1.Interpretation of the Shanghai Standards for the Development of Health-promoting Hospitals

The development of health-promoting hospitals is an important part of the Healthy China Initiative, facilitating the transformation of medical institutions from a primary focus on medical and surgical treatments to a patient-centered approach prioritizing public health. To promote the scientific, standardized, and sustainable development of health-promoting hospitals, the Shanghai Municipal Center for Health Promotion, together with Shanghai General Hospital and other institutions, has jointly developed the Standards for the Development of Health⁃promoting Hospitals (Standards) in accordance with the principles of scientific rigor, feasibility, and advancing with the times. The Standards outlines the requirements for the development of health-promoting hospitals across four aspects: organizational and institutional guarantees, creation of a healthy environment, implementation of health education and promotion, and evaluation. It is applicable to all levels and types of public medical institutions in Shanghai, and can also be referred to and implemented by other medical institutions. The introduction of the Standards is of great significance for promoting the formation of a long-term mechanism in health-promoting hospitals. This paper introduces the basis, principles, main content, precautions, and implementation suggestions of the Standards. Taking into account relevant policies and standards, it interprets the technical aspects in the process of developing health-promoting hospitals, which is helpful for medical institutions and their management to better understand and apply the standards in their work.
2.The Effect of heparin solution sealing with different tube volumes on coagulation function in patients undergoing continuous renal replacement therapy

Objective:To investigate the influence of various volumes of heparinized saline seal on coagulation function in patients undergoing Continuous Renal Replacement Therapy (CRRT) with non-tunneled hemodialysis catheters.Methods:A randomized controlled trial was conducted on patients requiring catheter sealing during CRRT in the Department of intensive care unit, Beijing Friendship Hospital Affiliated to Capital Medical University from March 2021 to December 2022. Patients were randomly assigned to either the experimental group or the control group, The control group used 100% catheter volume of heparinized solution, while the experimental group used 80% catheter volume of heparinized solution. Compare the activated partial thromboplastin time (APTT), prothrombin time (PT), prothrombin activity (PTA), fibrinogen (Fbg), hemoglobin, platelets, and poor prognosis events (mainly including clotting, bleeding, bleeding inclination) between two groups of patients.Results:A total of 121 patients were included in the study, with 59 in the experimental group and 62 in the control group. In the control group, there were 39 males and 23 females, with an average age of (66.6 ± 18.2) years, while in the experimental group, there were 29 males and 30 females, with an average age of (65.2 ± 17.5) years. After catheter sealing, APTT and PT in the experimental group were (41.2 ± 7.6) s and (14.0 ± 0.8) s, respectively, both lower than the corresponding values in the control group [(48.1 ± 14.7) s and (16.2 ± 4.0) s], with statistically significant differences ( t=4.32, 4.08, both P<0.05). In the experimental group, PTA, fibrinogen levels, and platelet were (77.9 ± 12.5) s, (90.3 ± 10.1) g/L, and (151.3 ± 89.4) × 10 9/L, respectively, higher than the corresponding values in the control group (65.3 ± 17.2) s, (85.2 ± 5.2) g/L, and (110.0 ± 21.6) ×10 9/L, with statistically significant differences ( t=-4.59, -5.03, -5.59, all P<0.05). After the closure of the tube, the bleeding incidence rate in the experimental group was 1.7% (1/59), which was significantly lower than that in the control group at 12.9% (8/62), with statistically significant differences ( χ2=0.12, P<0.05). Conclusions:Heparin solution sealing with 80% lumen capacity improves coagulation function, reduces bleeding risk, and does not increase the incidence of catheter blockage.
3.Status of health communication of medical institutions using new media platforms in Shanghai

ObjectiveTo investigate the current situation of health communication carried out by medical institutions in Shanghai through new media platforms such as WeChat, Weibo, Toutiao, Douyin, Kuaishou, Bilibili and WeChat Videos, and to propose targeted measures. MethodsBased on the systematic collection of new media accounts of medical institutions in Shanghai, and through the combination of keyword screening and manual audit, health communication data of medical institutions on new media platforms were determined. ResultsData from 1 117 new media accounts of 162 medical institutions in Shanghai were collected, including 610 WeChat official accounts, 105 WeChat video accounts, 89 Weibo accounts, 18 Bilibili accounts, 198 Douyin accounts, 37 Toutiao accounts, and 60 Kuaishou accounts, totaling 111 853 posts. After keyword sorting and manual screening, a total of 66 761 health science posts were collected, with WeChat Official Accounts, Douyin, and Weibo having the top three highest number of posts. Video-based new media such as Douyin, WeChat Videos, Kuaishou, and Bilibili had a better communication impact than text and image-based new media like WeChat Official Accounts, Weibo, and Toutiao. Among them, Douyin and Toutiao were the best platforms for video and text-image-based new media, respectively. ConclusionMedical institutions in Shanghai recognize the importance of new media in health communication and have made full use of various media platforms to carry out health communication,having a certain impact on health education. In the future, medical institutions should choose appropriate platforms based on target audiences and content characteristics, fully leverage the advantages of various platforms, explore innovative communication strategies, promote the dissemination of health knowledge, and enhance the health literacy of the public.
4.Advances in prostate cancer biomarkers.

