1.Speckle-tracking imaging and tissue Doppler strain imaging in assessing left atrium mechanical function: A study on patients with transcatheter device closure of atrial septal defect

Objective To compare the value of speckle-tracking imaging and tissue Doppler strain imaging in evaluating left atrial (LA) mechanical contraction. Methods Forty patients after successful percutaneous atrial septal defect (ASD) closure were enrolled in this study. Two-dimensional strain imaging (2D strain echocardiography, 2DSE) and tissue Doppler imaging (TDI) were performed one weak after ASD closure. Off-line analysis was done for atrial longitudinal peak systolic strain on the interatrial septum, in correspondence of the device, and on the lateral wall of the left atrium from apical 4 chamber view with 2DSE and color-coded strain imaging, respectively. Results There was no statistical difference of the inter- and intra-observer viability as well as time consumption on the off-line analysis between 2DSE and TDI. LA lateral wall strain was much higher than that of LA septum assessed by both color-coded strain imaging (47.31%±27.25% vs 30.06%±14.29%, P<0.01) and 2DSE (43.49%±25.55% vs 12.74%±9.16%, P<0.001). LA lateral wall strain was similar with either method (P=NS). However, at the site of ASD occluder, a non-contractile element, deformation was significantly higher in color-coded strain imaging than 2DSE (P<0.001). Five patients (12.50%) presented absence of deformation on ASD occluder with 2DSE, whereas there was no patient with TDI assessment (χ~2=5.33, P=0.027). Conclusion Speckle-tracking imaging is superior to tissue Doppler strain imaging in assessing left atrium mechanical function.
2.Assessment of fetal ventricular volume and systolic function using real-time three-dimensional echocardiography

Objective To discuss the worthiness of real-time three-dimensional echocardiography in researching normal fetal heart ventricle growth and systolic function. Methods End-systolic volume(EDV),end-diastolic volume(ESV), struggle volume(SV) and ejection fraction(EF) of 54 normal fetal were acquired from 3-D data by Qlab software. The relation between ventricular growth with pregnant week was analysed and the difference of the volume and systolic function between left ventricle with right ventricle were compared. Results Ventricular volume of normal fetal heart (EDV,ESV,SV) were all increased with the gestational ages,there was linear relativity between them,while EF was not increased with pregnant weeks,there was no linear relativity between them. There was no statistics difference on EDV and ESV between left ventricle and right ventricle, while there was statistics difference on SV, EF between them. Conclusions The image of fetal endocarium could be derived clearly by real-time three-dimensional echocardiography, which help to get fetal heart ventricular volume and to study fetal heart growth and function.
3.Diagnosis of fetal complete atrioventricular block with echocardiography

Objective To evaluate the application of M-mode echocardiography and Doppler echocardiography in diagnosing complete atrioventricular block (CAVB) in fetus. Methods M-mode and Doppler echocardiography were used to screen the fetuses and bradycadia was established as CAVB in 10 cases. Atrial and ventricular rhythm was measured by M-mode echocardiography. Flow of mitral valve, left ventricular in-flow and out-flow tract, venous duct was measured by Doppler echocardiography. The characteristics and prognoses of CAVB fetus were compared and analyzed. Results CAVB was established as independence in rhythm of atrium and ventricle. The rhythm of atrium could be in normal range, while the rhythm of ventricle should be slower than normal. Enlarged chambers were observed in 6 cases, cardiac dysfunction in 5 cases, and pericardial effusion in 7 cases. Tricuspid regurgitation and mitral regurgitation existed in 5 and 1 case, respectively. All of the CAVB fetus in this study underwent abortions. Conclusions Fetal echocardiography is proven to be a useful tool to diagnose CAVB, which greatly influenced the cardiac function in fetuses. Clear diagnosis as early as possible is crucial to the treatment of CAVB fetus.
4.Retrospective analysis of echocardiography on criss-cross heart

