1.Curative effect analysis of scapula lateral margin approach and standard Judet approach in the operative treat-ment of scapular fracture

Objective To compare the effect of scapula lateral margin approach and standard Judet approach in the operative treatment of scapular fracture. Methods Medical data of 48 patients with scapular fractures from January 2006 to February 2013 were retrospectively analyzed. They were divided into the observation group and the control group according to the surgical approach. 27 patients in the observation group were treated with operative treatment through scapula lateral margin approach and 21 patients in the control group were treated with sur-gical treatment through standard Judet approach. Compared the clinical effects and complications of the patients in two groups. Results To operation time and intraoperative hemorrhage in the observation group were less than that of the control group (P<0. 05),fracture healing time was shorter than that of the control group after operation (P<0. 05). Six months after operation,the excellent rate of the observation group was higher than that of the control group. Scores of daily activities and myodynamia in the observation group were higher than that of the control group,the differences were statistically significant(P<0. 05),and there was no significant difference in the scores of pain and joint motion between the groups(P>0. 05). Conclusion Scapula lateral margin approach in treatment of scapular fracture has the advanta-ges of easy operation,small trauma,definite effect,little complication and it can be used as the preferred scheme of scapular fracture operative treatment.
2.Effect of the Swimming Training and Tertram Ethylplyrazine on the Microcirculation in Cochlea of Guinea Pigs After Chronic Hypoxia

Objective To explore the effect of swimming training on the microcirculations of cochlea in guinea pigs with tertram ethylpyrazine(TMP) treatment after chronic hypoxia.Methods 28 guinea pigs were divided into normal control group outside of cabin (A) and chronic hypoxia model group (B). After four weeks the model group were divided into the model control group (B 1),TMP treatment group (B 2),swimming training group(B 3) and swimming training with TMP treatment group (B 4).Until the given time,LDF was used to investigate the blood flow of every guinea pig's cochlea.The spiral ligament spreaded-slice optical lens was used to observe guinea pig's vascular veins, capillary appearance and RBC counts.Results There was significant difference in cochlear blood flow(CBF) and RBC counts between B 2,B 4 groups and B 1 group.CBF and RBC counts of B 3 group was improved comparing with B 1 gorup,so did that of B 4 group comparing with B 2.But no significant difference was found.Conclusion Swimming training can relieve the swelling on the thin blood vessel and improve the effect of TMP on the microcirculation in cochlea of guinea pigs under chronic hypoxia environment.
3.Transumbilical single-port laparoscopic cholecystectomy: an analysis of 15 cases

Objective To summarize the operative experience of transumbilical single-port laparoscopic cholecystectomy. Method The clinical data of 15 patients underwent transumbilical singleport laparoscopic cholecystectomy between May to October in 2010 was reviewed. Results The operative time was 30-85(45±16) min,hospitalization time was (2.5 ± 1.1 ) d. No surgical complications such as bile leak or uncontrolled hemorrhage occurred postoperatively. All the incision were healed well and the surgical scar was virtually concealed whithin the umbilicus. Conclusions Transumbilical single-port laparoscopic cholecystectomy is feasible and safe when patients are well selected. Comparing with conventional laparoscopic cholecystectomy, it's more manipulatively difficult.
4.Comparison of the effects of ursodeoxycholic acid, dexamethasone, and S-adenomethionine on intrahepatic cholestasis induced by ethinylestradiol in pregnant rats

Objective To investigate the effects of ursodeoxycholic acid (UDCA), dexamethasone (DEX), and S adenomethionine (SAMe) on the pregnant rats with intrahepatic cholestasis induced by ethinylestradiol (EE). Methods A total of 50 15 day pregnant Wistar rats were randomly divided into five groups: control group, EE group, UDCA group, DEX group, and SAMe group. In EE group, rats were subcutaneously injected with EE (2.5 mg?kg -1 ?d -1 ) for 5 d. In the control group, rats received subcutaneous injection of appropriate volume of propylene glycol (PG) for 5 d. In UDCA, SAMe, and DEX groups, rats were administered with the same dose of EE plus UDCA (100 mg?kg -1 ?d -1 ), SAMe (100 mg?kg -1 ?d -1 ), and DEX (1 mg?kg -1 ?d -1 ) for 5 d, respectively. The serum levels of aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatases (ALP), glycocholic acid (GC), and total bilirubin were measured after cesarean. Morphologic changes of the liver of pregnant rats and the growth and development of the fetal rats were observed. Aim To explore the effect of 4-amino-2-trifluoromethyl-phenyl retinate(ATPR)on proliferation,differentiation activity in K562 cell line,and to research the mechanisms.Methods Cell proliferation was assessed by MTT assay.Cell differentiation index was analyzed by NBT reduction test.Morphologic changes were observed by Wright's staining in the light microscope. Cell cycle was determined by FCM.The mRNA expression of Cyclin D1,Cyclin E,CDK2,CDK4,CDK6,P21cip1,P27kip1,P57kip2,PCNA mRNA were detected by RT-PCR.While the protein expression of cyclin D1 and CDK4 was detected by Western blot.Results The growth of K562 cells was inhibited in a dose-dependent manner.NBT reduction test indicated that the ATPR could induce differentiation of K562 cells and increase the positive cell ratio.Morphologic changes were observed after Wright's staining using inverted phase contrast microscope.The proportion of cells in G0/G1 phase increased while S phase cells decreased.Cell cycle progression was blocked in the G1 phase.The expression of Cyclin E,cyclin D1,CDK2,CDK4,CDK6 mRNA decreased,while PCNA,P21 cip1,P27 kip1 change was not obvious,but P57 (kip2) mRNA expression was increased.Cyclin D1 and CDK4 protein expressions were reduced as well.Conclusions ATPR inhibits the growth of K562 cells and induces differentiation.P57 kip2 plays a key role in differentiation.Moreover,high level of P57kip2 is regulated via inhibiting its degradation through reducing proteasome-dependent proteolysis,and ATPR plays a role in cell cycle arrest.
6.Clinical study of nutritional support in patients with acute peritonitis caused by colonic perforation

