1.Effects of high flexion total knee prosthesis treatment of senile knee osteoarthritis in elderly

Objective To investigate the elderly knee osteoarthritis high-flexion total knee prosthesis re-placement therapy clinical efficacy .Methods 68 patients(68 knees) were randomly divided into two groups in ac-cordance with the order of treatment ,each group was treated with high flexion total knee prosthesis replacement therapy and the general treatment of posterior stabilized prosthesis ,compared before and after surgery in March and after a in HSS,WOMAC knee function , maximum knee flexion and knee activity before the pain incidence .Results Two groups of patients HSS and WOMAC after treatment ,3 months,1 year compared with before treatment ,the differences were statistically significant(t=6.28,7.36,all P<0.05),the control group after treatment,3 months and 1 year compared with before treatment,there was statistically significant(t=5.23,7.35,all P<0.05);HSS in observation group after 1 year compared with the control group ,the difference was statistically significant (t=4.69,P<0.05), WOMAC after 3 months compared with the control group,the difference was statistically significant (t =6.49,P<0.05);Two groups of patients knee activity after treatment ,3 months,1 year compared with before treatment , the differences were statistically significant (t=5.24,4.66,all P<0.05),the control group after treatment ,3 months and 1 year compared with before treatment ,the differences were statistically significant (t=5.81,4.64,all P<0.05);ob-servation group after 1 year maximum flexion and range of motion and control group ,the difference were statistically significant(t=6.64,5.33,all P<0.05).The incidence of postoperative anterior knee pain of observation group and control were 2.94%,14.71%,there was statistically significant difference (χ2 =5.23,P<0.05).Conclusion Eld-erly high flexion knee osteoarthritis total knee prosthesis replacement therapy can improve long -term postoperative knee function and range of motion ,which also can reduce knee pain occur .
2.Expressions of epidermal growth factor receptor (EGFR) and Ki-67 in vulvar dystrophy

ObjectiveTo explore the expressions of EGFR and Ki-67 in vulvar dystrophy and their clinical significance MethodsTissue specimens were collected from 160 patients with vulvar dystrophy.Immunohistochemical method was used to detect the expressions of EGFR and Ki-67 in these specimens.The expression levels of EGFR and Ki-67 were compared among different pathological subtypes of vulvar dystrophy,and the relationship of these expressions with clinical pathological characteristics was assessed.ResultsNo significant differences were observed in the expressions of EGFR or Ki-67 between vulvar dystrophy of squamous cell hyperplasia type,lichen sclerosus type and mixed type(P > 0.05).The expression level of EGFR was positively correlated with that of Ki-67 in vulvar dystrophy (r =0.66,P < 0.05).There was a significant difference in the expression rate of EGFR and Ki-67 between patients with different courses of vulvar dystrophy(both P <0.05),but not between patients with different age at onset of vulvar dystrophy(both P > 0.05).Conclusion The expressions of EGFR and Ki-67 are different among patients with different courses of vulvar dystrophy.
3.Practice and Experience of Clinical Pharmacists Participating in One Case of Cancer Patients with Infection Induced by Peripherally Inserted Central Catheter Complicated with Deep Venous Thrombosis

Objective:To explore the methods for drug therapy regimen formulation and pharmaceutical service of clinical pharma-cists for the infection induced by peripherally inserted central catheter ( PICC) complicated with deep venous thrombosis in cancer pa-tients. Methods:Referring to the related guide, clinical pharmacists put forward concrete opinions on how to choose anti-infective drugs and anti-thrombotic drugs for a cancer patient with PICC infection complicated with deep venous thrombosis. Clinical pharmacists sug-gested that vancomycin be used for the infection and low molecular weight heparin sodium for prophylactic anticoagulation. Meanwhile, pharmaceutical care for blood clotting function should be strengthened. Results:The proposals of clinical pharmacist were partly adopt-ed by clinicians. After the therapy, the temperature of the patient returned to normal, and the deep venous thrombosis was well con-trolled. The patient was out of hospital smoothly. Conclusion:Through the participation in clinical practice, clinical pharmacists can assist physicians in optimizing treatment plan and summarize the pharmaceutical care mode for the PICC infection and deep venous thrombosis in cancer patients, which can provide instructions for pharmaceutical care in the future.

Although the concept of united airway disease has been widely accepted, most scholars emphasize only the effect of rhino-sinusitis while ignoring the pharyngeal factors to the lower airway, especially to the allergic pharyngitis (AP), which still lacks enough awareness. First of all, absence of unified diagnostic standard leads to the lack of epidemiological data, which, results in doctors' personal experience but no guideline in treatments. In addition, it is still not clear that the role of AP in the allergic airway diseases and its relationship with asthma. However, the number of patients with AP has been increasing obviously in daily clinic practice. Combined with the previous observation, this paper does a systematic review about the clinical problems of AP, expecting to give a hand to the clinical diagnosis and treatment of AP.

