3.Application effect of clinical pathways in children with bronchopneumonia: a Meta-analysis

Objective To evaluate the application effect of clinical pathways (CP) in children with bronchopneumonia.Methods The literature of randomized controlled trials (RCTs) in children with bronchopneumonia were retrieved by searching PubMed,Cochrane Library,Web of Knowledge,Wanfang database,VIP,and CNKI from their inception date to March 2014.Meta-analysis was performed by Revman 5.2software.Results Six RCTs involving 24 559 patients were finally selected,with 12 269 cases in the CP group and 12 290 cases in the traditional treatment group.The results of Meta-analysis showed that there were significant differences between the CP group and the traditional treatment group in the cure rate [OR =1.67,95 % CI (1.03,2.77),P =0.04],length of hospital stay [MD =-1.21,95 % CI (-1.95,-0.48),P =0.001],hospitalization expenses [SMD =-3.68,95 % CI (-5.04,-2.32),P < 0.000 1],days of antibiotic using [MD =-1.20,95 % CI (-1.51,-0.89),P < 0.000 01] and patient satisfaction [OR =6.12,95 % CI(3.12,12.04),P < 0.000 01].Conclusion CP is superior compared with the traditional treatment for bronchopneumonia,and is worthy of spread in clinical.
5.Relation of autophagy and postherpetic neuralgia in older adults

Objective To investigate the relation of autophagy and postherpetic neuralgia in elderly patients,and provide new methods for prevention and treatment of postherpetic neuralgia.Methods One hundred thirty-five specimens were obtained from elderly patients with herpes zoster,containing 83 patients with herpes zoster plus neuralgia and 52 cases with non-neuralgia herpes zoster.The level of autophagy was determined by acridine orange-stain.The protein expressions of autophagy marker protein LC3 and autophagy related protein Beclinl were detected by Western blot.Results Flow cytometer detection after acridine orange-stain showed that mean fluorescence intensity (MFI) of herpes zoster plus neuralgia patients was 0.775±0.068 and non-neuralgia herpes zoster patients was 0.342±0.025.The level of autophagy of non-neuralgia herpes zoster patients was significant lower than herpes zoster plus neuralgia patients (P<0.05).The expressions of autophagy marker protein LC3 and autophagy related protein Beclin1 detected by Western blot also showed the level of autophagy of non-neuralgia herpes zoster patients was decreased compared with herpes zoster plus neuralgia patients.Conclusions High level autophagy is risk factor for postherpetic neuralgia in elderly patients.
6.Influence of self-management and education on behaviors of medical compliance of the patients undergoing vitrectomy combined with tamponade

Objective To investigate the influence of self-management and education on behaviors of medical compliance of patients undergoing vitrectomy combined with tamponade.Methods Sixty-four patients undergoing vitrectomy combined with tamponade were randomized equally into the control group and observation group in the order of admission.The control group received routine care and health education,while the observation group received self-management education follow-ups besides the routine nursing.The two groups were compared in terms of the cumulative time of the correct position,compliance behavior and the incidence of postoperative complications.Results The cumulative time of the correct position of observation group was longer than that of the control group after operation.The observation group did better in compliance behavior than the control group and the incidence of postoperative complications in the former group was lower than the latter(P<0.05).Conclusion Self-management and education can enhance patients’behaviors in medical compliance and decrease postoperative complications and promote the success of the surgery.
7.Effect of decitabine combined with daunorubicin on apoptosis of HL-60 cell line

Objective To observe the cell proliferation inhibition of DNA methyltransferase (DNMT) inhibitors decitabine (DAC) combined with daunorubicin (DNR) in human leukemia cell line HL-60.Methods The effects of DNR and DAC were examined in HL-60 cells by cell viability using MTT method,and cell death using flow cytometric (FCM).Results DAC,DNR single drug application showed their effects on cell proliferation was dependent of dose and time,the inhibition effect of combined treatment group was much clearer [inhibition ratio of 72 hours was (80.23±1.71) %,P < 0.001].The highest apoptosis rate was at 5.0 μmol/L DAC combined with 1.0 μmol/L DNR for 72 hours,which was statistic significant (F =30.199,P < 0.001).Combinations of different concentrations of DAC and DNR increased expression of PTEN mRNA in concentration-dependent manner,which was significantly higher than the control group and DNR single drug group (F =578.218,P < 0.001).Conclusions DAC can significantly inhibit the proliferation of HL-60 cells and induce apoptosis,synergistic effect can be observed when DAC combined with DNR.The underlying mechanism can be due to DAC demethylation effect to increase PTEN mRNA expression.
8.Effects of intracerebroventricular injection of bicuculline or NMDA on the amnestic effect induced by etomidate

Aim To investigate the relationship between GABAA receptor or NMDA receptor and the amnestic effect induced by etomidate.Methods Amnestic model was established by intraperitoneal injection of etomidate(3 mg?kg-1) in mice before intracerebroventricular injection of different doses of bicuculline or NMDA,then the error times,step down latency and step through latency were observed and recorded in the step down test and step through test.Results Bicuculline(2,4 ?g) instead of NMDA by intracerebroventricular injection could decrease the error times and increase the step down latency and step through latency of amnestic mice in the step down test and step through test.Conclusion GABAA receptor rather than NMDA receptor may be an important target for the amnestic effect induced by etomidate.

Aim To observe the effects of ketamine on Na+,K+-ATPase,Ca2+-ATPase and NOSase activity in different cerebral cortex in convulsive mice.Methods The mice were randomly divided into blank group,normal saline(NS) group and ketamine 25 mg?kg-1 (KetⅠ),50 mg?kg-1(KetⅡ) group. The animals of blank group were killed directly.Convulsion was induced by intraperitoneally(ip) strychnine(1.5 mg?kg-1) in other groups,and correspond drugs were administered ip before five minutes.Action variety of mice was observed. Animals were killed on 30 minutes after strychnine injection.The activity of Na+,K+-ATPase,Ca2+-ATPase,TNOSase and iNOSase were assessed by spestrophotometric analysis in different cere-bral cortex(forehead,parietal and occipital area).Results Ketamine group could decrease mortality completely. The duration of tonic state in KetⅡ group was significantly shorter than that in KetⅠgroup.Compared with blank group,Na+,K+-ATPase and Ca2+-ATP ase activities were decreased in the group of NS and KetⅠ,and recovered normal level in the group of KetⅡ at parietal and occipital area. Aim To investigate the relationship between amnestic effect of enflurane or isoflurane and NMDA receptor.Methods Amnestic model was established by intraperitoneal injection of enflurane(0.4 ml?kg-1)or isoflurane(0.3 ml?kg-1)respectively in mice before intracerebroventricular injection of different doses of NMDA(25,50,75 ng),then the error times,step down latency and step through latency were observed in the step down test and step through test.Results NMDA(50,75 ng)by intracerebroventricular injection could decrease the error times,and increase the step down latency and step through latency of amnestic mice induced by enflurane or isoflurane in the step down test and step through test.Conclusions NMDA by intracerebroventricular injection can improve amnestic effect of enflurane or isoflurane partially.NMDA receptor may be an important target for amnestic effect of enflurane or isoflurane.
