1.Early repair and reconstruction for the wounds of face and joints of mass burn casualties

Objective To repair and reconstruct severe burn wounds on face,hands and joints with the principles and techniques of plastic and aesthetic surgery.Methods Twenty three burn victims in a single batch were transferred from a remote area to our Burns Institute on post burn day 2,June 28,2006.All the patients were males aged from 17 to 46 years,with a mean of 24.4?6.3 years,the mean total burn area was 15.7%?13.9% TBSA(ranged from 6% to 75%),with moderate to severe inhalation injury,and fourteen of them had received tracheostomy.The burn areas on face ranged from 0.5% to 3% TBSA,and all of them were deep second or third degree burn.The burn areas on hands ranged from 1% to 4% TBSA of deep second or third degree.On post burn day 3,the patients received escharectomy or tangential excision of eschar on hands followed by split thickness skin grafting with the principles and techniques of plastic and aesthetic surgery.During post burn week 2 to 3,the eschar on the faces were peeled off,followed by split-thickness skin grafting according to the principles and techniques of plastic and aesthetic surgery.Results All the wounds on the face and joints were repaired successfully,the configuration and functions were satisfactory.Conclusion It is feasible to repair the burn wounds on face and joints with the principles and techniques of plastic and aesthetic surgery for the burn patients of mass fire catastrophe.
2.Expression of granulocyte macrophage-colony stimulating factor in patients with psoriasis

Objective To investigate the expression of granulocyte macrophage-colony stimulating factor(GM-CSF)in skin lesions and sera of patients with pustular psoriasis and psoriasis vulgaris.Methods Tissue specimens were obtained from the lesions of 15 patients with pustular psoriasis,15 patients with psoriasis vulgaris and 15 normal human controls.Immunohistochemistry and dual antibody sandwich enzyme-linked immunosorbent assay(ELISA)were carried out to detect the levels of GM-CSF in the tissue and serum specimens from the patients and normal human controls,respectively.Results Significantly higher levels of GM-CSF were observed in the tissue and serum specimens from patients with pustular psoriasis and psoriasis vulgaris compared with the normal controls(all P < 0.01),as well as in those from the patients with pustular psoriasis compared with the patients with psoriasis vulgaris(both P < 0.01).Conclusion GM-CSF may be involved in the pathogenesis of psoriasis.
3.Study of emotion recognition under anxiety based on physiological signals by relief method.

Anxiety is usually generated because of the threatened feeling. The data of electrocardio, respiration, blood volume pulse and skin conductance signals were collected. The arithmetic of Relief were used for the feature selection and combined with k-Nearest Neighbor (kNN) arithmetic and Support Vector Machine (SVM) arithmetic for classification. The results show that the combination of Relief-SVM is better than combination of Relief-kNN on the recognition of anxiety state. The emotion recognition based on multi-physiological signals is superior to that based on one single signal.

6.Protective effect of creatine phosphate sodium pretreatment on circulatory function in total knee replace-ment surgery using bone cement implantation

Objective This study is to investigate the protective effect of creatine phosphate so-dium on circulatory function in unilateral total knee replacement surgery using bone cement implanta-tion.Methods Forty patients undergoing unilateral total knee replacement surgery with bone cement were randomly assigned into 2 groups:test group (group CP,n=20)and control group (group NS, n=20).The easophageal doppler ultrasound probe was placed at the level of the third rib transorally after induction in each patient.In group CP,creatine phosphate sodium (2 g in 100 ml normal saline) was infused (within 30 min)at 30 min before the operation,as well as normal saline 100 ml in group NS.SBP,DBP,HR,SpO2 ,PET CO2 and BIS were monitored and recorded before (T0 )and 1 (T1 ),3 (T2 ),5 (T3 ),10 (T4 )min after insertion of bone cement.Cardiac output (CO),stroke volume (SV)and left ventricular end-diastolic volume (LVEDV)were simultaneously monitored and recorded with esophageal doppler ultrasound.WhileⅡ-lead electrocardiogram were recorded to monitor ven-tricular arrhythmia perioperatively.Results SBP,DBP and HR decreased at T2 and T3 in group NS, and were lower than those in group CP (P <0.05 );Meanwhile,compared with group CP,CO and SV were significantly lower,while LVEDV was significantly higher in group NS (P <0.05 ).Com-pared with T0 ,CO and SV decreased and LVEDV increased at T2 and T3 in group NS (P <0.05 ). After insertion of bone cement,the incidence of arrhythmia in group CP was obviously lower than that in group NS (P < 0.05 ).Conclusion Pretreatment with creatine phosphate sodium can effectively prevent the incidence of bone cement implantation syndrome (BCIS)by stabilizing hemodynamic in elderly patients undergoing total knee replacement with bone cement.
7.The role of TNF-αand sICAM-1 in periodontitis of rat model exposed to chronic intermittent hypoxic

