nerve fibers in multiple endometriosis lesions and pelvic pain.MethodsFrom Sept.2007 to Sept.2008, 120 endometriosis patients treated in Peking Union Hospital were enrolled in this study, which including 19 cases with stage Ⅰ , 29 cases with stage Ⅱ , 44 cases with stage Ⅲ and 28 cases with stage Ⅳ.The pain symptom was evaluated by visual analogue scales(VAS) score and nerve fibers in multiple endometriosis lesions were detected by immunohistochemical staining.Results The number of nerve fibers in multiple endometriosis lesions were (29.74 ± 17.33)/mm~2 in uterosacral ligament, (24.53 ± 13.34)/mm~2 in vaginal septum, (17.09 ± 10.09)/mm~2 in uterus rectum crux, (6.77 ± 4.21)/mm~2 in peritoneal endometriosis lesions, (0.07 ± 0.25)/mm~2 in endometriosis ovarian cyst wall.The number of nerve fibers in uterosacral ligament was mostly correlated with the degree of pain (r = 0.56).The nerve fibers of uterus rectum crux and vaginalseptum were correlated with defecation pain (r = 0.58 and 0.41) and dyspareunia (r = 0.82 and 0.67),which were significantly higher than those in endometriosis leision in peritoneum and ovary.There was no significant different number of nerve fibers among different stage disease (P > 0.05).Conclusion There was significantly different distribution of nerve fibers in multiple endometriosis lesions, which was correlated with dysmenorrhea, anus pain, dyspareunia and chronic pelvic pain, not with clinical staging.
7.Expression of IL-1βand IL-17 on mast cells in human periapical diseases

AIM:To identify and quantify the expression of IL-1βand IL-17 in mast cells ( MCs) in different types of human pericapical diseases using double immunofluorescence staining .METHODS: The specimens ( n=102 ) , including healthy control ( n=35 ) , periapical cyst ( n=35 ) and periapical granuloma ( n=32 ) , were involved in the present study .The tissue samples were fixed in 10% buffered formalin for at least 48 h and then embedded in paraffin . Serial 5-μm-thick sections were deposited onto SuperFrost/Plus microscope glasses .Routine staining of the sections using hematoxylin&eosin ( HE) was performed for morphological evaluation .The number of IL-1βand IL-17 positive MCs was identified by double immunofluorescence staining .RESULTS:Compared with the healthy controls , the inflammation score of periapical lesions was significantly increased in the periapical patients (P<0.01).The density of IL-1βand IL-17 posi-tive MCs in the periapical lesions were obviously higher than that in the healthy controls (P<0.01).However, no signifi-cant difference between periapical cyst and periapical granuloma was observed .The Pearson correlation analysis showed that there was a positive correlation between the density of IL-1βand IL-17 double positive MCs and inflammation score in dif-ferent groups of specimens (P<0.01).CONCLUSION:There is significantly increased number of MCs , along with in-creased density of IL-1βand IL-17 positive MCs in human periapical lesions .The increased density of IL-1βand IL-17 positive MCs has the similar tendency as the severity of tissue inflammation in human periapical lesions , suggesting that IL-1βand IL-17 positive MCs may play an important role in the pathogenesis of human periapical diseases .
8.Safety and efficacy of low-dose recombinant tissue plasminogen activator in Chinese patients with acute ischemic stroke

