1.The relationship of self-management skills and the quality of life among solitary elderly patients with hypertension

Objective To investigate the relationship of self-management skills and the quality of life among solitary elderly patients with hypertension.Methods 110 cases of solitary elderly patients with hypertension were measured with self-designed Self-management of Hypertension Scale and Short Form Health Survey (SF-36) and the correlation of them was analyzed.Results The levels of self-management skills of elderly hypertensive patients was at a middle level,and the mean score of self-management skills was (78.6 ± 5.2) points.The total score of SF-36 of patients with good self-management skills was higher than those with bad self-management skills.The therapy management,catering management,psychosocial management,sports management and overall quality of self-management of elderly patients with hypertension were positively correlated with survival quality.The factors influencing self-management ability of patients incuded education,gender,social resource utilization,psychological conditions.Conclusions Community health nurses should pay attention to self-management skills such as diet,exercise,therapy,social and psychological aspects in elderly hypertensive patients in order to improve their quality of life.Furthermore community nurses should strengthen intervention for solitary eldedy males with lower education level,poor utilization of social resources to improve their self-management skills.
2.Effect of dexmedetomidine hydrochloride on senile patients with hypertension during cataract surgery

AIM:To investigate the effect of dexmedetomidine hydrochloride on hemodynamics, sedation and analgesia effect during cataract surgery for senile patients with hypertension.METHODS: Totally 90 senile patients with hypertension receiving cataract surgery were randomly divided into two groups, 45 cases in study group during the surgery by intravenous infusion of dexmedetomidine hydrochloride, 45 cases in control group were given normal saline for intravenous infusion.Blood flow dynamics index level were detected in two groups at five time points of before surgery (T0), beginning of the surgery (T1), 10 min after beginning (T2), 20 min after beginning (T3), the end of surgery (T4).Analgesia and sedation scores were compared in two groups, the incidence of adverse reactions and complications were recorded in two groups.RESULTS: Heart rate (HR), diastolic blood pressure (DBP), systolic blood pressure (SBP) increased significantly at the time points of T1, T2, T3, T4 compared with T0 time point in the control group (P<0.05).HR, DBP, SBP decreased significantly at the time points of T1, T2, T3, T4 compared with T0 time point in the study group (P<0.05), which of the study group were significantly lower than the control group at the same time (P<0.05).Analgesia scores at the time points of T1, T2, T3, T4 were showed different degrees of decline in the two groups, and analgesia scores in the study group were significantly lower than that of the control group at the same time (P<0.05).Sedation scores at the time points of T1, T2, T3, T4 were showed different degrees of decline compared with T0 time point in the control group (P<0.05).Sedation scores at the time points of T1, T2, T3, T4 were showed different degrees of rise compared with T0 time point in the study group (P>0.05).Sedation scores in the study group were significantly higher than that of the control group at the same time (P<0.05).The incidence of adverse reactions and complications in the study group was significantly lower than that of the control group (P<0.05).CONCLUSION: Application of dexmedetomidine hydrochloride to monitoring anesthesia during senile cataract patients with hypertension surgery, can stabilize the hemodynamics and has obvious sedation and analgesia effect.
3.Transurethral ultrasonography in the diagnosis and staging of bladder tumor

Objective To evaluate transurethral ultrasonic scanning in the diagnosis and staging of bladder tumor. Methods Form 1996 to 2000 transurethral ultrasonography(TUUS) was carried out for 70 cases of bladder cancer and the findings were compared with cystoscopy,transabdominal ultrasonography(TAUS),CT,MRI and pathological studies. Results The diagnosis rate of TUUS has been 99.1% being better than cystoscopy and TAUS.The accurate staging rate was 100.0% in stage T 1 tumors, 95.8% in stage T 2,82.4% in stage T 3 and 55.6% in stage T 4. Conclusions TUUS can clearly reveal the depth of tumor infiltration and therefore is an important method for the diagnosis and staging of early bladder tumor.
4.Isolated Rate and Drug Resistance of Mycoplasma from Genitourinary Tract

