193 patients with irritable bowel syndrome (IBS) were divided into 3 groups stochastically:73 cases were treated with both trimebutine malate and doxepin. 56 cases were only treated with doxepin, and this group was given doxepin to take orally. The other 64 cases were only treated with trimebutine malate. This group were given trimebutine malate to take orally. Doxepin was given 12.5-25.0 mg tid po, and trimebutine malate was given 200 mg tid po. This is for a course of 4-week-therapy. The rates of the total response of the 3 groups were 95.9%, 83.9% and 81.3% respaefively. The effect of the combination of the 2 drugs is much higher than the other two (P<0.01), and especially it has the highest effect on diarrhea type IBS. The rates of the improvement of nervos and mental symptoms were 87.0%, 79.0% and 71.6% respectively. The effect of combination use is the highest. It suggests that both the single use and combination use of doxepin and trimebutine malate has the effect on the treatment of IBS, but the combination use can improve the effect obviously.

Objective To evaluate the effect of treatment and compliance of rebeparzole sodium enteric coated capsules, amoxicillin and clavulanate potassium tablets and furazolidon or levofloxacine treatment of helicobacter pylori infection.Methods Two hundred and forty-nine cases of positive helicobacter pylori infection were randomly divided into three groups.Eighty-one cases of Group A were treated with rebeparzole sodium enteric-coted capsules (20 mg one time), amoxicillin and clavulanate potassium tablets (3 tablets one time) and furazolidon(100 mg one time) ,twice a day before breakfast and supper.Eighty-three cases of Group B were treated with rebeparzole sodium enteric-coted capsules (20 mg one time), amoxicillin and clavulanate potassium tablets (3 tablets one time) and levofloxacine (200 mg one time), twice a day before breakfast and supper.Eighty-five cases of Group C were treated with rebeparzole sodium enteric-coted capsules (20 mg one time) ,clarithromycin capsules(500 mg one time) and amoxicillin dispersible tablets(1 000 mg one time) ,twice a day before breakfast and supper.Period of treatment was 10 days.Results The helicobacter pylori eradication rate of Group A,B and C were 93.83％(76/81) ,89.16％(74/83) and 81.18％(69/85).The eradication rate of Group A was higher than that of Group C (P＜0.05).The comparison between Group A and Group B and between Group B and Group C had no statistically significant difference(P＞0.05).Total integral in average of main digestive symptoms respectively decreased from 12.52±2.24, 12.30±2.42,12.61±2.01 to 3.16±2.23, 3.58±2.39,4.78±2.28 after four-week treatment.The effect of treatment of Group A and Group B was much better than that of Group C(P＜0.05).The incidence of adverse reaction were 11.11％, 30.12％ and 62.35％respectively.Group A and Group B were much lower than Group C (P ＜ 0.05), and Group A was lower than Group B(P＜0.05).In the comparison of the spread of compliance, Group A was the best, the next was Group B,and the last was Group C.The comparison result of those 3 groups was statistically significant (P ＜0.05).Conclusion Ray rabeprazole sodium enteric coated capsules and amoxicillin and clavulanate potassium dispersion tablets combined with furazolidone furazolidone triple therapy in the treatment of Helicobacter pylori Helicobacter pylori infection effect is the best.

[Objective]To evaluate the role of mobile bearing knee prosthesis in TKA and the clinical results.[Method]A total of 301 TKAs by using mobile bearing knee prosthesis were gotten average 48.7 months follow-up.HSS knee scores and X-ray films were to used to evaluate the results.[Result]The mean HSS score improved from 51.3 before operation to 91.3 after operation.The ROM improved from extension/flexion(-9.5?/64.3?)before operation to extension/flexion(0?/97.5?)after operation.The maximum varus/valgus(25?/15?)before operation were improved to the mean valgus 5.3?.Two cases of septic loosening and 2 cases of femoral fracture were found.[Conclusion]The CR type mobile bearing knee prosthesis not only had advantages in kinematics and wear experiments,but also had achived good clinical result.

