1. Malignant tumor-related mortality in crocidolite-contaminated area of Dayao County in Yunnan Province in China: A 27-year retrospective cohort study

Objective: To investigate the association between environmental exposure (living environment and production environment) to crocidolite and the cancer mortality risk. Methods: A cohort including 1 249 healthy residents recruited from crocidolite-contaminated area in Dayao County, Yunnan Province in 1984 was retrospectively analyzed. The COX regression model was used to estimate hazard ratio (HR ) and 95% confidence interval (CI ) for cancer mortality according to time of exposure to crocidolite and the crocidolite exposure in the production and living environments of blast-furnace manufacturing, material processing, whitewashing the wall, masonry stove building, and starching clothing with slurry. Results: A total of one hundred and twenty-one deaths were identified by April 30, 2011. After adjusting for the confounding factors including age, gender, marriage, education, smoking and alcohol consumption, it was found that as compared with population exposed to crocidolite in five years or less, HRs (95% CIs ) for cancer mortality in populations exposed to crocidolite in 6-10, 11-15 and more than 15 years were 2.898 (1.496-5.616), 4.159 (2.169-7.975), and 4.233 (1.182-9.886), respectively, and a dose-response relationship (P trend<0.05) was observed; compared with population not engaged in production of blast-furnace, the HRs (95% CIs ) for cancer mortality in populations engaged in sporadic production and mass production of blast-furnace were 2.533 (1.549-4.142) and 2.541 (1.642-3.935), respectively; compared with population not engaged in material processing, the HRs (95% CIs ) for cancer mortality in populations using crushing machine in material processing and handcrushing in material processing were 1.811 (1.211-2.709) and 1.785 (1.072-2.793), respectively; the HRs (95% CIs ) for cancer mortality in populations whitewashing the wall for 4 times or more, building the masonry stove and starching clothing with slurry were 1.736 (1.000-3.012), 2.229 (0.97-5.122) and 3.307 (1.522-7.184), respectively. Conclusion: Exposure to crocidolite in living environment and production environment may increase the risk for cancer mortality. Copyright © 2012 by TUMOR.
2.The biocharacterization of intrasplenic transplantation of gene modified mouse fetal liver cells

Objective To develop a genetically modified fetal liver cells (FLC) based transplantation system that can release therapeutic levels of hematopoietic growth factors into the system circulation which can facilitate treatment of patient receiving cytokine therapy following chemotherapy. Method Examine adeno virus mediated gene transfer to isolated murine FLC and evaluate the biocharacterization of intrasplenic transplantation of gene modified murine FLC. Results Substantial transfection rate of 80%~85% were achieved at a ratio of 50 for 2 hr of exposure. Gene modified FLC (FLC GM) labeled with 111 In were injected into the allogenic mice, spleen, the %ID/g of liver was 20%~25% at 24 hr and 50%~55% at 48 hr after transplantation. In addition, serum concentration of GM CSF in mice with intrasplenic transplantation reached its maximum at 48 hr [(356 ?58 ) pg/ml]. Conclusion Intrasplenic transplantation of FLC GM can be predominantly localized in liver and spleen, and engraft rapidly and maintain normal function, which represent a critical step toward successfully accomplishing liver directed gene therapy.
3.Effect of Patient-controlled Epidural Analgesia and Patient-controlled Intravenous Analgesia on Pulmonary Function after Thoracotomy

ObjectiveTo compare the effect of patient-controlled epidural analgesia (PCEA) and patient-controlled intravenous analgesia (PCIA) on pulmonary function in post-thoracotomy patients.Methods33 ASA Ⅰ~Ⅱ patients undergoing selective esophagectomy were randomly divided into the PCEA group (n=16, treated with morphine plus bupivacaine) and PCIA group (n=17, treated with morphine plus droperidol) for 3 days postoperatively. Pulmonary function indices including respiratory rate (RR), tidal volume (Vt), vital capacity (Vc) and pulse oximetry (SpO2) were recorded before operation and on the first 2 days after operation. Pain scores with visual analogue scale (VAS) at rest, deep breathing and with cough, and adverse effects were also recorded.ResultsRR increased, Vt , Vc and SpO2 decreased markedly in both groups postoperatively compared with the base line (P<0.01), but there were no significant differences between two groups. VAS scores were much lower in PCEA group, especially, when the patient was at deep breathing or during coughing (P<0.001).ConclusionPCEA is superior to PCIA in pain relief, but contributes no more than PCIA in improving pulmonary function in post-thoracotomy patients.
4.Effect and Mechanism of Music Exposure on Fearful Behavior of Prenatal Stressed Rats

