Androgens and chronic inflammation, which play essential roles in the development of benign prostatic hyperplasia (BPH), are considered to be important factors in disorders of prostate homeostasis. These two factors may lead to pathological hyperplasia in the prostate transition zone of patients with BPH. However, few studies have examined the mechanism of how dihydrotestosterone (DHT) affects chronic inflammation in prostate tissue during the progression of BPH. This study examined the performance of DHT in lipopolysaccharide-treated M1 macrophages and the subsequent effects on the proliferation of prostate stromal and epithelial cells. We found that DHT increased secretion of the pro-inflammatory factor tumor necrosis factor (TNF)-α from M1 macrophages differentiated from THP-1 cells. The supernatant of M1 macrophages promoted the proliferation of WPMY-1 prostate stromal cells by upregulating B-cell lymphoma-extra large (Bcl-xL) and cellular Myc (c-Myc) levels by activating TNF-α-mediated nuclear factor-kappa B (NF-κB) and mitogen-activated protein kinase (MAPK) pathways. Moreover, this supernatant increased the expression of androgen receptor in WPMY-1 cells, which was TNF-α-independent. Additionally, TNF-α protein expression was significantly higher in patients with BPH and a large prostate volume than that in those with a small prostate volume. Further analysis showed that higher serum testosterone combined with prostate-specific androgen concentrations was related to TNF-α expression. This study suggests that DHT modulates the inflammatory environment of BPH by increasing TNF-α expression from lipopolysaccharide-treated M1 macrophages and promotes the proliferation of prostate stromal cells. Targeting TNF-α, but not DHT, may be a promising strategy for patients with BPH.

Shanghai Journal of Preventive Medicine 2023;35(8):799-803
ObjectiveA high-risk group of diabetes in a community in Shanghai was followed up for 2 years. The level of blood glucose control was monitored, the incidence and risk factors of the high-risk group were analyzed, and the incidence and risk factors distribution in the community were studied. The results will provide a basis for the community to formulate strategies for early prevention and treatment of diabetes in Shanghai. MethodsA total of 580 subjects were collected, according to the criteria of high-risk groups of diabetes. Among them, 77 people whose blood glucose had reached the diagnostic criteria for diabetes entered the patient group, and the rest 503 people entered the high-risk group. Corresponding intervention methods such as outpatient follow-up and health education were given, and blood glucose monitoring was repeated at the 12th month and 24th month after enrollment. Blood glucose control, new-onset diabetes and the effect of intervention were analyzed. ResultsThe patients’ venous fasting blood glucose level at the 12th and 24th month was significantly lower than that at the baseline survey, and there was no significant change in body mass index (BMI) and waist circumference. Subjects in patient group were older and had a higher proportion of a history of impaired glycemic regulation, family history of diabetes, hypertension, and hyperlipidemia than those in high-risk group. In the high-risk group of 503 cases, 74 (14.7%) were new-onset diabetics during the follow-up period, A higher proportion of new-onset diabetics were male, BMI, a combined history of impaired glucose regulation and gestational diabetes history of gestational diabetes mellitus (women only) was a contributing factor to the onset of diabetes. ConclusionHigh-risk groups are more susceptible to diabetes; patients in the diabetes group have better control of fasting blood glucose levels during the 2-year follow-up period, and outpatient follow-up combined with comprehensive interventions helps diabetic patients to control blood glucose.

Chinese Journal of Medical Instrumentation 2019;43(2):146-149
OBJECTIVE:
To establish a set of evaluation system of digital radiography clinical value and provide foundation for the maturity assessment of digital radiography.
METHODS:
The evaluation system of clinical value of digital radiography was established by literature survey, expert consultation, and percentage weight method.
RESULTS:
The expert authority coefficients were 0.81 and 0.88,respectively.After two rounds of consultation, variation coefficients of each item ranged from 0 to 0.207,and the coefficient coordination were 0.599.The index system consisted of 5 first-level indexes and 12 second-level indexes. The weights of first-level indexes such as image quality, safety, usability, economic and social indicators share of the weight are 0.298,0.294,0.199,0.121 and 0.088 respectively.
CONCLUSIONS
A completed and scientific evaluation system was established, which provides a scientific assessment tool for clinical value of digital radiography.

BACKGROUNDRepetitive transcranial magnetic stimulation (rTMS) and event-related potentials (ERPs) are a noninvasive technique that widely used in neurophysiological field. Although rTMS has shown clinical utility for a number of neurological conditions, Recently, there was little understanding of the the efficacy of rTMS on Schizophrenia(SZ) and the change of ERP between before and after rTMS treatment. The objective of this study was to investigate the characteristics of N400, mismatch negativity (MMN), and P300 before and after treatment with rTMS in SZ.
METHODSOne hundred and twenty-seven SZ patients hospitalized in Shanghai Mental Health Center from March 2015 to July 2017, divided into two groups (85 patients were recruited as rTMS group and 42 were recruited as sham rTMS [ShrTMS] group) and 76 normal controls (NCs) who were the staff and refresher staff in our hospital were recruited at the same time. A Chinese-made rTMS and a Runjie WJ-1 ERPs instrument were used in the present experiment. N400 was elicited by congruent and noncongruent Chinese idioms. After rTMS treatment, N400, P300, and MMN characteristics were compared with those before treatment and NC group.
RESULTSCompared with NC, the SZ patients exhibited delays in N400, P300, and MMN latency and decreased N400, P300, and MMN amplitudes in their frontal area (P < 0.05). After 25 rTMS treatments, N400 amplitudes in the frontal area (elicited by idioms with same phonic and different shape and meaning and with different phonic, shape, and meaning) were increased in the SZ patients (P < 0.05). However, there was no significant change in N400 before and after treatment with ShrTMS in SZ patients (P > 0.05). Amplitudes for MMN and target P300 also increased in SZ patients after rTMS treatment (P < 0.05).
CONCLUSIONSBased on our preliminary findings, we believe that the combined usage of N400, MMN, and P300 could be a valuable index and an electrophysiological reference in evaluating the effects of rTMS treatment in SZ patients.
