1.Comparison of accuracy of SVV, CVP and PAWP in monitoring changes in blood volume in patients undergoing renal transplantation

Objective To compare the accuracy of stroke volume variation (SVV),central venous pressure (CVP) and puhnonary arterial wedge pressure (PAWP) in monitoring the changes in blood volume in the patients undergoing renal transplantation.Methods Sixteen patients with chronic renal failure,of American Society of Anesthesiologists physical status Ⅱ or Ⅲ,aged 18-55 yr,scheduled for elective allograft renal transplantation under general anesthesia,were enrolled in the study.SVV was continuously monitored with the FloTrac/Vigileo monitor,and CVP,PAWP and stroke volume index (SVI) were continuously monitored with the volumetric pulmonary artery catheter during surgery.The parameters of hemodynamics were recorded at 30 min after induction of anesthesia,5 min before renal artery opening,5 and 30 min after renal artery opening,and at the end of surgery.Hydroxyethyl starch 130/0.4 electrolyte solution 6 ml/kg was infused over 15 min via the central venous catheter to perform fluid responsiveness starting from 30 min after induction of anesthesia.Positive fluid responsiveness was defined as the change in SVI ≥ 15%.The relationship between SVV and CVP and between SVV and PAWP was analyzed using the Pearson correlation analysis.The receiver operating characteristic curve for CVP,SVV and PAWP in monitoring the changes in blood volume was drawn,and the area under the curve was calculated.Results Compared with the value at 5 min before renal artery opening,SVV was significantly increased after renal artery opening (P<0.05),and no significant change was found in CVP and PAWP after renal artery opening (P>0.05).SVV was negatively correlated with CVP,and r=-0.82 (P<0.01);SVV was negatively correlated with PAWP,and r=-0.77 (P<0.01).The area under the curve of SVV in monitoring the changes in blood volume was 0.87,and of CVP and PAWP was 0.69 and 0.66,respectively.Conclusion SVV provides better accuracy than CVP and PAWP in monitoring the changes in blood volume in the patients undergoing renal transplantation.
2.Expressions and relativity research of Cyclic B1 and Cyclic D1 in severe reflux esophagus, Barrett's esophagus and esophageal adenocarcinoma

Objective To explore the role of Cyclin B1 and Cyclin D1 on Barrett esophagus,Barrett's esophagus mixed with atypical dysplasia and esophageal adenocarcinoma.Methods Cyclin B1 and Cyclin D1 were examined with immunohistochemistry.76 esophageal tissues of patients werB collected,including severe reflux esophagitis(RE,25 cases),Barrett esophagus(BE,35 cases),Barrett esophagus mixed with atypical dysplasia(DY,8 cases),esophageal carcinoma(CA,8 cases).Ten cases with normal esophageal mucosa were examined as the control. Results Cyclin B1 and Cyclin D1 were high expression in the specimens of the BE,DY and CA groups and very low expression in the control and RE group.Statistieal difference Was showed(P<0.01).Expression of Cyclin D1 was increasing gradually from the tissues of intestinal metaplasia,atypical dysplasia to adenocarcinoma(50.04 vs 67.94 vs 74.31).There Was significant difference among these three groups(P<0.01).Conclusion Cyclin B1 and Cyclin D1 as markers of tumour development could evaluate the risk from Barrett esophagus to adenocarcinoma.Perhaps it is the earlv event in the development of esophageal carcinoma.
3.Microvessel density and expression of VEGF and AR in the prostates of men who received re-operation after TURP for benign prostatic hyperplasia

Objective To discuss microvessel density (MVD) and expression of vascular endothelial growth factor(VEGF), androgen receptor(AR) in the prostates of men who received re-operation after TURP. Methods Fifty cases were performed re-TURP (re-TURP group) and the remaining 50 cases served as controls. 150 specimens were collected. Sections were stained for CD34 and VEGF, AR by immuno-histo-chemistry(S-P). Statistical analysis of the results was performed using t-test or Pearson Chi-Square test Results The expression of VEGF, AR and MVD were significantly higher in the re-TURP group compared to controls(P<0. 05),but in re-TURP group, difference in VEGF and AR expression as well as MVD were not found to be significantly different between the first and the second TURP(P>0.05). Conclusion Over expression of VEGF and AR as well as high MVD in prostatic tissue might play an important role in the pathological process of BPH after TURP.
4.Predictive effect of serum CDH13 methylation for progression after TURBT in patients with non-muscle invasive bladder cancer

