1.Color Doppler ultrasound in the assessment of the retinal detachment

Objective The aim of the study was to determine the diagnostic value of color Doppler ultrasound in the assessment of the retinal detachment. Methods The changes of ultrasonograph of the retinal detachment were retrospectively observed in 35 cases of 31 opei-ation proven retinal detachment patients. Results Color Doppler ultrasound had an accuracy of 97.1% (34/35) for diagnosing retinal detachment. In 33 cases of retinal detachment, the results of color Doppler flow imaging(CDFI) showed the color blood-flow signal in the vitreous membrane echo,extending with central retinal artery and vein. Conclusion For the color blood-flow signal in the vitreous membrane echo by CDFI ,extending with central retinal artery and vein, has a specificity of diagnostic value for retinal detachment, and the color Doppler ultrasound has a high accuracy for diagnosing retinal detachment.
2.Color Doppler ultrasound in the assessment of the breast mass

Objective The aim of the study was to determine the diagnostic value of color Doppler ultrasound in the assessment of the breast mass.Methods The results of ultrasonograph of the breast mass were retrospectively observed in 79 operation and pathological proven breast mass parients,in which 47 patients had proven malignant breast mass and 32 patients had proven benign malignant breast mass.Results Color Doppler ultrasound had a sensitivity of 100%for diagnosing breast mass,accuracy of 93.7%(30/32)and 91.4%(43/47)for diagnosing malignant and benign mass.Conclusion Color Doppler ultrasound has high diagnostie value in distinguishing malignant from benign mass.

Objective To evaluate the diagnostic value of serum albumin (Alb),total protein (TP),body mass index (BMI),Mini Nutritional Assessment (MNA) and hand strength in the nutritional status in maintenance hemodialysis (MHD) patients.Methods A total of 126 MHD patients were included in this study who had been on MHD for at least 3 months.Depending on the levels of Alb,patients were divided into two groups:normal nutrition group (group A) and malnutrition group (group B).TP,BMI,MNA and hand strength were also detected at the same time.Independent samples t test,Spearman correlation analysis,ROC curve were used to analyze their difference between the two groups and evaluate their diagnostic value in nutritional status in MHD patients.Results Age,sex,height,weight and dialysis ages had no statistical significant difference (P > 0.05) between group A and group B,while Alb,TP,BMI,MNA and hand strength had statistical significant difference (P < 0.05) between two groups.After adjusting for age,sex and hemodialysis age,Alb,TP,BMI,MNA and hand strength were positively correlated with each other (P < 0.05).Since the area under the ROC curve of BMI was the smallest,BMI had the lowest diagnostic value in evaluation of the nutritional status in those patients.Conclusions Alb,TP,MNA and hand strength are good indexes in evaluation of the nutritional status in MHD patients but BMI is not.
6.Evaluation of blood flow perfusion of different function transplanted kidney with conventional ultrasound and CEUS

Objective To compare blood flow perfusion conditions of renal allograft of different renal function with conventional ultrasound and contrast enhanced ultrasonography (CEUS). Methods Sixty patients underwent kidney transplatation were divided into normal group (SCr≤110 μmol/L) and abnormal group (SCr>110 μmol/L). Renal structure and inner diameter of renal artery were messured with two-dimensional ultrasound. The hemodynamic parameters (PSV, EDV and RI) were messured with CDFI. Data of renal blood flow perfusion (AT, TTP, △I and Increase), data of renal blood flow perfusion (AT, TTP, △I and Increase) were quantitatively analyzed with CEUS combined with time intensity curve. All the data were compared between two groups.Results There was no significantly difference of above indexes for B-mode ultrasound and CDFI between two groups, except EDV of arcuate arteries. CEUS parameters of patients with normal renal function were better than those with abnormal function.Conclusion Microcirculation perfusion changes of transplanted kidney with abnormal function could be detected with CEUS. The quantitative indexes of transplanted kidney with normal function were better than those with abnormal function.
7.Application of conventional ultrasound and contrast-enhanced ultrasonography in comparison of the two different artery remodling techniques for kidney transplantation

