1.Study on cytomegalovirus gene mutations conferring resistance to ganciclovir in heart and lung transplant patients

Objective To establish a rapid and convenient assay to screen for emerging ganciclovir(GCV) resistant mutations in the UL97 gene of human cytomegalovirus (HCMV) from transplant recipients. Methods Thirteen spefcific oligonucleotide probes were designed according to the target sequence encoding HCMV-UL97 gene. A nested polymerase chain reaction (PCR) amplifying UL97 gene was employed. Line probes assay (LiPA) was configured to detect relevant non-synonymous mutations at codon 460,520,594,595,603, and 607. In parallel, nested-PCR amplicons were subsequently sequenced directly. Results Four of 16 heart and lung transplant recipients had been infected with HCMV resistant strains during GCV therapy. The mutant codons of UL97 gene were at codon 520、595 and 603. All amplicons detected by LiPA were fully concordant with the nucleotide sequenced.Conclusions Long-term prophylaxis and treatment with GCV in heart and lung transplant recipients may develop HCMV resistant strains. HCMV-LiPA proves to be an alternative method for the resistance genotype analysis of HCMV.

3.The application of case management based WeChat platform in the psychological intervention of patients with laryngeal cancer

Objective To explore the significance of case managers using WeChat platform in the treatment process and extended care for patients with laryngeal cancer.Methods From November 2014 to May 2015, 30 patients with partial resection or total resection of laryngeal cancer in the department of head and neck surgery were selected as a research subjects.Case managers provided fully supports,such as establishing WeChat group,replying online consultations,popularizing health education knowledge,providing psychological counselings.Psychological assessment of the patients was conducted by social support scale(SSRS)and anxiety scale(SAS)in the beginning of case management.Results comparison was performed before and after three months intervention.Results The score of subjective support of patients before intervention was (25.13 ±4.81)points,which after intervention was (28.33 ± 4.41)points.The score of support availability before intervention was (7.20 ±1.99)points,which after intervention was (35.27 ±5.90)points.The differences were statistically significant(t =-2.685,-5.280,2.917,all P <0.05).Conclusion The WeChat platform mediated by the case management is effective in the management of the psychological health of patients with laryngeal cancer,which can effectively promote the rehabilitation of patients.
4.Changes in plasma levels of D-dimer in patients with prostate hyperplasia complicated with acute urinary retention and its clinical significance

Objective To investigate changes in plasma levels of D-dimer in patients with prostate hyperplasia(BPH)complicat-ed with acute urinary retention(AUR),and to explore its clinical significance.Methods 102 cases of patients with prostate hyper-plasia complicated with acute urinary retention,treated in this hospital from July 2013 to December 2014,were selected and divided into the simple BPH group(56 cases)and BPH complicated with AUR(BPH+AUR)group.The plasma levels of D-dimer of these patients were detected and comparatively analysed.Results Compared with the simple BPH group,the plasma levels of D-dimer of patients in the BPH+AUR group was increased,had statistically significant difference.(P <0.01).Conclusion The plasma levels of D-dimer in patients with BPH complicated with AUR increase significantly,which indicates that patients are with secondary in-creased fibrinolytic activity.Measures should be taken to relieve urinary retention as soon as possible,so as to protect the vascular endothelial function of patients and reduce the incidence of complications.
5.Cord blood as third party cells for prophylaxis of graft versus host disease in allogeneic hematopoietic stem cell transplantation

