1.Systematic Review of Efficacy of Tibolone Tablet in the Treatment of Senile Vaginitis

OBJECTIVE:To systematically review the clinical efficacy of Tibolone tablet in the treatment of senile vaginitis, and provide evidence-based reference for clinical treatment. METHODS:Retrieved from PubMed,Medline,CJFD,CBM,VIP and Wangfang Database,randomized controlled trials(RCT)about Tibolone tablet in the treatment of senile vaginitis were collct-ed. Meta-analysis was performed by using Rev Man 5.2 software after data extract and quality evaluation. RESULTS:Totally 10 RCTs were enrolled,involving 909 patients. Results of Meta-analysis showed Tibolone tablet can significantly improve the cure rate of senile vaginitis [RR=1.46,95%CI(1.32,1.61),P<0.001],maturation vaginal cell index score [SMD=1.83,95%CI(1.65, 2.01),P<0.001] and vaginal health score [SMD=1.56,95%CI(1.21,1.92),P<0.001],reduce recurrence rate [RR=0.07,95%CI (0.02,0.18),P<0.001] and vaginitis score [SMD=-1.44,95%CI(-1.79,-1.08),P<0.001]. CONCLUSIONS:Tibolone tablet has good clinical efficacy in the treatment of senile vaginitis,it can improve the vaginal mucosa estrogen and vaginal environment, reduce recurrence rate.
2.Updated diagnosis and management of chronic orchialgia.

Chronic orchialgia (CO) is a common complaint in urology or andrology. Due to its complicated pathogenesis, the diagnosis and treatment of CO are quite challenging. Based on different etiologies, CO can be idiopathic or secondary. Idiopathic CO accounts for approximately 50% of the cases and is probably associated with Wallerian degeneration in the spermatic cord nerves and peripheral sensitization. Secondary CO can be attributed to direct causes and its treatment focuses on the pathologic condition identified. The main methods for the treatment of CO include conservative and surgical strategies, among which microsurgical spermatic cord denervation ( MSCD) is an effective and minimally invasive option, while orchiectomy is but the last alternative when no other means is left.

3.Myoepithelioma of nasal cavity: a case report.

Myoepithelioma is an uncommon benign tumor that most likely occurs in the salivary gland. Extra-salivary myoepithelioma are sporadic and rare, especially the quantities occur in the sinonasal cavity are fewer in related literature reported. In this paper, we present a rare case of benign myoepithelioma that occurred in the nasal cavity, and the related process of pathogenesis, diagnosis, treatment and prognosis are discussed.

4.Giant ossifying fibroma of nasal cavity and nasal sinuses: one case report.

A 39 years old male patient was admitted with the right of facial abnormalities and gradually protruding in right eye more than 30 years. CT shows high mixed density, clear boundary, and the right eye was crowded to the right. Giant tumor had completely been excised, and the result was fine. The outline figure and function of patients were obviously improved. The pathological examination after resection showed ossifying fibroma.

5.Progress of high density lipoprotein in resisting atherosclerosis

Coronary atherosclerotic heart disease is the leading cause of death in the world of non-infectious diseases.High density lipoprotein ( HDL ) plays an important role in the progression of anti-atherosclerosis.Recent studies have found that HDL can carry microRNA, and the anti-inflammation function of endothelial cell is achieved by the transport of microRNA.The specific functions of different HDL subtypes in the anti-atherosclerosis have been gradually recognized.At the same time, a series of traditional HDL anti-atherosclerotic functions also have been newly discovered, such as reverse cholesterol transport, antioxidant, anti-inflammatory and the protection of vascular endothelium.All of these advances provide a new basis for the clinical application of HDL.

