Objective To investigate the levels of serum tumor necrosis factor-alpha (TNF-?琢) and soluble tumor necrosis factor receptor (sTNF-R) and sTNF-R/TNF-?琢 ratios in patients with severe lupus nephritis (SLN) and the impact of double pulse therapy with methylprednisolone (MP) combined cytoxan (CTX). Methods Serum TNF-?琢, soluble tumor necrosis factor receptor type Ⅰ(sTNF-RⅠ) and soluble tumor necrosis factor receptor type Ⅱ (sTNF-RⅡ) levels were determined in 38 cases of SLN patients and 35 health controls by double antibody sandwich enzyme-linked immunosorbent assay (ELISA) before and after the double pulse therapy with MP and CTX. Anti-dsDNA antibody was detected by ELISA. Antinuclear antibody (ANA) was detected by indirect immunofluorescence experiments. Complement C3 and complement C4 were detected by rate nephelometry. Objective To investigate the effects of microencapsulated NGF expressing NIH3T3 cells on the repair of peripheral nerve defect in rats.Methods Thirty SD male rats with 10 mm defect of sciatic nerve were randomly divided into three groups(n=10).Group A: The nerve defect was bridged with heterogeneous nerve,the neural regeneration room was formed and filled with microencapsulated NGF expressing NIH3T3 cells.Group B: Autogenous nerve grafting was performed.Group C: The nerve defect was bridged with heterogeneous nerve,the neural regeneration room was formed and filled with microencapsulated NIH3T3 cells.The walking track analysis,electrophysiological and morphological observation were performed 12 weeks after operation.Results There were no significant differences of sciatic nerve function index,nerve concluctive velocity,histological changes of regenerative nerve,maturation degree of regenerative nerve fiber between group A and group B 4,8,12 weeks after operation;group A and B were surperior to group C(P

Objective To explore the efficacy of retroperitoneal laparoscopy in the treatment of parapelvic cyst.Methods From August 2005 to December 2008,16 cases of parapelvic cyst were treated by retroperitoneal laparoscopic deroofing and decompressing in our hospital.Under general anaesthesia,an operational space was established via retroperitoneal route.And then,along the urethra,the parapelvic cyst was explored.Afterwards,the cyst was unroofed 3 to 5 mm from the normal renal tissues.Results The operation was completed in 15 of the patients with a mean operation time of(75?20) minutes and mean blood loss of(28?8) ml.No injuries to the kidney pedicle or pelvis occurred.Another patient was converted to open surgery because of difficulty in exposure of the cyst.This series were followed up for a mean of 13 months(range,6 to 20 months).No one had recurrence during the period.Conclusions Retroperitoneal laparoscopic deroofing is minimally invasive,safe and effective or parapelvic cyst.It should be the first choice for the disease.

Objective To investigate effect of curcumin on NT-pro-BNP, IL-6, IL-10 and pulmonary function in patients with sepsis induced by lung injury.Methods 40 patients with purulent septic lung injury were collected in the hospital and divided into control group and experimental group according to different treatment, 20 cases in each group.The control group were given conventional treatment and the experimental group were given curcumin capsule on the basis of control group for a consecutive treatment of one weeks.After treatment, the serum levels of NT-pro-BNP, IL-6, IL-10 and pulmonary function were detected of two groups.ResuIts Compared with control group after treatment, serum IL-6 and IL-10 levels of patients in experimental group decreased significantly ( P<0.05 ) , serum NT-pro-BNP levels in experimental group decreased significantly ( P<0.05 ) , vital capacity (VC), maximal voluntary ventilation (MVV), and forced expiratory volume in one second (FEV1) levels in experimental group increased significantly (P<0.05).ConcIusion Curcumin could significantly reduce serum NT-pro-BNP, IL-10 and IL-6 levels in patients with lung injury, improve the pulmonary function, and has a guiding significance for clinical use.