Prostate cancer is one of the most common malignant tumors in men and posing a serious threat to men's health. Detection methods such as prostate-specific antigen (PSA), prostate biopsy, and magnetic resonance imaging are widely used for prostate cancer screening, but they have low specificity, high cost, and significant risks. Therefore, there is an urgent need to develop highly specific, low-cost, easily obtained, stable, and reliable biomarkers, and use them as the basis to establish non-invasive screening and diagnostic methods for prostate cancer. This paper reviewed the recent advances in the use of prostate cancer biomarkers and combined detection methods for prostate cancer diagnosis and prognosis assessment and provides an in-depth analysis and comparison of different biomarkers and combined detection methods, as well as points out the directions and challenges for future research. The paper emphasizes the importance of developing efficient, cost-effective and easy-to-implement biomarkers to increase the early diagnosis rate of prostate cancer, improve patient prognosis, and reduce the waste of healthcare resources. This paper provides an important theoretical basis and technical guidance for early diagnosis, precise treatment and prognostic evaluation of prostate cancer, and has important reference value for promoting clinical research and practice of prostate cancer.

Objective To evaluate the value of multi-slice spiral computed tomography angiography(MSCTA)scanning and reconstruction technology in detecting anomalous origin of coronary artery(AOCA).Methods Retrospective analysis was done in 3 856 patients who accepted MSCTA.Volume rendering(VR),multi-planar reformation(MPR),curved planar reformation(CPR)and maximum intensity projection(MIP)were used to observe the origin and course of coronary artery.Results 42 patients with AOCA were detected among 3 856 objects,and the detection rate was 1.09%.The detection rates had no statistically significant difference between male(1.17%)and female(0.98%).The rate of patients with anomalous origin of left coronary artery was 30.95%(13/42), and 9 objects(69.23%,9/13)of them had the anomalous origin of left circumflex.The rate of patients with anomalous origin of right coronary was 66.67%(28/42),and 35.71% of them(17/28)were found to have the anomalous origin of right coronary artery from the left sinus of valsalva.Conclusion MSCTA scanning and reconstruction technology is noninvasive,rapid,accurate and intuitive.
7.Prognostic Evaluation of Nasopharyngeal Carcinoma with Bone-Only Metastasis after Therapy.

PURPOSE: To evaluate the prognosis of nasopharyngeal carcinoma (NPC) patients who developed bone-only metastasis after primary treatment and the stratification of these patients into different risk groups based on independent prognostic factors. MATERIALS AND METHODS: Eighty NPC patients who developed bone-only metastasis after definitive radiotherapy from October 2005 to December 2010 were enrolled. All these patients received palliative treatment for bone metastasis, including chemotherapy and/or radiotherapy. Clinical features, treatment modality, and laboratory parameters were examined with univariate and multivariate analyses. RESULTS: The median follow-up time was 15.5 months (range, 2-67 months) for the whole cohort. The median overall metastatic survival (OMS) time and the 2-year estimate OMS rate were 26.5 months and 52%, respectively. Multivariate analysis indicated that patients with short metastases-free interval, multiple bone metastases sites, high serum lactic dehydrogenase levels, and treated with radiotherapy or chemotherapy alone had significantly worse outcomes. Patients were stratified into three different risk groups based on the number of adverse factors present. The OMS curves of the three groups were all significantly different (p<0.001). CONCLUSION: Severl prognostic factors were found to be associated with worse outcomes. According to the number of adverse factors present, bone-only metastasis patients can be stratified into three risk groups with significantly different prognoses. Such grouping may help in improving the design of clinical trials and in guiding individualized treatment for NPC patients with bone-only metastasis.

8.Stem cell therapy for degenerative disc diseases:present status and prospects

BACKGROUND:None of the current treatment strategies has been focused on relieving or reversing the disk degeneration process after degenerative disc diseases. In recent years, more and more scientists try to treat degenerative disc diseases using stem cel therapy. OBJECTIVE:To explore the research status and prospects of stem cel therapy for degenerative disc diseases. METHODS: A computer-based online search of PubMed database between January 2004 and December 2014was performed to search related articles with the key words of “stem cel, intervertebral disk” in English. Literatures related to stem cel therapy for degenerative disc diseases were selected; in the same field, the articles published lately in authoritative journals were preferred. RESULTS AND CONCLUSION: A total of 342 articles were primarily selected, and 43 articles were involved in result analysis according to inclusion criteria. Stem cel therapy is a newly treatment for degenerative disc diseases. Cels appropriate for stem cel therapy include embryonic stem cels, induced pluripotent stem cels, mesenchymal stem cels, human umbilical cord mesenchymal stem cels and chondrocytes or nucleus pulposus cels. Although cel leakage, intervertebral disc infection and tumorigenesis are the main chalenges, stem cel therapy for degenerative disc diseases is promising in the future.
9.Preventive effect of Polygonum Multiflorum on deteriorated micro-structure and biomechanical properties induced by prednisone