OBJECTIVE To investigate the characteristics of mycoplasma infection and antifungal resistance to guide the clinical reasonable usage.METHODS Mycoplasma was isolated and its antibiotic susceptibility was detected by Mycoplasma IES test kit.RESULTS Among the 1103 samples,418(37.8%) were positive,including 268 with Ureaplasma urealyticum(Uu)(64.1%),13 with Mycoplasma hominis(Mh)(3.1%),and 137 with mixed infection(Uu and Mh)(32.8%),Young people with the age from 20 to 45 were the highest group of infection(80.1%),Sensitive antibiotics to mycoplasma in precedence order were doxycyline,minocycline,tetracycline,josamycin,and clarythromycin.CONCLUSIONS Mycoplasma is a common pathogen resulting in urogenital tract infection.We should pay more attention to monitor mycoplasma infection and use antibiotics properly and to guide clinically antibiotics usage.
5.Synchronizing presentation of cyclins and merlin in single cell in vestibular Schwannoma tissues with confocal laser microscopy

Objective To explore the expression of merlin and cyclins and the nucleocytoplasmic shuttling of merlin in abiogenous vestibular schwannoma tissues. Methods Four paraffin imbedding vestibular Schwannoma samples were included in the study.The cyclins and merlin were analyzed by immunofluorescence at the same time,and the expression and location were synchronously observed with confocal laser microscopy(CLM) in a single schwannoma cell. Results Cyclins and merlin were perspicuously and synchronously presented in a single cell in vestibular schwannoma tissues.In early G1 phase of the tumor cell,merlin was mainly stained in the nucleus,followed by perinucleus and cytoplasm.In G1/S phase,merlin was concentrated in the perinucleus,less in the nucleus and cytoplasm.Merlin staining showed uniform distribution in the nucleus in G2/M phase,while was absent from the nucleus and concentrated in the perinucleus in late G2/M phase. Conclusion The expression of merlin and various kinds of cyclins can be synchronously presented in abiogenous vestibular schwannoma cells with immunofluorescence and CLM.Merlin is shuttling among the nucleus,cytoplasm and perinucleus along with the progression of cell cycle.
6. Preparation of Epalrestat Solid Dispersion by Hot-Melt Extrusion Technique

OBJECTIVE: To prepare epalrestat (EP) solid dispersion by holt-melt extrusion (HME) technique in order to enhance its dissolution rate. METHODS: Optimal formula was obtained by screening the carrier type and drug proportion. The dispersion state of EP in the solid dispersion and physical mixture were studied with differential X-ray diffraction (X-RD), Fourier transform infrared spectroscopy (FT-IR) and differential scanning calorimetry (DSC) analysis techniques. RESULTS: Poloxamer 188 was selected as the carrier with the optimal ratio of Poloxamer188 to EP of 1∶3. The final solid powder product was homogeneous dispersion in light yellow color without any agglomerate. CONCLUSION: The carrier of EP solid dispersion has significant impact on the dissolution behavior and state of EP. Using Poloxamer 188 as the carrier, holt-melt extrusion is an effective technology for improving the in vitro dissolution of EP.
7. Determination of calcitriol and 5, 6-trans-calcitriol in soft capsules by RP-HPLC

OBJECTIVE: To establish a new method for determination of the main active ingredient and its isomer in commercially available calcitriol capsules by reverse phase-high performance liquid chromatography-diode array detector (RP-HPLC-DAD). METHODS: The chromatographic separation was performed on a Symmetry C18 column (4.6 mm × 250 mm, 5 μm) with gradient elution of water-acetonitrile-methanol. The mass spectrometer was equipped with diode array detector. Using Chem Station chromatography workstation, the main ingredient and its isomer in commercially available calcitriol capsules were analyzed. RESULTS The linear ranges of calcitriol and its isomer were 0.171 4 - 1.36 and 0.161 3 - 1.28 μg·mL-1, the detection limits were 39.75 and 40.90 ng·mL-1 and the limits of quantification were 141.6 and 136.4 ng·mL-1, respectively. The specificity of the method was good, the RSD values of the precision and stability tests were less than 1.2%, and the recoveries were greater than 98%. The average contents of calcitriol capsules from three manufacturers were 72.71%, 80.35%, and 78.18%, respectively. CONCLUSION: The method is simple, rapid, accurate, and easy to be popularized, which is suitable for the determination of calcitriol and trans-calcitriol in calcitriol capsules.
8.A comparative study of two common optic nerve crush models in rats