Objective To investigate the safety and efficacy of low-dose recombinant tissue plasminogen activator (rtPA) in Chinese patients with acute ischemic stroke. Methods Consecutive patients with acute ischemic stroke received rtPA intravenous thrombolysis within 4. 5 h after symptom onset were enrol ed retrospectively. According to the dosage of rtPA, they were divided into either a low-dose group (0. 5-0. 8 mg/kg, the maximum dose 50 mg) or a standard dose group (0. 9 mg/kg, the maximum dose 90 mg). The incidences of symptomatic intracranial hemorrhage ( sICH) ( the National Institute of Neurological Disorders and Stroke [NINDS], and European Cooperative Acute Stroke Study Ⅱ [ECASSⅡ] , European Safe Implementation of Thrombolysis in Stroke-Monitoring Study [ SITS-MOST ] ), 90 d mortality, 90 d good outcome (the modified Rankin Scale [mRS] score 0-1), and life self-care ability (mRS score 0-2) were compared between the two groups. Results A total of 163 patients were enrol ed, including 74 patients in the low-dose group and 89 in the standard dose group. The constituent ratios of hypertension (44. 6% vs. 68. 5%; χ2 =9. 490, P=0. 002) and diabetes (5. 4% vs. 28. 1%; χ2 =14. 216, P<0. 001) in the patients of the low-dose group were significantly lower than those of the standard group, while the constituent ratios of smoking (56. 8% vs. 38. 2%; χ2 =5. 590, P=0. 018 ) and smal artery occlusive stroke (21. 6% vs. 10. 1%; χ2 =4. 122, P=0. 042 ) in patients of the low-dose group were significantly higher than those of the standard group. After adjusting for age, National Institutes of Health Stroke Scale (NIHSS) score, hypertension, diabetes, smoking, and ischemic stroke typing, multivariate binary logistic analysis showed that there were no significant differences in NINDS defined sICH (10. 8% vs. 9. 0%, odds ratio [OR] 1. 077, 95%confidence interval [CI] 0. 338-3. 436), ECASS Ⅱdefined sICH ( 9. 5% vs. 9. 0%; OR 0. 976, 95% CI 0. 296- 3. 221 ), SITS-MOST defined sICH (8. 1% vs. 4. 5%; OR 2. 269, 95% CI 0. 522-9. 852), 90 d mortality (17. 6% vs. 14. 6%; OR 1. 720, 95% CI 0. 578-5. 119), 90 d good outcome (35. 1% vs. 32. 6%;OR 0. 780, 95% CI 0. 356-1. 709), and life self-care ability (48. 6% vs. 42. 7%;OR 0. 936, 95% CI 0. 441-1. 987) between the 2 groups (al P>0. 05). Conclusions The program of intravenous thrombolysis for the treatment of Chinese patients with acute ischemic stroke with low-dose rtPA is safe and effective.
9.Response of potassium channels to estrogen and progesterone in the uterine smooth muscle cells of adenomyosis in vitro

Objective To investigate the expression of potassium channels and the influence of estrogen and progesterone on the cultured uterine smooth muscle cells (USMC) of adenomyosis in vitro.Methods There were 22 cases of adenomyosis hysterectomy in the adenomyosis group and 12 patients with cervical intraepithelial neoplasia Ⅲ removal of the uterus in the control group.USMC were separated and cultured in vitro, incubated with different concentrations of estrogen and progesterone.We used reverse transcription-PCR to dectect the expression of large-conductance calcium-and voltage-sensitive potassium channel α subunit (BKCa α) and voltage-gated potassium channel 4.3 (Kv4.3).Results The mRNA expression of BKCa α and Kv4.3 in the adenomyosis group (4.43 ±2.05 and 4.52± 1.97) were significantly higher than those in the control group (0.83±0.25 and 0.86±0.19, P<0.05).In the control group, Kv4.3 mRNA decreased after treated with 0.1 nmol/L (0.17±0.10) and 1.0 nmol/L (0.13±0.08) estrogen than before (0.55±0.29, P<0.05).In the adenomyosis group, BKCa α mRNA decreased significantly after treated with 10.0 nmol/L estrogen (0.56±0.27 versus 1.01±0.35, P<0.05).0.1 μmol/L progesterone elevated both BKCa α mRNA (0.44±0.24 versus 0.16±0.09) and Kv4.3 mRNA (1.29±0.51 versus 0.55±0.29) in the control group (all P<0.05);however, there were no significant difference in adenomyosis group of different concentration of progestrone (P>0.05).Conelusuion There is an abnormal expression of potassium channels in the adenomyosis USMC, which is regulated by high concentration of estrogen and might be resistant to progesterone.
10.Preoperative immune state of gastric cancer patients

Objective: To explore the relationship between the preoperative immune state and the progress, the prognosis in patients with gastric carcinoma. Methods: The NK cell activity (lactdehydrogenase releasing),the level of T cells subgroup (FACS) were detected preoperatively. The data of intra operative findings, pathologic observations and the results of clinical follow up were also studied. Results: The preoperative immune function in cancer patients was significantly reduced compared with the control group, and it had a significantly negative correlation with the stage of the cancer, but a positive correlation with the prognostic results. Conclusion: The detection of the preoperative immune state can play an important role in the estimation of the disease progress and the prognosis in cancer patients.
