1.Clinical Study on Operation for Rupture of Aortic Sinus Aneurysm

Objective To summarize the ruptured types, operational ways and therapy effect for rupture of aortic sinus aneurysm.Methods The operational cured 55 cases with aneurysm of aortic sinus were retrospectively analysed.Results There were 46 cases of right aortic sinus aneurysm, 45 cases among them rupture was peretrated into right ventricle, one rupture was peretrated into right atrium;there were null aneurysm of aortic sinus, 5 cases among them ruptured into right atrium, 2 cases ruptured into right ventricle; 2 cases of left aneurysm of aortic sinus, ruptured into left ventricular. There were 24 cases with ventricular septal defect, 32 cases with aortic valvular imcompetence, 6 cases with other diseases, 8 cases complicated with infective endocarditis. All cases underwent operational treatment, one patient who had severe left heart failure and huge left ventricle before operation died from refractory arrhythemia after operation, another who had severe infective endocarditis,and patients with cardic function degree Ⅳ were died from low cardic output syndrome, others were better.Conclusions The right aneurysm of aortic sinus were the most types, and it always ruptured into right ventricle; It suggests that it is important for aneurysm of aortic sinus should be operated as early as possible in order to avoid losing the chance of operation, particular in the patients complicated with infective endocarditis .
2.Analysis of the Causes for Misdiagnosis and Therapeutic Errors of Tracheobranchial Foreign Bodies

Objective To discuss the causes of misdiagnosis and therapeutic errors of tracheobronchial foreign bodies.Methods 139 cases of tracheobronchial foreign bodies hospitalized in our department during 5 years were reviewed. Retrospective analysis was performed, on statistics of case history, clinical manifestation, location of foreign bodies, the time of initial diagnosis and the time for definite diagnosis.Results 89 cases (64 03%) were misdiagnosed, and were given inappropriate treatments. The main causes include ignored or vague history of foreign body inhalation, the atypical or complex clinical manifestations, and underestimation of the importance of bronchoscopy.Conclusion We should pay more attention to history of foreign body ,bronchoscopy examination action in diagnosis and treatment and more aware of the various manifestations of tracheobronchial foreign bodies,these could contribute to decrease the misdiagnosis and therapeutic errors.
3.The Relationship between Behavior Types and Plasminogen Activation System in Patients with Coronary Heart Disease

Objectives: To study the difference of Plasminogen Activation System in patients with coronary heart disease by different behavior types. Methods: 81 patients with coronary heart disease and 59 normal control were evaluated by Type A Behavior Scale. t-PA and PAI-1 activities of both groups were also measured. Results: The activity of PAI-1 was higher and t-PA was lower in patients with coronary heart disease, and this difference is also found between Type A and Non Type A patients. Conclusion: The activity of t-PA is decreased and PAI-1 increased in patients with CHD, this phenomenon are also related to Type A Behavior traits.
4.Surgical treatment of double outlet ventricle

Objective: To study the results of surgical treatment for double outlet ventricle. Methods: The results of 72 cases of double outlet ventricle were retrospectively analyzed. Results: There were 71 cases of double outlet right ventricle, including 64 cases of type SDD, 3 type ILL, 3 type SDL, 1 type IDD. Only one case was double outlet left ventricle type ILD. Surgical procedures included left ventricle-aorta intraventricular tunnel connection in 61 patients, total cava-pulmonary artery connection in 2, left ventricle-aorta intraventricular tunnel and right ventricle-pulmonary artery extracardial tube repaired in 3. Six cases underwent bi-directional Glenn procedure. Two cases died from operation. Residue shunt of VSD was observed in one case and re-operation was done to repair the shunt. There were no long-term death and other complications. Conclusion: It is important to choose the right time and proper procedure. Reconstruction left and right ventricle is the essential factor to the surgical success.
5.Cloning of TPA Gene and Establishing of Expressing Model in vitro

Objective To recombine the tissue plasminogen activator (TPA) gene and establish the expressing model in vitro to provide both theoretical basic and technical guidance for gene therapy of ischemic heart disease and prevention of postoperative vessel re-stenosis. Methods Expression vector pcDNA3.1 TPA was constructed, and transfected into Chinese hamster ovary(CHO) cells. Then the exogenous TPA expression was observed. Results The expression quantity of TPA in transfected CHO cells was higher than that in non-transfected cells. The exogenous TPA activity was 12 296 IU/10 6cell/24hr in the transfected cells, when measured by chromogenic substrate assay, while that of non-transfected cells was 3 176IU/10 6cell/24hr. The TPA quantity of non-transfected cells was 9 608 ng/10 6cell/24hr, when measured by enzyme-linked immunosorbent assay (ELISA), while that of transfected cells was 586 172 ng/10 6 cell/24hr, and the latter was the 60 times of the former. Conclusions When pcDNA3 1(+)TPA was transfected into CHO cells, exogenous TPA can efficiently be expressed, which provides theoretical basic for TPA clinical gene therapy.

