1.Clinical Study of Chromosomal Abnormalities by Puncturing Umbilical Cords

Objective:To evaluate the application of puncturing the umbilical cord under the ultrasound guidance in prenatal diagnosis,clinical high risk factors of fetal chromosomal abnormalities were discussed.Methods:283 pregnant women who had the percutaneous umbilical cord blood sampling were recruited and the karyotyping was done.The detection rate of chromosomal abnormalities and correlate factors in different subgroups of prenatal diagnosis were analyzed.Results:The success rate of puncture was 100%.Major complications were bleeing at the puncture site(33 cases,11.67%)and fetal heart brachycadia(5 cases,1.77%).25 chromosomal abnormalities were found and the detection rate was 8.83%.There was significant correlation between the couple's chromosomal abnormalities and the fetal's(r=22.348,P=0.000).Conclusions:The ultrasound guided percutaneous umbilical cord blood sampling proves to be valid in the prenatal diagnosis,especially when couples have chromosomal abnormalities.
2.Study for the influence of vulvovaginal candidiasis on the pregnancy outcomes

Objective To investigate the incidence of vulvovaginal candidiasis(VVC)in the third trimester,and the effect on the of perinatal outcome.Methods Vaginal discharge samples of 300 gravida were examined and cultured for fungus.According to the clinical feature,the patients were divided into slight VVC group and heavy VVC group.The termination of pregnancy and perinatal outcome were recorded 42 days after delivery.Results The incidence of VVC was 34%.There was significantly higher prevalence of premature rupture of membrane(PROM),afterbirth adherence,premature labor,break of perinaeum when intrapartum,disruption of wound after intrapartum and infection of newborn in the group of heavy VVC,with significant difference as compared with non-VVC group (P<0.01)but with no difference in puerperal infection(P>0.05).Conclusion Heavy VVC is disadvantageousto the mothers and their babies in gravida,and the therapy of VVC in pregnant women should be conducted actively before and after delivery.
3.Expression and significance of Th1/Th2 cytokines in ankylosing spondylitis

Objective To investigate the changes and significance of Th1/Th2 cytokines in ankylosing spondyhtis (AS).Metbotis Serum and synovial fluid levels of TNF-α,IL.17.IL-10 and IL-4 were detected by en-zyme linked immunosorbent assay(ELISA)in AS cases.Results The serun levels of TNF-α,IL-17 and IL-10 were significantly higher,while the serum level of IL-4 Was significantly lower in AS group than in normal group.The levels of TNF-α.IL-17 and IL-4 in synovial fluid were significantly higher than in serum.Conclusion Disequilibrium exists in Th1/Th2 cytokine network of AS.and there is a strong predominance of Th1.
4.Association of PITX3 polymorphism with Parkinson's disease in Chinese patients

ObjectiveTo investigate the relationship between polymorphism in the PITX3 gene and hereditary susceptibility of Parkinson's disease (PD). MethodsThree PITX3 single nucleotide polymorphisms ( SNPs ),including rs2281983,rs4919621 and rs3758549 were examined in 509 late-onset PD patients ( LOPD ),290 early-onset PD(EOPD) and 494 healthy controls.Genotyping was carried out in all subjects using a ligase detection reaction( LDR).ResultsAllele and genotype frequencies did not differ between the 799 PD patients and 494 controls ( P values of genotype were 0.494,0.343,0.951 ; P values of allele were 0.369,0.297,0.823 ),between 509 LOPD patients and 494 controls ( P values of genotype were 0.522,0.350,0.630 ; P values of allele were 0.413,0.328,0.571 ),between 290 EOPD patients and 494 controls ( P values of genotype were 0.499,0.492,0.552; P values of allele were 0.321,0.301,0.931 ),and between 509 LOPD and 290 EOPD patients ( P values of genotype were 0.577,0.710,0.127 ; P values of allele were 0.346,0.472,0.077 ) for all three SNPs (rs2281983,rs4919621 and rs3758549).There were no association petween the three PITX3 SNPs and PD.ConclusionThree PITX3 SNPs do not contribute to the risk of developing PD in Chinese population.
5.Effect of Lactobacillus delbrueckii fermented supernatant on Candida albicans biofilm formation

