[Objective]To discussthe clinic effective and the application value of the fibula fixation in treating the complex and comminuted fracture of the tibiofibular.[Method]At first,we reduced and fixed up the fibula fracture anatomically as the support to reduce the tibia fracture indirectly,then we fixed up the tibia fracture by using intramedullary nail or external fixators.[Result]In the 23 cases of the tibiofibular fracture,10 cases of the fibula fracture were almost anatomically reduced,13 cases were reduced functionally,the wounds were cured without parenchyma infection or bone infection.The follow-up time was 13 months averagedly.The time of clinical healing was from 13 weeks to 21 weeks.The cases which were excellently cured was 20,which were well cured was 2,which was tolerably cured was 1 by the Johner-Wrushs grade standard.[Conclusion]This method was designed reasonably,operated simply,acceded with the minimal-invasive and biological rule of the bone fractuer treatment,profited to the heal of the fracture,it was a prefect mean to cure the complex and comminuted fracture of the tibiofibular.

OBJECTIVE To reduce the cross infection during gastroscopy and probe into the(administrative) measures in preventing infection.METHODS The hidden infection of sterilizing gastroscope was analyzed in order to work out the measures to prevent infection.RESULTS Choosing the correct methods of sterilizing gastroscope could(avoid the) incomplete sterilization and reduce the cross infection.CONCLUSIONS Be strict in sterilizing(procedure)s and protecting measures,and prevent the interactive infection in the hospital during gastroscopy.

[Objective]To explore the curative effect of the three diamen sional external fixation frame and limited internal fixation in treating the comminuted fracture of the shaft of humerus.[Method]Twenty-eight patients of the comminuted fracture of the shaft of humerus were treated from Jan 2000 to May 2006.According to the AO/ASIF,there were 5 cases of the B1 type,3 cases of the B2 type,9 cases of the B3 type,6 cases of the C1 type,4 cases of the C2 type,1 case of the C3 type.All of them were treated by the three diamensional external fixation frame and limited internal fixation of the screws or wires,and all of them were given the reasonable rehabilitation training at 3 day after the operation.The curative effect of the operation,the complications,the fracture healing and the function appraisal were analyzed.[Result]The 28 patients were followed-up for 10～15 months(average 13 months),27 cases had the frature healed and the healing rate was 96.4%.The healing period of the fracture lasted from 12 weeks to 33 weeks,average 19.5 weeks.Only 1 case was not healed.The functions of the shoulder and the elbow were normal,the excellent rate was 92.8%.[Conclusion]The three diamensional external fixation frame and limited internal fixation is a better method in treating the comminuted fracture of the shaft of humerus.It fits the rule of minimal-invasive-biology. The operation is simple,and the fixation is reliable,the function is satisfactory.

Objective To screen and identify proteins that interact with receptor for advanced glycation end products (RAGE) with T7 select phage display system. Method GST-tagged fusion protein of RAGE was coated on glutathione suporflow resin and binding screens were performed with human lung T7 phage library. Results After 6 rounds of biopanning, 64 independent plaques were scraped and processed with EDTA. The inserted fragments in these plaques were amplified by PCR and the PCR products were purified by a gel recovery method. The sequences of the insertions were identified and analyzed with BLAST program in GeneBank. Twenty-five clones of encoding proteins were found. Conclusions T7 select phage display system is a convenient, rapid and effective method for screening binding proteins. This study provides an approach to explore complicated functions and novel mechanisms of RAGE-related signal transduction pathways.