Objective To describe the characteristics of the criss-cross heart with comprehensively segmental echocardiographic scanning in order to explore the diagnostic value of the echocardiography.Methods Ten cases of criss-cross heart were retrospectively reviewed with 2-dimensional,color Doppler echocardiography to summarize the main diagnostic methods.Results The right ventricle was located superiorly and the left ventricle inferiorly in 9 patients and paralleled alignment in 1.The solitus situs was observed in 9 patients and dextrocardia in 1;the atrioventricular connections were concordant in 8 patients and discordant in 2.Echocardiography revealed that 5 patients presented with transposition of great artery and 5 with double outlet of right ventricle as well as all with ventricular septum defect.Conclusions Clear visualization is available with echocardiography to show the crossing inflow streams at the level of atrial-ventricular valve,abnormal alignment of the ventricular septum as well as the associated abnormalities.
5.Realization of dose verification tool for IMRT plan based on DPM

Objective To build a Monte Carlo dose verification tool for IMRT Plan by implanting an irradiation source model into DPM code and to extend the ability of DPM to calculate any incident angles and irregular-inhomogeneous fields.Methods The virtual source and the energy spectrum unfolded from the accelerator measurement data were used,in combination with optimized intensity maps,to calculate the dose distribution of the irradiation irregular-inhomogeneous field.The irradiation source model of accelerator was substituted by a grid-based surface source.The contour and the intensity distribution of the surface source were optimized by IMRT.The dose calculation was realized by combining the position of the emitter with the fluence map from the IMRT plan.The weight of the emitter was decided by the grid intensity.The direction of the emitter was decided by the combination of the virtual source and the emitting position.The weighted fraction of the emitter was also combined with the flux grid intensity based on the particle transport model of DPM code.Results The accuracy of calculation was verified by comparing with the measured data.It was illustrated that the differences were acceptable (< 2% inside the field,2-3 mm in the penumbra).The dose calculation of irregular field by DPM simulation was also compared with that of FSPB (Finite Size Pencil Beam).The passing rate of gamma analysis was 95.1% for peripheral lung cancer.The regular field and the irregular rotational field were all within permissible range of error.The calculation time of regular fields were less than 2 h,and that of the test of peripheral lung cancer was 160 min.Conclusions The adapted DPM code with its simple irradiation source model is faster than that with classical Monte Carlo procedure.Its computational accuracy and speed satisfy the clinical requiremcnt,and it can be useful as a Monte Carlo dose verification tool for IMRT Plan.
6.Protection against acute hypoxic/reoxygenation injury to kidney for rabbit with morphine hypoxic preconditioning by observing the expression of caspase-3 protein.

The maintenance of the balance between oxygen supply and oxygen consumption is a key measure in preventing acute kidney hypoxic/reoxygenation injury. Morphine can inhibit metabolism and reduce the oxygen consumption. We tried to investigate the protective effects of morphine hypoxic preconditioning on acute kidney hypoxic/reoxygenation injury in rabbit and its influence on expression of caspase-3 protein. Kidney hypoxic and reoxygenation were induced by making the tested rabbits inhale 8% oxygen for three hours firstly, and then putting them in the air to breathe in normal oxygen for another three hours. Morphine hypoxic preconditioning was induced by administering morphine 3 mg/kg, and then hypoxic of 8% oxygen was induced. Caspase-3 protein expression in renal tissue was assessed by immunohistochemical method. In the present study, the expressions of caspase-3 protein were significantly higher in saline-control hypoxic group than in morphine hypoxic preconditioning group ((29.3+/-5.7)% vs. (12.16+1.23)%, P<0.05). These observations suggested that morphine hypoxic preconditioning can protect rabbit against acute kidney hypoxic/reoxygenation injury by decreasing expression of caspase-3 protein.