ObjectiveTo investigate the efficacy of co-infusing cord blood (CB) as the third party cells on graft versus host disease (allo-GVHD) prophylaxis after unrelated or haploidentical donor allogeneic hematopoietic stem cell transplantation (allo-HSCT).MethodsFrom 2007 to 2011,41 patients receiving unrelated or haploidentical donor allo-HSCT were analyzed retrospectively.Twenty-five patients received one unit of HLA 4/6-6/6 matched CB one day before SCT as CB group,and median MNC dose was (1.64 ± 0.49) × 107/kg.Sixteen cases not receiving CB served as control group.All patients received antithymocyte globulin,cyclosporine,methotrexate,and mycophenolate mofetil as GVHD prophylaxis.The incidence and severity of aGVHD,and treatment-related mortality were compared between two groups.ResultsThe main clinical characteristics in both groups were comparable.The cumulative incidence of aGVHD in CB group and control group was 44.0% versus 68.8% respectively (x2 =2.403,P>0.05).The cumulative incidence of grades Ⅲ to ⅣV aGVHD in CB group and control group was 16.0% and 37.5% respectively (x2 =2.445,P>0.05).The 100-day treatment-related mortality in CB group and control group was 12.0% and 12.5% respectively (x2 =0.002,P>0.05).ConclusionCord blood as the third party cells might reduce the incidence and severity of aGVHD in unrelated or haploidentical donor HSCT.The efficacy and the mechanism of this strategy need to be further explored by prospective randomized controlled trials.
6.A study on the transfer of mild cognitive impairment to Alzheimer’s disease among the elderly in Nanchang City

Objective To explore the incidence rate of mild cognitive impairment (MCI) among the elderly which transferred to Alzheimer disease (AD) and to analyze the related influencing factors. Methods 10 urban communities were selected through stratified cluster sampling as the research sites where 361 patients with MCI were screened from 1 942 residents aged over 60 years old. Questionnaires and laboratory assays were used to collect data from subjects, including characteristics of demographic, life style, medical history, Alzheimer-associated neuronal thread protein,(AD7C-(NTP)), amyloid β-protein 42(Aβ42) and amyloid β-protein 40(Aβ40), etc. Patients with MCI were followed up for three years to determine whether they progressed to AD. Results 121 of 361 patients of MCI converted to AD, and the incidence rate of MCI to AD was 9.49% person-years. According to the results of Logistic regression analysis model, elder (80-89 years) (OR=3.651,95% CI:1.295-10.297, P<0.001),female (OR=2.603,95% CI:1.136-5.966, P<0.001), heavy drinking(OR=1.479,95% CI:1.343-1.627, P<0.001), increased ADL score (OR=1.790, 95% CI:1.443-2.220, P=0.031) and smoking (OR=1.157,95% CI:1.091-1.224, P<0.001) were the risky factors of the transition of MCI to AD. The increase of Moca score (OR=0.766,95% CI:0.681-0.861, P<0.001) was the protective factor of the transition. Conclusions We should strengthen our monitor on elder female MCI patients, and promote healthy lifestyles among the elders, encouraging them to actively participate in physical exercises and reading, in order to delay patients' transition of MCI to AD.
7.Germination and propagartors of artificial seeds of Dendrobium huoshanense.

The artificial seeds of Dendrobium huoshanese was produced with Auxiliary buds, Protocorm-like bodies, and adventitious shoots. By using orthogonal experiment, we studied the effect of Maltose (%), hormone rate between 6-BA (mgx L(-1)) and NAA (mgL(-1)), active carbon (%), sodium alginate (%), time of ion exchange (min) on germination rate of artificial seeds of D. huoshanese. Then the leaking rate of maltose and variation of pH value of artificial seed capsule during vegetating of artificial seeds of D. huoshanese was measured. The results show that maltose played the most important role in inducing D. huoshanese artificial seeds to germinate. The optimal combination was: maltose 4%, hormone rate between 6-BA (mg x L(-1)) and NAA (mg x L(-1)) 10:1, active carbon 0.1%, sodium Alginate 4%, time of ion exchange is 10 min. Protocorm-like bodies were appropriate propagartor, the germination rate of artificial seeds of D. huoshanese takeing Protocorm-like bodies as the propagartors is 90.1%. After germination, the survival rate of seedlings of artificial seeds was 80.6%, the leaking rate of maltose of artificial seed capsule was 0.52%, and the pH value of artificial seed capsule decreased during the process of vegetation of artificial seeds. After having been stored at 4 degrees C for 20 d, the germination rate of artificial seeds of D. huoshanese takeing Auxiliary buds, Protocorm-like bodies, Adventitious shoots as the propagartors were 3.3%, 10.6%, 5.2%. Under natural conditions the germination rate was 13.8% after 10.0 g/L carbendazim was appended into artificial seed capsule. This result provides a foundation of manufacture and further study of the artificial seeds of D. huoshanese.