Objective To investigate the effect of Lactobacillus delbrueckii fermented supernatant on Candida albicans biofilm and explore its possible mechanism and effective components.Methods Lactobacillus delbrueckii was isolated from vaginal Lactobacillus capsules (Ding Junsheng) and identified by mass spectrometer and sequencing.Lactobacillus delbrueckii fermented supernatant was prepared.The effect of Lactobacillus delbrueckii fermented supernatant on Candida albicans biofilm was tested by microplate crystal violet staining method and catheterization tube modeling method.Biofilm morphological changes were analyzed by crystal violet staining and Live/ Dead fluorescent dyes.Exopolysaccharides of Candida albicans biofilm was measured by the sulphruic acid-phenol method.The effect of Lactobacillus delbrueckii fermented supernatant on the adherence of Candida albicans biofilm was tested by the method of colony counting on plate.The fermented supernatant was treated with NaOH,catalase,typsin and proteinase K respectively.Then the treated fermented supernatant's effect on Candida albicans biofilm formation was detected.One-way ANOVA or Wilcoxon Rank Sum Test was used as statistical analysis for quantitative data.Results Lactobacillus delbrueckii was successful ly isolated and identified from Ding Junsheng.Lactobacillus delbrueckii fermented supernatant could significantly and dose-dependantly inhibited the formation of Candida albicans biofilm(F=10.804,P=0.000).The morphological changes showed that the fermented supernatant could decrease the formation of biofilm.When treated with Lactobacillus delbrueckii fermented supernatant,Candida albicans biofilm expolysaccharides was significantly inhibited (F=17.140,P=0.000) and adherence ability was reduced.Compared to untreated fermented supernatant (A490=0.486±0.112),the biofilm inhibition effect of fermented supernatant treated with NaOH (A490=0.675 ± 0.095),catalase(A490=0.577 ± 0.118),typsin(A490=0.600 ± 0.044) and proteinase K (A490-0.495±0.084)only decreased slightly,but when compared to the control group (A490=1.079 ± 0.158),the treated fermented supernatant still showed significant biofilm inhibition effect (x2=26.052,P=0.000),which suggested that the effective components of fermented supernatant were organic acid and bacteriocin-like substance.Conclusions Lactobacillus delbrueckii fermented supernatant can effectively inhibit Candida albicans biofilm formation.The action mechanisms are related with expolysaccharides decreasement and adherence reduction.The function components of fermented supernatant are possibly organic acids and bacteriocin-like substance.
6.Clinical characteristics and associated factors for depression in patients with Parkinson's disease

Objective:To investigate the characteristics and associated factors of depression in elderly patients with Parkinson's disease(PD).Methods:A total of 1138 elderly PD patients from Xuanwu Hospital of Capital Medical University were consecutively enrolled.The Hamilton Depression 17-item Scale(HAMD-17)was used to diagnose depression(scores ≥14). The depressive characteristics of PD patients with depression and those without depression were compared, and Logistic regression model was used to analyze the clinical risk factors of depression in PD patients.Results:Among the 1 138 PD patients, 233 cases had depression.The incidence of depression in PD patients was 20.5%, and the treatment rate was only 6.0%(14 cases). The main symptoms of depression in PD patients included sleep disturbance(101 cases, 43.3%), depression(57 cases, 24.5%), work and interest loss(49 cases, 21%). Compared to PD patients without depression, PD patients with depression were more likely women(49.4% vs36.3%), and had lower cognitive scores[(25.56 ± 4.22)scores vs(27.07 ± 3.08)scores], higher rate of freezing of gait(51.0% vs24.0%), higher incidence of disability(87.1% vs28.5%)( P<0.05). A comparison of each part of the unified Parkinson's disease rating scale(UPDRS)between the two groups revealed that the scores of activities of daily living[(16.52 ± 8.71)scores vs(10.15 ± 5.48)scores], the motor examination[(33.78 ± 19.48)scores vs(22.87 ±13.08)scores]and the complications of therapy[2.0(0.0, 6.0)scores vs0.0(0.0, 2.0)scores]were higher in the PD patients with depression group than in the PD patients without depression group( P<0.05). Logistic regression analysis showed that women( OR=1.532, 95% CI: 1.073-2.187, P=0.019), disability( OR=6.357, 95% CI: 4.399-9.186, P<0.001), activities of daily living evaluated by UPDRS( OR=1.093, 95% CI: 1.043-1.146, P<0.001)and motor complications( OR=1.100, 95% CI: 1.014-1.193, P=0.022)were independent risk factors for PD depression. Conclusions:Depression was common in PD patients and usually manifested as sleep disturbance and low motivation, women, motor complications, disability and decreased quality of daily living were independent risk factors for depression.
7.Effect of gold micro needle radiofrequency combined with syringe needle subsicion therapy on scar release operation

