1.Mitral annulus velocities and time intervals for evaluation of global left ventricular diastolic function in patients with coronary artery disease

AIM:To detect and compare the longitudinal mitral annulus diastolic velocity and time interval changes by pulsed Doppler tissue imaging(DTI)in patients with angina pectoris(AP)and myocardial infarction(MI),and to explore the value of mitral annulus diastolic velocities and time intervals for evaluation of global left ventricular diastolic dysfunction.METHODS:Fifty patients with established coronary artery disease were divided into AP group(16 cases)and MI group(34 cases).Sixteen age-matched healthy individuals served as the control group.The septum,lateral,anterior and inferior walls of the mitral annulus were displayed,and selected for DTI spectrum sampling.Peak early and late diastolic velocities and their ratio,time to the onset and peak of the early diastolic wave,and regional isovolumic relaxation time were measured,and the average values of the four mitral annular sites were calculated and presented as Em,Am,Em/Am,QEm,TEm and IVRTm,respectively.RESULTS:Compared with the control group,Em and Em/Am were significantly lower in both the AP and the MI groups(P<0.01).Em was even lower in the MI group than that in the AP group(P<0.01).QEm,TEm and IVRTm were significantly longer in the AP and the MI groups than those in control group(P<0.01 or P<0.05).IVRTm was even longer in the MI group than that in AP group(P<0.01).IVRTm had significantly negative correlation with Em(r=-0.64,P<0.01).CONCLUSION:Em,Em/Am,QEm,TEm and IVRTm as measured by pulsed DTI may be promising indexes for quantitative assessment of global left ventricular diastolic dysfunction in patients with coronary artery disease.Em and IVRTm may indicate the severity of ischemic myocardial damage.
2.Meta-analysis on Jinlianqingre Effervescent Tablet for acute upper respiratory tract infection in children

Objective To systematically review the efficacy and safety of Jinlian Qingre Effervescent Tablet (JQET) compared to Ribavirin for acute upper respiratory tract infection in children.Methods CNKJ,CBM,WanFang Data,VIP,PubMed,EM base,Web of science,and Cochrane Library databases were searched from the date of establishment to December 2016 for all randomized controlled trials (RCTs) and quasi-RCT on the use of JQET in children with acute respiratory infections.Meta-analysis by using Rev Man 5.3.Results A total of seven RCTs involving 782 patients were included,while the group of JQET involving 392 patients,Ribavirin involving 390 patients.The results ofmeta-analysis indicated that the efficacy in Jinlianqingre group was superior to that of Ribavirin control group,such as clinical effectiveness [RR=l.26,95%CI=(1.18,1.34),P<0.000 01],fever subsidence time [MD=-1.54,95%CI (-1.79,-1.30),P<0.000 01],the time of subsided cough [MD=-1.53,95%CI (-1.79,-1.27),P<0.000 01],the disappearance time of pharyngalgia [MD=-1.29,95%CI (-1.88,-0.70),P<0.000 1],and Pharyngeal congestion disappearance time [MD=-2.80,95%CI(-3.11,-2.49),P<0.000 01].The difference was statistically significant.There were three adverse reactions reported in JQET group.Conclusion JQET is superior to the Ribavirin control group in clinical effectiveness,fever subsidence time,time of subsided cough,disappearance time of pharyngalgia,and pharyngeal congestion disappearance time to treat acute upper respiratory tract infection in children.However,these results should be carefully interpreted,and this conclusion has to be further verified by high quality,large scale RCTs.

Objective To investigate the effects of radix salviae miltiorrhiaze(RSM) on the expression of Fas, perforin and bcl 2 mRNA in liver ischemia reperfusion(I R). Methods 45 patients were divided into three groups: Control group(group 1, n=5); I R group(group 2, n=20), and RSM pretreatment group(group 3, n=20). The patients in group 2 and group 3 subjected to hepatic resection with “Pringle procedure” for 15?min. Expression of Fas, perforin and bcl 2 mRNA were examined with reverse transcription polymerase chain reaction(RT PCR) after liver reperfusion for 40?min. Objective To measure the organ weights , blood physiological and biochemical parameters , and immune cells of BALB/cA-nu mice .Methods BALB/cA-nu mice at five and ten weeks of age were selected , and the organ weights , blood physiological and biochemical parameters were observed .The percentages of CD 3+, CD4+, CD8+, CD19+, B220+, NK1.1+, and CD11b+were detected by FCM in BALB/cA-nu mice at six weeks of age in terms of its T, B lymphocyte cells and NK cell activity .Results At the same age, the weights of the body , liver, and kidney in male mice are significantly higher than females .The HGB、MCH、HCT in male mice are significantly higher than females (P <0.05).The weights of the lung, liver, and kidney, and the parameters of TP、GLOB、CHO、TBIL、UN are significantly lower at five weeks of age comparing with ten weeks (P <0.05).The BALB/cA-nu mice lacks T cells(0.18 ±0.06)%、CD4+T cells(0.26 ±0.08)%、CD8+T cells(0.13 ±0.04)%、The percentage of B cells is CD19+B cells (30.10 ±2.74)% and B220 +B cells(30.55 ±2.77)%.The percentage of NK cells is (1.35 ±0.29)%, and the percentage of granulocytes is (47.90 ±5.48)%.The BALB/cA-nu mice lack T cells and the group of cells .Conclusion The study suggests that the organ weights , blood physiological and biochemical parameters are affected by age and gender in BALB/cA-nu mice that lack T cells immune function .The studied parameters of BALB/cA-nu mice are similar with the same strain in other countries .
6.Clinical observation of ozone therapy combined with gemcitabine and cisplatin regimen in patients with advanced non-small cell lung cancer