Objective To study the influence of sufentanil on analgesic effect and hemodynamics during recovery period of general anesthesia in elderly patients.Methods Eighty patients with general anesthesia were randomly divided into 4 groups (20 cases in each group).Ten min before surgery ended patients in each group were intravenous injected different dose of sufentanil:group S1 with sufentanil 0.2 μg/kg,group S2 0.4 μg/kg,group S3 0.6 μg/kg,and group C (control group) with saline 5 ml.The time of spontaneous breath restores,the time of summon opening eyes and extubation,the cases occurred cough,nausea,vomiting during recovery period were recorded and the VRS pain scores and Ramsay score 10 min after extubation were also recorded.Mean arterial pressure (MAP) and the heart rate (HR) were recorded before the anesthesia induction(T0),stopped anesthetic (T1),pre-extubation (T2),5 min and 10 min after extubation (T3 or T4)Results As group S1 and S2 compared with group C,there was no significant difference on the time of spontaneous breath restores,the time of summon opening eyes,and the time of extubation and leaving the operating room (P ＞ 0.05).However,there was significant difference on the time of spontaneous breath restores ((7.3 ± 4.1) min vs (10.2 ± 4.7) min),the time of summon opening eyes ((8.1 ± 3.1) min vs (11.2 ± 3.7)min),the time of extubation ((12.3 ± 3.1) min vs (16.3 ± 5.9) min),and the time of leaving the operating room((21.4 ±3.0)min vs (24.2 ±3.5)min) between the group S3 and group C(P＜0.01 or P ＜0.05)Patients occurred cough in group S1,S2 and S3 were less than those in group C (P ＜0.05),but there was no significant difference on the occurrence of nausea and vomiting between group S1-3 and group C (P ＞ 0.05)There was significant difference on VRS pain score and Ramsav score between group S1-3 and group C(P ＜0.01).MAP and HR were increased in all points of time as compared with T0 (P ＜ 0.05 or P ＜ 0.01).And there was significant difference on MAP and HR between group S1-3 and group C (P ＜ 0.05 or P ＜ 0.01).Conclusion Before the end of anesthesia sufentanil 0.2 ～ 0.4 μg/kg intravenous injection could be able to enhance analgesic effect,stabilize hemodynamics and do not affect regain consciousness in elderly patients.

Background and Purpose:In recent years,along with marked rise in the incidence of lung cancer,the incidence of brain metastasis from lung cancer has increased year by year.The main treatment strategy of lung cancer with brain metastasis is irradiation,while so far there are only few researches concerning chemotherapy combined with radiotherapy for these patients.The aim of this study is to evaluate the therapeutic effect,survival rate and toxicity of chemotherapy with VM_(26)+DDP regimen given concurrently with whole-brain radiotherapy in lung cancer with brain metastasis.Methods:From Sep.2000 to Oct.2001,forty-one patients with lung cancer with brain metastasis were divided randomly into two groups: 20 patients(14 male,6 female) received concurrent chemoradiotherapy(chemoradiotherapy group),the other 21 patients(14 male,7 female) received only radiotherapy(radiotherapy group).In the chemoradiotherapy group,the average age was 50 years with range 40 to 70 years,16 patients were non-small-cell lung cancer,4 patients were small-cell lung cancer.In the radiotherapy group,the average age was 52 years with range 40 to 73 years and 19 patients were non-small-cell lung cancer,2 patients were small-cell lung cancer.For both groups,the same radiation technique was given with conventional fraction.Radiotherapy was delivered by 6MV.Fractionations of 3Gy/fraction/day was delivered 10Gy/5 factions/week.The total dose was 30Gy/10Fr/2W.For chemoradiotherapy group,the patients were also given concurrent chemotherapy(VM_(26) 60mg/m~(2)/ day iv on days 1-3,cisplatin 60 mg/m~(2) iv on the 1~(st)day).Results:The response rate and complete response in the chemoradiotherapy group was significantly higher than that in the radiotherapy group(75% ?? 38.10%,P

Extended liver resection may provide longterm survival in selected patients with hilar cholangiocarcinoma.In May 2013,a patient with hilar cholangiocarcinoma received right hemihepatectomy combined with caudate lobectomy under the guidance of precision hepatectomy technique.The porta hepatis was clamped in the operation for 3 times intermittently.The operation time was 8 hours,and the volume of intraoperative blood loss was 600 mL.The patient was discharged at postoperative day 14.The results of pathological examination confirmed that the patient had moderately-highly differentiated adenocarcinoma with full-thickness infiltration of the bile duct and tumorfree margins.No tumor recurrence was detected during the followup which was lasted for 6 months.The operation was carried out based on the three dimensional reconstruction,liver reserve function evaluation,intraoperative controlled low central venous pressure and precision hepatectomy technique,and the results were satisfactory.