Objective To investigate the expression and significance of intercellular adhesion molecule-1 (ICAM-1) in graft reperfusion injury after cardiac transplantation in rats. Methods Twenty-four Wistar rats heterotopic cardiac transplantation models were randomly divided into control group (n=6) and experimental group (n=18). In control group, the ischemic period of the grafts was 60min without reperfusion. In experimental group, the grafts were respectively reperfused for 3h, 6h and 24h after 60min ischemic period. 7.The Effect of Mesenchymal Stem Cells Transplantation on Angiogenesis and Secretion of Cytokines After Rat Myocardial Infarction

Objective To investigate the effect of mesenchymal stem cells transplatation on angiogenesis and secretion of cytokines after rat myocardial infarction. Methods Harvested rat mesenchymal stem cells were cultured and labeled in vitro. And at the same time, myocardial infarct model was set up by liquid nitrogen cryoinjuring the left ventricular free wall. 4 weeks later, 2?10 6 mesenchymal stem cells or cold D-Hanks liquid were injected into several different points of infarcted area. At 1, 2 and 4 weeks after mesenchymal stem cells transplatation, specimens were acquired from infarcted area. Then cell morphology and capillary density were measured, and vascular endothelial growth factor(VEGF) and transforming growth factor ?(TGF-?) expression levels were assayed by RT-PCR. Results Mesenchymal stem cells were alive at 1, 2 and 4 weeks after transplantation and had a trend toward differentiation and maturation in vivo. The number of capillary vessels in per infarcted area (0 1 mm 2) in experiment group increased compared with control group. Objective To investigate the effect of mesenchymal stem cells transplantation on heart function and the expression of Ca 2+ -ATPase (ERCA) mRNA after rat myocardial infarction. Methods Wistar inbred rat mesenchymal stem cells were cultivated, proliferated and labeled in vitro. Myocardial infarct models were set up by liquid nitrogen cryoinjuring the free wall of left ventricle. 4 weeks later, 2?10 6 mesenchymal stem cells or cold D-Hanks solution were injected into several different points of infarcted myocardial area of experiment group and control group rats. In addition, 5 Wistar inbred rats served as normal group only got thoracotomy twice. 4 weeks after transplantation, all rats received cardiac function test and specimens from infarcted area were got for the expression assay of SERCA2 mRNA by RT-PCR. Results Mesenchymal stem cells were alive at 1, 2 and 4 weeks after transplantation and had a trend toward differentiation and maturation in vivo. Traumatic dynamic measurement was applied to assay heart function by Buxco system, which showed that SLVP and +dp/dt were the highest in normal group, next in experiment group, and the lowest in control group, which LVEDP was the lowest in normal group, next in experiment group, and the highest in control group. 9.Construction of Expression Vector pcDNA3.1(+)TPA and Study of Its Expression Activity in Human Umbilical Vein Endotheliocytes

Objective To establish the expression of exogenous tissue plasminogen activator (TPA) gene in human umbilical vein endotheliocytes to provide both theoretical basis and new method for gene therapy of ischemic heart disease and prevention of postoperative vessel re-stenosis. Methods Expression vector pcDNA3 1(+)TPA was constructed, and transfected into human umbilical vein endotheliocytes cultivated in vitro by lyposome. The exogenous TPA expression was observed. Results The expression vector pcDNA3 1(+)TPA could efficaciously express TPA in human umbilical vein endotheliocytes. The protein quantity of TPA in the transfected cells was 568 6ng/10 6cell/24h, when measured by enzyme-linked immunosorbent assay, while that in the non-transfected cells was 17 8ng/10 6 cell/24h. The exogenous TPA's activity in the transfected cells was 108 8IU/10 6cell/24h, when measured by chromogenic substrate assay, while that in the non-transfected cells was 5 6 IU/10 6cell/24h. Conclusion When pcDNA3 1(+)TPA was transfected into human umbilical vein endotheliocytes, exogenous TPA could be expressed efficaciously, which provides theoretical basis and new method for TPA gene therapy of ischemic heart disease.
10.Operation for 23 Cases of Coronary Artery Fistula

Objective To observe the operative effects of coronary artery fistula. Methods Malformations and operative results of 23 cases with coronary artery fistula were retrospectively analyzed. Results There were 13 cases of fistula of right coronary artery, 9 cases of fistula of left coronary artery, one cases of fistula of two coronary artery; 9 cases of coronary artery fistula opened into right ventricle,6 cases opened into right atrium, 3 cases opened into pulmonary artery, 2 cases opened into the sinus of coronary veins, 2 cases opened into left atrium, 2 cases opened into left ventricle, 3 cases were multiple fistulas. All cases received surgical treatment, excepttion for one cases of multiple fistulas had small residue fistula, the others were better. Conclusion At pressent,the operation treatment is the best method for fistula of coronary artery .