8.Effect of cardiopulmonary bypass for 30 min on structure and mechanical properties of erythrocyte membrane surface

AIM:To observe and analyze the effect of cardiopulmonary bypass ( CPB) for 30 min on surface ultra-structure and mechanical properties of the erythrocyte membrane by atomic force microscopy (AFM).METHODS:Ten cases of elective patients in cardiac surgery were selected in the study and divided into control ( CON) group and CPB group.The central venous blood (2 mL) before surgery and 30 min after CPB was collected with heparin anticoagulation . The non-circular red blood cells were counted under a stand fluorescence microscope .AFM was used to examine the ultra-structure of the membrane surface and measure the force curve of the erythrocytes .RESULTS:The percentage of non-cir-cular red blood cells in CPB group showed no statistically significant differences as compared with CON group .AFM images showed that the significant differences of membrane surface concave and convex , evenness , particle distribution , the sur-face average roughness (Ra), the surface root mean square roughness (Rq) and cell membrane adhesion between CPB group and CON group were observed .However, the membrane deformation resilience and curve slope had no significant difference between the 2 groups.CONCLUSION:Cardiopulmonary bypass for 30 min changes the morphology and ultra-structure of the erythrocyte membrane surface , and increases the adhesion between cells .
9.Cognitive and behavioral strategies of stress management among Chinese urban residents.

OBJECTIVETo identify the effective stress management strategies among the Chinese.
METHODSThe sample was selected from Hangzhou, Guangzhou, Chongqing and Taiyuan by using a multi-stage sampling procedure, including 3679 subjects. The data were collected using the household interviewing survey method. The Chinese perceived stress scales (CPSS) measured stress. Stress management strategies included the cognitive and behavioral ones, the former were further divided into positive, neutral and negative ones and the latter were divided into three kinds, i.e. looking for support, liberating and displacing, and relaxing and detracting. The frequency of their usage and their perceived effectiveness were assessed. Multivariable analysis was used to examine the association between various stress management strategies and stress.
RESULTSThe prevalence of health risk stress (HRS) was 44.54% (95% CI: 42.90% - 46.12%). Among the cognitive strategies, all the positive strategies and one of neutral strategies ("Suiyuan") were associated with lower HRS, and the rest of them had no effects. Among the behavioral strategies, all were associated with lower HRS except that of looking for support.
CONCLUSIONThe effective stress management strategies identified in this study might be used to develop a stress management program.

10.Study of the bacterial community structure of microbiota in bacterial vaginosis using amplified ribosomal DNA restriction analysis.

OBJECTIVETo study the bacterial community structure of the microbiota in the vaginal fluid from patients with bacterial vaginosis.
METHODSThe composition of bacteria in the samples of vaginal fluid from 3 patients with bacterial vaginosis and 1 normal premenopausal control was investigated by amplified ribosomal DNA restriction analysis(ARDRA).
RESULTSLactobacillus species were the predominant bacteria in the woman without bacterial vaginosis. Bacterial vaginosis was associated with higher concentrations of a variety of bacterial groups. Women with bacterial vaginosis had greater bacterial diversity, with 31 to 37 OTUs operational taxonomic units detected per sample. The species associated with bacterial vaginosis were Leptotrichia, Prevotella sp. and Megasphaera including several species with no close cultivated relatives.
CONCLUSIONSWomen with bacterial vaginosis have complex vaginal infections with many newly recognized species. ARDRA allows rapid analysis of the diversity of microorganisms in the vagina, and is capable of identifying potentially pathogenic bacteria that can not be identified by general culture.