In this study, retrospective multi-center study on medical records of Uyghur medicine diagnosis and treatment program of osteoporosis was done and demographic, diagnostic, therapeutic, efficacy evaluation and nursing data and other information were collected. The main and secondary disease manifestations of osteoporosis were analyzed inductively. Osteoporosis was divided into 2 types in Uyghur medicine field, which were abnormal Savda type and abnormal Balgham type. Uighur medicine syndrome type standards of osteoporosis were established. Treatment efficacy was evaluated according to bone pain, bone density, life quality, and improvement of clinical symptoms and Uyghur medical symptoms. On this basis, in accordance with the technical requirements of the national drug review, the study developed clinical research guidelines for the treatment of osteoporosis with new Uyghur medicine (draft).
8.Clinical observation of acupuncture combined with theJintiange capsule in the treatment of lumbar disc herniation

Objective To explore the clinical efficacy and safety of acupuncture combined with Jintiange capsule for the patients with lumbar disc herniation (LDH).Methods According to the random table method, 95 cases of patients with LDH were divided into the control group (n=47) and the research group (n=48) from June 2015 to March 2017. The patients in the control group were treated by acupuncture, while the patients in the research group were treated withJintiange capsule on the basis of the control group. The continuous treatment for 4 weeks was a course, and the treatment of two groups was 2 courses. The clinical total effective rate and symptom control time of two groups were observed. The pain intensity, JOA, the straight leg-raising angle and the serum level of inflammatory factors of two groups of patients were detected before and after treatment respectively. In addition, the incidence of adverse reactions of two groups was analyzed. Results The total clinical effectiveness of the research group was 93.75% (45/48), and the control group was 76.60% (36/47). The difference of two groups was statistically significant (χ2=5.562,P=0.018). The symptom control time (9.2 ± 2.3 dvs. 14.0 ± 2.3 d,t=-10.406) of the research group was significantly lower than that of the control group (P<0.05). After treatment, the straight leg-raising angle [(78.6 ± 17.6)°vs. (59.9 ± 3.2)°,t=5.869] and JOA (18.7 ± 2.1vs. 12.1 ± 1.9,t=16.152) of research group was significantly higher than those of the control group. While the VAS score (1.1 ± 0.3vs. 2.6 ± 0.6,t=-15.997), TNF-α (1.18 ± 0.22 ng/Lvs. 1.51 ± 0.21 ng/L,t=-7.476), IL-1β (25.14 ± 6.17 ng/mlvs. 40.26 ± 6.07 ng/ml,t=-12.038), IL-6 (93.85 ± 23.02μg/Lvs. 110.63 ± 24.07μg/L, t=-3.473) and TXB2 (299.36 ± 52.16 pg/mlvs. 429.17 ± 53.62 pg/ml,t=-11.961) of research group were significantly lower than those of the control group (P<0.05). Both groups had adverse events during the treatment.Conclusions Acupuncture combined withJintiange capsule could effectively improve the clinical symptoms, release pain and decrease the serum inflammatory factors of patients with LDH.

Uyghur Medicine syndrome type standards of acne vulgaris were established. At the same time, through evaluating skin integrity and morphology of lesions as well as changes in patients’ symptoms and quality of life and observing changes in patients’ internal environment respectively before and after treatment, disease and syndrome clinical criteria of acne vulgaris were established. On this basis, in accordance with the technical requirements of the national medicine review, this study developed clinical research guidelines for the treatment of acne vulgaris with new Uyghur Medicine.
10.Influencing factors of learned helplessness in patients with primary liver cancer treated by interventional therapy

Objective:To investigate the influencing factors of learned helplessness in patients with primary liver cancer treated by interventional therapy.Methods:This study was a cross-sectional study. The 221 patients with primary liver cancer treated by interventional therapy in the Second Affiliated Hospital of Air Force Military Medical University from August 2020 to October 2021 were selected as the subjects. General data statistics, Learned Helplessness Scale and Support in Intimate Relationships Rating Scale were used for questionnaire survey.Results:The total score of learned helplessness in the 221 patients with primary liver cancer treated by interventional therapy was 74.12 ± 6.55. Gender, education level, family per capita monthly income, number of interventions and perceived spouse support were the main influencing factors of learned helplessness in patients with primary liver cancer treated by interventional therapy (adjusted R2=0.891, F=65.65, P<0.05). Conclusions:The sense of learned helplessness of patients with primary liver cancer treated by interventional therapy is at a high level. According to patients with features of male, lower education level, lower family per capita monthly income, more number of interventional therapy or lower perceived spouse support, medical staff can use dialectical behavior therapy, symptom group intervention, spouse support intervention and other methods to reduce learned helplessness of patients.