Objective To investigate the effect of postnatal music exposure on fearful behavior of prenatal stressed rats in defensive withdrawal test and the expression of gastrin-releasing peptide receptor (GRPR) in the amygdala. Methods Wistar pregnant rats were divided in stress group (n=8) and control group (n=8). The stress group were immobilized and bright lighted 45 min, 3 times a day, when gestation 11-20 days, while the control group without any stress. The new born rats from the stress group were exposed in music (SM group) or white noise (SW group) for 45 min twice a day since 1 day after born, and the rats from the control (SC) group left undisturbed. They were tested with defensive withdrawal test 60 days after birth, and the GRPR levels in the amygdala were measured with immunofluorescence and real-time PCR. Results The latency was shorter and the frequence out of the chamber was the most in the SM group of the defensive withdrawal test (P<0.001), while GRPR-positive cells and GRPR mRNA increased in amygdala (P<0.001). Conclusion Postnatal music exposure can counteract the abnormal behavior response of prenatal stressed offspring and accompany with increased expression of GRPR in the amygdala.
5.Comparison of clinical effects among tramadol,buprenorphine and morphine in patient-controlled analgesia

Objective To compare the analgesic and side effects of tramadol, buprenorphine and morphine in equipotent dosage in patient-controlled intravenous analgesia (PCIA). Methods 53 patients (ASA I-II) scheduled for upper abdominal surgery were randomly allocated to receive 10mg/ml tramadol (group T) or 0.03mg/ml buprenorphine (group B) or 1mg/ml morphine (group M) plus 0.1mg/ml droperidol. Baxter APII PCA pump was used to administer the different analgesic according to the model of LCP, with the amount of loading dose, continuous infusion and bolus of 2.5ml, 0.5ml/h, 1ml respectively. At 4, 8, 16 and 24h postoperatively, pain scores in VAS, comfort scale in BCS, sedation scores with Ramsay score, and the incidences of nausea, vomiting, pruritus, respiratory depression were assessed. Drug consumption in the first 24h and the D/D ratio (demand/deliver) were also recorded. Results VAS and BCS scores showed no significant difference among the 3 groups, but drug consumption within the first 24h and the D/D ratio in group T were higher than those in other 2 groups (P0.05). 8 patients in group T, 5 in group B and 4 in group M suffered from nausea; 1 patient in group T, 2 in group B and 3 in group M complained of pruritus. No respiratory depression was observed in all cases. Conclusion Tramadol and buprenorphine can be safely used as PCIA in patients undergoing upper abdominal surgery, but they are not superior to morphine.
6.Press fit technique reconstruction of anterior cruciate ligament with different types of tendon transplants

[Objective]To explore the clinical effects of selecting differents type of tendon transplants to reconstruct anterior cruciate ligament(ACL) using press fit technique.[Method]A total of 101 cases of ACL injury were included in this study,62 cases were athletic injuries,22 cases were traffic injuries,and 17 cases were daily-life injuries.According to patient's condition using different types of tendon transplant in which hamstring-tendon-knot 42 cases,bone-bolt-hamstring tendon knot 32 cases,patella bone-quadriceps tendon 27 cases,the tendon-knot was completely fit-into the bottle-necked femoral tunnel,and the tibial tunnel were pulled ont and seperated into two bundles, then tie a knot to fix the tendons tightly onto the bone bridge.[Result]In this group 98 patients were followed-up,According to the knee joint effective evaluate standard,88 were excellent,6 good and 4 fine.Excellent and good rate was 85.7%.[Conclusion]According to patient's condition,different type of tendon transplant to reconstruct ACL by press fit technique should be selected carefully.It is an method of easy to manipulate,reliable fixation,minimized trauma,biological fixation,economical and free expensive implant technique.
7.Lateral retinaculum patellae solution and medial patellofemoral ligament reconstruction using the semitendinosus tendon free grafted for the treatment of recurrent dislocation of the patella