Objective:To explore the efficacy of gold micro needle radiofrequency combined with syringe needle subsicion therapy in the treatment of facial acne scars.Methods:A total of 58 patients with facial depression acne scars admitted to our hospital from May 2018 to May 2020 were selected as subjects. There were 28 cases in the control group and the rest was the experimental group. The control group was treated with gold micro needle radiofrequency, and the experimental group was given a small syringe needle on the basis of the control group. After the treatment, the treatment effect and delayed construction period were observed.Results:According to the results of two independent samples Wilcoxon rank sum test, the treatment results of the two groups were different ( Z=2.742 and P=0.006). The effect of experimental group (mean rank 34.74) was better than that of control group (mean rank 23.88). According to =0.05 level, there was no statistically significant difference between the two groups in terms of missed work time after three treatments. There was no significantly difference between the two groups. Conclusions:Gold micro needle radiofrequency combined with syringe needle subsicion therapy for the treatment of facial acne scar has a satisfactory effect, and it is worthy of clinical promotion.

In women with preeclampsia (PE), endothelial cell (EC) dysfunction can lead to altered secretion of paracrine factors that induce peripheral vasoconstriction and proteinuria. This study examined the hypothesis that PE sera may directly or indirectly, through human umbilical vein ECs (HUVECs), stimulate phospholipase C-gamma1-1,4,5-trisphosphate (PLC-gamma1-IP3) signaling, thereby increasing protein kinase C-alpha (PKC-alpha) activity, collagen I expression and intracellular Ca2+ concentrations ([Ca2+]i) in human umbilical artery smooth muscle cells (HUASMCs). HUASMCs and HUVECs were cocultured with normal or PE sera before PLC-gamma1 silencing. Increased PLC-gamma1 and IP3 receptor (IP3R) phosphorylation was observed in cocultured HUASMCs stimulated with PE sera (P<0.05). In addition, PE serum significantly increased HUASMC viability and reduced their apoptosis (P<0.05); these effects were abrogated with PLC-gamma1 silencing. Compared with normal sera, PE sera increased [Ca2+]i in cocultured HUASMCs (P<0.05), which was inhibited by PLC-gamma1 and IP3R silencing. Finally, PE sera-induced PKC-alpha activity and collagen I expression was inhibited by PLC-gamma1 small interfering RNA (siRNA) (P<0.05). These results suggest that vasoactive substances in the PE serum may induce deposition in the extracellular matrix through the activation of PLC-gamma1, which may in turn result in thickening and hardening of the placental vascular wall, placental blood supply shortage, fetal hypoxia-ischemia and intrauterine growth retardation or intrauterine fetal death. PE sera increased [Ca2+]i and induced PKC-alpha activation and collagen I expression in cocultured HUASMCs via the PLC-gamma1 pathway.