Objective To evaluate the efficacy of Xuebijing injection plus oral acitretin for the treatment of erythrodermic psoriasis.Methods Forty-eight patients with erythrodermic psoriasis were equally and randomly divided into two groups by a random number table:test group treated with Xuebijing injection once a day plus oral acitretin,and control group treated with oral acitretin.The dose of acitretin began at 0.5 mg per kilogram per day,and was modified according to the tolerance in and response of patients.After 8 weeks of treatment,clinical efficacy was evaluated by psoriasis area and severity index (PASI) score,response rate and recurrence rate.Adverse reactions were also recorded and evaluated.Results The difference in PASI score between pre-and post-treatment was significantly higher in the test group than in the control group (34.9 ± 2.2 vs.27.3 ± 1.7,t =3.37,P ＜ 0.05).The total response rate was 87.5％ in the test group and 62.5％ in the control group (x2 =4.87,P ＜ 0.05).There was a statistical decrease in the average onset time ((13.5 ± 2.4) d vs.(20.7 ± 3.1) d,t =3.67,P ＜ 0.05),daily dose and total dose of acitretin ((26.4 ± 3.3) mg vs.(34.7 ± 3.5) mg,(1854.5 ± 85.2) mg vs.(2768.8 ± 88.7) mg,t =3.07,4.32,respectively,both P ＜ 0.05) in the test group compared with the control group.The recurrence rate was 9.5％ (2/21) in the test group and 26.6％ (4/15) in the control group (x2 =5.23,P ＜ 0.05).Conclusion In the case of erythroderma psoriaticum,Xuebijing injection combined with oral acitretin is superior to oral acitretin alone in clinical efficacy,onset time and reducing recurrence.

BACKGROUND:A large number of clinical trials have found that the number of bone marrow stem cels at the femoral neck and proximal femur in patients with osteonecrosis of the femoral head is significantly reduced, accompanied by decreased activity, which causes a significant decrease in osteogenic capacity that the necrotic bone cannot be effectively repaired after absorption, leading to the colapse of the femoral head. OBJECTIVE:To probe into the early clinical efficacy of autologous peripheral blood mononuclear cel transplantation with porous core decompression for treatment of avascular necrosis of the femoral head. METHODS:Forty-five patients with avascular necrosis of the femoral head (49 hips) were enroled in this study, and underwent autologous peripheral blood mononuclear cel transplantation with porous core decompression. After treatment, pain scores, Harris hip score, scores on the satisfaction of patients were evaluated, as wel as X-ray, CT and MRI examinations. RESULTS AND CONCLUSION:Al the patients received a folow-up visit of 11-14 months, averagely (12.5±0.6) months. During the folow-up, there were no complications and serious adverse reactions. Postoperative pain scores and Harris scores were both improved significantly compared with preoperative ones (P < 0.05). At 12 months after treatment, the excelent satisfaction rate was up to 92%. Patient’s MRI low signal region accounting for a percentage of the volume of the femoral head was decreased from (40.1±7.34)% preoperatively to (20.23±5.4)% at 6 months postoperatively, and there was a significant difference (P < 0.05). These findings indicate that autologous peripheral blood mononuclear cel transplantation with porous core decompression for treatment of avascular necrosis of the femoral head has significantly clinical effects at early stage, which can obviously reduce joint pain, improve and restore hip joint function, and delay progression of disease.

Objective To explore the role of interleukin (IL)-1β in the pathogenesis of gout.Methods Real-time quantitative polymerase chain reaction (RT-qPCR) and enzyme-linked immunosorbent assay(ELISA) were used to measure the expression of IL-1β mRNA in peripheral blood mononuclear cells (PBMCs) and IL-1β in plasma samples from 120 acute gouty (AG) arthritis,70 chronic gouty (CG) arthritis,80 intercritical gouty (IG) arthritis patients and 96 healthy control subjects respectively.Results The expression of PBMCs IL-1β mRNA and plasma concentration of IL-1β were both much higher in gout patients than those in controls (P ＜ 0.01,respectively).And the plasma levels of IL-1β mRNA and IL-1β significantly increased in the AG group compared with CG and IG groups (P ＜ 0.01,respectively) and much higher in the CG group than those in the IG group.Positive correlations existed between plasma concentration of IL-1β and the levels of white blood cell,neutrophil,monocyte,erythrocyte sedimentation rate,blood uric acid,globulin and PBMCs IL-1β mRNA (P ＜ 0.01,respectively) while negative correlation between plasma IL-1β and plasma level of apolipoprotein in gout patients (P ＜ 0.05).Conclusion Elevated plasma level of IL-1β may be involved in the pathogenesis of acute and chronic gouty arthritis.