Objective To observe the efficacy and adverse reaction of ozone therapy combined with gemcitabine and cisplatin (GP) regimen in patients with advanced non-small cell lung cancer.Methods Fifty-five patients with advanced non-small cell lung cancer were enrolled and allocated to treatment group (28 cases) and control group (27 cases).The patients in treatment group received ozone therapy combined with GP regimen,and the patients in control group received GP regimen only.The efficacy,quality of life,adverse reaction and cellular immune function after treatment was compared between two groups.Results There was no significant difference in the efficacy between two groups (P > 0.05).The quality of life after treatment in treatment group was better than that in control group (P < 0.05).The liver function damage in treatment group was lower than that in control group (P < 0.05).The cellular immune function in treatment group was stronger than that in control group (P < 0.05).Conclusion Ozone therapy combined with GP regimen can effectively alleviate adverse induced by GP regimen chemotherapy and significantly improve the quality of life in patients with advanced non-small cell lung cancer.
7.Blood-saving effect of low central venous pressure combined with acute hypervolemic hemodilution in patients undergoing hepatic lobectomy

Objective To evaluate the blond-saving effect of low central venous pressure(CVP) combined with acute hypervolemic hemedilution(AHHD)in patients undergoing hepatic lobectomy.Methods sixty ASA I orⅡpatients of both sexes aged 32-48 yr weighing 47-72 kg undergoing hepatic lobectomy for primary malignant hepatonm under epidural combined with general anesthesia were randomly divided into 3 groups(n=20 each);group I control(C);group 1I AHHD and group Ⅲ low CVP+AHHD.Group C received crystalloid and coloid in a ratio of 1.5:1 during operation.In groupⅡ4% suecinylated gelatin was infused at 50 ml·kg-1·h-1 for 30 min after tracheal intubation (AHHD);while inⅢ group low CVP was induced and maintained by epidural administration of a mixture of 1.5% lidnoaine +O.2% bupivacaine 6-8 ml combined with intravenous infmion of propofol at 6 mg·kg-1·h-1 until 10 min after hepatic lobectomy was completed.then 4% succinylated gelatin was infused at 50 ml·kg-1·h-1 for 30 min.Blood glucose,Hb,Hct, WBC count,blood coagulation (PT,AVIT,Fib),shtmic-pyruvic transaminase (GPT) and renal function (BUN,Cr) were determined before operation (baseline),immediately before skin incision,immediately before and 10 min after liver lobe was removed,at the end of operation and 7 d after operation.Urine output,intraoperative blood loss and blood transfusion and complications were recorded.Results The glood glucose concentration.WBC count and GPT levd were significantly lower;the amount of fluid infused and urinary output before hepatic lobe resection and the percentage of the patients with allogeneic blood transfusion during operation were less;Hb,Hct and the amounl of fluid infused and urinary output after hepatic lobe resection were uigher in grolp Ⅲ than in group I and ⅡⅡⅡ.There were no significant differences in blood coagulation,renal function,the total amount of fluid infused and urine output among the 3 groups.No patient developed any complication.Conclusion The low CVP hefor combined with AHHD after hepatic resection can decrease intraoperative blood loss and allogeneic blood transfusior and is safe.

AIM: The aim of this study was to investigate the possibility of establishing a chronic atrial fibrillation model with long-term rapid atrial stimulation ( 1 000 bpm), which was performed in rabbits in vivo. METHODS: 20 rabbits were randomly divided into 2 groups: 1) control group (n=10): pacemaker was implanted but no pacing; 2) experimental group (n=10): a left intercostal thoracotomy was performed and the pericardium was opened to expose the heart and a steel-wire pacing electrode was fixed on the epicardium of the left atria in 10 rabbits. Then the rapid pulse generator was implanted subcutaneously in the left abdominal region and rapid atrial pacing ( 1 000 beats/min) was initiated and continued for 30 days. Electrocardiogram (ECG) was monitored and recorded on day 1, 3, 5, 7, 14, 21 and 30. The atrial effective refractory period (AERP) was measured before pacing and at the time of fibrillation. AIM: To investigate the relations between the proliferation and phenotype change of adventitial cells and the expression of T GF-?1 in adventitia after arterial injury. METHODS: After abdominal artery injury in rabbits, adventitial cel l proliferation and phenotype were identified by immunohistochemistry, transmissi on electron microscope and in situ Hybridization. RESULTS: Three days and 7 days after injury, the PCNA-positive ce lls were significantly increased. The previously weakly actin-positive adventiti al cells became strong-actin staining, the typical myofibroblast featurses of ad ventitial cells were detected by TEM 7 and 14 days after injury. Three days afte r vascular injury, there was a marked increase in the nu mber of cells that demonstrated the presence of TGF-?1 transcripts within the adventitia. Seven days after arterial injury, adventitial cells continued to ex press TGF-?1. on day 28, the number of TGF-?1-expressing cells was markedly r e duced in the adventitia. CONCLUSION: This study demonstrates that the proliferation and p henotype modulation of adventitial cells are associated with induction of TGF- ?1 expression in adventitia after arterial injury.