Objective:To investigate the effect of different postoperative nutritional support on the gastrointestinal function and nutritional status in acute colon peforation patients.Methods:60 cases of acute emergency operation patients with perforation of the colon,according to the given nutritional support treatment of the different ways,were divided into enteral parenteral joint nutrition (EN + PN) group and total parenteral nutrition (TPN) group.Clinical therapeutic effects of two groups were compared.Results:The results of EN + PN group were significantly better than the those in TPN group (P < 0.05).Conclusion:EN + PN model can improve the postoperative nutritional status and accelerate the recovery of patients with acute colonic perforation.
7.Relationship between the overlap volume of organ at risk and target and the dose to organ at risk in intensity-modulated radiotherapy

Objective To explore the relationship between the overlap volume of organ at risk (OAR) and target (Voverlap) and the mean dose to OAR (DmOAR) in intensity-modulated radiotherapy (IMRT).Methods Fifty randomly selected patients undergoing IMRT for nasopharyngeal carcinoma (NPC) and an equal number of patients undergoing radical IMRT for cervical cancer in our hospital were enrolled as subjects.The relationship between Voverlap and DmoAR in IMRT was analyzed.The Matlab software was used to generate function to fit the relationship between Voverlap and DmOAR for different OARs.Results The Voverlap varied among patients with NPC or cervical cancer.The ratio of Voverlap to the volume of OAR (VOAR) was positively correlated with the ratio of DmOAR to the prescribed dose (Dp) (all P=0.01).A function was generated to fit the correlation between Voverlap/VOAR ratio and DmOAR/Dp ratio.Conclusions In IMRT,patients have different Voverlap/VoAR ratios due to varicd target volume,disease stages,OAR filling status,and OAR volume.As criteria for plan verification,a specific DmOAR value for each OAR should be calculated before radiotherapy planning based on the corresponding correlation function and the Voverlap/VoAR ratio with a fixed dose prescription.It can be used to reduce the subjective influence on the optimization of radiotherapy planning.
8.Comparative Study on the Vitro Antibacterial Activity of Domestic and Imported Teicoplanin against 224 Strains of Enterococci

OBJECTIVE:To study the vitro antibacterial activity of domestic teicoplanin against224strains of enterococ-ci.METHODS:Determination of MIC of domestic teicoplanin against169strains of entercococcus faecalis and51strains of E.faecium was tested by agar dilution method,and its antibiotic effect was compared with that of the imported teicoplanin and other antibiotics.RESULTS:The MIC 50 of domestic teicoplanin against169strains of E.faecalis and51strains of E.faecium were0.125,0.25?g/ml respectively,MIC 90 were2,1?g/ml respectively;The MIC 50 of imported teicoplanin against E.faecalis and E.faecium were0.25,0.25?g/ml respectively,MIC 90 were1,0.5?g/ml respectively.CONCLUSION:Two kinds of te-icoplanin have strong antibacterial activity against224strains of enterococci;the sensitivity of224strains of enterococci to both kinds was100%.
9.Impact of intervention of weight management on readmission rate of patients with congestive heart failure

Objective To explore the impact of intervention of weight management (WM)on readmission rate of patients with congestive heart failure(CHF).Methods sixty-six CHF patients were divided into the control group(34 cases)and the intervention group(32 cases).The patients in the intervention group received the weight management intervention for 6 months after discharge,while the control group was carried on the conventional education and follow-up.The ability of WM and readmission rate of patients due to onset of CHF were compared between the two groups.Results Six months after the intervention,significant differences were found on the scores of weight monitoring,WM related-knowledge,belief and practice in the intervention group,but only WM related-practice was improved in the control group.The readmission rote due to onset of CHF was reduced in the intervention group.Conclusions WM intervention could significantly improve the ability of WM and reduce readmission rate due to CHF onset of CHF patients.
10.Hepatic failure patient's serum before and after plasmapheresis induces the differentiation of umbilical cord mesenchymal stem cells into hepatic cells

BACKGROUND: Umbilical cord mesenchymal stem cells (UC-MSCs) have made certain curative effect on hepatic failure, but little is reported on the effect of hepatic failure patient's serum microenvironment on UC-MSCs differentiation ability. OBJECTIVE: To investigate the effect of hepatic failure patient's serum on the differentiation of umbilical cord mesenchymal stem cells into hepatic cells. METHODS: UC-MSCs were isolated by tissue adherent method and the cell morphology and phenotype identified by microscope and flow cytometry. Alpha-MEM media with serum before/after plasmapheresis were used to culture the hirdgeneration of UC-MSCs, and regular fetal bovine serum culture medium acted as control group. Inverted microscope was used to observe the cell morphology in three groups. Immunohistochemical method was used to measure expression level of alpha fetoprotein, albumin and cytokeratin 18. RESULTS AND CONCLUSION: A great amount of high-purity UC-MSCs could be obtained using tissue adherent method, which highly expressed CD44, CD73, CD90, CD10, but did not express CD45. Hepatic failure patient's serum could change the morphology of UC-MSCs and induce UC-MSCs to express alpha fetoprotein, albumin and cytokeratin 18. The positive expression of alpha fetoprotein, albumin and cytokeratin 18 was significantly increased after plasmapheresis (P < 0.05). To conclude, hepatic failure patient's serum after plasmapheresis exert more benefits to induce UC-MSCs to differentiate into hepatic cells than that before plasmapheresis.