Background Optic nerve crush (ONC) model is an available tool in the basic research on the mechanism and treatment of optic nerve injury.The opening optic nerves sheath crushing and via bulbar conjunctiva lateral canthus optic nerves crushing are frequently used ONC modeling methods.However,the comparison between these models is not elaborated.Objective This study was to compare the outcomes between opening sheath ONC model and via bulbar conjunctiva lateral canthus ONC model.Methods Twenty-four male SD rats were randomized into four groups.ONC models were established via superorbital rim opening sheath to crushing optic nerve for 20 seconds in the opening sheath ONC group,or via bulbar conjunctiva lateral canthus cutting to crush optic nerve for different time in the via bulbar conjunctiva ONC 20-second,40-second and 60-second groups,respectively.All models were monocular created in the rats,and the fellow eyes served as controls.In 14 days after modeling,flash visual evoked potential (F-VEP) were recorded,optic nerve and retinal sections were prepared in the rats.The histopathology of the samples was examined by hematoxylin and eosin staining.The expression of Brn-3α in the retinal ganglion cells (RGCs) was detected by immunofluorescence technique and the number of Brn-3α+ RGCs was counted.The modeling procedure and outcomes were compared between the two approaches.Results The latencies of P1 waves were significantly extended in the opening sheath ONC group,via bulbar conjunctiva ONC 20-second,40-second and 60-second groups in comparison with the corresponding control eyes (t =-11.64,-8.04,-6.50,-10.84,all at P<0.01).The P1 latencies were longer in the opening sheath ONC group than those in the via bulbar conjunctiva ONC 20-second,40-second and 60-second groups (P =0.01,0.02,0.05),but no significant differences were found in the amplitudes of P1 waves between model eyes and corresponding control eyes (all at P>0.05).The Brn-3α+ RGCs numbers were evidently decreased in the model eyes in comparison with the fellow control eyes.The Brn-3 α + RGCs numbers were (13.60 ± 2.14),(18.74 ± 3.61),(15.84 ± 2.31) and (14.58 ± 3.23)/field in the opening sheath ONC group,via bulbar conjunctiva ONC 20-second,40-second and 60-second groups,which reduced to 47.49%,67.70%,56.69% and 50.17% of the fellow eyes,respectively.No significant differences were seen in the Brn-3α+ RGC numbers between the opening sheath ONC group and via bulbar conjunctiva ONC 40-second or 60-second groups (both at P>0.05).The disorder of glial cell arrangement,vacuolization of the cell matrix and infiltration of inflammatory cells were displayed in various model groups,with the prominent findings in the opening sheath ONC group.Conclusions Compared with the via bulbar conjunctiva ONC models,the morphological and functional damage of optic nerve is more obvious,and the survival rate of RGCs is lower in the opening sheath ONC models.
9.Model of immune-mediated inner ear disease and efficacy of dexamethasone by systemic application or round window administration

Objective To establish a model of immune-mediated inner ear disease(IMIED) and to compare the efficacy of dexamethasone between systemic application and round window adminstration. Methods The guinea pigs were injected with keyhole limpet hemocyanin(KLH) mixed with complete Freund's adjuvant subcutaneouly in the back,and were boosted with KLH mixed with incomplete Freund's adjuvant two weeks later.Meanwhile,a small piece of gelatin sponge full of KLH was delivered on the round window through operation.Thus established a model of IMIED.Micro-osmotic pumps were used to deliver dexamethasone through the round window(PBS control group also set),and systemic application was implemented by intraperitoneal injection.The efficacy was compared by recording auditory brain-stem response(ABR) before and after the treatment. Results None of the 6 guinea pigs in the control group experienced significant hearing loss,while 22 of the 39 guinea pigs in the experiment group suffered from hearing loss beyond 10 dB.Seventeen guinea pigs with hearing loss no less than 15 dB were divided into three groups randomly and were given different treatment.Those(n=6) treated by local application through the round window enjoyed the efficacy and the mean ABR threshold decreased 13.3 dB.Four out of the 6 treated by systemic application enjoyed the efficacy and the mean ABR threshold decreased 13.7 dB.No efficacy was observed in the PBS group. Conclusion The model of IMIED can be successfully induce by KLH through the round window,and dexamethasone administered through the round window is as efficacious as systemic application.
10.Comparison of Electrogastrogram and Gastroscopy in 136 Children with Gastric Disease

Objective To study and evaluate the consistency and diagnostic value of electrogastrogram(EGG) and gastroscopy in children′s gastrosis diagnosis.Methods A total of 136 children were checked with gastroscopy and EGG during the same period,then compared the diagnostic accordance rate with 2 ways,and analyzed the characteristics of EGG.Results EGG showed certain characteristics in different gastrosis.Diagnosis of EGG in the group of functional illness was consistence with gastroscopy at 95.8%;others were less than 40%.Conclusion EGG can be used as a method of special value to diagnose children′s functional gastrosis and a screening means in general gastrosis.