9.Application of tranexamic acid in pre-hospital emergency care of patients with craniocerebral trauma

Objective To investigate the feasibility and clinical effect of tranexamic acid in prehospital emergency care of patients with craniocerebral trauma.Methods A randomized,placebo controlled trial was carried out on 77 craniocerebral trauma patients [Glasgow Coma Scale (GCS) ≤12 points]enrolled between May 2015 and December 2016.There were 45 males and 32 females,with an average age of 36.5 years (range,19-73 years).Among the patients,37 cases were caused by traffic accidents,19 falling from high places,11 falling when walking,and ten by being hit.According to the random number table method,they were divided into control group (39 cases) and treatment group (38 cases).The treatment group received 1 g of tranexamic acid by intravenous injection within ten minutes on the scene and another 1 g of tranexamic acid within eight hours at the hospital.The control group received 0.9% isotonic saline.The operation and medication followed the routine process.The arrival time of ambulance and the time of first medication use were recorded.The plasma fibrin degradation products (FDP) and D-dimer at admission and 1 d post-trauma,the percentage of cranitomy operation,case fatality rate,red blood cell transfusion,length of stay in the neurosurgical intensive care unit (NICU),and the Glasgow Outcome Scale (GOS) at day 28 were all recorded and analyzed.Results No significant differences were found between the two groups in gender,age,injury causes,GCS,arrival time of ambulance,and the time of first medication use (P > 0.05).The FDP and D-dimer at admission of the two groups were similar (P > 0.05).One day after admission,the expression of plasma FDP was significantly lower in treatment group than that in control group [6758 (4732,13661) μg/L vs.11740 (8516,21756) μg/L] (P < 0.01).The expression of D-dimer was significantly lower in treatment group than that in control group [1074 (849,1414) μg/L vs.1722 (1389,2330) μg/L] (P < 0.01).Between group differences were insignificant in the percentage of craniotomy operation,case fatality rate,and red blood cell transfusion (P > 0.05).However,treatment group showed shorter stay in NICU [4 (1,12)days vs.2 (0,4)days] and higher GOS [4 (3,5)points vs.5 (4,5)points]than control group (both P < 0.01).Conclusions Tranexamic acid can be applied conveniently in pre-hospital emergency care of craniocerebral trauma patients.It can effectively realize the synchronization of pre hospital transport and treatment,eventually reducing the time of NICU treatment as well as improving the prognosis.
10.Diagnostic characteristics and survival analysis of screen-detected colorectal cancer in Shanghai,2013-2015

Objective:To analyze and compare the survival rate between patients with screen-detected and non-screening detected colorectal cancer in Shanghai from 2013 to 2015. Methods:Patients with screen-and non-screening detected colorectal cancer from 2013 to 2015 were collected from Shanghai Colorectal Cancer Screening Program and the Population-Based Cancer Registry.The results presented were based on data collected by December 31,2020.Survival rates were stratified by year of diagnosis,gender,site,age-group,stage at diagnosis and histopathologic category when analyzed.The 5-year observed survival rates were calculated based on the life table,and then the cumulative expected survival rates were calculated according to the Ederer Ⅱ method.Finally,the 5-year relative survival rate was calculated.The COX proportional hazard regression model was used for the multivariate analysis. Results:2 108 patients with screening-detected colorectal cancer and 14 154 patients with non-screening colorectal cancer from 2013 to 2015 were included in the analysis,and the proportions of phase Ⅰ were 35.53%and 9.33%,respectively.The stage distribution of colorectal cancer was significantly different between patients with screen-detected and non-screening detected colorectal cancer(P<0.05).The 5-year relative survival rates of patients with screen-detected and non-screening detected colorectal cancer were 84.66%(95%confidence interval:82.87%-86.45%)and 63.51%(95%confidence interval:62.64%-64.38%),respectively.The patients with screen-detected colorectal cancer had a significantly improved 5-year relative survival rate in comparison with the patients with non-screening detected colorectal cancer.The survival rates of females in both groups were higher than those of males.The relative survival rate decreased with the increase of age and gradually decreased with the increase of stage at diagnosis.The relative survival rate of patients with non-screening detected colon cancer was significantly lower than that of patients with rectal cancer. Conclusion:Patients with colorectal cancer found at screening had a significantly improved survival rate compared to patients with non-screening detected colorectal cancer.Staging at diagnosis is a key factor,which indicates that enhancing screening and early diagnosis has important meaning to further improve the survival of patients with colorectal cancer and reduce the burden of disease.