ObjectiveTo assess long-term effect of two different types of arterial reanastomoses for kidney transplantation [an end-to-side anastomoses (ESA) of renal artery to external iliac artery and an endto-end anastomoses (EEA) of renal artery to internal iliac artery] by comparing conventional ultrasound and contrast-enhanced ultrasonography(CEUS) after five years of renal transplantation.MethodsSixty patients underwent kidney transplatation were included and divided into EEA group or ESA group based on the type of anastomoses. Data of renal blood flow perfusion,hemodynamic parameters,and renal anatomical structure were collected and analyzed in each groups using CEUS,color Doppler flow imaging(CDFI) and B-mode ultrasound.ResultsThe patients of the two arterial reanastomoses had no significantly difference in B-mode ultrasound and CDFI. The ESA patients had a slightly better perfusion than the EEA ones in parameters of CEUS.Conclusions Transplant tissue perfusion imaging was performed by CEUS. The perfusion of ESA grafts was slightly better than that of EEA.
8.Initial application of virtual touch tissue quantification in diagnosis of chronic allograft nephropathy

Objective To investigate the feasibility of virtual touch tissue quantification(VTQ) for the assessment of chronic allograft nephropathy(CAN).Methods 48 patients with normal renal function and 50 patients with CAN were checked by color Doppler and VTQ technique.All the results were compared between two groups.Results Mean VTQ-values were significantly different between the two groups( P < 0.05).ROC curve displayed that VTQ value of 2.51 m/s could be used to diagnose CAN,the sensitivity,specificity,positive predictive value and negative predictive value were 76.0%,52.1%,60.3% and 65.7%,respectively.The diagnostic efficiency was 62.2%.Conclusions Parenchymal stiffness measured by VTQ is stable and repeatable,which can be used to diagnose patients with CAN and to monitor renal function.
9.Surgical treatment of vascular injury

Objective To study the management of vascular injury.Methods Retrospective analysis was made on the clinical data of 59 cases of vascular injury,including 55 cases of vascular injury in neck and(extremity) and 4 cases of portal vein and vena cava injury.Among them,21 cases had femoral artery injury with infection and 4 cases had vascular injury due to intervention therapy.All patients with vascular wound of extremity or neck had undergone hemostasis by compression and antishock treament before hospital admission.All cases of femoral artery injury with infection underwent hemostasis by arterial ligation and incision and(drainage) of abscess.Vascular anastomosis was performed in 11 cases,vascular grafting in 12 cases,and(vascular) repair in 14 cases.Results There were 2 deaths.5 cases had amputation(including a case of(femoral) embolism due to intervention trerapy).Postoperative intermittent claudication,decreased skin(temperature) and other signs of ischemia occurred in 21 cases of femoral artery injury with infection,but none developed limb gangrene. The other cases were discharged in good health.Conclusions In the treatment of vascular injury,wound hemostasis and antishock treatment should be done first to save the patient′s life and the management of the vascular injury depends on the situation,with the aim to try by all means to save the extremity.Vascular reconstruction is the main method for treatment of vassular injury.Vascular ligation can be done in cases of femoral artery injury with infection.
10.Twenty-year experience in the diagnosis and treatment of hepatic trauma

Objective To summarize the experience in the diagnosis and treatment of hepatic trauma. Methods The clinical data of 260 patients with hepatic trauma admitted from January 1988 to December 2007 were retrospectively reviewed with regard to degree of trauma, treatment methods, therapeutical effects, complications and SO on. Results One hundred and fifty-three eases were treated by operative management,1 07 cases by nonoperative management.236 cases were cured,24 cases died,and the case fatality rate was 10.2%.There were no death among 139 patients with hepatic trauma grades Ⅰ~Ⅱ,22 death among 119 grades Ⅲ~Ⅴ patients, all death of 2 in grade VI, which demonstrated the correlation between death and hepatic trauma grade was statistically significant. Complications appeared in 82 eases, mainly including Secondary hemorrhage, abdominal infection and so on. Conclusions Nonoperative management is suitable for hepatic trauma with stable hemodynamics. Operative management is rapidly selected when the hemodynamics aren't stable. The cooperation of many specialities can enlarge the application of nonoperative management and decrease complications.