Objective To know the situation of upper GI diseases among the residents in Dong tai city. Methods With three levels examination methods of Occult Blood Bead Detector,Gastroscope and pathological examination,residents aged over 30 yrs in Dongtai were screened during Aug 2005 to Sept 2006. Results 70 809 residents,about 6.13% of whole city population,were taken the screen with occult blood bead detector. There were about 28 392 people positive; the positive rate was 40.10%. Among the positive people,there were 8 121 people taking gastroscope inspection,and while 2 294 of them also took the pathological examination. The gastroscope results showed that 4 636 people suffered from chronic superficial gastritis,1 431 rotten gastritis,565 esophagus gastritis,339 chronic & atrophic gastritis,24 suspicious cancers,53 stomach cancer,44 esophagus cancer. Of these 97 cases with cancers diagnosed by gastroscope,87 cases were confirmed by pathological examination,the false positive rate of gastroscope examination was about 10.31%; Among 2 197 of the non-cancer subjects diagnosed by gastroscope,there were 25 cases with cancer diagnosed by pathological method,the false negative rate of gastroscope was 1.14%. For those 112 cancer cases diagnosed by pathology,there were 83 cases (74.11%) who were in the early or middle phase of cancer. Conclusion The prevalence of upper GI diseases in Dongtai City was higher. It should strengthen the education and the improvement of the health situation.

Objective:To establish a determination method of Bergenin content in Dicha Kechuan oral liquid by HPLC.Methods:The separation was performed on SHIMADZU VP-ODS C18 column(150mm?4.6mm,5?m),the mobile phase was carbinol-water(20:80) with a flow rate of 1.0ml/min.The column temperature was room temperature and the detection wavelength was 275nm.Results:The linear range of Bergenin was 0.2575-5.150?g,the regression equation of working curve was Y=812702.2X-80626.3,r=0.9998,the average recovery was 101.4% and RSD 1.67%(n=5).Conclusion:This method is simple,reproducible,and can be used for quality control of Dicha Kechuan Oral Liquid and the semifinished product.

Objective To investigate the abnormal characteristics of electrocardiogram and its early diagnostic value in acute pulmonary embolism (PE) of different positions.Methods A total of 147 hospitalized patients of acute PE diagnosed by the pulmonary artery CT angiography (CTA) were enrolled in this study and divided into the following two groups:pulmonary trunk or main pulmonary artery (MPA) embolism (group A) and lobar artery or remote branch embolism (group B).ECG,D-dimer,BNP,cTnT were collected and determined,the varieties of abnormal ECG were counted.Then,the relationships between the severities of the PEs at different positions and the corresponding ECG abnormalities as well as the degree of right ventricular hypertrophy (RVH) were analyzed.Results There were significant differences in dyspnea,syncope,in-hospital mortality and the level of cTnT,BNP between the two groups (P ＜ 0.05).There were significant differences in the occurrence of SIQ Ⅲ T Ⅲ,right bundle branch block (RBBB),ST segment depression (STD) in leads Ⅲ and aVF,ST segment elevation (STE) in lead aVR,negative T waves (NTWs) in leads Ⅲ and aVF,STD in leads V1-V3/V6,and STE in leads V1-V3 in combination with STD in leads V4-V6 between the two groups (P ＜ 0.05).The proportion of RVH diagnosed via ECG has significantly different between the two groups.The result of correlation analysis showed that the incidence of pulmonary trunk or MPA embolism was significantly related to the number of ECG abnormalities (r =0.782,t =-7.086,P ＜ 0.05).Conclusions The number of abnormal ECGs increase and the RVH is more serious when PE occurring in pulmonary trunk as well as in the MPA,early recognition of electrocardiographic abnormalities is of greater value in the diagnosis of acute pulmonary trunk and MPA embolism.

OBJECTIVE To investigate genes associated with the drug-resistance of ?-lactam antibiotics in Pseudomonas aeruginosa(PAE) isolated from clinical patients. METHODS ?-Lactamase genes including TEM,SHV,OXA-10,PER,VEB,GES,CARB,IMP,VIM,SPM,GIM,DHA,FOX,MOX and oprD2 were detected by PCR amplification in 33 PAE isolates. RESULTS TEM,SHV,GES,CARB and VIM genes were positive in 100%,6.1%,6.1%,9.1% and 12.1% of 33 isolates,respectively.The deletion of oprD2 gene was found in 22 isolates.Other ?-lactamase genes were absent in all isolates.By PCR amplification,DNA sequencing and BLASTn comparison analysis,the CARB genes of 2 strains were demonstrated to be CARB-3 and the VIM genes of 2 strains were VIM-2. CONCLUSIONS P.aeruginosa carries various beta-lactamase genes in clinical PAE patients,and the deletion ratio of oprD2 gene is high.