6.Relevance between inflammatory factors and intradialytic hypertension in patients with maintenance hemodialysis

Objective To study the relationship between intradialytic hypertension (IDH) and inflammatory factors in patients with maintenance hemodialysis.Methods Forty maintenance hemodialysis patients were involved,including 20 patients with IDH (IDH group) and 20 patients without IDH(non-IDH group) during hemodialysis.All of them had no sign of acute infections or other progressive disease.The levels of C-reactive protein (CRP),interleukin (IL)-1 β,IL-6,tumor necrosis factor (TNF)-α were measured by enzyme-linked immunosorbent assay (ELISA) before hemodialysis and 1,2,4 h after hemodialysis.The correlation of hypertension to CRP,IL-1 β,IL-6,TNF-α was analyzed.Results The levels of CRP,IL-1β,IL-6,TNF-α were increased in IDH group with the progress of hemodialysis,but were stable innon-IDH group.Logistic regression analysis indicated that there were positive correlations between CRP (r =0.3652),IL-1β (r =0.3261 ),IL-6(r =03156),TNF- α (r =0.3324) and blood pressure during hemodialysis (P < 0.01 ).Conclusion The levels of CRP,IL-1β,TNF-α,IL-6 are positively related to the IDH in maintenance hemodialysis patients.
7.Effect of nateglinide or sequential treatment with metformin on glycemic stability in newly diagnosed type 2 diabetic patients

The effect of nateglinide or sequential treatment with metformin on glycemic stability in newly diagnosed type 2 diabetes was investigated. Thirty-four cases of newly diagnosed type 2 diabetes received nateglinide therapy, or sequential treatment with metformin according to fasting and postprandial blood glucose, and were classified into isolated nateglinide therapy group(n=14) and sequentially treated with metformin group(n=20). Glycemic stability, reflected by mean amplitude of glycemic excursions(MAGE) and HbA1C, was determined in all patients before and after therapy for three months. HbA1C and MAGE in two groups were all improved after treatment(P<0.05). The therapy of nateglinide alone or combined with metformin can significantly improve glycemic stability in newly diagnosed type 2 diabetes.
8.Effects of Particle Size of Ticagrelor Crude Drug on in vitro Dissolution Behavior of Ticagrelor Tablets

OBJECTIVE:To study the effects of particle size of ticagrelor crude drug on in vitro dissolution behavior of Ticagre-lor tablets. METHODS:Ticagrelor crude drug and different particle size of ticagrelor powder A,B,C,D,E after smashing for dif-ferent time(15,30,40,60 s)were used to prepare the tablet by wet granulation method. Accumulative in vitro dissolution rate of prepared tablets within 60 min were determined by UV spectrophotometry at 300 nm(using 0.2% tween as medium,paddle meth-od). Using original tablet as reference preparation,the similarity factor(f2)method was used to compare the similarity of dissolu-tion behavior between 5 prepared tablets and original tablet. RESULTS:d(0.9)of powder A,B,C,D,E were 69.181,40.778, 24.805,12.611,3.083 μm,respectively. The corresponding f2 were 27.77,36.79,50.06,67.68,79.99. CONCLUSIONS:The par-ticle size of ticagrelor crude drug is much smaller,and the dissolution behavior of prepared tablet is closer to that of original tablet. The in vitro dissolution rate of Ticagrelor tablets is improved remarkably by micronization technology. In order to produce Ticagre-lor tablets with the same bioavailability as original tablet,particle size of ticagrelor crude drug powder should be controlled with d(0.9)≤20μm.
9.14q32 miRNA cluster in hepatocellular carcinoma

Hepatocellular carcinoma ( HCC) , as one of the most common malignant neoplasms , has a relatively high morbidity and mortality rate worldwide.MicroRNAs (miRNAs),a type of comparatively conserved endogenous small non-coding RNAs, function as pivotal regulators involved in various biological functions through the post -transcriptional regulation of gene expression .Some miRNA genes are arranged into a intandem model and reside close together on the same chromosome , forming miRNA clusters . These clustered miRNAs are mostly located on polycistronic transcripts and expressed at similar levels.In the human imprinted 14q32 region, 52 miRNA genes are organized as a large cluster which spans about 220 kb of the long arm ( q) .Lines of evidence show that dysregulation of miRNAs in this cluster are involved in the development of HCC .This review summarizes the structural characteristics of 14 q32 miRNA cluster as well as its impact on HCC in initiation and progression .
10.Effect of contrast-enhanced ultrosound imaging with CCR2 targeted microbubbles on murine breast tumor model

Objective To develop and validate a ultrasonographic (US)imaging agent with targeted microbubbles that attaches to chemokine receptor 2 (CCR2)and to compare the US single obtained from targeted microbubble with that from control microbubble in murine breast tumor model.Methods The microbubble which carried CCR2 antibody (MBCCR2 )and isotype-macthed immunoglobulin G-labled control microbubble (MBcontrol ) were prepared. The microbubble size and distribution were assessed by AccuSizer780.Binding specificities of targeted microbubble compared with control microbubble were tested with murine microvascular endothelial cells (bEnd.3 ).Orthotopic breast tumor model was estabished in BALB/c mice with mouse breast cancer 4T1 cell.In vivo imaging signals of contrast material-enhanced ultrasound by use these two different types of microbubble which were injected respectively into each mouse at random order and 30 min interval.Tumor tissue was stained for CCR2 and CD3 1 .Results Automatic Particle Sizer showed size uniform of two kinds of microbubbles,and narrow distribution of particle size (mean diameter of about 1 -2 μm),which were not significantly different (P >0.05).Adhension to bEnd. 3 endothelial cells was significantly higher (P < 0.001 )for MBCCR2 (mean,9.50 ± 1 .5 1 )than that for MBcontrol (mean,0.01 ±0.01).Imaging signal in the murine tumor model was significantly higher for MBCCR2 [mean,(6.76±0.26)dB]than that for MBcontrol [mean,(1 .06 ±0.62)dB,P <0.001 ].Immunofluorescence confirmed expression of CCR2 on tumor vasculature.Conclusions The targeted microbubbles with CCR2 monoclonal antibody had been successfully prepared,which precisely targeted to CCR2 of tumor angiogenesis in the murine breast cancer xenograft tumor models in vivo.These results suggest that the targeted microbubbles as a kind of ultrasound molecular imaging agent with a better specificity can be used for both evaluating tumor neovascularization and monitoring therapeutic effect of anti-angiogenesis.