Aim To investigate the preventive effect of Polygonum Multiflorum (PM)on the deteriorated mi-cro-structure and biomechanical properties induced by prednisone.Methods Ninety 6-month-old male Sprague-Dawley rats were randomly divided into nine groups,which were control,prednisone,CAL,30%ethanol eluent of the PM(H,M,L),PM(H,M,L). Prednisone was gavaged to rat for 21 weeks as model group of osteoporosis.Meanwhile,tested herbal ab-stract were orally administrated to the modeled rats in-duced by prednisone.At the end of the experiment, the right femur was collected for micro-CT scanning, three-dimensional reconstruction and biomechanical test.Results Compared with the control group,mod-el group showed destruction of bone microarchitecture, BV /TV fell 28.6%(P <0.05),bone biomechanical parameters decreases,and stiffness fell 29.7%(P <0.01 ). Compared with the model group, positive group had significantly improved effect on bone micro-architecture,and biomechanical parameters,BV /TV increased 46.7%(P <0.01 ),and stiffness increased 25.9%(P <0.01 ).30% ethanol eluent of the PM (M,L)dose may improve bone microstructure by in-creasing BV /TV 46.7% (P <0.01 ),40.0% (P <0.05)respectively,PM(H)may improve the biome-chanical parameters by increasing stiffness 24.7%(P<0.05),and 30% ethanol eluent of the PM(H)and PMhigh-dose may improve the biomechanical parame-ters,but not as positive group.Conclusions Predni-sone reduces biomechanical properties of rat femur and deteriorates femoral microstructure.30% ethanol eluent of the PM(M,L)and PM(H)plays a preventive role in the changes of micro-structural and biomechanical properties by prednisone,and increases BMD,whereas other groups have no significant preventive effect.
10.Micro-titanium plate fixationversus suture suspension fixation in cervical posterior expansive open-door laminoplasty:a meta-analysis

BACKGROUND:Many studies concern the comparison of micro-titanium plate fixation and suture suspension fixation during cervical posterior expansive open-door laminoplasty, but the sample size of many studies has limitations. There is lack of objective evaluation on advantages and disadvantages of micro-titanium plate.
OBJECTIVE:To systemicaly evaluate the efficacy and safety of micro-titanium plate fixationversus suture suspension fixation in cervical posterior expansive open-door laminoplasty.
METHODS: English and Chinese randomized controled trials were searched by two reviewers. They retrieved the Cochrane Central Register of Controled Trials (CENTRAL), PubMed, EMbase, the ISI Web of Knowledge Database, CNKI, CMB, VIP and Wanfang database for randomized controled trials addressing micro-titanium plate fixationversus suture suspension fixation in cervical posterior expansive open-door laminoplasty published from database foundation to March 1, 2015. The references were also searched by hand. Meta-analyses were performed by using the Rev-Man 5.3 software, provided by the Cochrane Colaboration.
RESULTS AND CONCLUSION: A total of 9 studies involving 642 patients were included. The results of meta-analyses showed that: (1) safety: There were no significant differences between the two groups in operation time [SMD=-0.02, 95%CI (-0.57, 0.54),P=0.95 > 0.05], and intraoperative blood loss [SMD=0.07, 95%CI (-0.26, 0.40),P=0.69 > 0.05]. (2) Efficacy: compared with suture suspension fixation, Japanese Orthopaedic Association Scores were higher [SMD=0.26, 95%CI (0.10, 0.42),P=0.001 < 0.05], the angle of the opened laminae was bigger [SMD=0.25, 95%CI (0.02, 0.48),P=0.04 < 0.05], cervical curvature was better [SMD=0.46, 95%CI (0.27, 0.65),P < 0.000 01], and incidence of axial symptoms was lower [RR=0.40, 95%CI(0.29, 0.56),P< 0.000 01] after micro-titanium plate fixation. These findings suggest that during expansive open-door laminoplasty for treatment of cervical spondylosis, micro-titanium plate fixation and suture suspension fixation can obtain good clinical outcomes. However, Japanese Orthopaedic Association Scores were higher and the angle of the opened laminae was better in micro-titanium plate fixation than in suture suspension fixation. Micro-titanium plate fixation could effectively prevent loss of cervical curvature and reduce the incidence of axial symptoms. For the poor quality of the original studies and smal sample size, a prudent choice is suggested. More high-quality large-sample studies are needed for further verification.