To explore the relationship between the perioperative change in erythrocytic pyruvate kinase (PK) activity and surgical hyperglycemia in the patients undergoing upper abdominal surgery under intravenous procaine balanced anesthesia. Seventeen patients with ASA grade Ⅰ~Ⅱ, being scheduled for selective cholecystectomy or subtotal gastrectomy, were randomly enrolled in our study. PK activity of erythrocytes and its pertinent modulators, such as adenosine triphosphate (ATP), adenosine diphosphate (ADP), inorganic phosphate (Pi), magnesium, plasma glucose and serum insulin were dynamically assayed in the surgical patients receiving intravenous procaine balanced anesthesia. The results showed that PK activity was decreased significantly at 10 min and 24 hours after operation as compared with that of preoperative period. Changes in PK activity were positively correlated with ATP/ADP ratio( r =0 680, P 0 05). It is our supposition that the decreased PK activity and the disturbance of glycolytic pathway might be directly or indirectly induced by anesthesia and surgical trauma, leading to hindrance of utilization of glucose and Pi, as well as synthesis of ATP. Therefore, an inhibition of glycolytic reaction is one of the important mechanisms of "surgical hyperglycemia".
8.A New Family of Regulators of Calcineurin (RCANs)

Calcineurin is a serine-threonine protein phosphatase that plays a pivotal role in a wide series of crucial physiologic processes such as T-cell activation, apoptosis, skeletal myocyte differentiation, and cardiac hypertrophy. A new family of regulators of calcineurin (RCANs) has been shown to modulate calcineurin activity through direct binding of it in vivo. Calcineurindependent signals are transduced to the nucleus by nuclear factor of activated T-cells (NFAT) transcription factors that undergo nuclear translocation upon dephosphorylation and promote transcriptional activation of target genes. The recent researches have revealed that RCAN1 modulating catalytic activity of calcineurin can function as an endogenous backfeed inhibitor during the calcineurin-NFAT signalling pathway. RCANs have now been implicated in several pathological conditions including Alzheimer’s disease, down syndrome and cardiac hypertrophy. In addition, the RCAN family is a rational, functional name for RCAN gene and it is proposed in 2007. It is, therefore, necessary to review the RCAN gene, RCANs and the roles of RCANs in a wide variety of diseases especially including Alzheimer’s disease. It is suggested that regulation of RCAN expression may be a new target on neurodegeneration disease.
9.Clinical analysis of 179 patients of community-acquired pneumonia in out-patients

Objective To analyse the clinical features of community-acquired pneumonia(CAP)in out-patients.Methods A retrospective study was performed on 179 consecutive patients with CAP in fever clinic of Xuanwu Hospital during two months.Results Most of CAP patients were young(aged 14 to 30 years,accounting for 58.1%)and had no-existing diseases(72.6%).Within 2 to 4 days after onset of fever,50%~80% of patients were diagnosed as CAP.About 1/3 of patients had no respiratory symptoms except for fever.Cough and sputum production was seen in 52.5% of patients.WBC was normal in 63.1%.Unilateral pneumonia often occurred(accounting for 82.1%).After treatment with antibiotics,fever in 80%(42/52)of cases disappeared within 3 days in follow-up patients.Conclusion In fever clinic,most patients are young and have no-existing diseases.They can be identified as CAP quickly and have good outcomes.
10.Chemical constituents of Abrus mollis

Objective To study the constituents of Abrus mollis.Methods The constituents were isolated by chromatographic methods,their structures were elucidated by spectroscopic evidences.Results Eleven compounds were purified and their structures were identified,they were identified as ?-sitosterol(Ⅰ),stigmasterol(Ⅱ),nonacosanyl caffeate(Ⅲ),daucosterol(Ⅳ),betulinic acid(Ⅴ),vanillic acid(Ⅵ),inositol methyl ether(Ⅶ),sucrose(Ⅷ),soyasaponin Ⅰ(Ⅸ),kaikasaponin Ⅲ(Ⅹ),and dehydrosoyasaponin Ⅰ(Ⅺ).Conclusion Compounds Ⅰ,Ⅲ,Ⅳ,Ⅵ,and Ⅶ-Ⅺ are isolated from this plant for the first time.