Objective To investigate the outcomes of arthroscopically lateral retinaculum patellae release and medial patellofemoral ligament reconstruction using the semitendinosus tendon autograft for the treatment of habitual or recurrent dislocation of the patella. Methods 18 cases of recurrent dislocation of the patella were included in this group, among which there were 6 males and 12 females. The average age was 20 years old(range from 17 to 24 years). Arthroscopic investigation found tensed lateral retinaculum patellae and damaged cartilage at patellar medial margin and femoral lateral condyle. There were 6 cases of Ⅱdegree cartilage injuries, 5 cases of Ⅲ degrees injuries, and 7 cases of Ⅳ degree injuries caused by patellofemoral joint impact. For cases of Ⅳ degree injuries, the damaged cartilage was shaved and microfractured. For cases of Ⅱ or Ⅲ degree injuries, radiofrequency system was used for debridement. After the lateral retinaculum patellae was extensively released, semitendinosus tendon autograft was used to reconstruct the medial patellofemoral ligament. The knee joint was flexed and extended. Patella's dynamic motion, the tension of the tendon and relationship between the femoral trochlea and patellar bone were arthroscopically observed. Interface screw was used to fix the tendon ends within the bone tunnel on condylus medialis femoris. Results All patients were followed up for 9 to 19 months averaging 13 months. No patellar dislocation occurred postoperatively. Apprehensive test, patella tilt test were negative; and Apley test were negative in 16 patients and positive in 2 patients. One case of inferior pole of patella fractured. Axial view of X-ray and MR images of patella showed 15 cases regained normal anatomical relation of patellofemoral joint. 3 cases had mild semi-dislocation. Lysholm's score averaging (78.0?4.5) points preoperatively and (94.0?3.5) points postoperatively. Conclusion Lateral retinaculum patellae solution and medial part reconstruction under arthroscope can effectively cure recurrent subluxation of the patella and improve knee joint function.
8.Analysis on the Chinese disease control and prevention institutions` laboratory testing capacity

Objective: The objective of this study is to understand the construction of laboratory testing capacity of the Chinese Disease Control and Prevention institutions at all levels and to expound the problems and puts forward the related suggestions and countermeasures which can provide the basis for scientific and corresponding improvement of laboratory construction.Methods: The whole procedures were completed through descriptive epidemiological studies using SAS 9.2 Software and Excel 2010 for conducting statistical and trend analyses.Results: There is an increasing trend in the number and proportion of inspections in the disease control institutions at all levels in China.The number of disease control and prevention institutions` special testing laboratories increased significantly in 2012 and then the growth rate undergone a slow-down.The owning rate of equipped laboratory appliance rapidly rose in 2011 and started to level off after that.The average number of equipment in eastern region is greater than that drawn in the central and western regions.According to rate of compliance with the standard of basic laboratory projects, there is a trend in the provincial, district and county level disease control and prevention institutions where the eastern region comes first, followed by the central and then the western region closes the line.However, in general, this rate of those provincial level institutions show a state that the central region holds the highest rate, the eastern region comes second, and the western last.Conclusions: The qualities of health inspection personnel still need to be further improved in disease control and prevention institutions at all levels in the country.In the aspect of laboratory equipment possession rate, the provincial-level CDCs have greater and absolute advantage than the county-level and district level institutions.The testing projects of CDCs in the central and eastern regions of China have a relatively good level, while those tracked in the western region are relatively weak.In view of this, China should strengthen the support to western region in terms of human resource, equipment and funding.The provincial-level disease control and prevention institutions` laboratory testing and diagnosis projects are the best in balance and fairness.Improving the testing capacity is an effective approach to promote the development of disease control and prevention.Hence, the state should pay much attention to the laboratory construction works and management strategies and flows.
9.Developing aptamers into tumor diagnostics and therapeutics

Aptamers are small single-stranded nucleic acid molecules that bind a target protein with high affinity and specificity. Due to their stability, low toxicity and immunogenicity, as well as improved safety, aptamers are attractive alternatives to antibody and are therefore suitable for in vivo applications. Aptamers are typically isolated, through a process termed SELEX (systematic evolution of ligands by exponential enrichment), from combinatorial libraries with desired proteins. In the present review, the recent non-conventional aptamer selection process will be discussed together with an overview on the aptamer application in cancer diagnosis and therapy.
10.Acceleration of the Recovery of Chemotherapy-Induced Immune Suppression by the Intrasplenic Transplantation of GM-CSF Gene-Transfected Fetal Liver Cells

After murine fetal liver cells (FLC) were transfected with granulocyte-macrophage colony-stimulating factor (GM-CSF) gene by recombinant adenovirus and intrasplenically transplanted into allogeneic mice, the effects of GM-CSF gene-transfected FLC on the recovery of immune response inhibited by chemotherapy were observed. The number of CD4 + cells and the ratio of CD4 + /CDS + cells from peripheral blood lymphocytes increased significantly. The cytotoxicity of the NK cells and the proliferation response of splenocytes to ConA, LPS elevated markedly, but the same results were not from bone marrow. These data demonstrated that intrasplenic transplantation of GM-CSF gene-transfected FLC could effectively accelerate the recovery of immune response after high-dose chemotherapy.