Objective:To investigate the expression of tumor necrosis factor-α(TNF-α)and soluble intercellular adhesion molecule -1(sICAM-1)in blood serum and gingival tissue in rat periodontitis model in normoxia and chronic intermittent hypoxia environment. Methods:32 male SD rats were randomly divided into 4 groups(n =8):normoxia control group(group A),normoxia periodontitis group(group B),hypoxic control group(group C)and hypoxia periodontitis group(group D).The periodontitis models were estab-lished by ligating the bilateral maxillary second molar and raised by periodontitis diet.The rats in hypoxia groups were raised under chronic intermittent hypoxia environment,while those in normoxia groups were raised under normoxia environment.After 8 weeks, plaque index(PLI),bleeding index(BI)and attachment lost(AL)were measured,TNF-αand sICAM-1 concentrations in blood serum and gingival tissue were measured by ELISA.Results:TNF-αand sICAM-1 concentrations in blood serum and gingival tissue in group A were higher than those in group B,C(P <0.05);in group D were lower than in group B and C(P <0.05).TNF-αand sICAM-1 levels in blood serum and gingival tissue were positively correlated with AL(P <0.05).Conclusion:In chronic intermittent hypoxic environment TNF-αand sICAM-1 may aggravate periodontitis,and promote the inflammatory response to peripheral vascular system.
8.Effect of tea polyphenols on oxidative stress and micro inflammatory condition in maintenance hemodialysis patients

Objective To investigate the influence and the possible mechanism of tea polyphenols on maintenance hemodialysis (MHD)patients’ oxidative stress and micro inflammatory condition.Methods 60 cases of MHD patients were randomly divided into routine group and tea polyphenols group.30 healthy subjects were chosen as normal control group.After 3 months,MDA,AOPP,SOD,TNF-α,CRP,IL-6 and NF-κB of routine group and tea polyphenols group were detected.Results Before treatment,compared with normal control group,MDA and AOPP of routine group and tea polyphenols group increased (P <0.01 ),SOD decreased (P <0.01 );after treatment,compared with routine group,MDA and AOPP of tea polyphenols group decreased (P<0.01),SOD increased (P<0.01).After treatment,compared with normal control group,NF-κB,TNF-α,CRP, IL-6 and oxidative index of routine group and tea polyphenols group increased,and the difference was statistically significant (P<0.01);compared with routine group,oxidative index of tea polyphenols group decreased,and the difference was statistically significant (P <0.01 ).Conclusion Tea polyphenol can improve MHD patients’oxidative stress and micro inflammatory state.
9.ALK-positive large B-cell lymphoma: a case report and review of the literature

Objective To study clinicopathologic features of ALK-positive large B-cell lymphoma.Methods The clinical data,histopathological characteristics,immunophenotype and fluorescence in situ hybridization (FISH) result of a patient with ALK-positive large B-cell lymphoma were analyzed and discussed combined with related literatures.Results A 30-years-old male patients with the left neck lymphadenectasis was studied.Histological evaluation revealed the tumor grew in sheets in the nodal,with round nuclei,dispersed chromatin,a single prominent central nucleolus and moderate amounts of eosinophilic to amphophilic cytoplasm.The neoplastic cells exhibited immunoblastic/plasmablastic morphology.Immunohistochemistry measurement showed that the tumor cells were marked positively by CD138,ALK-1,CD45RO,CD4,Perforin,CD117 and Kappa proteins,while negatively by CD3,CD8,CD20,CD30,CD38,CD57,CD79a,Pax-5,EMA and AE1/AE3 proteins.FISH test demonstrated the presence of ALK gene translocation.The patient was given 4 cycles of CHOP chemotherapy after surgery.However,the conditions deteriorated after 4 months.Now the patient continued to receive treatment.Conclusion ALK-positive large B-cell lymphoma represents a distinct variant of diffuse large B-cell lymphoma,and the tumor has special histological features along with a distinct immunophenotype and ALK gene rearrangement.
10.Quality of life and coexisting depression and anxiety of female nurses in general hospitals

Objective To investigate the relationship between coexisting depression and anxiety (CDA)and quality of life of female nurses in general hospitals.Methods Seven hundred and ninety-three female nurses completed Zung' s Self-rating Depression Scale (SDS),Zung' s Self-rating Anxiety Scale (SAS) and the World Health Organization' s Quality of Life Questionnaire-Brief Version (WHOQOL-BREF).According to the cut-off scores of SDS and SAS,all subjects were divided into 4 groups (non-depression and non-anxiety,pure depression,pure anxiety,and CDA).Group differences in SDS,SAS and WHOQOL-BREF scores were compared.Results Of these 4 groups,CDA group had the highest SDS and SAS scores (P < 0.001) (SDS scores:(34.3 ±5.4),(47.1 ±4.0),(39.4 ± 2.4) and (49.7 ±4.2)) and the lowest WHOQOL-BREF scores (P≤0.033)(WHOQOL-BREF physical domain scores:(15.6 ± 2.0),(13.8 ± 2.0),(13.6 ± 1.6) and (12.1 ± 2.0)).The corresponding stratification analysis (controlling for confounding factors) showed consistent results.Conclusion CDA nurses have the most severe depressive and anxious symptoms and the worst quality of life,and thus are the key target population of mental health service.