Objective To investigate the predictive effect of serum CDH13 methylation for the progression after TURBT in the patients with non-muscle invasive bladder cancer.Methods Ninety-eight patients with non-muscle invasive bladder cancer treated by TURBT in this hospital from January 2010 to January 2012 were selected.The methylation specific PCR was used to detect the methylation status of serum CDH13.Then its correlation with the clinicopathological data as well as postoperative progression situation was analyzed.Results Serum CDH13 methylation was detected in 52 cases (53.1%),moreover serum CDH13 methylation was closely correlated with tumor size,grade and number (P<0.05).During follow-up,20 cases (20.4%) appeared the tumor progression.The Kaplan-Meier analysis and log-rank test found that the patients with serum CDH13 methylation had shorter progression-free survival rate than the patients without serum CDH13 methylation,and the difference was statistically significant (P=0.007).The Cox regression analysis showed that serum CDH13 methylation was an independent risk factor for the progression after TURBT in non-muscle invasive bladder cancer.Conclusion Serum CDH13 methylation can serve as a predictive indicator of the progression after TURBT in non-muscle invasive bladder cancer.
5.Effect of transcutaneous electrical acupoint stimulation on intracranial pressure in patients undergoing laparoscopic hysterectomy: measurement of optic nerve sheath diameter

Objective:To evaluate the effect of transcutaneous electrical acupoint stimulation (TEAS) on the intracranial pressure in patients undergoing laparoscopic hysterectomy by measuring the optic nerve sheath diameter (ONSD) via ultrasound.Methods:Forty-two American Society of Anesthesiologists Physical Status classification Ⅰ or Ⅱ patients, aged 18-60 yr, with a body mass index of 18-28 kg/m 2, scheduled for elective laparoscopic hysterectomy, were divided into 2 groups ( n=21 each) using a random number table method: control group (group C) and TEAS group (group T). In group T, TEAS was applied to the Yintang (EX-HN3) and Taiyang (EX-HN5) acupoints at 30 min before anesthesia, the stimulation used alternating dense-disperse waves at a frequency of 2/100 Hz, with the current intensity starting at 1 mA and increasing to the maximum tolerable level just below the pain threshold, and stimulation was maintained at the Hegu (LI4), Quanliao (SI18), and Fengchi (GB20) acupoints during surgery until the procedure was completed. Patients had electrode pads applied to the corresponding acupoints without electrical stimulation in group C. The ONSD was measured upon entry into the operating room (T 0), 3 min after anesthesia induction (T 1), 5 min after trendelenburg position (T 2), 30 min after trendelenburg position (T 3), 60 min after trendelenburg position (T 4), and immediately after the end of operation (T 5). Results:Compared to the baseline at T 0, the ONSD was significantly increased at T 3-5 in group C and at T 4, 5 in group T ( P<0.05). The ONSD was significantly lower at T 4, 5 in group T than in group C ( P<0.05). Conclusions:TEAS can reduce the intracranial pressure to some extent in patients undergoing laparoscopic hysterectomy.
6.A study on the correlation of phthalate metabolites in umbilical cord blood of 161 newborns with birth indicators in Beijing

Objective:To analyze the correlation of phthalate metabolites in neonatal umbilical cord blood with birth indicators in Beijing, 2015.Method:From February to July in 2015, 161 pregnant women and their newborns who met the criteria were recruited from the Maternal and Child Health Hospital in Haidian District, Beijing. Questionnaires were used to collect the demographic information of pregnant women such as age, smoking, drinking, and cord blood after delivery. An ultra-high performance liquid chromatography-tandem mass spectrometry was used to determine the concentration of phthalate metabolites in umbilical cord blood. The multiple linear regression model was used to analyze the correlation of phthalate metabolites in umbilical cord blood with the neonatal weight, length, and ponderal index.Result:The age of 161 pregnant women was (30.3±3.0) years. The weight, length and ponderal index of 161 newborns were (3 447.2±413.0) kg, (50.2±1.1) cm, and (26.7±2.2) kg/m 3; 51.6% of newborns (83 cases) were boys. The concentrations of seven phthalate metabolites detected in umbilical cord blood, i.e., mono-methyl phthalate (MMP), mono-ethyl phthalate (MEP),mono-(2-isobutyl) phthalate (MiBP), mono-n-butyl phthalate (MBP), mono-ethylhexyl phthalate (MEHP), mono-(2-ethyl-5-oxohexyl) phthalate (MEOHP), and mono-(2-ethyl-5-carboxypentyl) phthalate (MECPP), was (3.50±0.28), (2.65±0.47), (4.31±0.55), (6.26±0.57), (1.71±0.13), (1.10±0.09) and (0.47±0.06) ng/ml, respectively. The result of multiple linear regression model analysis showed that the concentrations of seven phthalate metabolites were not related to the neonatal weight, length, and ponderal index (all P values> 0.05). Conclusion:The concentrations of phthalate metabolites in neonatal umbilical cord blood are low, and they are not related to the neonatal weight, length, and ponderal index.
